[Postoperative adjuvant M-VAC chemotherapy for invasive bladder cancer].

Between 1986 and 1989, 14 patients undergoing cystectomy for bladder cancer, in pathological stage high risk pT2 group, pT3-4 and/or with N+ disease, received postoperative adjuvant methotrexate, vinblastine, doxorubicin and cisplatin (M-VAC) chemotherapy. Of the 14 patients 10 were alive with no evidence of disease for an average of 41 months. Tumor recurrence was seen in 4 patients (bone in 2, lungs in 1, brain in 1 patient). Of the 4 patients, 3 patients died of cancer progression at an average of 26 months and 1 patient was alive with tumor for 30 months. Their actual survival rate at 64 months was 70%, which was significantly higher than that of the historical control groups (1974-1981: 18%, 1982-1985: 46%). Although postoperative adjuvant M-VAC chemotherapy for invasive bladder cancer seemed effective in this study, a controlled randomized study will be necessary to conclude if it could be of real benefit for these patients.

Hepatocyte growth factor is a potent mitogen for cultured rabbit renal tubular epithelial cells.

Hepatocyte growth factor (HGF), which is a potent growth factor of adult rat hepatocytes in primary culture, also strongly stimulated DNA synthesis of rabbit renal tubular epithelial cells in secondary culture. Its mitogenic activity was dose-dependent, being detectable at 3 ng/ml and maximal at 30 ng/ml. Over 20% of the cells were shifted to the S-phase by HGF alone, judging by the labeling index. HGF had additive effects with EGF, acidic fibroblast growth factor (a-FGF), and insulin. Transforming growth factor-beta 1 (TGF-beta 1) strongly inhibited DNA synthesis of renal tubular cells stimulated by HGF. The growth of renal tubular epithelial cells was also regulated by cell density: DNA synthesis stimulated by HGF was high at lower cell density and was strongly suppressed at high cell density. These results suggest that HGF may act as a renotropic factor in compensatory renal growth or renal regeneration in vivo.

Immunohistochemical prostatic acid phosphatase level as a prognostic factor of prostatic carcinoma.

To determine whether prostatic acid phosphatase (PAP) immunoreactivity in prostatic adenocarcinoma is a reliable prognostic factor, the PAP immunohistochemical distribution has been examined in 78 prostatic carcinoma cases. The intensity of PAP immunostaining was graded from 0 to 2, and the scores of the primary and the secondary staining patterns were added to assess the extent of the PAP expression in needle biopsy specimens. As a result, a higher cancer-specific survival rate was observed in patients showing a greater PAP immunostaining (P less than 0.01). Further, a multivariate analysis was made of possible prognostic factors (age, stage, Gleason score, serum PAP, PAP-immunostaining score, and the initial treatment) to estimate the extent of their impact on cancer-specific survival. Results have confirmed that the difference in PAP immunoreactivity is an excellent, independent prognostic factor for prostatic carcinoma.

Multiloculated renal cell carcinoma.

Multiloculated renal cell carcinoma and multilocular renal cyst with renal cell carcinoma are uncommon diseases. We report on 2 cases of multiloculated renal cell carcinoma. To our knowledge 16 cases of multiloculated renal cell carcinoma and multilocular renal cyst with renal cell carcinoma have been previously described in detail. The authors review the literature and discuss the aetiology and diagnosis of the disease.

Development of bacillus Calmette-Guérin-induced antitumor activity in peripheral blood mononuclear cells.

Peripheral blood mononuclear cells (PBMNCs) after bladder instillation of bacillus Calmette-Guérin (BCG) to patients with superficial bladder carcinoma showed lytic activity against natural killer (NK)-sensitive and NK-resistant target tumor cells. PBMNCs in normal volunteers were isolated and cultured with BCG (5 micrograms/ml) in vitro. Maximum cytotoxicity was obtained within 24 h. Cytosine arabinoside as a DNA synthesis inhibitor did not inhibit this killer cell activation. Monoclonal antibody treatment revealed that both precursor and effector cells were NK cell type. However, effector cells had a wider spectrum of target cells than usual NK cells. The recognition specificity from cold target competition experiments was selective.

Transient increase in renal epidermal growth factor content after unilateral nephrectomy in the mouse.

A sensitive enzyme-linked immunosorbent assay for mouse epidermal growth factor was established for measurement of the content of epidermal growth factor in the remaining kidney after uninephrectomy. In 5-week-old male mice, the renal epidermal growth factor content before uninephrectomy was 355 +/- 97 ng/g wet tissue with a 2.1-fold increase on the first day after uninephrectomy, whereafter it gradually decreased. In 15-week-old mature male mice, the renal epidermal growth factor content increased 1.7-fold on the first day after uninephrectomy. Immunohistochemical analysis showed that epidermal growth factor was present in the distal tubular cells and that the staining intensity was increased on the first day after uninephrectomy. During the course of compensatory renal growth, no significant alteration of epidermal growth factor content was observed in plasma or in the submaxillary gland. Our data suggest that the increase in renal epidermal growth factor content after uninephrectomy is due to an increased production of epidermal growth factor in the kidney itself. The significance of this phenomenon is discussed.

