Left ventricular size and performance during graded supine exercise in normal subjects.

To investigate left ventricular size and performance during graded submaximal exercise, 14 normal subjects with a mean age of 21 years exercised in a supine position to achieve the target heart rate. Using two-dimensional echocardiography, we recorded and analysed the left ventricular (LV) cross-sectional area and internal dimension at the level of the tips of the mitral valve at rest and during mild, moderate and severe exercise. The heart rate and systolic blood pressure increased substantially from rest to peak exercise (71 +/- 11 to 162 +/- 10 beats/min, 122 +/- 10 to 204 +/- 22 mmHg). The end-diastolic cross-sectional area and internal dimension (EDA & EDD) increased by 1.1-2.2 cm2 (7-13%) and 0.2-0.3 cm (4-7%), respectively, from mild to moderate exercise, (p less than 0.05-0.001). At peak exercise, however, these decreased and showed no statistically significant difference from the values at rest. The end-systolic cross-sectional area and internal dimension (ESA & ESD) decreased by 1.1 to 1.6 cm2 (14-20%) and 0.2-0.3 cm (7-10%), respectively, from moderate to severe exercise (p less than 0.01-0.001). However, the end-systolic values during mild exercise were not significantly different from those at rest. The stroke area (EDA-ESA) and dimension (EDD-ESD) increased by 1.6-2.6 cm2 (19-31%) and 0.2-0.6 cm (25-38%), respectively, during all levels of graded exercise (p less than 0.05-0.001). The percent change of LV cross-sectional area and internal dimension during systole increased gradually from rest to moderate exercise (51.0 +/- 7.1 to 61.9 +/- 4.4%, 35.4 +/- 3.9 to 45.0 +/- 3.7%), respectively, and showed no further increase during peak exercise. The mean circumferential fiber shortening velocity increased sharply from rest to peak exercise (1.27 +/- 0.14 to 2.25 +/- 0.21 circ/sec). These results suggest that the Frank-Starling mechanism operates during mild to moderate exercise, and contractility increases markedly at moderate to severe exercise levels as cardiac performance is augmented during graded submaximal exercise.

[Clinical efficacy of cefotaxime in the field of internal medicine].

Fifty-eight patients were treated with cefotaxime (CTX, Claforan) intravenously. Almost all patients (54 patients) had underlying diseases that were 16 cases of diabetes mellitus, 10 cases of respiratory diseases, 8 cases of cerebral vascular disturbance, 6 cases of renal diseases and blood diseases, 5 cases of carcinoma and hypertension, 4 cases of cholelithiasis, 3 cases of heart diseases and 7 cases of other diseases. The clinical efficacy of CTX in 34 cases of RTI, 11 cases of UTI, 8 cases of BTI and 5 cases of other infection was excellent in 11 cases, good in 27 cases, fair in 12 cases, poor in 4 cases and unclear in 4 cases. The over all clinical effectiveness was 70.4%. No adverse reaction was observed except for 2 cases (general fatigue in 1 case and eruption and itching in another). These results obtained should support the usefulness of CTX.

Increased sensitivity of cultured fibroblasts from colon cancer-prone rats to cytotoxicity of carcinogens and to viral transformation: a comparison with fibroblasts from patients with adenomatosis coli.

Lethal effects of mitomycin C (MMC), 4-nitroquinoline 1-oxide (4NQO) and ultraviolet light (UV) on fibroblast cell lines derived from a colon cancer-prone substrain of Wistar-Furth rats (WF/OSAKA rats) were measured in terms of the cellular colony-forming ability, and compared with the sensitivity to these agents of human fibroblasts from patients with adenomatosis coli and rectum (ACR). All 6 fibroblast strains from the cancer-prone WF/OSAKA rats were significantly more sensitive (though to various extents) to MMC as well as 4NQO than normal rat fibroblasts derived from the parental WF Hiroshima rats. These WF/OSAKA cell strains were slightly more sensitive to UV than normal rat cell strains. Similarly, 5 out of 6 fibroblast strains derived from ACR patients were hypersensitive to both MMC and 4NQO. Further, the WF/OSAKA cell strains were more susceptible to morphological transformation induced by Kirsten murine sarcoma virus than normal rat strains. The observed higher sensitivity to chemical agents and to viral transformation suggests a close similarity in cellular terms between the colon cancer-prone WF/OSAKA rats and human individuals affected with ACR.

