Peritubular endothelial cell proliferation in mice during compensatory renal growth after unilateral nephrectomy.

The proliferation of peritubular endothelial cells during compensatory renal growth (CRG) following unilateral nephrectomy in mice was investigated using a labeling index. The labeling index of peritubular endothelial cells increased 6 h after uninephrectomy and decreased to the normal level within 72 h. Immunohistochemical study revealed that c-myc protein was expressed in the nuclei of both cortical tubular cells and peritubular endothelial cells. Furthermore, intravenous injection of anti-basic fibroblast growth factor (bFGF) neutralizing antibody just after uninephrectomy led to significant inhibition of proliferation of peritubular endothelial cells, but not tubular cells. These results indicate that peritubular endothelial cells proliferate transiently during CRG and that bFGF plays an important role for the growth regulation of that cell in the kidney.

[Prognostic factors in primary lymphoma of the thyroid--a review of 74 cases].

Prognoses and prognostic factors in 74 patients with primary lymphoma of the thyroid gland were analyzed. From 1968 to 1991, 74 patients with non-Hodgkin's lymphoma originating in the thyroid gland were treated at Noguchi Thyroid Clinic Hospital. Patients consisted of 53 females and 21 males, with a median age of 64.5 years. Survival curves were calculated by the Kaplan-Meier method and significance was tested by generalized Wilcoxon test. The overall 5-year and 10-year survival rates were 81.8% and 62.5%, respectively. The survival rate in stage II disease was significantly (P = 0.004) lower than that in stage I. By the LSG classification, there was no significant difference in survival curves between follicular lymphoma and diffuse lymphoma. Using the Working Formulation, there was no significant difference in survival curves between cases with low and intermediate grade histology. Histologic subtype of the tumor did not appear to be a significant determinant of prognosis. Patients with vocal cord paralysis had poorer survival rates than others (P = 0.001). Extrathyroidal spread of the tumor by direct soft tissue invasion had a distinctly adverse effect on prognosis.

[Assessment of the quality of life of prostate cancer patients].

Assessing the QOL in cancer clinical trials is becoming increasingly important. However, a suitable device of assessment of QOL has not been developed yet for prostate cancer patients in Japan. We tried to assess the QOL of prostate cancer patients using the EORTC questionnaire translated in Japanese, and examined its validity and reliability. Thirty-six patients filled in a questionnaire. The reliability of this device was confirmed by the results of test-retest reproducibility. Good correlation was shown between the results and patients performance status, and between the results and clinical stages, which support the validity of the device. As for the results of assessment of QOL, these patients were severely damaged in sexuality. Those factors of functional status, physical symptoms and fatigue/malaise were closely related to disease activity and clinical stage.

[Two cases of ganglioneuroma discovered as "incidentaloma"].

Two cases of adrenal ganglioneuroma discovered as "incidentaloma" are presented Case 1 was in a 29-year-old female who had a right adrenal tumor incidentally found by abdominal ultrasonography. The tumor was 26 g and 5.1 x 3.6 x 3.0 cm in size. She underwent surgical exploration and histopathologic examination revealed adrenal ganglioneuroma. Case 2 was in a 75-year-old man who had a right adrenal tumor detected during examination of microhematuria due to right renal stone by abdominal computed tomography. The tumor was 51 g in weight and 5.3 x 5.0 x 5.2 cm in size. He underwent right adrenalectomy and histopathologic examination showed adrenal ganglioneuroma. Both patients had no abnormal endocrinological findings and the DNA histogram showed a diploid pattern.

Transforming growth factor-beta in rat kidney during compensatory renal growth.

Transforming growth factor-beta (TGF-beta) inhibits the DNA synthesis and stimulates the alkaline phosphatase activity of cultured renal cortical tubular cells. When unilateral nephrectomy was performed, TGF-beta activity content gradually decreased in the remaining kidney. An immunohistochemical study of the remaining kidney after uninephrectomy, however, showed that the expression of TGF-beta increased by the third day after uninephrectomy in the proximal tubular cells and an autoradiographic study revealed that TGF-beta expressing tubular cells were different from proliferating cells during compensatory renal growth. These results indicate that the total renal TGF-beta content decreases after uninephrectomy, while the expression of TGF-beta in the proximal tubular cells may increase and act as a brake for compensatory renal hyperplasia.

Anti-epidermal growth factor antibody inhibits compensatory renal hyperplasia but not hypertrophy after unilateral nephrectomy in mice.

The effect of anti-epidermal growth factor (EGF) antibody on the compensatory renal growth including hyperplasia and hypertrophy was investigated. The antibody used in this study blocked the stimulatory effect of EGF on the DNA synthesis of cultured rat hepatocytes. When this antibody was injected into mice intravenously after unilateral nephrectomy, the labeling index of the renal cortical tubular cells decreased significantly at the second day after uninephrectomy, but the antibody caused no decrease in remaining kidney weight. Immunohistochemical study revealed that injected anti-mouse EGF rabbit IgG was positively stained at the renal cortical tubular cells. EGF would thus appear importantly essential to compensatory renal hyperplasia.

Secretion of autocrine growth-promoting activity by renal-carcinoma cells treated with 5-fluorouracil.

