T cell repertoire selection in T cell receptor transgenic mice.

T cell receptor (TCR) gene segments begin to rearrange in CD4-8-thymic lymphoblasts. In severe combined immune deficiency (scid) mice the development of T cells is arrested at this early stage as the scid thymus does not contain any CD4+ or CD8+ lymphocytes. This block in T cell development can be overcome by introducing productively rearranged TCR genes into the scid strain which results in the formation of CD4+8+ lymphocytes. While this early differentiation step requires TCR's of any specificity, later developmental stages depend on the specificity of the TCR: in scid mice, a transgenic TCR restricted by Db class I major histocompatibility complex (MHC) antigens allows the formation of CD4-8+ but not CD4+8- lymphocytes in Db positive but not Db negative animals. Thus, a TCR-MHC interaction in the absence of nominal antigen is required for the generation of mature T cells, and this interaction determines the CD4/CD8 phenotype. If both nominal antigen and presenting MHC antigen are present developing T cells are deleted at an immature CD4+8+ stage preventing the formation of more mature and functional autoaggressive T cell progeny. These experiments indicate that the immune system first learns about self by positive and negative selection of self recognizing lymphocytes from a continuously turning over pool of lymphocytes without requirement for idiotypic network interactions.

Nuclear magnetic resonance imaging of a benign cystic teratoma in the retroperitoneum.

A retroperitoneal teratoma arising from the vicinity of the upper pole of the right kidney in an 18-year-old woman is described. The plain anteroposterior abdominal radiographs showed irregular calcific shadows with tooth-like calcifications within the right upper quadrant of the abdomen. The findings of a primary cystic teratoma in the retroperitoneum examined by computerized tomography and magnetic resonance imaging of the abdomen are presented. Sagittal and coronal magnetic resonance imaging scans were useful to determine the extent of the mass, and to distinguish the mass from adjacent structures and organs.

Human aldolase A deficiency associated with a hemolytic anemia: thermolabile aldolase due to a single base mutation.

Fructose-1,6-bisphosphate aldolase A (fructose-bisphosphate aldolase; EC 4.1.2.13) deficiency is an autosomal recessive disorder associated with hereditary hemolytic anemia. To clarify the molecular mechanism of the deficiency at the nucleotide level, we have cloned aldolase A cDNA from a patient's poly(A)+ RNA that was expressed in cultured lymphoblastoid cells. Nucleotide analysis of the patient's aldolase A cDNA showed a substitution of a single nucleotide (adenine to guanine) at position 386 in a coding region. As a result, the 128th amino acid, aspartic acid, was replaced with glycine (GAT to GGT). Furthermore, change of the second letter of the aspartic acid codon extinguished a F ok I restriction site (GGATG to GGGTG). Southern blot analysis of the genomic DNA showed the patient carried a homozygous mutation inherited from his parents. When compared with normal human aldolase A, the patient's enzyme from erythrocytes and from cultured lymphoblastoid cells was found to be highly thermolabile, suggesting that this mutation causes a functional defect of the enzyme. To further examine this possibility, the thermal stability of aldolase A of the patient and of a normal control, expressed in Escherichia coli using expression plasmids, was determined. The results of E. coli expression of the mutated aldolase A enzyme confirmed the thermolabile nature of the abnormal enzyme. The Asp-128 is conserved in aldolase A, B, and C of eukaryotes, including an insect, Drosophila, suggesting that the Asp-128 of the aldolase A protein is likely to be an amino acid residue with a crucial role in maintaining the correct spatial structure or in performing the catalytic function of the enzyme.

Primary testicular carcinoid tumor in a 19-year-old boy.

This report describes a primary carcinoid tumor of the testis arising in a 19-year-old boy. Symptoms suggestive of a carcinoid syndrome were not noted in this case. The pathological specimen did not contain teratomatous components. Histochemically, the argyrophil reaction was positive. The immunohistochemical finding of immunoreactive cells for 5-hydroxytryptamine (serotonin) was also positive. To our knowledge, this is the youngest case to be reported.

[Clinical trials on cefuzonam in obstetrics and gynecological infections].

