The effect of heparin-free dialysis on abnormal lipid metabolism in patients on regular dialysis treatment.

To elucidate the contributory role of heparin to abnormal lipid metabolism in patients on regular haemodialysis (RDT), six week haemodialysis without heparin was performed using gabexate mesilate in combination with low dose aspirin. Total cholesterol and beta-lipoprotein were significantly increased at the fourth week after the start of the study. No significant changes were observed in high density lipoprotein cholesterol, phospholipid, triglyceride, lipoprotein phenotype and post heparin lipolytic activity. Routine dose heparin in RDT might have a favourable effect on abnormal lipid metabolism by inducing post heparin lipolytic activity in circulating blood, not by reducing the lipid removing activity in peripheral tissues.

Deterioration of renal function in non-dialytic uraemics caused by 1 alpha (OH)D3 which was not attributable to hypercalcaemia.

Ten patients with renal failure not on dialysis were subjected to our study to evaluate the effect of 1 alpha (OH)D3 on calcium metabolism and its influence on renal function. The results suggest the possibility that 1 alpha (OH)D3 might be effective in bone disease due to secondary hyperparathyroidism. On the other hand, 1 alpha (OH)D3 depressed creatinine clearance without any increase of Ca, Ca x P product or PRA. 1 alpha (OH)D3 is to be prescribed to patients not on dialysis only with careful attention to the dosage and the duration, and frequent observation of renal function is necessary.

Clinical use of tobramycin in patients with surgical infections due to gram-negative bacilli.

Twenty-three patients with acute peritonitis, intraabdominal abscess, wound infections (including infection of serious burns), sepsis accompanying renal transplantation, or urinary tract infection associated with cancer were treated with various dosage regimens of tobramycin, and their clinical responses were analysed. For the conditions studied, the optimal regimen was 3 mg/kg per day in three divided doses. No clinical effectiveness was noted in this study for doses of greater than or equal to 4 mg/kg per day.

[Revaluation of gentamicin in laboratory and clinical studies (author's transl)].

The MIC of gentamicin and other antibiotics against clinical isolates was determined, and the following sensitivity distribution was obtained. a) Against 54 strains of Staphylococcus aureus, GM had the lowest MIC, followed by TOB, DKB, BB-K8 in order. The distribution range of 0.1-6.25 mug/ml was noted with GM. The peak of MIC was 1.56 mug/ml. b) Against 54 strains of E. coli, the lowest MIC was obtained with GM, and 53 strains (98%) were in the range of 0.4-6.25 mug/ml. The peak of MIC was 3.12 mug/ml. GM was followed by TOB, CL, DKB in order of superiority. c) Against 27 strains of Klebsiella pneumoniae, GM and TOB had the lowest MIC, showing the peak of MIC of 0.8 mug/ml. No significance difference was noted between GM and TOB. GM and TOB were followed by DKB, CL, BB-K8 in order of superiority. d) Against 27 strains of Proteus, GM had the lowest MIC, followed by TOB, DKB and BB-K8 in order. But the peak was 6.25 mug/ml even in GM. The activity of antibiotics against Klebsiella pneumoniae was generally low. e) Against 54 strains of Pseudomonas, the peak of MIC of GM was 3.12 mug/ml and 49 strains (91%) were in the range of 1.56-12.5 mug/ml. Whereas TOB had the peak value of 0.8 mug/ml and 49 strains (91%) were in the range of 0.4-3.12 mug/ml. TOB was followed by DKB, GM, BB-K8 in order of superiority.

Studies on the pathogenesis of rheumatic heart disease. An immunological relationship between the polysaccharide of group A streptococcus and the glycoprotein of heart valve.

The immunization of rabbits with bovine heart valve homogenates induced the formation of antibody which reacted with CTC soluble extracts and also proteolytic enzyme treated fractions. Analysis by fractionation of materials revealed 2 peaks and both peaks contain monosaccharides. The first peak was more antigenic to anti-valvular sera. The cross-reactivity between the valvular glycoprotein and group A streptococcal polysaccharides was confirmed by the immunological inhibition tests. Widespread deposits of gamma globin were observed in valve of patients with recurrent rheumatic valvulitis who has valve replacement. This work was performed under scientific-grants-in-aid from the Ministry of Education. Portions of this study were presented at the 38th annual meeting of the Japanese Circulation Society, Yamaguchi, April, 1974.

An epidemiological study of laryngeal cancer in Japan.

This report presents the findings of an epidemiological study of laryngeal cancer in Japan, based on 6,360 cases treated and registered during the 10-year period from 1960 through 1969. The number of the patients increased 1.5 times from 1960 to 1969. Geographically the absolute number of recorded cases was proportional to the population density; in other words, it was larger in heavily urbanized areas and smaller in rural regions. An anatomical classification revealed that the incidence of supraglottic and glottic cancer was about equal (3,121 and 3,176 cases), and subglottic cancer was only 1 percent of the total. Histologically, 98.6 percent were of the squamous cell carcinoma and 1.4 percent were of the basal cell carcinoma, adenocarcinoma, transitional cell carcinoma, etc. As to the age and sex distribution, the majority of the patients were 50 to 70 years old, and those in their 60's were most numerous. The ratio between men and women was 9.6 to 1. 56.1 percent of the patients surveyed were found to have a blood relation with a history of cancer and 31 percent were engaged in occupations where they used their voices frequently. Smoking habits were found in 96 percent of the patients surveyed and as many as 52 percent came under category of heavy smokers. Cigarette smoking might be regarded as a significant factor in laryngeal cancer, while alcohol consumption was not as significant a causal factor as smoking. The living environment had no explicit relationship to laryngeal cancer in the above-mentioned period of years.