Postinfarction cardiac rupture. A pathogenetic consideration in eight cases.

Eight fatalities due to cardiac rupture, of 40 consecutively autopsied myocardial infarctions, were clinicopathologically examined. Total duration of infarctions in 8 cases ranged from 9 hours to 14 days. The longer the period, the more the infarction extended, and aneurysmal dilatation of infarcted wall and tissue weakness were striking. The weakness was caused by the lack of replacement of necrotic muscle fibers by granulation, and was related to aneurysmal structure per se and also to its paradoxical pulsations. Postinfarction cardiac rupture occurring in relatively later phase was considered to be mainly due to weakness of infarcted wall, with less influence factors of mechanical stress such as physical exertion, hypertension, or intracardiac pressure. In the case where cardiac rupture took place in a relatively early stage of the episodes, infarction covered only a small range and aneurysmal change of the ventricle was slight, for which dynamic factors, represented by the degree of a localized strain elicited by myocardial contraction of non-infarcted site, seemed to play a significant role.

[Quantitative evaluation of thallium-201 exercise myocardial scintigraphy and its display on the two-dimensional view of the left ventricular wall as the functional image].

We developed a new method for the quantitative evaluation of 201T1 exercise myocardial scintigraphy. Final diagnostic images obtained by this method were displayed on the two-dimensional views of the left ventricular wall. Five projection images obtained 10 min and 3 hrs after exercise were recorded respectively in the magnetic disk. Each image was divided into seven fan-shaped segments utilizing the apical image as the reference which was placed in the middle. The corrected uptake indices were examined in 20 normal controls and served as the normal data. Each segment was then judged to be intact or ischemic or infarcted by matching the segmental uptakes of the images given 10 min and 3 hrs after exercise. The results were displayed on the computer-assisted color monitor as the two-dimensional view of the left ventricular wall. In order to evaluate its diagnostic usefulness in coronary artery disease, comparative coronary angiography was performed in 25 patients. The present technique proved to have high sensitivity and specificity both in left anterior descending artery and right coronary artery diseases (88 and 78%, respectively in the former and 83 and 89%, respectively in the latter).

Morphology and distribution of microtubules in the presecretory and secretory ameloblasts of the rat incisor.

The morphology and distribution of microtubules in the ameloblast of the rat incisor were examined. Two types of microtubules distribution were observed: isolated single tubules and multiple tubules. The latter formed bundles of tubules and filaments or sheets of filaments. The thickness of the filament or sheet was almost the same as that of the wall of the microtubule. The filament or sheet as well as the microtubule were negatively stained when treated with tannic acid. The microtubules of the single type were small in number at the undifferentiated stage of the cell. They increased in number in the distal portion of the cell during differentiation, and their number reached a peak in the proximal portion of Tomes' process at the secretory stage of the cell. On the other hand, the multiple microtubules showed a different distribution in the secretory stage of the cell, i.e., they were mostly distributed between the Golgi region and the distal terminal web.

Combined epidermoid and adenocarcinoma in diffuse interstitial pulmonary fibrosis.

This report documents a case of combined epidermoid and adenocarcinoma with idiopathic diffuse interstitial fibrosis developing in the lung. This type of carcinoma with diffuse interstitial fibrosis occurs only rarely in the lungs, in contrast to such carcinoma without fibrosis, which occurs less rarely. A 79-year-old man died of respiratory insufficiency three years after the was diagnosed as having diffuse interstitial pulmonary fibrosis. Six months prior to his death. a tumor shadow was noticed on a radiograph of his chest. Postmortem examination revealed diffuse interstitial pulmonary fibrosis and a primary lung tumor situated peripherally in the right lower lobe. The histologic features of the tumor closely resembled those of mucoepidermoid carcinoma of the major bronchi. However, the tumor had no relation to any bronchi or bronchial glands, and it was evident that no relation to any bronchi or bronchial glands, and it was evident that it had originated from the surface epithelium of the abnormally altered distal airspaces of the honeycomb lung. It is suggested that the malignantly transformed cells originally possessed the potential for bidirectional differentiation to epidermoid and mucous cells.

