Estimate of the excessive spinal cord dose in AP-PA thorax treatment.

By providing quick reference graphs, we have attempted to answer the question "Does this patient need a compensating filter to keep the spinal cord dose within a certain limit?" Such graphs were obtained using several parameters that influence compensating filtration when treating the mediastinum with (anterior-posterior) parallel opposed fields. For typical 5 and 10% excess spinal cord doses above the tumor dose, the reference graphs were designed to provide quick answers for patients with the following measured parameters: (1) patient thickness at the beam axis, (2) difference in source-to-skin distances, (3) air gap between the posterior skin and the couch top, and (4) posterior spinal cord depth. 4, 10, and 18 MV photon beams were used to illustrate the results.

Melibiosylceramide as the sole ceramide dihexoside from the eggs of the sea urchin, Anthocidaris crassispina.

Melibiosylceramide (Gal alpha 1-6Glc beta 1-1Cer) was found as the sole ceramide dihexoside from the eggs of the sea urchin, Anthocidaris crassispina. Ceramide monohexoside of the eggs consisted only of glucosylceramide (Glc beta 1-1Cer). These lipids were purified by successive column chromatographies on DEAE-Sephadex A-25, silicic acid and Florisil, and identified by gas-liquid chromatography, negative-ion fast atom bombardment mass spectrometry and proton nuclear magnetic resonance spectroscopy as well as methylation analysis. Long-chain base compositions of both lipids were almost identical and comprised n-C18-phytosphingosine and small amounts of its homologs (C17-C19). Fatty acid compositions were qualitatively very similar, but the glucosylceramide contained more 2-hydroxy fatty acid than the melibiosylceramide. Although the chain length of fatty acids was distributed over a wide range, six major fatty acids, namely 22:1, 23:1, 24:1, 22h:1, 23h:1 and 24h:1, constituted more than 92% of the fatty acid content in these lipids.

Immunohistochemistry of desmin and vimentin in smooth muscle tumors of the digestive tract.

The expression of desmin and vimentin in 35 "smooth muscle tumors" of the digestive tract was investigated using formalin-fixed and paraffin-embedded materials and the indirect immunoperoxidase method. Fourteen of 15 esophageal tumors and two of 15 gastric tumors were categorized as benign leiomyoma with low cellularity, and immunohistochemically were desmin-positive and vimentin-negative as were normal muscle layers of the digestive tract. The remaining tumors showed moderate to high cellularity: Six tumors seemed to be malignant on the basis of frequent mitosis and/or nuclear atypia, while 11 were considered to be of borderline malignancy. This cellular tumor group exhibited consistent vimentin immunoreactivity, but desmin was negative or only weakly and/or focally positive. S-100 protein was fundamentally negative in these tumors. The pitfalls of studying desmin immunohistochemistry as a routine diagnostic tool in surgical pathology are discussed.

[Dissecting aortic aneurysm of DeBakey type II diagnosed by color Doppler flow imaging and treated by a Bentall surgical procedure: a case report].

A 33-year-old man with known annuloaortic ectasia was admitted to the hospital because of chest pain. A dissecting aortic aneurysm was strongly suspected, but angiography was not performed because of his history of allergic reaction to iodine contrast material. Two-dimensional echocardiography demonstrated a markedly dilated basal aorta. The aortic wall immediately above the right sinus of Valsalva appeared to divide the aortic lumen into anterior and posterior channels. A defect in the intimal flap with a diameter of two cm was also detected six cm above the aortic ring. Color Doppler flow mapping showed blood flow passing through a probable entrance tear from the true lumen into the false lumen. Reentry could not be detected by Doppler imaging. All major aortic tributaries were presumed patent and supplied via the true lumen. The patient underwent successful emergency Bentall surgical procedure and recovered. It is emphasized that in some cases of DeBakey type I or II aortic dissection, color Doppler flow imaging is helpful for determining indications for emergency surgery.

[Survey of keratinophilic fungi from soils and mating behaviour of Microsporum canis from human ringworm cases in Hokkaido Prefecture in Japan].

