Superovulation of Holstein heifers by a single subcutaneous injection of FSH dissolved in polyvinylpyrrolidone.

This study was undertaken to determine whether a single injection of porcine FSH (pFSH) would induce a superovulatory response in cattle. Holstein heifers were given a single injection of pFSH (30 mg, s.c.) dissolved in saline (Group 1, n = 5); 50% polyvinylpyrrolidone (PVP; Group 2, n = 5); or 25% PVP (Group 3, n = 4). Group-4 heifers (n = 5) were given multiple intramuscular injections of pFSH every 12 h for 3 d at decreasing doses, for a total of 30 mg. All animals received a single injection of 750 microg PGF2 alpha 48 h after the initiation of pFSH treatment. Animals exhibiting estrus were artificially inseminated twice throughout estrus. Ova and embryos were recovered nonsurgically. Ovaries were examined by transrectal ultrasonography or by palpation per rectum on Day 7 or 8 of estrus. Plasma concentrations of pFSH, bovine FSH progesterone, estradiol-17 beta and inhibin were determined by specific radioimmunoassays. The number of corpora lutea (CL) and the numbers of total and transferable embryos which were detected and recovered in Groups 2 and 3 were equivalent to the numbers detected and recovered in Group 4. In Group 1, however, only 1 of 5 animals ovulated even a single oocyte. The present study demonstrated that only a single injection of pFSH dissolved in PVP was capable of inducing a superovulatory response by maintaining a high plasma FSH concentration to allow for the recovery of a sufficient number of embryos for transplantation.

Brain stem cheiro-oral syndrome: neurological signs for brain stem lesions.

Cheiro-oral syndrome (COS) is characterized by a sensory disturbance in one hand and the ipsilateral oral corner. It is usually due to a lesion in the parietal cortex, thalamocortical projections or thalamus. Brain stem lesions may rarely produce COS. We present two COS patients with midbrain infarction and pontine hemorrhage, respectively. In our patients, unilateral oculomotor nerve palsy or medial longitudinal fasciculus syndrome concurred with sensory disturbances of cheiro-oral distribution. COS produced by cortical/thalamic lesion is not accompanied with such oculomotor signs. Thus, oculomotor deficits are decisive in differentiating brain stem from cortical or thalamic COS.

Expression of 18.6/CD23 antigen on human lymphoid progenitor cell lines and phorbol 12-myristate 13-acetate (PMA)-induced microglia-shaped cells.

The nature of lymphoid progenitors and factor(s) determining commitment to either the T- or B-lymphocyte pathway are poorly understood in the human system. In this study, we generated a monoclonal antibody (MoAb), 18.6, that recognizes a cell surface antigen on a human lymphoid progenitor cell line (FL4.4). MoAb 18.6 reacted with lymphoid progenitor lines, B lymphoid cell lines, and myelomonocytic cell lines. It did not react with any T cell or erythroid leukemic cell lines. Two color FACS analyses of normal lymphoid tissues showed that MoAb 18.6 reacted with a majority of CD20+ mature B cells and a minority of CD64+ monocytes. Molecules of 3 different sizes with MW of 34, 45, and 68 Kd were precipitated with MoAb 18.6 from the lymphoid progenitor cell line. The 18.6 antigen was not expressed on a fetal liver-derived lymphoid progenitor-like cell line, FL1.4, which has the capacity to differentiate into microglia-shaped cells upon PMA-stimulation. Stimulation of FL1.4 cells with PMA induced expression of the 18.6 antigen within 24 hr and the microglia-shaped cells stained positively with MoAb 18.6. Finally, cloning of a cDNA that encoded the 18.6 antigen revealed that the 18.6 antigen is identical to the CD23 antigen. Taken together, these data suggest that the 18.6/CD23 antigen is expressed on lymphoid precursors at a very early stage of differentiation.

Treatment and posttreatment effects of fluoride mouthrinsing after 17 years.

