[Prosopagnosia and disturbance of color recognition during recovery from cortical blindness].

Neuropsychologic findings during recovery from cortical blindness are described in four right-handed patients (two female, two male) aged 53 to 70 years. The lesions were due to occlusion of both posterior cerebral arteries (Case 1) and vascular spasm following subarachnoidal hemorrhage (Case 3) or angiography (Cases 2 and 4). Complete blindness lasted for 3 months until death in Case 1, 4 weeks in case 2, and 1 to 2 days in Cases 3 and 4. Confusional states and visual hallucinations were pronounced in three cases. Anosognosia (Anton's syndrome) was most pronounced in a patient with occlusion of both posterior cerebral arteries and less obvious in the remaining patients. During the recovery phase, symptoms of right hemisphere involvement were prominent with left-sided hemianopsia and diminished optokinetic nystagmus to the left, prosopagnosia in two cases, and dysmorphopsia with altered physiognomic recognition in one case. Transient disturbances of color recognition occurred in three patients. The observations are discussed with reference to clinical and neuropathologic findings of the literature.

"ADAM complex" (amniotic deformity, adhesions, mutilations)--a pattern of craniofacial and limb defects.

We report eight patients with the craniofacial defects and limb anomalies of the (amniotic deformity, adhesions, mutilations) ADAM complex. Facial abnormalities comprise clefts and distortion and dislocation of craniofacial structures; limbs show various combinations of amputation, secondary syndactyly, and constriction. From previous reports and our cases it is obvious that the clinical picture of the ADAM complex varies enormously; a less severe type combines cleft lip and palate with amputations or amniotic bands. Clinical and experimental data suggest that these malformations are of symptomatic (exogenous) origin. Nosologic differentiation from other conditions phenotypically similar but of genetic etiology is important for genetic counseling. Observation of the ADAM complex in two members of a family suggests that genetic factors might operate in some cases.

Trends and aspects in clinical chemistry.

This paper first seeks to evaluate current trends in methodology and technology and discusses possible implications for the future of clinical chemistry, whose range and diversity of tasks are carefully defined. It is thought that the remarkable development in the field of immunological methods, in the general sense of the work, will continue for the years to come. One other principle, which it is predicted will play an increasing role, is thermometry for the determination of enthalpy. Similarly, new developments in the fields of bio- and chemoluminescence offer interesting possibilities. The impact of the computer in clinical chemistry is also discused. Secondly, the double role of clinical chemistry as a diagnostic speciality and as an academic field is analyzed. On the one hand, the need for closer cooperation both with other diagnostic subspecialities and with other medical clinical disciplines is stressed. In particular, newly organized Departments of Laboratory Medicine encompassing microbiology, cytology, nuclear medicine, toxicology etc. might contribute greatly to interdisciplinary understanding. On the other hand, problems of clinical medicine and of certain areas within biological research should be at the center of interest of the future research-oriented clinical chemist.

[The importance of ionic strength and protein concentration in the dialysis performance of autoanalyzer systems for the enzymatic determination of uric acid (author's transl)].

An earlier method for the determination of uric acid (this journal 13, 89--96, (1975) was reevaluated. It was confirmed that it is necessary to add NaCl and a detergent to the wash fluid in order to obtain full sensitivity and linearity. Protein components in the samples or in secondary standard solutions also increase sensitivity and straighten the calibration curve of primary standard solutions. The effects are compared with results reported in the literature, and the possible mechanisms of these effects are discussed.

IgM and IgG antibody profiles after immunization of man with a HDCS-vaccine (preliminary communication).

HDCS vaccine was given to 20 healthy, not rabies-exposed people on days 0, 3, 7, 14, 28 and 90. Ten of them received in addition 20 IU/kg of human antirabies immunoglobulin. Vaccine potency in NIH tests was 1.8 (WHO). Most individuals did not show any measurable neutralizing antibody titer on days 3 and 7. On days 14, 28, 90 and 300 the mean of titres was above 1:200; with one exception, no titer was lower than 1:50. The sera of days 14 and 90 were separated on "Sephadex G-200" columns. On day 14 at least 46% of the rabies antibody was found in the IgM fraction, while 15% was recovered from the IgG fraction. On day 90 the proportion recovered from the IgM fraction had decreased to 17%; the IgG antibody had increased to 30% of the titres found in whole serum. Almost no antirabies activity was found in the IgG fractions.

[The posterior opening of the pterygopalatine fossa and the position of the pterygopalatine ganglion].

