[Digital intravenous subtraction angiography in the diagnosis of renal hypertension].

Indirect aortographies conducted in 123 patients via intravenous digital subtraction angiography (DSA) enabled satisfactory assessment of the renal arteries in 93% of the cases. The present state of the art, therefore, permits us to claim that DSA is of satisfactory diagnostic value in screening diagnosis of renal hypertension. It is well suited for confirming or excluding renal arterial stenosis and does not place any additional burden on the patient over and above the strain already exercised by excretion urography.

[The occult fracture in the roentgen picture and its detection using bone scintigraphy].

A retrospective study was made of patients with initially negative radiographs and positive radionuclide bone images. Comparison of conventional radiography and bone scanning in traumatology shows that the masked bone fracture can be seen in the bone scan but a focal accumulation of the radioactive material may not correspond to a bone fracture in every case. The ligamentous avulsion of a bone chip and/or the periosteum can yield the same image. Usually bones respond to trauma with an abnormal focal tracer accumulation because of irregular metabolic processes and the disturbance of the local blood circulation. By using a highly sophisticated technique (3-phases bone scanning) the focal disturbances can be seen. The greatest problem in scanning is the nonspecificity of abnormal tracer accumulation. Although bone scanning is very important in the diagnosis of any traumatic lesion of the bone it cannot replace the conventional radiograph.

[Determination of free L-triiodothyronine (FT3) in serum. Comparative tests of 2 FT3 radioimmunoassays].

A trial of the routine use of two test procedures for the evaluation of free L-triiodothyronine (FT3) was carried out (1070 patients). The normal values given by the manufacturers are not correct with respect to our patient group. In order to avoid false hyperthyroid (FT3-Amerlex) or false hypothyroid (FT3-DPC) values, the normal spectrum was redetermined for each. Under these conditions, both tests could show a definite difference between normal and overfunctioning of the thyroid. In substitution therapy with L-thyroxine, the quality of adjustment may be assessed according to the FT3 value. An extension of the normal spectrum, as for FT4, is not necessary. Since any additional assessment of the carrier protein concentration becomes irrelevant, the FT3 test is advisable also for economic reasons.

[Demonstration of Wegener's granulomatosis in the mammogram].

Communication about two patients with Wegener's granulomatosis of the mamma. In both cases the rare disease was detected on the occasion of a mammographic examination. The results of the mammography could cause misinterpretations, e.g. it might be mistaken for an advanced mamma carcinoma. Up to now, the manifestation of a Wegener's disease of the female breast was neither mentioned in collective statistics of more than 324 cases nor it could be found in examining the publications of additional 231 cases. Both patients showed an unusual course of the disease with a slow progression as well as an unimportant affection of the respiratory tract. The regression of the tumor like lesions in the breast was remarkable.

[Gallstones as depicted in the sonogram and radiogram. A comparative study].

Eighty-seven unselected gall stones were examined radiologically and by ultrasound in vitro. No relationship of diagnostic value could be established between the amount of calcification and stone demonstration, the sonogram being influenced by the size of the stone, its surface contour and its position in the ultrasound beam. Sonography is therefore not suitable for selecting those patients on whom litholysis could be carried out. Sonography appears to be useful only in a negative way by establishing the exact size of the concretion.

[Circulatory and adverse effects of nonionic contrast media in arteriography. Results of a double-blind study].

The effect to the local toleration of different concentrations of new non-ionic contrast media (Iopamidol 300 and 370) has been examined by means of a randomized intraindividual double-blind study and by the use of uniform parameters of application. In all cases the pain and heat effect were tolerable for the non-anaesthesized patients. The application of the higher concentration of the contrast media did not strengthen or intensify these side effects, which were formerly connected with the arteriography of the extremities. In cases, where the circulation is heavily disturbed, the opacification of the peripheral branches of the leg arteries can be improved by the use of the higher concentration. The hemodynamic effect was low and disappeared after a few minutes.

[Pharmacologic effects of contrast media in angio-densitometry].

Angio-densitometry is a very sensitive method in the evaluation of vascular effects of a contrast medium. For that purpose the renal circulation is especially suitable. The method is based on the zero or subtraction method. Thus, the majority of the well known interference sources will not be recorded. Only changes of density caused by the passing blood are measured. Four different contrast media are compared. Their effects on the circulation cannot be predicted from a single measurement. The mean value of several measurements limits the scattering range and allows comparisons to be made. Accordingly there are some differences between the examined conventional contrast media and their influence upon the kidney circulation. They all slow down the blood velocity in the renal artery, but the extent is different. The causes responsible for the circulatory effects of the contrast media are discussed on the basis of current scientific knowledge. In addition the results of 144 angio-densitometric studies are reported. The analysis of the pharmacological effect of meglumine iothalamate (Conray 60) revealed only an unimportant influence upon the renal circulation.

