[Prevention of severe hypotension caused by epidural anaesthesia for transurethral resection of the prostate (author's transl)].

Severe art. hypotensions are one of the most frequent complications of epidural anaesthesia. In 62 geriatric patients undergoing transurethral prostatic resection two methods of prophylaxis for hypotension were investigated: 1. 500ml HES before epidural anaesthesia and supine position until operation. 2. Lithotomy position immediately after EDA without plasma substitute. Blood pressure, heart rate and necessity for the application of vasoactive substances show that the lithotomy position immediately after EDA is the statistically better method. The problem of plasma substitutes in transurethral prostatic resection will be discussed.

[Metabolic behavior in pre, intra, and postoperative total parenteral feeding with a carbohydrate combination and amino acid solution].

Ten metabolically healthy patients who had undergone gastric resection were fed intravenously from the preoperative to the 5th postoperative day. No metabolic alternations were observed by using a low dosis of 0.12 mg/kg.h of glucose, fructose and xylitol in combination with amino acids. We were able to limit the so-called postaggressive syndrome by eliminating the fasting state. These results were proven by evaluating the most important metabolic parameters like triglycerides, free fatty acids, lactate and by measuring the carbohydrate-nitrogen balances.

[A study of the carbohydrate and electrolyte balance of 10 surgical patients during 7-days' infusion therapy (author's transl)].

For postoperative parenteral feeding fructose proved preferable to glucose and xylitol. Although the infusions were started pre-operatively sodium deficiency and a fall in serum osmolarity developed during the postoperative stage. Administration of potassium to normalize lowered serum potassium levels was needed only on the first and second postoperative day. Serum calcium levels fell during the 7-days' period of observation but remained within the limits of normal. If parenteral feeding is to be continued for longer periods determination of the phosphorus balance is advisable to avoid excessive depletion of the endogenous phosphorus stores. Estimations of urinary osmolarity and urine volume indicated that fluid uptake was adequate.

[Ultrasonic diagnosis in urology].

In recent years ultrasonography has been established as a reliable diagnostic method in various urological disorders. The first and most important indication is the differentiation between renal cysts and solid tumors. But ultrasound is suitable, too, for the diagnosis of haematomas, abscesses and hydronephrosis in the postoperative period and for the evaluation of renal transplants to detect early signs of rejection. Ultrasound allows the examination of patients with renal insufficiency and of patients with unilateral absence of contrast medium excretion in urography, because it is independent of contrast medium and renal function. In these patients as well as in traumatized patients ultrasound can be used as a bed-side method. Furthermore, it is possible to diagnose retroperitoneal tumors causing obstruction, megaureter, and tumors of the bladder. For the diagnosis of the prostate the rectal application of an ultrasonic transducer seems to be the best method. With an ultrasonic Doppler probe the diagnosis of testicular torsion is possible on ground of the absence of intratesticular arterial pulsation. Finally the possibility of ultrasonic targeted percutaneous puncture of the kidneys, tumors and cysts as a diagnostic and occasionally therapeutic approach has to be mentioned.

[The influence of abnormal vertebral column on failure in epidural anaesthesia (author's transl)].

In 387 pat., we observed the interdependence of abnormal vertebral column and failures in epidural anaesthesia by medial tap with the "los of resistance" method. We could show that in contradistinction to other opinions scoliosis induces no higher rate of failure of epidural anaesthesia than the normal vertebral column. In patients with kyphoscoliosis and ossified ligaments the epidural space was identified less frequently than in the normal group, perforations of the dura were encountered more frequently. These results are statistically significant. In patients who had or have pain in the back (p.e. lumbago) and those with ossified ligaments we found significantly more pain during operation although analgesia was tested before. In these three abnormalities of the vertebral column we recommend to judicously balance advantages against disadvantages of epidural anaesthesia and alternative anaesthetic procedures.

[Histone phosphorylation of human lymphocytes after PHA stimulation in relation to age (author's transl)].

Lymphocytes of young and old donors are stimulated by phytohemagglutinin (PHA). The 32P-incorporation into the histones which occurs in the early phase after PHA-stimulation is determined. In lymphocytes of old persons the 32P-incorporation in the histones is diminished by about 50% with respect to the cells of young donors. Furthermore, it could be shown that in the histones prepared from old cells the amount of H1 is much increased. It is postulated that, in old age, the other 4 histones are poorly extractable.

[Parenteral feeding in cases of impaired liver function (author's transl)].

Patients with hepatic failure show pathological plasma amino acid chromatograms which closely correlate to the course of hepatic encephalopathy or hepatic coma. An improvement of the encephalopathy could be achieved by normalising the plasma amino acid levels. The conventional amino acid solutions used for feeding patients having hepatic failure do not allow for the special metabolic features of this condition, and with regard to hepatic encephalophathy they should not be used in parenteral nutrition. For the low-risk parenteral feeding of patients with hepatic failure, amino acid solutions must be used which meet both the problems of hepatic encephalopathy and the principles of parenteral nutrition.

[Determination of splenic size by ultrasonic scanning (author's transl)].