Ten patients (3 cases of abscess of vaginal cuff, 1 case of abscess of vaginal cuff complicated with parametritis, 2 cases of pyosalpinx, 1 case each of abscess of abdominal wall, pelvic cellulitis, pyometra with cervical cancer and paraovarian abscess were treated with cefuzonam (CZON), which was administrated by intravenous drip infusion at a dose of 1,000 mg twice a day for 3 to 10 days (6 g to 19 g total). The clinical effectiveness reached 70.0% including 1 excellent case, 6 good cases and 3 poor cases. Bacteria were detected in all the 10 cases, and with CZON treatment, bacterial eradication were obtained in 3 cases, bacteria decreased in 3 cases, no change in 2 cases and bacterial replacement occurred in 2 cases. No abnormal laboratory findings and side effects were noted. From the above results, CZON seemed to be a highly effective and useful agent for gynecological infections.

[Heart metastasis of gallbladder cancer--a case report].

A 42-year-old man with gall bladder cancer was given a pancreatoduodenectomy. Chest-roentgenograms revealed a prominence of the left cardiac silhouette 6 months after operation. Two months later, the patient developed exertional dyspnea and chest pain. A diagnosis of heart metastasis of gall bladder cancer was established by aspiration cytology. Intravenous administration of 5-FU and cis-DDDP proved ineffective in controlling the disease. The patient died 14 months after operation. An autopsy revealed metastasis to the left ventricle of the heart. This is the 6th such case of gallbladder cancer in the literature.

Direct surgical treatment of intracavernous internal carotid artery aneurysms: report of four cases.

The authors have operated directly on four cases of intracavernous internal carotid artery aneurysms by opening the cavernous sinus. Surgery was performed using a semisitting position, in which the upper half of the patient's body was elevated approximately 20 degrees from the horizontal plane. The superior wall of the cavernous sinus was opened through the pterional approach, and an aneurysmal neck clipping was successfully performed on three patients. A coating of the aneurysm was carried out on one patient. The results obtained were excellent in all cases. The authors believe that the operative technique reported is useful for surgical treatment of intracavernous lesions.

Beta-2 adrenergic receptors are not only for circulating catecholamines in ventricular muscles of carp heart (Cyprinus carpio).

Whether beta-2 adrenergic receptors are only for the circulating catecholamines in isolated ventricular muscles of carp heart was studied. Isoproterenol, epinephrine and norepinephrine had concentration dependent positive inotropic effects. The ED50 values for isoproterenol, epinephrine and norepinephrine were 6.23 +/- 1.9, 87.3 +/- 27.3 and 4500 +/- 580 nM, respectively. Phentolamine did not alter the inotropic effects. Tyramine increases the force of contraction and this action was completely blocked by propranolol and reserpine, but not by atenolol. Norepinephrine levels, in comparison with those of epinephrine, were similar in plasma and higher in ventricular muscles. However, the norepinephrine levels in ventricular muscles of carp heart were markedly lower than those in ventricular muscles of rat heart. These results suggest that ventricular muscles of carp heart contain adrenergic neurons, and that a catecholamine released from the nerve terminals, presumably epinephrine, may stimulate beta-2 adrenergic receptors.

Serum levels of coenzyme Q10 and lipids in patients during total parenteral nutrition.

Serum levels of coenzyme Q10 (CoQ10) as well as lipids were determined in patients during total parenteral nutrition (TPN). The mean CoQ10 levels (M +/- SD) were 0.77 +/- 0.30 microgram/ml for 108 normal subjects and 0.59 +/- 0.35 microgram/ml for 95 patients before TPN. The mean CoQ10 level of the patients decreased significantly to 0.35 +/- 0.23 microgram/ml one week after the start of TPN, and then remained almost unchanged during TPN for up to 6 weeks. When the patients receiving TPN (TPN patients) were grouped according to their clinical diagnoses, the mean CoQ10 level of patients with cancer was significantly lower than that of the other patients without cancer in 4 week therapy, but there was no difference in the levels between the patients with and without diseases of the gastrointestinal tract. Serum levels of total cholesterol (T-Chol) and esterified cholesterol in TPN patients also declined below their respective normal ranges, but not to the same extent in comparison to CoQ10. The levels of triglycerides (TG), phospholipids (PL), non-esterified fatty acids, low density lipoproteins, very low density lipoproteins, chylomicrons, and cholesterol in the high density lipoprotein fraction in serum of TPN patients were within their normal ranges. The levels of CoQ10 in TPN patients were correlative to those of T-Chol, TG, and PL, and decreased rapidly prior to the latter levels.