OBJECTIVES: This study assessed the treatment and posttreatment effects of a school-based, fluoride mouthrinse regimen. METHODS: Children in a nonfluoridated community in Japan participated in a daily rinse program using a 0.05 percent NaF solution in nursery and primary schools, and a weekly rinse with 0.2 percent NaF in junior high school. Students were examined at least annually for dental caries and dental treatment was provided in a public dental clinic through the ninth grade. Incipient carious lesions with no cavitation were not restored. RESULTS: The percent of children in grades one through nine (6-14 years of age) with caries-free permanent teeth increased from 13.4 percent in 1974 to 73.0 percent in 1991, while the mean DMFT decreased by 86 percent during this period. For 12-year-olds, mean DMFT scores declined to about one tooth per child after 1982. For adults 20 years of age, there was a 64 percent difference in DMFS between the treatment group who started the rinse regimen at 4 years of age and continued for 11 years, and the controls who lived in different districts and did not participate in a fluoride rinse regimen. CONCLUSIONS: Children who began rinsing at 4 or 5 years of age benefited the most from the program. The program was inexpensive, simple to implement and well accepted by families and teachers. The conservative treatment policy in the public clinic likely contributed to the benefits derived by participants.

Clinical value of argyrophilic organizer region in bladder transitional cell carcinoma.

For the purpose of elucidating whether or not argyrophilic organizer regions (AgNORs) predict the biological behaviors of bladder tumors, the AgNOR was analyzed in 80 primary bladder transitional cell carcinomas (TCCs), using the computer-assisted image analyzer. The AgNOR parameters used were the mean AgNOR count (C-AgNOR) and the percentage of cells exhibiting more than 3 AgNOR dots or 4 AgNOR dots within nuclei (3P-AgNOR and 4P-AgNOR, respectively). The correlation of C-AgNOR with 3P-AgNOR and 4P-AgNOR reached a high statistical significance (p < 0.0001, respectively). All of these three AgNOR parameters correlated with histological grade, whereas the difference between grade I and II proved to be higher in 3P-AgNOR and in 4P-AgNOR (p < 0.01, respectively) as compared with that in C-AgNOR (p < 0.05). Similarly, these three parameters correlated with histological stage, whereas the statistical significance of the correlation was less prominent in C-AgNOR. Moreover, the prognostic relevance was noted in C-AgNOR (p = 0.041), whereas it was indicative in 3P-AgNOR as well as in 4P-AgNOR (p = 0.090 and p = 0.061, respectively). These results suggest that 3P-AgNOR and 4P-AgNOR reflect the difference in the proliferative activity of individual TCCs more precisely than C-AgNOR. Since the biological behaviors of individual TCCs cannot be predicted by the proliferative activity alone, the AgNOR in TCCs has a rather limited value regarding the prediction of survival.

Neoadjuvant chemotherapy for locally advanced urothelial cancer of the upper urinary tract.

Cisplatin-based multiple-drug chemotherapy is currently considered the most effective treatment for advanced and metastatic urothelial cancers. We treated 15 patients with locally advanced urothelial cancers of the upper urinary tract using the cisplatin-based multiple-drug regimen in a neoadjuvant setting. The regimens administered were: M-VAC (methotrexate, vinblastine, doxorubicin and cisplatin); MEC (methotrexate, etoposide and cisplatin), or M-VEC (methotrexate, vinblastine, epirubicin and cisplatin). Total nephroureterectomy was performed in all patients and response was evaluated pathologically Of 15 patients 2 (13%) achieved a pathological complete response, 6 (40%) a pathological partial response, for an overall response rate of 53% (95% confidence limits 29-77%). The median durations of response were 54 months for patients with a pathological complete response and 15.5 months for patients with a pathological partial response. One of six patients with a pathological partial response and 4 of 7 with no remission died of cancer. While a positive relationship between the pathological response and prognosis was observed, adequate follow-up is needed to assess the ability of neoadjuvant chemotherapy to improve the prognosis of patients with locally advanced urothelial cancer of the upper urinary tract.

