[Two cases of budgerigar breeder's lung].

We report 2 patients with Bird breeder's lung associated with keeping budgerigars. Patient 1 kept a budgerigar indoors for 13 years. Histological examination revealed fibrosis and granulomatous changes in the alveolar septa. Among T lymphocyte subsets in BALF, helper T cell were increased. In the serum, precipitating antibodies to budgerigar dropping extract were observed. Patient 2 had kept Budgerigar for 7 months. Histologically, alveolitis was the primary finding, and suppressor T cell in the BALF were increased. Precipitating antibodies to budgerigar dropping extract was observed only in the BALF but not in serum. In patient 1 who showed a chronic course and granuloma formation, helper T cell in the BALF were increased while in patient 2 who showed an acute course and alveolitis, suppressor T cell in the BALF were increased, and precipitating antibody was observed only in the BALF. These findings suggest that T lymphocyte subsets in the BALF differ depending on the stage of hypersensitivity pneumonitis, and specific local antibody production precedes systemic antibody production.

[Antagonism of diazepam-induced sedative effects by flumazenil in patients after surgery under lumbar spinal or epidural anesthesia].

The efficacy and general safety of flumazenil (YM684), a specific benzodiazepine antagonist, have been evaluated. Forty-seven patients scheduled for surgery under lumbar spinal or epidural anesthesia with diazepam sedation received flumazenil at the end of the procedure. Criteria of efficacy were the degree of sedation and antegrade amnesia. Before injection all patients were heavily sedated with the mean dose of 27 mg of diazepam (range 10-50 mg). After the mean dose of 0.21 mg of flumazenil (range 0.2-0.4 mg), all patients were awake or drowsy within 1-6 min but sedation recurred in one patient 1 hour later. The amnesia was eliminated by flumazenil in 34 patients (72%). No serious side-effects or hemodynamic changes were observed after flumazenil. It is concluded that flumazenil is an effective antagonist of sedation induced by diazepam.

Malignant Zollinger-Ellison syndrome. Stabilization of liver metastasis after gastrectomy with resection of primary tumor.

A case of malignant gastrinoma of the jejunum and jejunal mesentery, with findings of metastasis in the liver was studied. Tumor resection and remnant gastrectomy, as well as liver biopsy, were performed. Of all the tumors removed, the metastatic tumor had the lowest gastrin content, which was 1.5 times that found in normal human antral mucosa. The gastrin distribution was of four peaks and one peak for the main tumor, versus two peaks for the metastatic tumor in which G17 was the main component. Seven years after the operation, computed tomography (CT) scan indicated there was stabilization of the liver metastasis, and now, ten years after the total gastrectomy, the patient is well and working full-time. Such an analysis can be of value in determining the prognosis in cases of malignant gastrinoma with liver metastasis.

Electrophysiologic effects of E-4031, a class III antiarrhythmic agent, on re-entrant ventricular arrhythmias in a canine 7-day-old myocardial infarction model.

Effects of E-4031, a class III antiarrhythmic agent, on re-entrant ventricular arrhythmias were studied in eight dogs with a 7-day-old myocardial infarction. Epicardial mapping and local refractory periods were obtained using 47-channel bipolar electrodes attached to the epicardium. The induction of sustained ventricular tachycardia by programmed electrical stimulation was not suppressed by i.v. infusion of E-4031 at 1 microgram/kg/min, but was suppressed markedly by infusion at 10 micrograms/kg/min in six of seven dogs. During the infusion of E-4031 at 10 micrograms/kg/min, epicardial conduction velocity in the normal ventricle did not change (0.7 +/- 0.12 to 0.71 +/- 0.13 m/sec, n = 6), whereas slowed conduction in the infarct zone improved (0.58 +/- 0.10 to 0.77 +/- 0.13 m/sec, n = 6). E-4031 at 10 micrograms/kg/min prolonged effective refractory periods (ERP) in the normal zone (139 +/- 8 to 164 +/- 18 msec, P less than .01, n = 8), nontransmural infarct zone (145 +/- 7 to 177 +/- 15 msec, P less than .01, n = 8) and transmural infarct zone (156 +/- 14 to 191 +/- 22 msec, P less than .01, n = 8). The degrees of ERP prolongation were almost equal in all zones. On epicardial mapping, the areas of longer ERP and delayed conduction were observed to become inexcitable after the administration of E-4031. These results demonstrated that E-4031 effectively prevented the induction of re-entrant ventricular tachycardia in canine myocardial infarction model, and suggested that E-4031 rendered re-entrant circuits inexcitable by marked ERP prolongation in both normal and infarct zones.

Developmentally regulated expression of vascular smooth muscle myosin heavy chain isoforms.