[Skeletal scintigraphy as an addition to the roentgenological examination in traumatology (author's transl)].

Considering various indications a review was made of 141 scintigraphic examinations for skeletal trauma. It was shown in 122 patients that bone scanning is an important method to help conventional X-ray diagnosis but that it cannot replace it. The alteration of the local metabolic bone processes is reliably indicated; and the limits and possibilities of bone scintigraphy and its use in traumatology are determined. An acute bone fracture can be proven scintigraphically. Errors are possible by the accumulation of 99mTc-activity in the soft tissue or by a negative scan in an early phase. Useful results can also be obtained when the interval from medical evaluation to the time of accident had to be evaluated. There were similarly useful results in the evaluation of fracture healing. Both those groups of cases, however, showed a high percentage of questionable results which brought no further information than the conventional X-ray findings. This is due to the irregular metabolic process at the fracture site and by much scattering in the area. Bone scanning was less informative when loosened metal plates (for osteosynthesis, however, showed a high percentage of questionable results which brought no further information than the conventional X-ray findings. Bone scanning was less informative when loosened metal plates (for osteosynthesis) or secondary osteomyelitis close to the fracture or the question of a recurrent fracture had to be evaluated. These groups of cases showed a higher portion of questionable results and misinterpretation. Totally, 65.9% correct results of skeletal scintigraphy brought some additional information to the roentgenological examination and were important for the surgeon. However, there were 21.3% questionable results. In 12.8% the interpretation of the bone scan was not correct.

[Crohn's disease of the colon (author's transl)].

Despite the availability of endoscopy, radiological examination remains an essential method in the investigation of Crohn's disease. Various authors agree that it has an accuracy of 75% (1, 8, 14, 20). In our experience accuracy is somewhere between 70 and 85%. These figures depend on the fact that in about 16% histology is also unable to make a definite diagnosis. Crohn's disease of the large bowel is of significance since it is often misinterpreted as ulcerative colitis, because in 10% of cases there is no involvement of the terminal ileum. The final diagnosis depends on the summation of the individual features whose individual significance must be considered. Malignant involvement of the peritoneum, which has not been extensively described in the radiological literature, may result in changes in the wall of the colon and lead to an appearance resembling Crohn's disease, with consequent errors in diagnosis.

[Angiographic diagnosis of haemoptysis with particular reference to angiomas of the bronchial arteries (author's transl)].

It is hardly ever assumed that pulmonary bleeding can be due to the presence of angioma racemosum arteriovenosum. This is not only because it is relatively rare, but also because it cannot be visualised in conventional x-ray films of the lung. It is also not accessible to more detailed and invasive lung diagnosis. Definite confirmation is only possible via selective bronchial arteriography, which is too rarely applied during diagnostic differentiation. The authors observed angioma racemosum arteriovenosum in seven out of 47 patients in whom angiography had been performed after single or repeated haemoptysis; in six of these patients, the angioma was removed by surgery. Bronchial arteriography not only enables visualisation of the characteristic vascular patterns, but also to differentiate this disease against other pulmonary disorders which may also be responsible for the occurrence of haemoptysis. Furthermore, the angiogram enables exact determination of the extension and localisation of the angioma, this being an important prerequisite of successful thoracosurgery.

Determination of renal circulation by means of angiocinedensitometry.

This new X-ray method is based on the densitometric measurement of the speeds and the flow of injected contrast media during an angiographic examination of the vessels and the parenchyma of the kidney. Pairs of highly sensitive photoconductive cells and a Wheatstone bridge circuit are used. Minimal doses of contrast media can be detected before vascular reaction influences the renal circulation. The measurement of various parameters allows the simultaneous hemodynamic analysis of kidney function and of factors related to the morphological substrate.

[Nuclear medical techniques in diagnostics of urological diseases (author's transl)].

We report on the evaluation of 2170 kidney scintigrams and isotope nephrograms under the aspect of a urological radiodiagnostic of the kidney. The possibilities of a nuclear medical demonstration for kidney tumours are discussed. Furthermore we refer to the importance of the isotope nephrogram to recognize intermitting obstructions of the ureters.

[Angiomas and angioma-like changes of the bronchial arteries (author's transl)].

Hyperplastic changes of the bronchial arteries may take the form of a racemose arteriovenous angioma or result from chronic bronchitis. Clinically, recurrent haemoptysis is a striking feature occurring in otherwise fit patients. The two forms can be distinguished by bronchial arteriography because of the difference in the appearance of the vessels. In the one condition there are circumscribed areas of arterial and venous, vascular abnormalities, in the other there is vascular hyperplasia, which is less clearly circumscribed. In both forms, broncho-pulmonary, i.e. arterio-venous anastomoses can be demonstrated; these may be considered as the cause of the haemoptyses.