Follicular dynamics and secretion of inhibin and oestradiol-17 beta during the oestrous cycle of the hamster.

Plasma and ovarian levels of inhibin were determined by a radioimmunoassay (RIA) at 3-h intervals throughout the 4-day oestrous cycle of hamsters. Plasma concentrations of FSH, LH, progesterone, testosterone and oestradiol-17 beta were also determined by RIAs. In addition, hamsters were injected at various times with human chorionic gonadotrophin (hCG) to determine the follicular development. The changes in plasma concentrations of FSH after injection of antisera to oestradiol-17 beta (oestradiol-AS) and inhibin (inhibin-AS) on the morning of day 2 (day 1 = day of ovulation) were also determined. Plasma concentrations of inhibin showed a marked increase on the afternoon of day 1, remained at plateau levels until the morning of day 4, then increased abruptly on the afternoon of day 4 when preovulatory LH and FSH surges were initiated. A marked decrease in plasma concentrations of inhibin occurred during the process of ovulation after the preovulatory gonadotrophin surges. An inverse relationship between plasma levels of FSH and inhibin was observed when the secondary surge of FSH was in progress during the periovulatory period. Plasma concentrations of oestradiol-17 beta showed three increase phases and these changes differed from those of inhibin. Changes in plasma concentrations of oestradiol-17 beta correlated well with the maturation and regression of large antral follicles. Follicles capable of ovulating following hCG administration were first noted at 2300 h on day 1. The number of follicles capable of ovulating reached a maximum on the morning of day 3 (24.8 +/- 0.6), and decreased by 0500 h on day 4 (15.0 +/- 1.1), corresponding to the number of normal spontaneous ovulations.(ABSTRACT TRUNCATED AT 250 WORDS)

[Acid phosphatase (ACP)].

The acid phosphatases are a group of enzymes capable of hydrolyzing esters of orthophosphoric acid in an acid medium. Acid phosphatase activity is widely distributed in human tissues and acid phosphatases represent a heterogeneous group of enzymes containing many isoenzymes, each specific for one type of tissue. The human prostate is particularly rich in this enzyme (PAP) and serum enzyme levels have been used as a tumor marker of prostate cancer. While PAP was markedly increased in patients with bone metastases of prostate cancer, it is unable to detect earlier stage tumors reliably. The sensitivity and specificity of serum acid phosphatases and PAP are low in diagnosing, staging and following patients with prostate cancer. Presently, prostate-specific antigen (PSA) is superior to PAP for diagnosis, screening, and monitoring prostate cancer. PAP may have an adjuvant value in the management of prostate cancer because a combination of PSA and PAP testing has revealed a high sensitivity and specificity in detecting prostate cancer.

Growth regulation of retinal pigment epithelial (RPE) cells in vitro.

Studies of growth factor production by chick embryo retinal pigment epithelial (RPE) cells and the effects of cytokines on chick and human RPE cells were performed in vitro. RPE cell growth was evaluated by tritiated thymidine uptake. Chick RPE cells produced growth factors whose molecular weights were 15 and 108 kD. These fractions contained interleukin (IL)-1-like activity which stimulated murine thymocyte proliferation. 100-150 U/ml human IL-1 beta, interferon (IFN)-beta, IFN-gamma and tumor necrosis factor (TNF)-alpha stimulated the growth of both RPE cells. The activity of IFN-beta was the most potent of these cytokines, while IL-2 and IFN-alpha had no effect in chick RPE cells. 100-150 U/ml transforming growth factor (TGF)-beta 1 suppressed cytokine-induced growth of both RPE cells. RPE cells therefore produce growth factors, respond to several growth factors and are regulated by a network of cytokines.

Effect of cytokines and prostaglandins on the growth of chick retinal pigment epithelial cells.

