[Family planning-the pill or natural method of birth control (author's transl)].

Observations made at the Family Planning Clinic and a review of the literature show that there is no increased tendency towards the natural contraceptive method among advice-seeking couples. On the contrary-apart from a few exceptions-the most effective method is always chosen. The natural method presupposes a high degree of motivation, of readiness to communicate and of clarification. These prerequisites underly the relatively high rate of failure because not all couples have the right degree of understanding. Nevertheless, to many people this method is the only acceptable and morally justifiable one. Thus, precise and detailed instructions and frequent check-ups would be necessary especially during the first few months, until adequate experience is gained.

[The importance of second-look surgery following radio-chemotherapy for advanced ovarian cancer (author's transl)].

Fifteen patients with advanced, initially inoperable ovarian cancer (stages III and IV) were subjected to second-look surgery between May 1976 and August 1979 following combined radio-chemotherapy. Each case had shown a distinct reduction in tumour size and a demarcation of the foreign tissue, which is considered to be prerequisite for second-look surgery. The diagnostic goal of the operation was to verify the success of the preceding radio-chemotherapeutic treatment by means of exact determination of the extent of the residual tumour. The subsequent therapeutic regimen can, if necessary, be modified accordingly. The therapeutic goal is complete surgical removal of the tumour, if at all possible, since the success of subsequent chemotherapy is dependent on the achievement of this aim. The mean interval between initial and second-look surgery was three and a half months. Tumour tissue of varying extent was found in 14 out of 15 cases. In 12 cases total extirpation of the internal genitals was successfully carried out, tumour resection was performed twice, and only one case proved inoperable. The case in question was one of third-look surgery, when after two years of radio-chemotherapy and secondary removal of the internal genitals abdominal tumour recurrence occurred. Two years and six months after initial diagnosis this patient is now undergoing further chemotherapeutic treatment. We believe that this very case illustrates the importance of second-look surgery and of consistent continuation with radio-chemotherapy in cases of advanced inoperable ovarian cancer.

HPL and estriol serum levels of placental perfusion as indexes of placental function in EPH gestosis and placental insufficiency.

62 patients with EPH gestosis and 64 subjects suffering from placental insufficiency were monitored with estimation of placental perfusion and determination of radioimmunoassayable serum levels of human placental lactogen (HPL) and estriol. The utero-placental blood flow was measured by quick intravenous administration of 0.25-1.0 mCi of (113m)In, and the radioactivity uptake curve was evaluated by a computer program. Good correlation between perfusion type of serum levels of HPL and estriol was recorded. The present data combine to suggest that the recording of utero-placental blood flow is valuable aid in the diagnostic and prognostic evaluation of high-risk pregnancies.

[The effect of antibiotic prophylaxis in vaginal obstetric procedures (author's transl)].

202 cases of vaginal obstetric procedures (forceps and vacuum extraction, breech delivery and manual removal) were reviewed with regard to assessing the value of prophylactic administration of antibiotics. Criteria for the evaluation were fever post partum, duration of stay in hospital and involution of the uterus. The results showed that the prophylactic administration of antibiotics had no significant effect on these parameters, with or without the simultaneous administration of oxytocics. Hence, in view of the risk of allergic reactions, toxicity and the selection of resistant strains the prophylactic administration of antibiotics does not seem to be justified in vaginal obstetric procedures. Scrupulous aseptic precautions during all vaginal or intrauterine procedures are considered essential for the prevention of infection.

[Angiographic diagnosis of ovarian cancer (author's transl)].

The methods of examination which are usually used in cases of tumours of the lower abdomen are mostly not able to determine the site of origin of the lesion of its histology unequivocally. Angiographic examination techniques enable the definition of such tumours and very often allow the differentiation between benign and malignant lesions. Both hypogastric arteries are selectively or semiselectively catheterized using Seldinger's technique. Additionally, in most patients, aortography and selective catheterization of the inferior mesenteric artery is performed. So far the diagnosis of an adnexal tumour was made by angiography in 6 out of 30 patients. The typical angiographic criteria of ovarian carcinoma are cited and discussed. Our results show that in selected cases of suspected ovarian tumours angiography offers good results and, therefore, it deserves an important place among the different available clinical examination techniques.

[Clinical significance of arteriovenous shunt malformations in the uterine and adnexal region (author's transl)].

