[Benzamil and mucoviscidosis. Primary culture of nasal mucosa as an electrophysiologic in vitro model].

The genetic disease cystic fibrosis (CF) is characterized by a defect in the gene encoding for an epithelial chloride channel. This gene product, cystic fibrosis transmembrane conductance regulator (CFTR), in turn leads to a decreased chloride secretion. When this occurs sodium absorption is increased drastically because of an up-regulation of the epithelial sodium channel. In airway epithelia these changes in ion permeabilities result in dehydration of the mucus blanket and impaired mucociliary clearance, and subsequently lead to chronic infections of the airways. In this study we established a primary cell culture of human nasal epithelium from CF and non-CF patients. The increased transepithelial potential in CF exclusively was due to the greater sodium absorption. In previous studies, amiloride was found to specifically block the epithelial sodium channel. Inhalation of nebulized amiloride has been used clinically in the treatment of CF to inhibit sodium absorption and exert a secondary effect on water withdrawal. In the present cell culture model benzamil, an amiloride analogue, was found to inhibit sodium absorption completely, but at strikingly lower concentrations than amiloride. This raises the possibility of using benzamil for inhalation and to provide better efficacy in the symptomatic therapy of patients with CF.

Childhood asthma: mothers' affective attitude, mother-child interaction and children's compliance with medical requirements.

Maternal critical attitude (CA) and negative mother-child verbal interaction in families with an asthmatic child and in controls were investigated in the present study. Twenty-eight children with bronchial asthma (6-13 yr) and 23 healthy children with their mothers participated in the project. Mothers were asked to describe their child (Five Minute Speech Sample; FMSS). Mother and child were then asked to discuss a mutual problem (Problem Discussion; PD). Tape recordings were taken. The mothers' responses during the FMSS were used to assess CA. Negative verbal behavior during PD was coded by means of a reliable coding system for dyadic interaction. Significantly more mothers of the asthmatic children than of the controls showed CA during the FMSS. Mothers of the asthmatic children made significantly more critical remarks and offered more negative solutions during the PD. Within the asthma group mothers who made more critical remarks had to make more effort to have their child comply with the medical requirements. Additionally, mothers showing a higher total amount of negative verbal behavior had children with rather high IgE levels. There was no systematic difference between children having CA mothers with high vs low face-to-face contact.

[Croup syndrome].

Common causes of acute laryngotracheobronchitis (LTB) are viral infections. More rarely, bacterial germs, unspecific irritants, foreign bodies, rachitic laryngospasm, mild malformation, tumours, C1 esterase inhibitor deficiency, bilateral vocal cord paralysis, and psychogenic laryngospasm may be responsible for croup. Symptoms similar to epiglottitis may occur in pharyngitis based on common bacterial tonsillitis or infectious mononucleosis and peritonsillar abscess. It is decisively important to establish a precise diagnosis to provide for an appropriate therapy. Viral croup of mild degree is often sufficiently treated by cold and moistened air and--if necessary--prednisolone. In serious disease, oxygen insufflation and adrenaline (epinephrine) are useful. Recurrent croup is due to an unspecific hyperreactivity of tracheobronchial mucosa. It often leads to asthma. Consequently, preventive measures have to be considered similar to patients with bronchial hyperreactivity. Vaccination with haemophilus influenzae type b vaccine has proved effective and safe. The disease has therefore become impressively less frequent.

[Amiloride inhalation therapy in mucoviscidosis].

The defective regulation of the secretory chloride channel and the accelerated rate of sodium absorption in the airway epithelia of patients with cystic fibrosis (CF) contribute to the dehydration of airway secretions which is responsible for secondary damage in the respiratory tract. The sodium-blocker amiloride inhibits sodium absorption depending on the dosage of the drug. This effect is documented by means of measuring transepithelial potential difference which is decreased following to inhalative amiloride administration in CF patients. Clinical long term efficiency has been shown to be probable, but has to be confirmed. Investigation in this field is difficult, because there are some other important factors which influence viscosity of mucus. The use of amiloride can best be evaluated, if it is given immediately after birth. However, the answer to some important questions has to be found before.