From 383 (38.3%) of a total 1000 of soil samples, collected from Hokkaido prefecture, keratinophilic fungi have been isolated using hair baiting technique. The fungi isolated were as follows: 318 strains of Trichophyton ajelloi (including 90 strains of Arthroderma uncinatum), 59 of Microsporum cookei (including 13 of Nannizzia cajetani), 32 of Chrysosporium keratinophilum, 23 of C. tropicum, 15 of Chrysosporium anamorph of A. tuberculatum, 6 of T. terrestre (including 3 of A. insingulare and one of A. quadrifidum), 4 of Chrysosporium sp., 2 of M. canis (including one of N. otae), one of M. gypseum, and one of Ctenomyces serratus. This is the first isolation of N. otae from soil. Because of the successful isolation of N. otae (and its anamorph) from soil sample, (+) strain of M. canis was suggested to be popular in Hokkaido prefecture. The prevalence of (+) strain of M. canis was investigated using 279 isolates obtained from human ringworm cases. Among the 279 isolates, 257 were (-) mating type, 8 were (+) and 14 were nonreactive. Results of the present study indicate that the distribution of keratinophilic fungi from soils in Hokkaido prefecture is markedly different from that in the southern parts of Japan. The unexpected isolation of N. otae and its anamorph from soil samples as well as the significant population of N. otae(+) strain from human ringworms suggest the peculiar distribution pattern of N. otae (+) in Hokkaido prefecture.

Post-column derivatization system for the fluorimetric determination of guanidino compounds with ninhydrin by reversed-phase ion-pair high-performance liquid chromatography.

A post-column derivatization system for the fluorimetric determination of guanidino compounds by high-performance liquid chromatography was developed. A mobile phase containing ninhydrin was used as the fluorigenic reagent. Ten guanidino compounds were separated within 25 min on a Nucleosil C8 (5 microns) column (15 cm X 4.6 mm I.D.) by isocratic, reversed-phase ion-pair chromatography and detected as fluorophors derived from condensation with ninhydrin in an alkaline stream. In this simplified system only two pumps required to deliver the mobile phase and the alkaline solution. This method was applied to serum from patients on haemodialysis therapy.

Late diastolic mitral regurgitation studied by pulsed Doppler echocardiography.

To evaluate the usefulness of pulsed Doppler echocardiography in assessing late diastolic mitral regurgitation (MR) and to clarify the pathophysiology of MR, 226 consecutive patients who had undergone left ventriculography were studied. By investigating blood flow patterns at the left atrial outflow tract, late diastolic disturbed flow suggesting MR was detected in 15 patients (7%), including 10 (4%) with positive left ventriculographic findings. Among these 15 patients, 14 (93%) had atrial fibrillation and had late diastolic MR in the cardiac cycle with prolonged RR interval. The limitation in number of cardiac cycles that could be analyzed and the rapid heart rate eliminating appearance of the beat with prolonged RR interval may be the reasons for the paucity of late diastolic MR by left ventriculography. Ten patients (66%) with late diastolic MR, including 1 with sinus rhythm, had aortic regurgitation, 3 had high-grade systolic MR and 2 had atrial septal defect. Simultaneous recording of pulmonary artery wedge pressure and left ventricular pressure in 3 patients showed a reversal of pressure gradient in late diastole when the RR interval was prolonged. In conclusion, pulsed Doppler echocardiography was useful for detecting late diastolic MR and in reducing overestimation of systolic MR in left ventriculography induced by erroneous addition of late diastolic MR. The difference of the incidence of this flow between left ventriculography and Doppler examination indicated that this flow depends primarily on heart rate and may come and go in a patient.

Fluorometric determination of guanidino compounds by new postcolumn derivatization system using reversed-phase ion-pair high-performance liquid chromatography.

