[Number of complaints in cashiers at workplaces with conventional cash registers and those with barcode reader equipment].

115 cash register operators of supermarkets have been studied. Postural complaints are high, compared to office workers. The results obtained by questionnaire and by a rheumatological examination revealed that most cash register operators complain about pains in the back, in the left shoulder and in the left arm. The cashiers working on laser-scanner-systems have the same complaints, but they have significantly less pains in the right arm and the right hand because the cash register is only used infrequently. It seems to be better if cashiers have to do different tasks in a supermarket because the study shows that cashiers who work at different workplaces have significantly less pains then those who work only at the cash register.

Comparison of the effect of flurazepam, brotizolam and alcohol on psychomotor performance.

The effect of equal doses of brotizolam (2-bromo-4-(2-chlorophenyl)-9-methyl-6H-thieno [3,2-f]-1,2,4-triazolo [4,3-a]-1,4-diazepine, We 941, Lendormin) (0.25 mg) and flurazepam (30 mg) was studied in double-blind technique in a continuous performance test. Experiments were done 7 h after the last of a three-day ingestion. The task was composed of a compensatory tracking one and additional visual ones in the central visual field (visuo-motor processing time) and in the peripheral visual field (visuo-motor reaction time). The effects were compared with that of different concentrations of alcohol. The starting point of all parameters was similar in spite of the drug used. The drug influenced the values during the running of the single experiment. Visuo-motor coordination in a compensatory tracking task, visuo-mental processing time, visuo-motor reaction time and reading errors were not impaired with brotizolam. In contrast there were observed residual effects with 30 mg flurazepam. As well visuo-motor coordination (p1 less than 0.01, p2 less than 0.01), visuo-mental processing time (p1 less than 0.025), p2 less than 0.05), visuo-motor reaction time (p1 less than 0.05, p2 less than 0.01) and reading errors (p1 less than 0.10, p2 less than 0.05) were impaired compared to placebo (p1) and 0.25 mg brotizolam (p2). Moreover, events in the peripheral visual field were not detected. Subjective assessments of performance correlated with measured performance only for flurazepam was performance in comparison with placebo assessed as significantly worse both for visuo-motor coordination (p less than 0.01) as well as visuo-mental processing time (p less than 0.02).(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of hygiene among the uncircumcised.

Recommendations against routine neonatal circumcision are based on the assumption that good penile hygiene can offer the same advantages as this procedure. The purpose of this study was to investigate hygienic practices of uncircumcised subjects and the relationship of these practices to the condition of the prepuce and glans. Outcome was related to hygiene: subjects who retracted the foreskin when bathing were less likely to have smegma accumulation, inflammation, phimosis, or adhesions than those who did not. Significant correlations were also found between early instructions concerning hygiene and the type of hygiene practiced. These results support the recommendation that good hygiene can offer many of the advantages of circumcision and highlight the need for clear, early instructions on hygiene to uncircumcised patients.

[Consequences of presbyopia for the dynamics of sight and work at a terminal].

For 100 persons of different age the visual charge for the accommodation was examined by an objective procedure. Already at the age of 40 the dynamic range of accommodation decreases. This decrease could be a reason for the loss of mobility during work at VDUs, as it has been observed in a group of persons aged 40 to 55 years.

[Physiological effects of needle guns on the hand-arm system].

Physiological effects and sensations of four air actuated needle-pistols of different constructions on the hand-arm-system were explored (n = 4). As no reliable standards of injurious effects are known, only comparative conclusions can be drawn. All parameters showed a significant diminution of vibration effects when using a machine constructed with a shock absorber.

Influence of edge sharpness on the accommodation of the human eye.

Using a subjective method of laser refractometry, the accommodation behavior of six subjects toward real targets with various degrees of blur, dependent on the viewing distance, were investigated monocularly and binocularly. Despite the sharpness of the target, the nominal value is only reached incompletely with a lag of accommodation, because accommodation tends to be as minimal as possible. Increasing blurring leads to a drift of accommodation towards the resting position of accommodation. The resulting underaccommodation causes further physiological blurring of the retinal image of the targets. The approach of the targets to the resting position of accommodation leads to a higher precision of adjustment and less drift. Subjective components (accommodative laziness, cognitive demand) influence the accommodation and increase the phenomenons discovered.

Residual effects of flurazepam and brotizolam on psychomotor performance.