Two dimensional B-Scans make it possible to examine and measure the spleen. In 62 patients the size of the spleen was evaluated by ultrasound; the results were compared with the size of the organ after extirpation: In 58 of 62 cases there were correct results. Therefore, the ultrasonic determination of the splenic size is a reliable method for clinical questions.

[Ultrasonic diagnosis of space-occupying lesions of the kidney (author's transl)].

In 140 patients who were suspected of having space-occupying lesions of the kidney the ultrasound findings were analysed. There were no false negative results. Out of 4 false positives two could be explained by nontumourous focal renal disease. In a further 10 cases a space-occupying lesion could not be excluded with certainty on the grounds of the ultrasound investigation alone. Out of a total of 96 space-occupying lesions (48 solid tumours, 48 cystic processes) 83 were correctly diagnosed sonographically.

[Consumption coagulopathy in a case of cystic spleen (author's transl)].

A case of a 65-year old female patient is reported, dealing with consumption coagulopathy and secondary hyperfibrinolysis due to cystic spleen. Following treatment with heparin, antifibrinolytic agents and replacement of clotting factors prior to surgery, the patient attained a satisfactory condition to be operated on. After removal of the cystic spleen, which was interspersed with massive hemorrhages, blood coagulation returned to normal quickly and continuously. The pathogenesis of consumption coagulopathy in relation to cystic spleen is discussed and it is referred to the pathomechanism of similar blood disorders in hemangiomatous and cavernous lesions of Kasabach-Merritt-syndrome, Klippel-Trenaunay-syndrome and pachymeningeosis haemorrhagica interna.

[Effect of intramuscular dihydroergotamine on blood circulation in the early postoperative phase. A clinical study].

The effects of Dihydergot (1,0 mg i.m.) and a placebo on orthostatic regulation disturbances were examined during the mobilisation in the first post-operative phase. According to a modified Schellong-test, 100 fresh-operated patients with hyper- rsp. hypodiastolic regulation disturbances were checked on the first post-operative day. In comparison with the placebo, Dihydergot showed a stabilisation of the systolic blood pressure and a decrease of the pulse rate during orthostatic conditions. Neither Dihydergot nor placebo influenced the diastolic blood pressure. Orthostatically caused side effects were observed less frequent within Dihydergot - therapy than under placebo.

Possibilities and limitations of ultrasonic diagnosis of space occupying lesions in internal medicine.

Possibilities and limitations of ultrasonic tumour diagnosis in internal medicine (thyroid gland, liver, pancreas, kidney, malignant lymphomas) are discussed on the basis of a five years experience with about 4,000 examinations a year. A real time and gray-scale technique is used. The accuracy of the presented ultrasonic findings is proven by comparative studies. Besides the well known advantages of ultrasonography the independence from contrast medias must be stressed in comparison to diagnostic radiology. The main limitations of ultrasonography are the impossibility of diagnosing tumours smaller than 1.5-2 cm and the absence of an ultrasonic pattern typical of malignancy. To establish a morphobiological diagnosis, ultrasonically guided fine needle biopsy has proved to be a reliable method.

Biology of the twinning phenomenon: embryogenesis and teratogenesis.

The twinning phenomenon has always interested the great public, the artists, and naturally the scientists. Isidore Geoffroy St. Hilaire has established a classification still now valid. This classification considers the different types of double monsters which are found in nature in all classes of vertebrates, including man. To explain the twinning phenomenon, the experimental realizations have progressed by successive bounds after the preliminary attempts of different authors. Now, when it is question of experimental duplication, anybody thinks of Speman for the amphibians, of Lutz for the birds, of Seidel for the rabbit, and of Tarkowsky for the mouse. Now, it is possible to conceive a twinning resulting from the separation of the first blastomeres (amphibian, rabbit, mouse) and a twinning originating from the fissuration of the blastoderm (bird, mammal). All these experiments confirm the unicist-theory. If a total or partial regulation of the excedents may be experimentally realized, no argument can however support this theory in the realization of the double monstrosity.

Transgastroscopic ultrasonography.

A newly designed ultrasonography probe is presented which can be advanced through the instrumentation channel of an operative endoscope (TGF-Olympus). The A-scan allows the differentiation between solid and cystic masses in the upper abdomen leading to gastric impression. Guided puncture of pancreatic pseudocysts is based on a solid diagnosis and can be performed from the gastric lumen thus avoiding the development of pancreato-cutaneous fistulas.

[Ultrasonography of pancreatic diseases (author's transl)].

The use of a modern ultrasonic equipment with real time and gray-scale technique enables a riskless and less time consuming examination of the pancreas. The accuracy of ultrasonography in the diagnosis of pancreatic diseases is demonstrated by a clinical study in comparison to the endoscopic retrograde pancreaticography. By ultrasonography it is not only possible, to detect an enlargement of the pancreas, but also to differentiate between the various diseases of the pancreas in about 75% of the cases. Follow up controls in acute pancreatitis and the simultaneous examination of the neighbouring organs in all cases of a pancreatic disease are an important aid for the decision of surgical treatment. The ultrasonic targeted fine needle aspiration biopsy seems to be a suitable method to establish a morphological diagnosis in certain pancreatic diseases too.
