The phenolsulphthalein 60/60 plasma test (PSP 60/60) was evaluated in 30 control subjects (mean value 51 micrograms/100 ml +/- 49 micrograms/100 ml) and 60 patients with chronic renal disease with varying degrees of renal insufficiency. PSP 60/60 proved to be superior to determination of serum creatinine and urea nitrogen. PSP 60/60 correlated well with endogenous creatinine clearance and 131I-iodohippurate clearance in over 80% of cases. Of various exogenous and endogenous factors interfering with the test, drugs influencing renal excretory mechanisms are most important. Due to its technical simplicity, PSP 60/60 is a useful screening test for evaluation of renal function, especially as an office procedure.

The serum of a patient suffering from thrombocytopenia associated with Waldenström's macroglobulinemia was found to contain an IgM paraprotein with unusual characteristics. Following isolation of this IgM paraprotein, dilutions up to 0.01 mg/ml caused immunofluorescence of thrombocytes from man, sheep, rabbit, and horse. In control studies with isolated IgM paraprotein from another non-thrombocytopenic patient with Waldenström's disease, positive immunofluorescence could be elicited only when a 1000-fold greater concentration of the paraprotein was used. The IgM paraprotein from our thrombocytopenic patient and that from the control patient showed comparable differences in immunofluorescence of thrombocytes and megacaryocytes when bone marrow smears were used for incubation. Further studies following papain cleavage of the IgM paraprotein with antithrombocytic activity showed that the combining activity was located in the Fab fragment. This observation characterizes the IgM paraprotein from our patient as an antibody directed to a substance in human and animal thrombocytes. Paraproteins with antibody-like activity for thrombocytes have not been identified previously; the present observation suggests that this mechanism should be considered a possible cause of thrombocytopenia associated with macroglobulinemia.
The diagnostic and prognostic reliability of lipoprotein-X (Lp-X) in demonstrating or ruling out cholestasis has been evaluated in a group of 80 patients with diseases of the liver and/or the biliary tracts, and in 103 subjects with various other diseases. The results of Lp-X detection were compared with the so-called "enzymes indicating cholestasis": alkaline phosphatase, leucine arylamidase, and gamma-glutamyltranspeptidase. Where possible a histologic specimen of the liver was obtained. The correlation between Lp-X and "enzymes indicating cholestasis" was satisfactory in more than 90% of cases. When compared with the histologic findings, Lp-X proved to be more reliable than the enzymes. Despite this fact, Lp-X did not show absolute specifity in the detection of cholestasis as there were several negative results in cases with histologically proven cholestasis. Furthermore, the differentiation of intra- and extrahepatic cholestasis was not possible on the basis of Lp-X. In the control group of 103 patients with other than hepatobiliary diseases, a positive Lp-X result was found in 3 cases. Further investigations in these three patients revealed that primarily unsuspected hepatobiliary disease could not be ruled out. In the follow-up of a hepatobiliary disease the transition of Lp-X to negative indicates a trend towards improvement of cholestasis 1-2 weeks earlier than the enzymes mentioned above.
Uncalibrated flow in the vertebral arteries was registered with a bidirectional CW Doppler ultrsound apparatus. It was applied to 20 patients, results being compared with aortic arch angiograms in which the extracranial part of all four cerebral vessels was seen. In 14 the vertebral arteries had a symmetrical flow and lumen, a similar symmetrical result being obtained with the Doppler. Hypoplasia of one vertebral artery was present in two patients the distinct asymmetry being recorded by the ultrasound. Three patients with central stenosis in one subclavian artery and subclavian "steal" were found to have reverse flow by the Doppler method in the vertebral artery on the side of the stenosis. Endarterectomy was performed on the affected subclavian artery in two of these patients: post-operative vertebral ultrasound recordings demonstrated a return to physiological flow direction in both vertebral arteries.
An attempt has been made to classify the wide range of available antimicrobial chemotherapeutic agents from the practical viewpoint. A group of drugs with selective indications and a series of alternative chemotherapeutic agents are distinguished from chemotherapeutic agents for everyday medical routine (penicillin G and analogs, ampicillin, tetracyclines, sulfonamides). The indications for the main drugs and some practical guidelines for chemotherapy are summarized. The excessive commercial supply of ampicillin derivatives and cephalosporins comes in for special comment. It is emphasized that some valuable antibiotics (cephalosporins, carbenicillin, gentamycin, kanamycin) should be kept in reserve.

10 out of 1600 angiographies of the kidneys or abdominal aortograms showed a renal aneurysm and in one patient an arterio-venous fistula following renal biopsy. Renal artery aneurysms are found mainly at the branching of the main renal artery. Fibromuscular dysplasia plays a large part in its development. Because of the danger of rupture every renal aneurysm should, if possible, be treated by surgery.
A continuous flow method is described for the determination of uric acid in serum using uricase and peroxidase, with omicron-dianisidine as the chromogen. The method can be used with single channel systems and with the multichannel system SMA 12/60 (Technicon). The new method was compared with six other manual and mechanized methods for the dosage of uric acid, and the results were analyzed statistically. The results from all the enzymic methods showed a good correlation, whereas the "chemical oxidation" methods showed systematic deviations.
Eleven parameters in heparinzed whole blood and plasma samples were investigated for storage-dependent changes. Samples were stored at +25 degrees C, and analyses were performed with the SMA 12/60 at 0, 24, 48, 72 and 96 hours after sampling. In whole blood, the following parameters increased significantly during storage: potassium calcium, phosphate, total protein. The following decreased: chloride, CO2, uric acid and bilirubin. In plasma samples, analogous effects were observed for: calcium, CO2, total protein, uric acid and bilirubin.
