In the early extralymphatic affection of the organs without relevant clinical and roentgenological findings the diagnosis of the lymphogranulomatosis makes difficulties. It is referred to the peculiarities of the extra-nodal manifestation. The case of a lymphogranulomatosis with early participation of the heart is described and it is gone more fully into the differential diagnosis. Disturbances of the regression of excitation in the ECG are etiologically ambiguous and may be the first recognizable findings of a penetration of a tumour into the pericardium.

The principle of the ferrokinetics according to the catenary model and to the multilocular system of Pollycove and Mortimer is described in short. The most important results of the investigation of the iron metabolism of 15 patients with chronic myelosis, 31 with osteomyelofibrosis and 68 with chronic liver diseases according to the catenary model are reported. According to the multicompartment model the ferrokinetic investigations of 18 patients with chronic liver disease as well as of one patient each with chronic myelosis and panmyelopathy were evaluated. An increased depot iron metabolism and a decreased effective erythropoiesis could particularly clearly stated in the latter patient. The statements to various parameters according to the two model demonstrations are compared.
The incudostapedial joint was studied in tomograms of 370 normal and 80 diseased middle ears. The Guillen view was found to be superior to the anteroposterior view in demonstrating this joint in normal middle ears (85% and 62% visualization, respectively). In the presence of disease, visualization was markedly reduced to 19%; this lack of visualization is a nonspecific finding since early cholesteatomas, chronic otitis media, and retraction pockets could not be differentiated radiologically. Nonvisualization of the incudostapedial joint in the Guillen view is a more objective indicator of middle ear disease than such terms as "indistinct of hazy" middle ear cavities.
The control of Fasciola hepatica in dairy cattle may lead to considerable residue problems in dairying. In the GDR, a hexachloro-p-xylene-based fasciolicide has been approved till recently, and another, rafoxanide-based fasciolicide has been on trial. The investigations have shown that the two active principles are excreted in the milk for a prolonged period, rafoxamide being found more suitable. Their marked lipophil behaviour leads to accumulation in the fat phase and, thus, in high-fat milk products.
Determining granulocyte kinetics with DF32P allows various parameters to be gained during the in-vitro marking, such as the total blood granulocyte pool, circulating granulocyte pool, marginal granulocyte pool, daily granulocyte exchange rate and half decay period of granulocytes. The half decay period of granulocytes, bone-marrow reserve in myelocytes, metamyelocytes and band cells as well as polymorphonuclear neutrophils can be determined by in-vitro marking, with DF32P being intravenously injected. The combination of both procedures with DF32P will reveal the half decay period, pool sizes and exchange rates of the proliferating myelocyte compartiment in bone-marrow and mature blood granulocytes. If 51Cr is used for determining granulocyte kinetics the surface activities of various organs, such as heart, liver, spleen, and lungs, can mainly be determined in addition to the half-life of leucocytes, indicating the degradation or storage of cells in certain areas of the body. In addition to normal values those findings are principally presented which were obtained with in-vitro marking by DF32P and 51Cr in chronic myeloid leukaemia, osteomyelofibrosis or osteomyelosclerosis respectively and in hypersplenism.
36% of a total of chronic liver patients suffered from anaemia and 50.5% of patients affected with liver cirrhosis. In most cases the anaemias were normochrome and hypochrome or hyperchrome only in some cases. In analyzing possible single factors the reductions of vitamin B12 absorption could be made probable by means of the Schilling test and sometimes a folic acid deficiency in macrocyte anaemia with normal vitamin B12 absorption by determining the folic acid content in the serum and by successes of test treatment 82% of patients with liver cirrhosis showed a latent or manifest haemolysis. However, it was only in 1/3 of the patients with liver cirrhosis that the spleen turned out to be the place of an increased degradation of erythrocytes. In some cases an increased erythrocytoclasia into the liver could be identified. Predominantly, however, an increased degradation of erythrocytes in the total RHS had to be assumed. Twice an ineffective erythropoiesis could be found by ferrokinetic examinations. As a whole ferrokinetic examinations cannot be interpreted easily, because their static and dynamic values of iron transport in the plasma volume of liver patients will undergo considerable changes. Patients with disturbances of haematopoiesis and with haemolysis remaining in the latent stage may develop a manifest anaemia because of the influence of additional factors, such as increase of the plasma volume at lowered haematocrit value or microbleedings. The cause of anaemia cannot be concluded with sufficient probability from the type of anaemia; in a single case all pathogenetic factors will rather have to be analyzed. Therapeutic possibilities for hepatogenous anaemia of complex genesis are discussed.