Our material consisted of 100 adult skulls and 50 adult half-head sections where the posterior opening of the pterygopalatinate fossa and the position of the pterygopalatinate ganglion were investigated. The report contains the following specifics: 1. Course and contents of the pterygoidal canal as well as the topographical relations of the canal to the sphenoid sinus. 2. Positioning of the axis of the foramen rotundum to the various planes of the head and its contents. 3. Course of the vomerovaginal canal and its contents. 4. Distances of the pterygopalatinate ganglion to important reference points of the head and to the median-sagittal line.

[Experimental protracted shock. II. Blood coagulation and histological changes].

Twenty mongrel dogs are subjected to a standardized traumatic hemorrhagic shock. After reinfusion of the dogs' own blood the animals are continuously observed for the 72 hr. Deterioration of general hemodynamics and activation of intravascular coagulation is more pronounced in the nonsurviving animals. Correspondingly, histologic alterations particularly of the liver and intestine are more marked in the nonsurvivors. Increase of partial thromboplastin time and long-lasting increase of the prothrombin time (Quick) appear to be early signs of a fatal prognosis.

[Attempts at postexposure active and passive human rabies immunization].

For several years now in Switzerland, postexposure prophylaxis against rabies has been performed with duck embryo vaccine containing inactivated virus. According to WHO recommendations, 14 consecutive daily injections are given subcutaneously followed by 3 booster injections 10, 20, and 90 days later. In addition, a recently developed human diploid cell vaccine has also become commercially available in Switzerland. This study is concerned with the question whether, by the use of the latter vaccine, sufficient antibody titres can be achieved with a smaller number of injections given at longer intervals. Furthermore, the question was studied whether simultaneous administration of homologous human hyperimmune gamma-globulin together with the first vaccination dose inhibits antibody production. "Vaccin rabique inactivé Mérieux", cultivated on human diploid cells, was injected on days 0, 3, 7, 14, 28, and 90. A group of 10 healthy volunteers received active vaccination only, while another group of 10 volunteers received Rabuman Berna, 20 U/kg, in addition. Antibody titrations were done by means of a neutralization test on monolayer cell cultures. On day 14, antibody titers of at least 1:100 (corresponding to more than 3 IU/ml) were found in both groups. There was no indication of significant inhibition of antibody production as a consequence of the combined active/passive vaccination.

Quantitative detection of antibodies to Mycoplasma suipneumoniae in pigs' sera by an ezyme-linked immunosorbent assay.

A new method for quantitative detection of antibodies to Mycoplasma suipneumoniae in pigs' sera has been developed by the application of an enzyme-linked immunosorbent assay (ELISA). The results show that the ELISA is specific and highly sensitive. In experimentally infected pigs antibodies could be detected several weeks before the clinical manifestation of enzootic pneumonia.

[Serum protein values in uremic patients, dialysis-patients and patients with transplanted kidneys].

Within three patient groups (1. advanced renal failure, 2. patients under chronic hemodialysis, 3. patients with renal transplants) 16 serum proteins were determined immunochemically employing MANCINIS method. The group mean values for each protein were compared, and the statistical significance of intergroup differences was examined. The most noteworthy results were the following: a distinct increase in prealbumin in all groups, least marked in the dialyzed patients; frequent elevation of alpha2-macroglobulin in the patients with transplanted kidney, but not in the other two groups; a significant increase in beta2-glycoprotein in all groups. Other changes affected several components of the acute phase reactants which were, in part, markedly elevated. On the other hand, alpha1-antitrypsin and transferrin remained largely within the normal range. Possible mechanisms accounting for some of these changes are discussed.

Critical evaluation of the Obscure Figures Test as an instrument for measuring "cognitive innovation".

The Obscure Figure Test has been conceptualized by Acker and McReynolds (1965) as a measure of Cognitive Innovation. As a general concept Cognitive Innovation refers to an integration of different kinds of behavioral systems, e.g., creativity and exploratory behavior. Considering recent research about the relationship between curiosity and creativity as well as basic assumptions underlying this test, it is hypothesized that this is an instrument for measuring creativity rather than curiosity. 41 boys and 41 girls ranging from 7 to 10 yr. of age were given a battery of tests of curiosity and creativity. There were significant correlations between Obscure Figures Test and creativity measures. No such relationships were found between obscure figures and curiosity measures. A factor analysis yielded two factors which could be interpreted as "visual exploration" and "creativity", the obscure figures being a marker variable on the latter factor. Results are discussed in terms of the theoretical assumptions underlying the concepts of curiosity, creativity, and Cognitive Innovation. Further implications with respect to the requirements for an operationalization of Cognitive Innovations are mentioned.