Two types of smooth muscle myosin heavy chain (MHC) isoforms, SM1 and SM2, were recently identified to have different carboxyl termini (Nagai, R., Kuro-o, M., Babij, P., and Periasamy, M. (1989) J. Biol. Chem. 264, 9734-9737). SM1 and SM2 are considered to be generated from a single gene through alternative RNA splicing. In this study we investigated expression of vascular MHC isoforms during development in rabbits at the mRNA, protein, and histological levels. In adults, all smooth muscle cells reacted with both anti-SM1 and anti-SM2 antibodies on immunofluorescence, suggesting the coexpression of SM1 and SM2 in a single cell. In fetal and perinatal rabbits, however, only anti-SM1 antibody consistently reacted with smooth muscles. Reactivity with anti-SM2 antibody was negative in the fetal and neonatal blood vessels and gradually increased during 30 days after birth. These developmental changes in SM1 and SM2 expression at the histological level coincided with mRNA expression of each MHC isoform as determined by S1 nuclease mapping, indicating that expression of SM1 and SM2 is controlled at the level of RNA splicing. However, sodium dodecyl sulfate-polyacrylamide gel electrophoresis of myosin from fetal and perinatal aortas revealed the presence of large amount of SM2. Interestingly, fetal SM2 did not cross-react with our anti-SM2 antibody on immunoblotting. We conclude that expression of SM1 and SM2 are differentially regulated during development and that a third type of MHC isoform may exist in embryonic and perinatal vascular smooth muscles.

An Epstein-Barr virus-transformed B cell line produces autoregulatory interleukin-1 that regulates bone remodeling.

In this study, we demonstrate that an Epstein-Barr virus-transformed B cell line, A-11, produced interleukin-1 (IL-1), a cytokine that regulates bone remodeling. A-11 cells produce IL-1 in a cell dose- and culture time-related manner. The IL-1 activity was neutralized by recombinant human IL-1 (rhIL-1) alpha antiserum, but not by rhIL-1 beta antiserum. The IL-1 was semi-purified by (NH4)2SO4 precipitation, Superose prep 12 gel filtration, and anion-exchange chromatography strongly stimulated in vitro bone resorption. The stimulatory effect of the purified IL-1 on bone resorption was prostaglandin independent. Purified IL-1 inhibited DNA and collagen synthesis in the osteoblastic cell line MC3T3-E1. However, it enhanced significantly the cellular activity of alkaline phosphatase (EC 3.1.3.1), a marker enzyme for differentiation of osteoblasts. On the other hand, A-11 cell proliferation was inhibited by addition of rhIL-1 alpha antiserum, but not by rhIL-1 beta antiserum. And cell proliferation was stimulated by exogenous rhIL-1 alpha and -beta.

Effects of anesthetic and related agents on calcium-induced calcium release from sarcoplasmic reticulum isolated from rabbit skeletal muscle.

We have investigated the effects of anesthetic and related agents on Ca(2+)-induced Ca2+ release (CICR) in heavy sarcoplasmic reticulum isolated from rabbit skeletal muscle. The purpose of this study is to elucidate their possible role as triggering agents in malignant hyperthermia (MH). None of the agents (ketamine, procaine, lidocaine, succinylcholine, pancuronium and fentanyl) affected CICR at clinical concentrations. At higher concentrations, procaine, pancuronium and succinylcholine inhibited CICR, but ketamine rather potentiated it. It is unlikely that lidocaine is a potent facilitator of CICR at any concentrations. We conclude that procaine, lidocaine, non-depolarizing muscle relaxants and opiate can be used safely for MH susceptible patients and that ketamine and succinylcholine are not recommended.

Mapping of the Es-4 locus and linear order of esterase genes (linkage group V; LGV) in the rat.

There are three different linear orders of esterase loci of linkage group V (LGV) in the rat (Rattus norvegicus). The first is Es-2-Es-3-Es-1, the second Es-3-(Es-2,Es-4)-Es-1, and the third Es-3-Es-2-Es-1-Es-4. We carried out mating experiments to define the order clearly. Linkage analyses of the four esterase loci, Es-1, Es-2, Es-3, and Es-4, were carried out using two inbred strains carrying different alleles at the four loci. Six locus combinations examined in this study were as follows: Es-1-Es-2, Es-1-Es-3, Es-1-Es-4, Es-2-Es-3, Es-2-Es-4, and Es-3-Es-4. The recombination frequencies of each combination were 6.3, 6.3, 6.3, 5.2, 1.8, and 3.4%, respectively. The first recombination between Es-2 and Es-4 was observed. We propose that the esterase loci of LGV be classified into three clusters according to distances between the loci. The linear order of the four loci is shown to be as follows: [Es-3] (cluster II)-3.4 +/- 2.4%-[Es-4-1.8 +/- 1.7%-Es-2] (cluster III)-6.3 +/- 6.1%-[Es-1] (cluster I).

A case of Gilbert's syndrome combined with macroamylasemia.