In the present investigation the storage effect of AcD-AG and CPD-AG-stabilizers on thrombocytes was tested. The platelets were stored in platelet-rich plasma (PRP) at 4 degrees C or room temperature for 3 days. The concentrates gained by it were marked with Na251CrO4 and reinjected. The thrombocytokinetic parameters were evaluated. The results show that storage with the help of the mentioned stabilizers can be made to a certain extent only. Platelets stored in AcD-AG stabilizerhad a survival time of 2.7 +/- 1.1 days towards 9.0 +/- 1.0 days of fresh whole blood concentrates. The survival time of CPD-AG thrombocytes stored at 4 degrees C for 3 days amounted to 2.0 +/- 0.5 days. Storage of CPD-AG platelets at room temperature showed favourable results. Their survival time amounted to 6.2 +/- 0.6 days. Measurements of surface activity above the spleen and the liver indicate that degradation of stored platelets is mainly performed in the spleen. Problems of liquids storing in view of the significance of therapeutic thrombocyte substitution for hospitals are referred to.
40 patients with haemoblastosis were examined clinically and by cultures for presence of yeast mycosis. Moreover, we analyzed specific antibodies with the help of cell agglutination, the passive haemagglutination and the precipitation using Candida antigen. 29 test persons were found having a yeast mycosis, 3 patients without clinical symptoms in pharynx showed presence of yeasts by culture, 8 test persons were clinically and mycologically negative. In cell agglutination we recorded titres of less than 1:20 to 1:5 160 and in haemagglutination titres of less than 1:20 to 1:5 160 and in haemagglutination such of less than 1:20 to 1:320. 5 patients showed positive precipitation reaction with antigen extracted of membranes of Candida albicans cells. We observed different titre motions with these patients: during illness the titre either rised or decreased clearly, or the patients were in a condition of humoral anergy against the Candida albicans antigen. It is hinted at the necessity to watch over susceptible patients mycologically.
The use of nuclear-medical methods is especially described for the diagnostic of anaemias with different etiology. For the diagnosis of the chronic haemorrhagic anaemias the determination of the blood losses through the gastrointestinal tract with the help of the 51Cr-method and for the course of the acute haemorrhagic anaemias the automatized determination of the blood volume are emphasized. For the proof of a latent haemolysis often the knowledge of the apparant half survival time of the erythrocytes is necessary. Also in manifest haemolysis this investigation method gives differential-diagnostic references. For the megaloblastic anaemias a diagnostic routine programme and a differentiated investigation scheme are reported by Hoffbrand. For the clarification of the anaemias by insufficiency of the bone-marrow and ineffective erythropoiesis the radio-iron-kinetic investigations have proved. They should also be taken into consideration for the indication of the transplantation of the bone-marrow.
A short survey is given on iron deficiency, its pathophysiology, the causes, the clinical symptomatology and the therapeutic possibilities. On account of the abundance of the material only the most important facts could be treated. Hereby the clinical practice was particularly taken into consideration. Essential theoretical questions have been outlined only in short.
Out of 120 patients with chronic liver diseases (45 liver cirrhosis, 38 chronic hepatitis, 15 fatty degeneration of the liver and 22 stasis liver) the osmotic resistance of the erythrocytes after Hennemann and the mechanic resistance after Schubothe was determined. 24% of the patients had a decreased osmotic resistance of the erythrocytes, even 55% a decreased incubation resistance. 16% of the patients with liver diseases had a decreased mechanic resistance of the erythrocytes. The comparison of the groups of disease and the stages of the diseases resulted in an increased mechanical resistance of the erythrocytes only for the decompensated liver cirrhosis. A positive correlation was the result between the degree of the haemolysis and the spherical index of the erythrocytes.
Since the extensive veterinary-hygienic treatment of herds with insecticidal veterinary preparations may lead to milk contamination, the authors studied the behaviour of some active agents (butonate, dichlorvos (DDVP), trichlorphon, naled, carbaryl, hexachloro-p-xylene and rafoxanide) during the industrial processing of milk to fluid milk, cream, butter, cheese and milk powder, and during the storage of these products. Small-scale model experiments served to investigate the effects of pasteurization (74 and 95 degrees C.), separation and churning as well as of the processing to milk powder and cheese. Analyses for residues were performed by thin-layer chromatography, colorimetry and with the aid of the isotope technique. From the viewpoint of milk processing, the use of TCP and, in part, that of DDVP may be considered as less critical due to their hydrophilic properties and rapid degradation. In view of their lipophilic behaviour, the use of butonate, carbaryl, rafoxanide and hexachloro-p-xylene as active agents in veterinary preparations for milk cows must be regarded as problematic. The utilization of naled is also problematic due to the fact that the toxicology of its metabolites is not yet sufficiently known.
A step-by-step program had been established to replace the persistent insecticide DDT with other active principles in the GDR by 1975. Monthly thin-layer chromatographic residue determinations in butter samples from all the districts demonstrate that the DDT-residues have decreased gradually since 1973 from an average value of 0.45 mg/kg to an average value of 0.18 mg/kg (in 1976). The DDT intake of adults via the milk products (butter, milk and cheese) was, in 1976, only 2% of the ADI value, which evidences a very positive development. Furthermore, findings concerning the contamination with lindane and HCB are presented.