Two new slow-moving variants of human serum albumin.

Two new slow-moving variants of human serum albumin were found in members of two unrelated Japanese families. These variants, different from each other and from the slow-moving variants described previously, were designated albumin "Okinoerabu' and albumin "Akasaka'. The mode of inheritance of each variant was consistent with an autosomal codominant trait. A very rare type, homozygote for a variant, albumin "Okinoerabu', was also detected. The segregation data of albumin and Gc showed no discrepancy from the postulation of a linkage between these two loci.

Dissecting aneurysms of the coronary arteries.

This paper reports two cases of coronary dissecting aneurysms of the heart. These were found among 45 consecutive autopsy cases with cardiac diseases from January, 1978 to April, 1981. The first case, a 60-year-old woman, revealed dissecting aneurysm involving the aorta and three main coronary arteries in association with cystic medial necrosis. The second case, a 55-year-old man, revealed dissecting hematoma limited to the right coronary artery without any presumptive causal lesion to the dissection. The dissections could not be identified by either clinically or even macroscopic examinations of the autopsy materials. It is likely that the dissecting coronary aneurysm is not so uncommon nor fatal lesion as previously thought. Considerable number of the cases would be concealed in those of sudden death not autopsied or carelessly autopsied, or in those survived myocardial infarction not angiographied.

Light and electron microscopic changes of the exocrine pancreas in diabetic dogs induced by streptozotocin.

Light and electron microscopic studies on exocrine pancreas of diabetic dogs with duration of diabetes up to a year induced by streptozotocin were performed. In the pancreatic acinar cells of the long-term diabetic dogs, atrophy, hyperplastic nodules, and interacinar and interlobular fibrosis were exclusively observed by light microscopy and autophagic vacuoles, lipid droplets, fragmentation of rough endoplasmic reticulum, and decrease or varying size of zymogen granules were much more frequently found by electron microscopy compared with the control. The results might indicate the possibility that insulinopenia causes the injurious effect on exocrine pancreas to produce interacinar fibrosis.

Medullary carcinoma of the thyroid with ectopic ACTH syndrome.

An autopsied patient with medullary carcinoma of the thyroid and ectopic ACTH syndrome is reported. A microadenoma of pancreatic islet coexisted in this case, which is assumed to be of D cell origin. Immunohistochemical study revealed decreased number of pituitary ACTH cells. Some of them showed Crooke's degeneration. Hormone assay study of tumor tissue and plasma disclosed abnormal ACTH, beta-MSH as well as calcitonin. Somatostatin and Substance P were also demonstrated in tissue. Histologically the tumor showed solid alveolar pattern with a minor part consisting of small cell variant and this histologic variation is discussed.

A method for separating commercial colistin complex into new components: colistins pro-A, pro-B and pro-C.

Commercial colistin was separated into three major components as well as lipid components by reversed phase adsorption chromatography on Diaion HP-20 AG, a macroreticular styrene-divinylbenzene copolymer, without any inorganic salts or detergents, in aqueous-organic solvent as mobile phase. These expected components were colistins A, B and C; there were, however, appreciable differences between these components and colistins A, B and C, isolated by countercurrent distribution. The newly isolated components showed slightly higher potency than colistins A, B and C; their molecular weights, as determined by gel permeation chromatography of 2,4-dinitrophenyl derivatives on TSK-GEL G2000H (mobile phase: dimethylformamide), were also slightly higher. Accordingly, they were tentatively named colistins pro-A, pro-B and pro-C. During purification by countercurrent distribution (solvent system: sec-BuOH - n-BuOH - 0.1 N HCl, 30:6:40), colistin pro-A was converted to colistin A. Similarly, colistin pro-B was converted to colistin B, and colistin pro-C to colistin C. Therefore, we concluded that colistins A, B and C are artifacts.
