Study of the coefficient of glucose assimilation during muscular exercise in diabetic adolescents deprived of insulin.

In six insulin deprived adolescent diabetics, the influence of muscular effort equal to 50% of the VO2 max on the coefficient of glucose assimilation was evaluated. During an IVTT the coefficient of glucose assimilation at rest was (0.59). 10(-2) +/- 0.14 and it did not increase during physical activity. A minimal concentration of insulin is apparently indispensable to increase glucose utilisation during muscular effort. At higher concentrations other factors probably intervene to enhance glucose assimilation.

Definition of myocardial perfusion by continuous infusion of krypton-81m.

Krypton-81m, a radionuclide emitting a 190 kev gamma ray, has a half-life of 13 seconds. It is a radioisotope of an inert water-soluble noble gas and is produced at a constant rate by spontaneous decay of rubidium-81 in an 81Rb-81mKr generator-delivery system. Delivery is through a minibore Teflon catheter that can be threaded through a standard no. 7 or 8F angiographic catheter. The generator is eluted by 5 percent dextrose-in-water, delivered by infusion pump at 1.5 ml/min, and the eluate is infused intraarterially directly into any organ. Delivery and decay reach equilibrium within 2 minutes, producing a heterogeneous distribution that is proportional to the perfusibility of the tissue concerned and the time required to reach it. The ultrashort halflife of the radionuclide rapidly eliminates activity when delivery ceases; thus, experiments can be sequentially repeated at brief intervals. The radiation hazard for easily imaged doses is negligible. Preliminary studies in open chest dogs were visualized by scintillation camera, stored on digital data disk, processed, and displayed in dual channel, dual color mode on a video system. Images of myocardial perfusion defined relative levels of perfusion, collateral circulation between coronary arterial branches, equilibrium time of diffusible perfusion of the myocardium, focal defects in induced occlusion, collateral circulation to occlusion, and reactive hyperemia after release of induced coronary occlusion. The system and technique appear applicable to human subjects.

Permanent neonatal diabetes mellitus: a case report with plasma insulin studies.

The 11th case of permanent neonatal diabetes mellitus appearing during the first month of life is reported. A critical review of the literature is also presented. The permanence of diabetes is demonstrated by the duration of insulin therapy still necessary after 30 months. Insulin-stimulation tests have been performed some for the first time in such a young diabetic. They have shown a nearly total failure in beta-cell response, only very high doses of glucagon provoking a moderate insulin secretion. The absence of acetonuria is discussed. It can perhaps be explained by the hyperglycemia which, by a mass effect, brings about cellular glucose penetration and this stops liberation of Nefa's from adipose tissue.

[Comparison of various semi-quantitative tests for determination of urinary glucose in diabetics (author's transl)].

Estimations of urinary glucose in 178 diabetics using 4 tests, Ketodiastix, Tes-Tape, Clinitest 5/10 and Clinitest 2/10 were compared with polarimetric determinitation. Clinitest is more accurate and specific, and as reliable as enzymatic methods. The widened scale of Clinitest 2/10 permits the detection and satisfactory estimation of glycosuria ranging from 0,25% to 5%; it is specially suitable for monitoring the labile insulin dependent diabetes of children or adolescents. Enzymatic methods are suitable for the qualitative detection of glycosuria, for the control of stable, maturity-onset diabetes or to meet the need for rapidly available diagnostic information.