A new postcolumn derivatization system using 1,2-naphthoquinone-4-sulfonate as fluorogenic reagent for the fluorometric determination of guanidino compounds is described. The guanidino compounds were separated by reversed-phase ion-pair high-performance liquid chromatography with an isocratic mobile phase containing the fluorogenic reagent and octane-sulfonate as the counterion. Fluorophors were derived from a condensation of guanidino compounds with the fluorogenic reagent in an alkaline solution. The chromatographic system using the mobile phase containing the fluorogenic reagent was simplified because only two pumps were required to deliver the mobile phase and the alkaline solution. Separation of guanidino compounds was completed within 25 min using a Nucleosil C8 column (5 microns, 15 cm X 4.6 mm i.d.). This method was applied to serum obtained from patients on hemodialysis therapy.

Fluorometric determination of streptomycin in serum by high-performance liquid chromatography using mobile phase containing fluorogenic reagent.

A new postcolumn derivatization method for the fluorometric determination of streptomycin in serum by high-performance liquid chromatography is described. The serum was treated with 3.5% perchloric acid to precipitate proteins and the supernatant was directly injected into the chromatograph. Streptomycin was separated by reversed-phase, ion-pair chromatography with a mobile phase containing ninhydrin as a fluorogenic reagent, octanesulfonate, and 1,2-ethanedisulfonate as counterions, and was detected by fluorescence using continuous-flow, postcolumn derivatization in an alkaline stream with ninhydrin in the mobile phase. This method is sensitive to 1.0 microgram/ml using only 100 microliter of serum. Comparison with a fluorescence polarization immunoassay gave a good correlation coefficient of 0.976.

Indication of aorto-coronary bypass graft surgery for infarction area of myocardium considered from exercise thallium-201 myocardial imagings.

To assess accurately the indication and effects of aorto-coronary bypass graft surgery (ACBG) to the myocardium infarction area, 35 patients with myocardial infarction were analyzed by using exercise thallium-201 myocardial imagings. The patients were classified into group I (18 cases) and group II (17 cases) before ACBG. Group I consisted of patients with regional hypoperfusion in the initial image who showed redistribution in the delayed image to the infarction area of myocardium. Group II consisted of those with regional hypoperfusion in the initial image without redistribution in the delayed image. Group I showed a significant improvement in myocardial perfusion (p less than 0.01), pressure rate product (p less than 0.01), global left ventricular ejection fraction (p less than 0.01), segmental wall motion abnormality (p less than 0.01), and exercise tolerance when compared with those of group II before and after ACBG. The above study showed that the pre- operative redistribution of the infarction area in the delayed image suggests myocardial viability and ACBG will be effective.

[Thallium-201 scintigraphy of the myocardium during exertion in the indication for aortocoronary bypass of an area in the myocardium, the site of an infarct].

The purpose of this study is to precisely evaluate the indications and the results of direct revascularization surgery in a myocardial area previously infarcted. 35 cases were analyzed using thallium scintigraphy during stress. Before surgery, the patients were divided in two groups. Group I (18 cases) includes patients who presented initially a regional hypoperfusion and a reperfusion at the site of the infarction, on late scintigrams; group II (17 cases) also presented a regional hypoperfusion initially, but showed no reperfusion on late scintigrams. Concerning the results during stress, the thallium 201 scintigrams during exercise show alterations of the myocardial perfusion in the infarcted area, before and after surgery; in group I, a significant improvement (p less than 0.01) is noted in the entire group, i.e. 16 patients (88.9%), except in 2 cases where the graft was occluded, and group II shows an improvement in only 4 cases (NS) after surgery. Concerning the modifications of the product pressure X cardiac rhythm, there is a significant improvement (p less than 0.01) in group I (14 out of 18 patients (77.8%)); 2 out of 4 patients who were aggravated, presented occluded grafts after surgery. Group II shows an improvement in 10 out of 17 patients (58.8%) after surgery (NS). The left ventriculography demonstrates alterations in the global ejection fraction. Group I shows a significant improvement in 15 out of 18 patients (88.3%) (p 0.01); in group II there is improvement in 11 out of 17 patients (64.7%) (NS) after surgery.(ABSTRACT TRUNCATED AT 250 WORDS)
