Clinical and hemodynamic appearances of aortic regurgitation caused by serious aortic lesions (AR-Ao-group) seemed more deteriorated than those with isolated chronic aortic regurgitation (AR-group). Often postoperative course and operative outcome are smooth and satisfactory in the AR-Ao-group. To support this observation, serial changes of left ventricular (LV) function and LV sphericity were evaluated before and after operation in ten cases in the AR-Ao-group and compared with those in the AR-group. Before operation, there were no significant differences in LV function between the two groups. Postoperative improvement of LV sphericity and LV function were statistically better in the AR-Ao-group. The LV function reserve was considered sufficiently retained in the AR-Ao-group. Results with this study supported that clinical observation. Therefore, the total correction, at the same time of aortic incompetence and aortic lesions is recommended.
Two cases of carcinoma in a diverticulum of the bladder were experienced. The first case was of a 50-year-old male who presented in February, 1981, complaining of asymptomatic microhematuria. The excretory urogram revealed a diverticulum in the left lateral aspect of the bladder which was causing shift of the lower ureter to the median side. The cytology report of voided urine was class III. Diverticulectomy was performed and pathologic findings was a transitional cell carcinoma, grade 1, stage 0. The patient has been free of recurrence for the past 54 months. The second case was of a 67-year-old male with the chief complaint of pollakiuria. Non-papillary tumor in a diverticulum of the bladder was found by cystoscopy and computed tomography. Tumor biopsy and urinary diversion by ileal conduit were performed in the usual manner. The pathologic finding was transitional cell carcinoma of grade 11 malignancy. The patient died of intestinal obstruction on January, 19, 1984, about 15 months after the surgery. The 117 cases of carcinoma in a diverticulum of the bladder we found in the Japanese literature are reviewed briefly.
Effect of prednisolone on experimental allergic neuritis (EAN) in Lewis rats was studied. In order to exclude the influence of residual antigens, we evaluated the effect in EAN passively transferred with living lymphoid cells. Prednisolone, when administered after the passive transfer, prevented EAN both clinically and histologically, but when administered after the clinical onset, it had little influence on the course of the disease. Prednisolone was suspected to have suppressed the sensitized cells, but to have had no remarkable effect after the clinical onset.

It has been reported that the action of etoposide (VP-16) (14) as an antitumor agent is mediated through its interaction with DNA topoisomerase II which results in DNA breakage inside the cell. In order to understand the mechanism of action as well as structure-activity relationships of 14, several novel, synthetic and some naturally occurring analogues related to podophyllotoxin were examined for inhibition of the DNA topoisomerase II activity. Compound 2 exhibited enhanced activity and compound 5 slightly diminished activity relative to 14. A 4 beta-substituted ether at the C ring and O-demethylation at the E ring appear to enhance activity.

A clinicopathological study was undertaken in 15 cases of massive hepatic necrosis after shock. The GOT and GPT level exceeded 1000 units in 10 cases. The 15 cases consisted of 3 diagnosed as fulminant hepatitis clinically and 12 diagnosed as disseminated intravascular coagulation (DIC) or multiple systemic organ failure (MOF) from the unremarkableness of liver dysfunction. It was noteworthy that sepsis and surgery were closely associated with these lesions. The weight of the liver at autopsy ranged from 800 to 2,700 g. Liver necrosis was macroscopically characterized by clear demarcation of the necrotic areas sharply separated from the surrounding liver parenchyma, showing the appearance of so-called "map-like necrosis". Microscopically, the lesions in these subjects showed mainly the pattern of centrilobular necrosis. As observed in the burn shock case (case 12), the shock which provoked in different phases of time seemed to have repeated its attack. These liver necroses were considered to result from severe systemic circulatory disturbance or intrahepatic circulatory disturbance. The possibility is indicated that the generalized or univisceral Shwartzman reaction, and repeated and combined severe shock participated in the pathogenesis. Fibrin thrombi aggrevate tissue perfusion and accelerate anoxia. Heparin therapy seemed effective in these cases if administered at an appropriate time.
The distribution of thorium dioxide (Thorotrast) in the liver from a Thorotrast-administered autopsy case was examined stereographically by analytical scanning electron microscopy. Thorium particles were detected in the macrophages of the portal triad and hepatic sinusoid. These macrophages were irregularly shaped and tended to be aggregated. In the sinusoid, accumulation of the macrophages formed a thrombus-like structure. Furthermore, observed in the sinusoid were free Thorotrast particles that appeared to have been released into the sinusoid as a result of breakdown of the macrophages (Kupffer cells).
A 37-year-old G1-P1 was diagnosed by ultrasonography at 26 weeks of gestation as having an abnormally large placenta with hemangiomas and a fetus associated with exomphalos. Placental protein 5 levels were relatively high in placental protein levels in maternal serum. The infant, delivered by cesarean section at 34 weeks, had the typical clinical features associated with Beckwith-Wiedemann syndrome. The abnormally large placenta weighed 1,492 g, measured 25 X 25 X 5.1 cm, and featured multiple hemangiomas. Microscopic placental features included edematous villi, increased fibrin deposition, intervillous thrombi, and multiple angiomatous and cellular chorangiomas.
Serum antibodies to P2 protein, myelin basic protein (MBP) and galactocerebroside (GC) were examined to study the role of humoral factors in experimental allergic neuritis (EAN). Out of 19 outbred rabbits sensitized with peripheral nerve homogenate, 14 developed EAN and the remaining 5 rabbits did not develop the disease. Anti-P2 protein and anti-MBP antibodies were detected not only in the EAN rabbits but also in the nonresponders. On the other hand, anti-GC antibody was detected in 7 of 14 EAN rabbits but not in the nonresponders. This antibody appeared near the onset of the clinical signs. As 7 rabbits developed EAN in the absence of the antibody and the titer did not correlate with the clinical severity, it is unlikely that anti-GC antibody is the major factor in inducing EAN. However, this antibody, along with other factors, may play a role in EAN.

Clinicopathological features were examined in 150 hepatocellular carcinoma cases autopsied from 1977 to 1983 in the Philippines and were compared to those of cases reported in Japan. The characteristics of hepatocellular carcinoma in the Philippines were as follows. 1. The age of autopsied patients with hepatocellular carcinoma in the Philippines was approximately 10 years younger than in Japan. The Philippines cases included even 10- to 11-year-old patients. 2. The macroscopic type of hepatocellular carcinoma in the Philippines consisted predominantly of the massive type of Eggel's classification. On the other hand, an encapsulated tumor was found in only one case. 3. The histological characteristics of hepatocellular carcinoma in the Philippines were not especially well defined. Hyaline globules and fatty metamorphosis were found in 1 and 12, respectively, of 90 Philippine hepatocellular carcinoma cases. 4. The frequency of HBs antigen in hepatocellular carcinoma in the Philippines was 37%. This is almost equal to that in Japan.