Childhood asthma: paternal critical attitude and father-child interaction.

The study investigated paternal critical attitude (CA) and negative father-child communication in families with an asthmatic child and in controls. Fathers were asked to describe their child (Five Minute Speech Sample; FMSS). Additionally, father and child discussed a mutual problem (Problem Discussion; PD). Tape recordings were taken. Critical attitude (CA) was assessed from the fathers' responses during the FMSS. Negative verbal behavior during PD was coded for father and child. During the FMSS, significantly more fathers of the asthmatic children than of the controls showed CA. During the PD, fathers of the asthmatic children made significantly more critical remarks than their controls. Significantly more long sequences of negative verbal communication occurred in father-child dyads with an asthmatic child than in the control dyads. No significant relationship was found between either the fathers' attitudes or amount of negative verbal communication and the children's severity of asthma, compliance, or IgE status.

[The value of physical therapy with VRP 1-Desitin ("Flutter")].

UNLABELLED: Preliminary examinations with an innovative device for physiotherapy, VRP 1-Desitin ("Flutter"), in patients suffering from chronic mucus retention and bronchial collapse gave encouraging results. Therefore, we tried to confirm these findings and to evaluate the use of this new physiotherapy. It is based on oscillations of air in the respiratory tract during expiration (rate about 2 to 32 Hertz) diminishing adhesiveness of bronchial mucus and bronchial collapse. Pressure and flow changing depend on the position of mouth-piece and effort of breathing. In order to establish the efficiency of the VRP 1 a comparing investigation was initiated. 20 patients with cystic fibrosis, aged 7 to 28 years, performed physiotherapy with VRP 1 and autogenic drainage (AD), respectively, in a randomized order one after another, each physiotherapy taking twenty minutes. The expectorated sputum was weighed by means of a precision balance. Mean values during VRP 1 treatment were 5.0 g (range 0 to 12.0 g), during AD 4.8 g (range 0 to 11.7 g). There was no statistical difference (p < 0.05). The results were not influenced by the order of physiotherapy. CONCLUSIONS: VRP 1 physiotherapy is as effective as AD with respect to sputum elimination. In opposite to other methods not requiring a helping person it is simple to teach and to learn. It may be performed by young children (> or = 3 years) and adults, also in combination with inhalation. Thus, VRP 1 appears to be a real enrichment in physiotherapy.

[Quantitative assessment of partial flow-volume curves in respiratory healthy and sick infants and young children].

In 83 patients of 1 to 24 months of age the flow quotients were calculated at 75, 50 and 25% of the tidal volume. The subjects were classified into four groups, depending on their basic disease: 1. healthy patients in respect of respiration; 2. patients with intrathoracic obstruction; 3. patients with extrathoracic obstruction and 4. patients suffering from a combined obstruction. It was found that in particular the quotient Fi25/Fe25 is suitable for differentiation between the studied groups.

[Histamine provocation test in children].

Bronchial challenge testing by means of histamine inhalation is one of the classical methods to examine bronchial response in children. There are some special problems, in particular, a limited cooperation in forced breathing manoeuvres which are performed to control lung function during the challenge procedure etc. In addition, we must be aware of the difficulties to quantify the histamine dose due to a strong variability of breathing. The evaluation of results is aggravated by a marked age dependency of the threshold of bronchial response and by a strong overlapping of the histamine response between healthy children and those suffering from bronchial hyperreactivity. In school-children a bronchial hyperreactivity is established, if a bronchoconstriction occurs below a histamine concentration of less than or equal to 1000 mg/ml.

Prevention of bronchial hyperreactivity in children.

Hyperresponsiveness of the bronchi is defined as an excessive reaction of bronchial mucosa to irritants that do no harm to a healthy mucosa. Measures to prevent bronchial hyperreactivity in children with asthma and other diseases remain rather ineffective at present. Though progress has been made in some areas.