[Bladder involvement in HTLV-I associated myelopathy].

The HTLV-I infection was endemic in south western Kyushu. This human T-lymphotropic virus type I may cause HTLV-I associated myelopathy (HAM), a neurological disease characterized by a spastic paraparesis. And one of the minor diagnostic features of HAM is the presence of cystorectal disturbance. We experienced 35 HAM patients with a neurogenic bladder. A gradually progressive contracted bladder was observed in 3 of them. The main pathological finding in these patients was submucosal infiltration of lymphocytes. These findings suggest that immunologic mechanisms account for the development of bladder lesions.

[Bladder replacement with sigmoid colon for bladder cancer].

From 1972 to 1989, 21 patients underwent bladder replacement with sigmoid colon after cystectomy for bladder cancer. A portion of sigmoid colon (about 15 to 20 cm) was isolated and anastomosed to the urethral stump. Then the ureters were implanted in the sigmoid colon via submucosal tunnels. There were 20 men and 1 woman, ranging in age from 24 to 71 years (average 51 years). Preoperative investigations showed that all the cases were free of metastasis. Five years survival rate was 60.7% and there was no operative mortality. There was urethral recurrence in one case and in that case postoperative histopathology revealed carcinoma in situ with tumor. All patients were able to void by themselves without any difficulty. Five patients complained mild incontinence at daytime and all patients had mild incontinence during deep sleep at night, but all of them could maintain their normal daily life like before. The major postoperative complications were: leak at the site of anastomosis (between sigmoid colon and urethra) in 6 cases, hydronephrosis in 2 cases, VUR in 2 cases and bladder stone in 1 case. The patients who underwent bladder replacement with sigmoid colon were well satisfied, because there were no external stoma and the patients could void by themselves. From now, we want to improve our operative method in order to avoid postoperative incontinence.

Malignant lymphoma of the testis presenting as primary testicular tumor.

A case of malignant lymphoma of the testis is presented. A 49-year-old male presented with swelling and dull pain of his left scrotal content. The initial clinical diagnosis was left testicular tumor. Left high orchiectomy was performed immediately after admission and histopathology showed malignant lymphoma, large cell type, B-cell type according to the Lymphoma Study Group (LSG) classification. Extensive investigations revealed clinical stage lE, with no evidence of metastasis. Chemotherapy was initiated with vincristine, prednisolone, adriamycin and methotrexate (VEPA-M). After therapy, the patient is doing well without any evidence of metastasis.

Non-specific granulomatous prostatitis treated with steroids.

A case of non-specific granulomatous prostatitis is reported. The patient, who had a history of sudden onset of high fever and acute urinary retention, had a hard prostate on digital rectal examination that gave us an impression of prostatic cancer. Since repeated biopsy specimens from the prostate showed granuloma formation with fibrinoid necrosis, the case was diagnosed as non-specific granulomatous prostatitis. Steroid therapy promptly resolved clinical symptoms along with marked histopathologic improvement.

Expression of ras gene family as result of compensatory renal growth in mice.

We have examined several gene expressions during the process of compensatory renal growth in mice following unilateral nephrectomy. On the 3rd, 5th and 7th days after operation in young mice (age: 5w), unilateral nephrectomy induced weight gain of the remaining kidney, but not in adults (age: 15w). It also induced maximum expression of c-H-ras and c-K-ras on the 3rd day in both young and adult mice, but there was no increase in N-ras in either. The expression levels of c-H-ras and c-K-ras were higher in young mice than in adults. However no expression of c-myc was detected at any point. Expression of metallothionein-I (MT-I) gene was detected during the first 12 h after unilateral nephrectomy both in the liver and the contralateral kidney. These data suggest that c-H-ras and c-K-ras gene expressions are in some way related to compensatory renal growth in mice and may be strongly related to hyperplasia in the contralateral kidney.

Transforming growth factor beta-like activity in human hydrocele fluid.

In this study transforming growth factor beta (TGF-beta)-like activity in human hydrocele fluid was investigated. Inhibition of DNA synthesis of adult rat hepatocytes in primary culture and stimulation of colony formation of normal rat kidney (NRK) fibroblasts, clone 49F in soft agar were observed in all acidified hydrocele fluids and these activities were neutralized by the specific antibody raised against human native TGF-beta. In samples obtained from recurrent cases of hydrocele, TGF beta-like activity was observed in its active form (without acidification). These results suggest that human hydrocele fluid contains TGF beta-like activity and that the active form of TGF-beta in recurrent hydrocele fluid may be responsible for the recurrence of the disease even after repeated aspiration.

The effect of transforming growth factor-beta on the alkaline phosphatase activity in rabbit renal cortical tubular cell cultures.