A 30-year-old Japanese male, who had no remarkable family history, visited our hospital with a complaint of abdominal pain, and unconjugated hyperbilirubinemia and hyperamylasemia were observed. He showed negative hemolysis tests, positive nicotinic acid test, low hepatic bilirubin UDP-glucuronyltransferase activity, decreased bilirubin diglucuronide and increased bilirubin monoglucuronide in bile, and a decrease in serum bilirubin after phenobarbital administration. He also showed high serum amylase level, low urine amylase level, and low amylase-creatinine clearance ratio. Gel filtration of serum with Sephadex G-200 revealed the existence of macroamylase. Countercurrent immunoelectrophoresis proved binding of serum amylase to lambda type IgA. From these results, the case was diagnosed as Gilbert's syndrome combined with macroamylasemia.

Age-related changes in brush border enzymes under different dietary conditions.

We examined the age-related changes of renal brush border enzymes of male Donryu rats under different dietary conditions. High protein diet (51% casein) markedly decreased the activity of renal leucine aminopeptidase as compared with low protein diet (8% casein). No significant difference, however, was observed in the activity of renal maltase between high and low protein diets. Alkaline phosphatase activity from young adult (3-month-old) rats in the high protein diet group was significantly lower than that in the low protein diet groups. From these results, it is inferred that dietary conditions affect the age-related alteration of renal brush border enzymes. Together with the previous results, it is suggested that proteins of renal brush borders are altered differently during the aging process.

Age-related accumulation of lipofuscin in myocardium of Japanese monkey (Macaca fuscata).

Deposition of lipofuscin autofluorescent granules, is an important phenomenon in the aging process. Cardiac lipofuscin in Japanese monkey (Macaca fuscata) first appears at approximately two years of age. Thereafter, the amount of cardiac lipofuscin increases with increasing age. The first appearance of cardiac lipofuscin shows a good correlation with the maximum life span of animals. Moreover, the rate of lipofuscin accumulation is inversely correlated with life span energy potential (LEP) of animals. From these results, it is suggested that the rate of lipofuscin accumulation depends on the total life span energy expenditure of the tissue.

Parapharyngeal schwannoma of the cervical sympathetic chain in a child.

Parapharyngeal schwannomas of the cervical sympathetic chain are benign tumors that have not been reported in children, compared to parapharyngeal neuroblastomas. We present, to our knowledge, the youngest reported case of parapharyngeal schwannoma of the cervical sympathetic chain. The nerve of origin could be clearly ascertained when excision was made through a cervical approach.

Control of alpha-Amylase Development in Cotyledons during and following Germination of Mung Bean Seeds.

Developmental patterns of alpha-amylase in Vigna radiata cotyledons during and following germination were quite different depending on the differences in the treatments of cotyledons during the imbibitional stage. When axis-detached cotyledons were imbibed in water with seed-coats attached, alpha-amylase activity did not increase and remained low. On the other hand, when the cotyledons were imbibed in water after seed-coat removal, the enzyme activity increased markedly. If the axis was attached to the cotyledons, alpha-amylase showed a marked development even under the former imbibition conditions. These changes in the enzyme activity were in parallel with those in the enzyme content, and the content, in turn, was dependent upon the availability of mRNA for alpha-amylase. We propose that the regulation of the development of alpha-amylase in cotyledons may involve some factor(s) inhibitory to accumulation of alpha-amylase mRNA, which is present in dry cotyledons and can be removed from cotyledons by leakage or by the presence of the axis.

Immunohistochemical study of ras p21 expression in human gastric cancers and benign lesions.

Expression of ras p21 in human gastric cancers, benign lesions and normal tissues was immunohistochemically evaluated by the avidin-biotin peroxidase complex (ABC) method with anti-ras p21 monoclonal antibody rp-28. Positive p21 immunoreactivity was shown in 23 (77%) of 30 gastric cancers, in 13 (48%) of 27 benign lesions and in 10 (22%) of 46 normal mucosa cases. Among them, strong staining was demonstrated only in 11 (37%) of 30 gastric cancers, but not in benign lesions and normal tissues. Cases showing more than 20% positive cell ratio were observed in 22 (73%) gastric cancers, in 11 (41%) benign lesions and 6 (13%) normal mucosa cases. Further, intracellular distribution of ras p21 is heterogeneous in gastric cancers, while it is homogeneous in benign lesions or normal tissues. The ABC method with rp-28 could be helpful for clinical differential diagnosis between gastric cancers and benign lesions by investigating three factors: staining intensity, positive cell ratio and intracellular distribution of ras p21.

Effect of alpha-naphthylisothiocyanate on blood clearance of 99mTc-phytate in rats.

The effect of alpha-naphthylisothiocyanate (ANIT) on blood clearance of 99mTc-phytate (99mTc-P) in rats was examined, and blood clearance test of 99mTc-P was compared with the cases of serum transaminase test, serum bilirubin test or histological test of the liver. The disappearance rate of 99mTc-P from blood decreased with the increase in dose of ANIT and with the passage of time after the ANIT administration. Changes of the blood clearance of 99mTc-P after ANIT treatment in rats may be influenced by the disorder in the hepatocytes rather than in the bile ductule cells. The blood clearance test of 99mTc-P in rats showed a sensitive reaction for the acute hepatic dysfunction induced by ANIT equally to the serum transaminase test, the serum bilirubin test or the histological test of the liver.