This case report describes and discusses the entity of arteriovenous shunt malformations in the uterine and adnexal region. The diagnostic measurements and clinical importance of this disease is also outlined. The condition is rare, but should be considered in cases of severe hemorrhage when routine investigations have been normal. For the first time bloodgas-analytic measurements showing the extent of the shunting have been performed with such a disease.

[pH measurements following delivery from breech presentation (author's transl)].

In 1977 the adaptation capability of 22 out of 89 breech presentation new-borns was studied by way of punctual pH-measurements within the first 30 minutes after birth. Thus, it was possible to gain an insight into the extra-uterine adaptation, especially with respect to the mode of delivery. Further points of comparison were parity, maturity and a possible correlation with the Apgar Score. The study included a comparison with identical measurements of 20 spontaneous head position new-borns. According to the results under certain preconditions a vaginal delivery in breech presentation is also today by all means to be justified.

[The influence of ritodrine-hcl (prepar), bunitrolol (stresson) and oxytocin (syntocinon) on Uteroplacental perfusion (author's transl)].

The administration of betamimetics has proved to be useful for symptom-oriented treatment in patients with reduced utero-placental flow. In order to quantify the betamimetic effect flow measurements were carried out in a total of 12 sheep, yielding the following experimental results: 1) Betamimetics, up to a certain point which correlates with maternal heart rate, raise the uteroplacental flow-rate. 2) The use of cardio-selective betablocker prevents undesired betamimetic side-effects but there is also a loss of the rise in utero-placental flow rate. 3) In sheep pretreated with Oxytoxin the application of betamimetics not only offsets oxytoxin-action but also greatly improves utero-placental blood flow.

[Serum oestriol: a parameter of the function of the feto-placental unit (author's transl)].

The determination of serum oestriol is known to be a useful parameter of the function of the feto-placental unit. In order to derive the normal range on application of the radioimmunoassay technique, the serum oestriol level was determined in 242 gravidae during the second half of pregnancy. The pregnancies were all uncomplicated and ended with the delivery of a healthy baby at term. The statistical mean and 4 confidence intervals were calculated for each gestational week individually. During the second half of pregnancy there was a continuous gradual increase in serum oestriol; between the 34th and 38th gestational week there was a further steep increase, followed by a decrease from the 39th gestational week onwards. The usefulness of this method is illustrated on the basis of case reports of 10 high-risk pregnancies in which oestriol determination is compared with the clinical findings and HPL determination.

[Localization of the placenta by means of nuclear medical methods].

Accurate localization of the placenta in the cavity of the uterus is indicated prior to transabdominal amniocentesis and in cases where hemorrhages occur during pregnancy. Several nucleo-medical techniques are available for this purpose. Earlier a so-called placentography was performed, a method later replaced by placental scintigraphy. Today the gamma scintillation camera is ideal for localizing the seat of the placenta. In case of a normally embedded placenta, the profile view makes it possible to determine whether the placenta is attached to the anterior or posterior wall. In case of an anterior wall placenta it is possible with a radioactively labelled marker to draw the outline of the placenta on the abdomen of the patient so that the examiner is certain to puncture in an area that does not contain the placenta. Diagnosis of placenta praevia is assured in all cases by use of the gamma scintillation camera. Short-term introduction of an emitter into the posterior fornix vaginae makes it possible to determine the position of the internal orifice of the cervix uteri. If the caudal part of the placenta extends up to this mark, a placenta praevia is likely and all obstetrical precautions indicated have to be taken.

[Differential diagnosis of tumors in the pelvis of gynecologic patients by angiography (author's transl)].

The value of angiography in differential diagnosis of tumours of the pelvis is discussed based on 15 cases and the relevant literature. Inflammatory infiltrates and neoplasms arising from the sigmoid colon can be separated from gynaecologic tumours by angiography. In 4 cases with pelvic masses in which differentiation between a myomatous uterus and an ovarian tumour was not possible clinically, unequivocal evidence was cleaned from a selective injection into the internal iliac artery. Angiography is particularly well suited for the delineation of parietal pelvic tumours--especially of vascular malformation.

[The influence of the continuous epidural anaesthesia on uteroplacental blood flow (author's transl)].

Continuous lumbar epidural anaesthesia causes a drop in maternal blood pressure by decreasing flow resistance in the peripheral vascular bed. By infusion of 1000 ml NaCl blood pressure can be kept constant. Our work shows, that dilatation of the peripheral vessels has a positive effect in patients with reduced uteroplacental flow, as it results in an augmentation of placental flow. In women with normal placental function such an effect could not be seen. The practical use of these results is illustraded by a case report.