The effect of various growth regulators including epidermal growth factor and transforming growth factor-beta on the alkaline phosphatase activity of rabbit renal cortical tubular cells has been investigated in a serum-free culture. As a result, it was found that transforming growth factor-beta, known to be a growth inhibitor of renal tubular cells, increased the alkaline phosphatase activity of the tubular cells dose-dependently and that cycloheximide blocked any increase in the activity of this factor. In contrast, epidermal growth factor decreased the alkaline phosphatase activity in the tubular cells.

A study of growth regulators of renal cortical tubular cells in the rabbit liver.

Two growth regulators, a growth stimulator for cultured renal tubular cells and a growth inhibitor, were observed in this study in rabbit liver homogenate after a unilateral nephrectomy. These regulators appeared in the liver on the third day after this nephrectomy, decreased on the seventh day, and disappeared by the fourteenth day. The growth stimulatory activity, termed a tubular cell growth factor (TuCGF), was a heat- and acid stable 15 to 20 KDa protein. The additive effects of TuCGF on the epidermal growth factor (EGF), the insulin-like growth factor-I (IGF-I), and the fibroblast growth factor (FGF) were observed; the results suggest that TuCGF differs from EGF, IGF-I, or FGF. In contrast, the growth inhibitory activity, termed a tubular cell growth inhibitor (TuCGI), was a heat- and acid-labile protein with a molecular weight of about 150 to 200 KDa. This factor potently inhibited the DNA synthesis of tubular cells in the presence of insulin and EGF. These results suggest that TuCGI is not identical with transforming growth factor-beta (TGF-beta). The regulation of these activities by target cell density is also discussed.

[Measurement of blood flow in human tympanic membrane with spectrophotometry and laser speckle flow meter].

The purpose of this study is to evaluate the blood volume and blood flow in human tympanic membrane. Hemoglobin concentration (IHb) and oxygen saturation of hemoglobin (ISO2) were measured in 71 ears by using a tissue spectrum analyzer. Blood flow (Flow level) was measured in 40 ears by using a laser speckle flow meter. By using two fiber optic probes, these three parameters were measured in the tense part of the tympanic membrane. A comparison was made between the measurements made with fiber probe touching the tympanic membrane and the measurements not touching it. Between two measuring patterns, there was no significant differentiation in three parameters, so we took non-touching technique. IHb and Flow level had reproducibility between the first and second recordings. But ISO2 did not have clear reproducibility. In normal tympanic membrane, IHb and Flow level were not significantly correlated (r = 0.52). IHb was 16 +/- 1.3 (Mean +/- SEM) in normal cases, 17 +/- 4.6 in otitis media with effusion and 104 +/- 22.8 in acute otitis media. Flow level was 0.7 +/- 0.07 in normal cases, 0.6 +/- 0.07 in otitis media with effusion and 2.2 +/- 0.37 in acute otitis media. It is considered that the measurement of hemoglobin concentration by using tissue spectrum analyzer and the measurement of blood flow by using laser speckle flow meter are useful in the studies of the pathophysiology of human tympanic membrane.

[Measurement of human laryngeal mucosal blood volume].

To the best of our knowledge little if any information is available concerning human laryngeal mucosal blood volume presumably because the larynx is an anatomical position which renders the measurement of blood flow there difficult. With the advent of a reflectance spectrophotometer, however, studies of tissue blood flow have been facilitated to a great extent. As a result, attempts have been made to determine mucosal blood volume in various organs and tissues including the gastric mucosa. Recently, an ultra-thin fiber probe (1.8 m min diameter) for measurement of tissue blood volume has been developed. Using a laryngeal flexible fiberscope attached with this newly developed probe in the opening for the forceps, an attempt was made to measure laryngeal mucosal blood volume by the method of reflectance spectrophotometry, determining IHb and ISO2 the former of which is a parameter of hemoglobin in the tissue blood vessels and the latter, a parameter of hemoglobin oxygen saturation in the tissue blood vessels. IHb of the vocal folds is significantly low, about half those of the false folds and arytenoid region, whereas ISO2 is significantly higher in the false folds and arytenoid region than in the vocal folds. The hemodynamics in the vocal folds as a high-speed vibrator are specific, unlike in other laryngeal regions. We have plans to determine laryngeal tissue blood volume at more sites so as to investigate the laryngeal tissue distribution of blood volume in greater detail. Furthermore, tissue blood volume in morbid conditions will also be determined for information which will be of value in clinical applications.

[A case of non-Cushingoid Cushing's syndrome].

A 42-year-old female was admitted to our hospital with a chief complaint of hypertension. Endocrinologically, the plasma cortisol level was increased, but its diurnal rhythm had disappeared and the plasma cortisol level was not suppressed by dexamethasone loading at 2 mg or 8 mg. The plasma ACTH level was low. Computerized tomographic scan, echography and adrenocortical scintigraphy showed an adrenocortical tumor on the right adrenal gland. Physical examination did not reveal typical Cushingoid symptoms such as moon face, central obesity and violaceous striae. Thus we diagnosed this case as non-Cushingoid Cushing's syndrome and performed right adrenalectomy. Histological examination showed adreno-cortical adenoma without malignancy.