The effect of cytokines and prostaglandins on the growth of chick retinal pigment epithelial (RPE) cells was studied in vitro. The growth of RPE cells was evaluated by [3H]-thymidine uptake. Human recombinant interleukin-1 beta, interferon-beta and tumor necrosis factor-alpha stimulated the growth of RPE cells at 50-200 units/ml. Prostaglandins such as PGE1, PGE2 and PGF2 alpha suppressed RPE cell growth at 10(-5) approximately (-6)M. Indomethacin, an inhibitor of cyclooxygenase, did not affect the spontaneous RPE cell growth at 10(-7) approximately (-9) M. However, 10(-7) M indomethacin enhanced RPE cell growth stimulated by cytokines synergistically. The addition of 10(-6) M PGF2 alpha suppressed the enhanced growth of RPE cells which was induced with cytokines and indomethacin. Furthermore, RPE cells stimulated with cytokines produced PGE2. These results suggest that RPE cells produce prostaglandins which have a negative regulatory role in RPE cell growth.

[The expression of proliferating cell nuclear antigen (PCNA) immunostaining in renal pelvic and ureteral cancers and its prognostic value].

The expression of the proliferating cell nuclear antigen (PCNA) was analyzed in formalin-fixed paraffin-embedded materials of 20 primary renal pelvic and ureteral cancers. The relation between PCNA positive rate and clinical pathological stage and prognosis were investigated. The fraction of PCNA positive nuclei ranged between 1.6-64.4% (mean 25.1%). The PCNA positive rate was found to be related to established prognostic factors, but not to patient age, sex and length of time for paraffin-embedded. Grade 3 cancers showed a higher fraction of PCNA positive nuclei than grade 1 and 2 cancers (p < 0.05). Superficial cancers showed a significantly lower fraction on nuclei positive for PCNA than invasive cancers (p < 0.01). The cancers with lymph node metastases showed a higher fraction (p < 0.05). The cancers with distant metastases postoperatively, also had a higher fraction (p < 0.05). As PCNA immunostaining can be applied to routinely processed paraffin-embedded specimen, this method is very useful to assess the cell proliferation. The results suggest that the fraction of PCNA positive nuclei would be useful for investigating the malignant potential, of renal pelvic and ureteral cancers thereby providing prognostic factors of these diseases.

[Rebleeding of ruptured intracranial aneurysms in the acute stage].

Among the cases of ruptured aneurysms we have handled, some patients have rebled before surgery was performed. In this study, we examine the factors that contribute to the rerupture of cerebral aneurysms in the acute stage and suggest measures to prevent rerupture prior to surgery. We have encountered 32 cases of rebleeding prior to surgery. The rebleeding occurred within six hours of the initial subarachnoid hemorrhage (SAH) in 26 patients, among whom 21 rebled within three hours. Thirteen patients rebled during bed rest, 10 patients during angiography, 4 patients during CT scan and 5 patients in various other circumstances. Nine of the 10 patients who rebled during angiography had undergone the procedure within three hours of the initial SAH. Overall, the patients' condition deteriorated substantially after rebleeding. Considering these circumstances surrounding aneurysmal rerupture, we suggest the following measures for the prevention of preoperative rerupture in the acute stage: 1) maintenance of lowered blood pressure during the risky period; 2) intentional delay of performance of angiography until at least hours after the initial rupture, and 3) performance of surgery on an emergency basis.

[Inverted growth of transitional cell carcinoma in the ureter: a case report].

Inverted growth of transitional cell carcinoma in the ureter is reported. A 71-year-old female visited us for right flank pain and gross hematuria. Hematuria from the right ureteral orifice was found by cystoscopy. Intravenous pyelography revealed right hydronephrosis and a filling defect in the right ureter, but the filling defect moved to the lower ureter. Computerized tomographic (CT) scan demonstrated a tiny lesion in the right ureter. Under a tentative diagnosis of right ureteral tumor, flexible fiberoptic ureteroscopy was performed. As a pedunculated tumor with a smooth surface was found, biopsy was performed. Since pathological examination showed malignancy, right nephroureterectomy was performed. Histological diagnosis was transitional cell carcinoma, grade 1. Cancer was covered by normal transitional epithelium, and developed inverted growth. This is a rare case of inverted growth of the transitional cell carcinoma in the ureter.