A study was made of the auto-proliferative activity of human renal-carcinoma cells in the supernatant from a carcinoma-cell culture in serum-free medium to which an anticancer agent had been added (5-FU). The human renal-cancer cells used in this study were of 3 strains: ACHN, VMRC-RCW and NT. When each line was cultured in medium containing no 5-FU, the supernatant showed almost no activity for stimulating DNA synthesis. However, when the line was cultured in the presence of 5-FU, the supernatant showed autocrine growth-promoting activity which strongly stimulated DNA synthesis of 3 renal-cancer cell lines in a dose-dependent manner. Activity could be detected in a 4- to 6-kDa fraction by gel filtration. This fraction increased the DNA-synthesis-promoting activity of epidermal growth factor, transforming growth factor-beta, basic fibroblast growth factor and insulin, and was acid- and heat-stable. It was also stable against pepsin and dithiothreitol. DNA synthesis in BALB/c 3T3, adult rat hepatocytes and rabbit renal tubular cells was not affected by this fraction which was thus considered not to affect non-cancerous cells. Renal-cell carcinoma responds poorly to anticancer agents, and the autocrine activity of the fraction may possibly be a factor accounting for this resistance.

Significance of a new stratification of alkaline phosphatase and extent of disease in patients with prostate carcinoma with bone metastasis.

Bone is the most frequent site of metastatic prostate cancer and the prognosis of patients with bone metastasis is poor. The authors have investigated a semiquantitative system to evaluate bone metastatic lesions in terms of cancer-specific survival. Based on the extension of disease (EOD) grade proposed by Soloway and associates, a new EOD grading system obtained from bone scintigraphy alone and EOD score obtained from bone scintigraphy and alkaline phosphatase was studied in 164 patients with prostate cancer with metastatic bone involvement. In terms of a cancer-specific survival and prostate cancer death, both the new EOD grade and the EOD score were apparently superior to eight other items studied (age, medical score, gait disturbance, histologic grade, erythrocyte sedimentation rate, prostatic acid phosphatase, and alkaline phosphatase). Multivariate analysis revealed that the EOD score was better than the new EOD grade. This improvement was due to the elimination of false-positive or nonactive metastatic bone lesions on bone scintigraphy through the alkaline phosphatase evaluation.

Gene expression and immunohistochemical localization of basic fibroblast growth factor in renal cell carcinoma.

Renal cell carcinoma is known as a neoplastic condition of renal tubular cells and usually shows a hypervascular tumor in angiographic examination. We examined the presence of basic fibroblast growth factor (bFGF) in human renal cell carcinoma. To determine if alterations in bFGF gene expression are present in human renal cell carcinoma, paired samples of normal and neoplastic renal tissue from 6 patients were analyzed for bFGF mRNA content by Northern blot hybridization. In 4 out of 6 patients, tumor tissue expressed bFGF mRNA 2 to 4 times greater than corresponding normal tissue. Two patients showed minimal elevation of tumor bFGF mRNA. The localization of bFGF in the renal cell carcinoma tissue was also examined using immunohistochemical staining, and it was found that bFGF was positively stained at the nuclei of tumor cells and the cell surface. These results suggest that increased expression of bFGF may be associated with neoplastic growth in renal tubular epithelial cells and neovascularization.

Analysis of survival of prostate cancer patients in Japan and the U.S.A.

The prognosis of U.S.A. patients with prostate cancer based on reports from the American College of Surgeons and Roswell Park Cancer Institute was compared with that of Japanese in Nagasaki and Gunma. There was more than a 10% difference in survival rate between the two countries. Several clinical parameters were analyzed to determine the basis for this difference. In general, stage distribution, metastatic sites, bone pain and alkaline phosphatase in the USA patients with Stage D disease were more severe than in the Japanese patients. Moreover the prostate cancer death rate in the USA was higher than that in Japan. However, only the distribution of pathological grade in the USA was better than that in Japan. From these results, it is difficult to conclude that prostate cancer in the USA is more aggressive than in Japan. The possibility of selection bias in these studies is discussed.

Transforming growth factor-beta-like activity is secreted by rabbit renal cortical tubular cells in primary culture.

The activity of an autocrine growth factor in a medium conditioned by cultured rabbit renal cortical tubular cells was investigated. Little stimulatory growth activity for tubular cells was observed in the conditioned medium, and inhibitory activity was seen only in acidified conditioned medium. This factor stimulated the colony formation of NRK 49F cells in soft agar only with epidermal growth factor and inhibited the DNA synthesis of primary cultured rat hepatocytes, and its molecular weight was about 25 kDa. The factor was neutralized by the specific antibody to transforming growth factor (TGF)-beta 1. These results indicate that renal tubular epithelial cells can produce latent TGF-beta in primary culture.

Novel complications with HTLV-1-associated myelopathy/tropical spastic paraparesis: interstitial cystitis and persistent prostatitis.