Residual effects of brotizolam (0.25 mg) and flurazepam (30 mg) were studied in healthy young adults. Performance and subjective assessments were observed from 7 to 8 h after overnight ingestion, and effects were compared with that of placebo. Visuo-motor coordination, visuo-mental processing time, visuo-motor reaction time and reading errors were not impaired with brotizolam. There were residual effects with 30 mg flurazepam. Visuo-motor coordination, visuo-mental processing time, visuo-motor reaction time, and reading errors were impaired compared with placebo and 0.25 mg brotizolam. Subjective assessments of performance correlated with measured performance. There was a decrease with flurazepam (visuo-motor coordination and visuo-mental processing time) compared with placebo.

[Effect of caffeine on motor reactions and visual-mental processing time].

The effect of coffee without and with caffeine (200 mg) on the performance of "awake" respectively "fatigued" subjects was proved. 5 females and 7 males (22 to 32 years) were chosen for the cross-over double-blind study. Motor reaction time, visual-mental processing time, reading-speed and reading-errors were recorded. Further experiments under aggravated conditions were carried out. In the morning ("awake") caffeine increases the efficiency of all parameters excepted the reading errors. At night ("fatigued") the increase by caffeine is insignificant. Even in some subjects caffeine decreases the speed of reading and increases the number of errors. Under test-conditions, which were aggravated by strong acoustical disturbance, caffeine does not improve efficiency, compared with caffeine-free coffee. Overriding into the range of decreasing performance in spite of increasing central activity was not found under any test-condition.

The influence of 1,3,7-trimethyl-xanthine (caffeine) on the pain-caused dilation of pupil.

The pupil dilation in man caused by pain was investigated. The pain stimulus was induced electrically. The size of the pupil was measured according to the "flying spot scanning method". The accuracy of measurement was increased by computer-assisted addition of repeated responses as well as by elimination of the, usually not considered constriction due to light compensation over Maxwellian view. The pure psycho-sensory dilation shows no initial contraction. The central stimulant 1,3,7-trimethyl-xanthine (caffeine) does not influence the course and adaption of the pupil reaction by a pain stimulus over the threshold. The pupil reaction correlates with the subjective feeling of pain. This suggests that caffeine does not influence the feeling of pain.

[Fluctuations in the accomodation of the human eye to mono- and binocular fixation (author's transl)].

Using an objective infrared technique, the deviation of the mean of accomodation during mono- and binocular fixation and the degree of fluctuation were measured for four male probands. 1. For two men, the additional impulse of convergence in binocular vision reduced deviation and fluctuation. 2. In every case, fluctuation and deviation increased with increasing theoretical value of accomodation. 3. Increased values of accomodation were linked to greater fluctuations so that loss of image quality could be somewhat compensated for.

[Quantitative pupillometric determination of sensitivity to pain (author's transl)].

1. Pain-induced pupillary dilatation was investigated by means of a method which compensates for the pupillary light reflex. Electric pain stimuli of varied intensity were applied to the forearm. Pure tone (T), pure pain (S) and combined tone-pain (TS) stimuli were used in stochastic or periodic succession. 2. The amplitude of pupillary dilatation increased in the order T, S, TS, In these experiments the sum of the T and S reactions is lower than the TS reaction, in contrast to summation patterns in other nervous structures. The amplitudes of pupillary dilatation and the intervals between stimuli and maximal dilatation increased linearly with increasing stimulus intensity. The latency period, however, is not systematically influenced by the intensity of the stimuli. Adaptation is delayed by stochastic application of the stimuli.

[Algo-pupillometric investigation of the analgesic effect of tramadol (author's transl)].

1. The analgesic efficacy of 1-(m-methoxyphenyl)-2-(dimethylaminomethyl)-cyclohexan-1-ol (tramadol; Tramal) (75 and 100 mg) was investigated in 22 young, healthy volunteers by means of an algo-pupillometric method. 2. Tramadal itself causes a slight miosis which becomes statistically significant only 3 h after administration. 3. In the algo-pupillogram the effect of tramadol is shown to be biphasic during the first 80 min. Thereafter the pupillary reaction steadily decreases, reaching a minimum after about 3.5--4 h in both dosages. 4. At the time of maximum effect the pupillary reaction of subjects given 75 or 150 mg tramadol is calculated to be equal to that of untreated individuals to whom only 60% or 10%, respectively, of the stimulus intensity has been applied.