Cardiopulmonary resuscitation without intermittent positive pressure ventilation.

The purpose of this study was to examine whether tracheal insufflation of oxygen (TRIO) could be used as a substitute for intermittent positive pressure ventilation (IPPV) during cardiopulmonary resuscitation (CPR) in dogs with orotracheal intubation. Twenty-seven anesthetized, paralyzed and intubated dogs were used. The tip of the insufflation catheter was placed 1 cm distal to the top of the endotracheal tube. The effects of TRIO at a flow rate of 10 l/min with or without a continuous positive airway pressure (CPAP) of 5 cmH2O during external cardiac compressions were compared with those managed under the standard CPR. During CPR, TRIO without CPAP maintained adequate gas exchange. Peak airway pressures in the TRIO groups were significantly lower than that in the standard CPR group. No significant differences were observed in arterial, pulmonary artery and diastolic right atrial pressures during CPR among the three groups. However, the coronary perfusion pressures in the TRIO group with CPAP always tended to be low during CPR. The present study suggests that TRIO without CPAP should be a promising substitute for IPPV during CPR when IPPV is not feasible.

[Influence of cytotoxic chemotherapy on superoxide production by neutrophils in cancer patients].

The influence of cytotoxic chemotherapy on the number and function of peripheral neutrophils was studied in time sequence in 9 cancer patients; 6 patients with transitional cell carcinoma of urothelium treated with CAP, the combination of cyclophosphamide, adriamycin and cisplatin, and the other 3 with testicular tumor treated with PEB, the combination of cisplatin, etoposide and bleomycin. The neutrophil function was evaluated by the superoxide production, measuring chemiluminescence of the neutrophil suspension by a photometer. The peripheral neutrophil count (PNC) significantly (p < 0.01) decreased with the nadir count of 280 +/- 100/mm2 16.4 days after cytotoxic chemotherapy and recovered to the normal level by the next course. The neutrophil function declined significantly (p < 0.01) as PNC decreased, reached the minimum almost at the same time as PNC, and returned to the normal level by the next course. Of the 9 cases, the neutrophil function in 6 cases reached the minimum on the same day as that of PNC. The study shows that cytotoxic chemotherapy appears to impair the host defense mechanism in cancer patients not only in inducing neutropenia but also in deteriorating neutrophil function at the same time.

Evidence that inhibin is an important factor in the regulation of FSH secretion during the mid-luteal phase in cows.

To investigate the physiological importance of inhibin in the regulation of FSH secretion in cows, seven cyclic cows were treated with an inhibin antiserum raised against bovine 32 kDa inhibin in a castrated goat. The same animals treated with a castrated goat serum (control serum) served as controls. On day 12 of the oestrous cycle (day 0 = day of oestrus), four of seven cows were injected with 100 ml inhibin antiserum first, and the remaining three cows with 100 ml control serum first. Twelve days after the second oestrus following the first serum injection (42-46 days after the first serum injection), the former four cows were injected with control serum and the latter three with inhibin antiserum. Follicular development after the injections of control serum or inhibin antiserum was assessed by daily ultrasonographic examination. Treatment with inhibin antiserum resulted in a marked increase (P < 0.01) in plasma concentrations of FSH and oestradiol-17 beta but not LH or progesterone, compared with those after treatment with control serum. Plasma concentrations of FSH increased significantly (P < 0.01) at 8 h after injection of anti-inhibin serum when compared with the control value. Concentrations of FSH in the plasma remained high for 72 h, then declined to the control level by 84 h, concomitant with an abrupt decrease in the titre of free inhibin antibody in the plasma. High concentrations of oestradiol-17 beta were observed between 36 and 96 h after treatment.(ABSTRACT TRUNCATED AT 250 WORDS)