Lower urinary symptoms associated with HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) are common, but have been regarded as 'neurogenic' due to spinal involvements. However, in some cases, these symptoms are persistent, progressive, and not directly correlated with the severity of other neurologic symptoms of the lower spinal cord. These findings prompted us to locate organic lesions in the lower urinary tract and to correlate them with HTLV-1 infection. Among 35 HAM patients with lower urinary symptoms, we found 4 cases with the symptoms persistent and progressive: 3 with contracted bladder and another with persistent prostatitis. Histological or cytological examinations indicated local lymphocytic infiltrations in the lower urinary tract in all cases: 3 by the infiltration in the bladder and the other by a high concentration of lymphocytes in expressed prostatic secretions. Of 3 cases whose urinary samples were available, 2 showed significant increase in the concentration of urinary anti-HTLV-1 antibody of IgA class. The urinary IgA antibody of the third case was not elevated, but the sample had been obtained after resection of the affected bladder. None of the control cases showed significant anti-HTLV-1 IgA antibody in urine except for a case of gross hematuria due to chemotherapy directed against adult T-cell leukemia. We suggest inclusion of these processes into the spectrum of complications for HAM/TSP. The elevated excretion of anti-HTLV-1 of IgA class in urine may be an indicator of these complications.

[Usefulness of renal CT scan for analysis of nutcracker phenomenon].

Among 24 patients with confirmed left renal bleeding, 11 were diagnosed of having nutcracker phenomenon (NP) on the basis of the results of renal venography and pull-back pressure from the left renal vein to the inferior vena cava. Renal CT scan was performed for these 11 patients, and its usefulness as a means of diagnostic analysis was studied. The following results were obtained. 1. The left kidney was scanned at 5-mm intervals. The mean distance between the anterior surface of the abdominal aorta (Ao) and the posterior surface of the superior mesenteric artery (SMA) was 4.5 +/- 0.6 mm in the NP group, while it was 13.9 +/- 6.4 mm in the control group (p < 0.01). 2. Regarding the morphology of the left renal vein (LRV), tapering dilatation, i.e., funnel-like dilatation, from the left side of Ao was seen in the NP group. In the control group, the left renal vein showed a club-like shape continuing to the inferior vena cava and showed no dilatation. 3. Vascular structures indicative of collateral vessels were observed around the LRV in the NP group. If renal CT scan is performed at 5-mm intervals, the following CT finding are thought to be indicative of the nutcracker phenomenon; (1) a distance of 5 mm or less between Ao and SMA and (2) tapered dilatation of the LRV. This study suggests that renal CT scan is useful for diagnostic analysis of the nutcracker phenomenon.

[Percutaneous Bacillus Calmette-Guerin perfusion of the upper urinary tract for carcinoma in situ].

We performed percutaneous perfusion of the upper urinary tract with Bacillus Calmette-Guerin (BCG) in 3 patients. Two of them had undergone unilateral nephrectoureterectomy for ureter carcinoma in situ and one had undergone radical cystectomy with bilateral ureterocutaneostomy for invasive bladder carcinoma. However, they suffered recurrent upper urinary tract carcinoma in situ within 2 years after their operation. Under ultrasound control a percutaneous nephrostomy tube was placed in the patient. Before BCG perfusion unobstructed flow from the renal pelvis to the bladder was confirmed and pyelovenous or pyelolymphatic back flow was excluded under fluoroscopy. A dose of 240 mg BCG was dissolved in 150 ml 0.9% saline. The flask was placed 20 cm above the kidney of the resting patient. A continuous flow of approximately 1 ml per minute was maintained. The perfusion was stopped after 2 hours and the nephrostomy tube was closed. Therapy was repeated at weekly intervals for a total of 6 perfusions (1 treatment course). In each of them urine cytology results became negative after 1 treatment course. No severe side effects were observed. Further investigation is also needed to determine whether BCG perfusion of the upper urinary tract could become a conservative treatment for carcinoma in situ of the upper urinary tract.

[A case of prostatic carcinoma presenting as a metastatic orbital tumor].

A 66-year-old man presented with progressive proptosis of the left eye associated with ocular pain. A computed tomographic scan showed a high density mass in the posterolateral portion of the left orbit. The patient underwent surgical removal of the tumor and histopathological examination revealed adenocarcinoma of unknown origin. To find out the primary focus of the tumor the patient was referred to our department, where biopsy of the prostate revealed adenocarcinoma. Further, immunohistochemical examination of the orbital tumor was performed and prostatic acid phosphatase was identified. Finally, we made a diagnosis of orbital metastasis from prostatic carcinoma. This paper presents a rare case of prostatic carcinoma with orbital metastasis and reviews the literature of the subject.

Effects of cytokines on the growth of cultured rabbit renal cortical tubular cells.

The effects of various cytokines including interleukin-1 beta, -2, and -6, tumor necrosis factor-alpha, and granulocyte-macrophage colony stimulating factor on the DNA synthesis of cultured rabbit renal cortical tubular cells were investigated. Among these cytokines, it was found that only interleukin-1 beta could stimulate the DNA synthesis of cultured renal tubular cells, while interleukin-6 mildly suppressed the DNA synthesis of such cells. Interleukin-1 beta and -6 are produced by mesangial cells. The above results suggest that these cytokines may regulate the growth of tubular cells via the urinary flow in the nephron.
