[Multicentric early cancer of the stomach (author's transl)].

In more than 16000 gastroscopies we found 65 (0.4%) cases of early cancer (e.c.), that is an amount of 12.3% of the total diagnosed cancers of the stomach. In 8 patients we found two or three sites of involvement in the stomach (12.3%). The control reading of 53 gastrocamerafilms with e.c. shows, that 20% of suspicious areas have been missed by using the routine interpretation and gastroscopy. It is proposed, that before each stomach surgery for e.c., the upper gastric region be investigated carefully for missed second lesions. This is rather difficult with prograde optic gastroscops. The instrument of choice for that purpose is the gastrocamera.

Seizures and intraventricular conduction defect in propranolol poisoning. A report of two cases.

We have recently seen two cases of propranolol poisoning. In both instances, plasma propranolol levels were elevated. Both patients experienced generalized seizures and had a transient intraventricular conduction defect on electrocardiogram. The literature on intoxication of propranolol and other beta-adrenoceptor blocking drugs suggests that the effects of massive propranolol intoxication observed in our patients can be explained on the basis of the pharmacologic properties of propranolol. Therapy consisted of gastric lavage and agents to antagonize the beta-adrenoceptor blockade effect. Both patients recovered.

Emphysematous cholecystitis.

The radiographic spectrum of emphysematous cholecystitis can be divided into three stages. Diagnosis on plain abdominal film is difficult in the first stage. Two cases are presented in which a circular gas filled structure in the right upper quadrant led to a correct diagnosis in the earliest stage. This was confirmed by obtaining contrast studies to rule out other etiologies.

[Contrast media and renal function in multiple myeloma (author's transl)].

An acute deterioration of the renal function after injection of (diiodized) renal contrast media in patients with multiple myeloma was pointed out for the first time by Bartels et al. (1954). Since then only a few similar cases have been reported. Therefore in a retrospective study the records of 89 patients with multiple myeloma from 1959 to 1974 were evaluated. For 31 of these patients a total of 41 i.v. pyelographs was made. Corresponding to a previous study by Vix in none of our patients who underwent pyelography a reduction of renal function was observed in direct association with the administration of contrast media. We are in agreement with other authors that a recognized myeloma is not from the first a contra-indication for the application of contrast material containing iodine so as a dehydration of the patients can be carefully avoided.

Amyloidosis secondary to drug abuse and chronic skin suppuration.

Amyloidosis has not been included in the wide spectrum of medical complications associated with drug abuse. Chronic visceral infection is a recognized cause of amyloidosis, but chronic suppuration of the skin is not a well-appreciated cause of this condition. Furthermore, to our knowledge, chronic skin suppuration secondary to parenteral drug abuse has not been reported as a cause for systemic amyloidosis. We report a case in which subcutaneous injections of narcotic tablets led to chronic skin suppuration and systemic amyloidosis.
