Gamma-aminobutyric acid and homovanillic acid concentration in the CSF of patients with senile dementia of Alzheimer's type.

The investigation of enzyme and neurotransmitter levels and/or their metabolites in the CSF of patients with senile dementia of Alzheimer's type (SDAT) could become a promising approach for a clinical research and diagnostic procedure. To learn more about the metabolic reflections of central metabolism in the CSF of patients with SDAT, we measured CSF levels of gamma-aminobutyric acid (GABA) and homovanillic acid (HVA), the dopamine metabolite. In 16 female patients with SDAT and in eight matched control patients, CSF GABA levels were measured by ion exchange with fluorimetric detection, and HVA levels were measured fluorimetrically. The GABA content of the CSF was significantly reduced in patients with SDAT, whereas the HVA level was unchanged.

[Inpatient behavior therapy of depression--a multimodal approach in clinical practice].

Using a reinforcement-oriented model of depression, we applied a multimodal approach of behavior therapy in clinical setting. We modified the self-control-group-therapy of psychoreactive depression, developed by Fuchs und Rehm (1977) in order to reach applicability for inpatients of psychiatric hospital. Operant techniques, social skills training, self-control-techniques, and the cognitive techniques developed by Beck (1979) were used. In contrast to Becks cognitive therapy we emphasized both the motivational aspect of depression and the aspect of self-regulation and self-modification too. There was not only the therapist trying to change behavior, thinking, and feeling of the patient by cognitive techniques. The patient himself tried to develop cognitive and behavior coping strategies. The treatment of patients with severe depression in an early stage was controlled by the therapist, later on they learned to control themselves. A central point of the concept of self-modification is teaching well founded knowledge of behavior analyses to the patient. Self-modification is not very efficient without, and the patient's motivation to work on his own therapy is elevated significantly. In some cases we continue this form of therapy after the patient's leaving the hospital up to three months.

Mercy killing without consent. Historical comments on a controversial issue.

In recent discussions on euthanasia the question about mercy killing without consent of mentally ill persons has turned up, particularly in cases of hopeless suffering among senile or chronically insane patients. Possible analogies to the actions under the German Nazi-regimen and relations to the concept of Social Darwinism with suggestions about active eugenic control as propagated in the early part of this century are outlined in a historical review. The implications and obvious dangers of misuse of any such kind of active euthanasia in psychiatry are discussed on this background.

[Family care for the aged in multigenerational households (author's transl)].

Multigenerational household with widowed elderly above the age of 70 have been studied by interviewing the aged person and his/her daughter/daughter in law. The sample is described according to household composure and some sociodemographic characteristics. Data analysis focused on the relationship between variables in the elderly person and variables in the daughter/daughter in law. Psychic and somatic well-being in the relatives can be predicted by the degree of competence/dependency in the aged family member, but less so with a general health assessment. Organic brain conditions in the aged parents seem to have most adverse effects on the daughter/daughter in law when they are of a moderate degree.
