Computer simulation analysis of fracture dislocation of the proximal interphalangeal joint using the finite element method.

Stress is a proximal interphalangeal (PIP) joint model was analyzed by the two-dimensional and three-dimensional finite element methods (FEM) to study the onset mechanisms of the middle phalangeal base fracture. The structural shapes were obtained from sagittally sectioned specimens of the PIP joint for making FEM models. In those models, four different material properties were given corresponding to cortical bone, subchondral bone, cancellous bone and cartilage. Loading conditions were determined by estimating the amount and position of axial pressure added to the middle phalanx. A general finite element program (MARC) was used for computer simulation analysis. The results of the fracture experiments compared with the clinical manifestation of the fractures justify the applicability of the computer simulation models using FEM analysis. The stress distribution changed as the angle of the PIP joint changed. Concentrated stress was found on the volar side of the middle phalangeal base in the hyperextension position, and was found on the dorsal side in the flexion position. In the neutral position, the stress was found on both sides. Axial stress on the middle phalanx causes three different types of fractures (volar, dorsal and both) depending upon the angle of the PIP joint. These results demonstrate that the type of PIP joint fracture dislocation depends on the angle of the joint at the time of injury. The finite element method is one of the most useful methods for analyzing the onset mechanism of fractures.

Distribution of complement regulatory proteins, decay-accelerating factor, CD59/homologous restriction factor 20 and membrane cofactor protein in human colorectal adenoma and cancer.

To clarify the events related to complement-mediated immune responses in human colorectal cancers, we immunohistochemically examined the distribution of decay-accelerating factor (DAF), CD59/homologous restriction factor 20 (HRF20), membrane cofactor protein (MCP) and terminal complement complex (TCC) in human colorectal adenomas and cancers, and then compared the findings with their distribution in normal colonic mucosa. In the normal mucosa, TCC was not present on epithelial cells. Whereas DAF and CD59/HRF20 were present only occasionally on the apical surfaces of normal epithelial cells, MCP was diffusely distributed on the basolateral surfaces of most epithelial cells of the colon. These findings suggest that MCP has a primary role in the regulation of complement activation on these cells. In adenoma cells, the expression of both DAF and CD59/HRF20 was enhanced. In cancer cells, the expression of CD59/HRF20 and MCP was diminished, whereas DAF expression was markedly enhanced. Since DAF was frequently stained in the lumen of the cancer glands, it was suggested that DAF was released into the colonic lumen in patients with colorectal cancer.

Clinicopathological analysis of de Quervain's disease.

Excised extensor retinacula of the first compartment and tenosynovium from 35 patients (6 men and 29 women) with de Quervain's disease were examined by light and electron microscopy to investigate the pathogenic mechanism. The patients, aged from 22-78 years, averaging 50 years, comprised the study group. Two hundred and thirty-two specimens from cadavers of 95 men and 75 women were macroscopically examined as the control. In the study group, the extensor retinaculum and tenosynovium were macroscopically thickened, and were histologically classified into 4 groups based on presence or absence of septum, and the location of retinacular thickening. Morphologically, the thickening of the tenosynovium and retinaculum was due to fibrosis in every layer, although fibroses were seen mainly in the middle layer. The ratios of proliferation of fibroblasts, myxoid changes and/or hyaline degeneration, and vascular proliferation were varied between layers. Minimal round cell infiltration was found in the retinaculum as well as in the tenosynovium. The results also indicate that the Iwahara-Nozue test can be used to accurately predict relatively greater thickening of the retinaculum on the extensor pollicis brevis side. Based on clinicopathological analyses, it appears that de Quervain's disease is induced not only by extrinsic factors such as superficial friction but also by intrinsic factors.

Statistical analytical methods for comparing the incidence of tumors to the historical control data.

Statistical analysis for comparing the incidence of tumors in treated groups to historical control data is not generally performed. In the present study, a number of data exhibiting the lowest, low, moderate and highest incidence of tumors from long-term rodent bioassay studies has been compared with the historical control data. In studies exhibiting the lowest incidence (less than a few percent) of tumors, the Kastenbaum and Bowman test was found to be relevant since it takes into account the sample size of both the historical control data base and each treated group in the study. In studies where a wider range of tumor incidence was exhibited, a statistical method which employs a rejection limits based on the range of incidence in the historical data is recommended. When malignant tumors are evident in treated groups, no matter how low the incidence, they should be analyzed statistically and compared with the incidence in historical control data as well as those in the concurrent control group. Statistical analytical comparisons of study results to historical control data may contribute to more meaningful evaluations in carcinogenicity studies by eliminating possible false positive or false negative results.

Capillary leak syndrome likely the result of granulocyte colony-stimulating factor after high-dose chemotherapy.

Two cases of malignant lymphoma complicated with capillary leak syndrome following super high-dose chemotherapy and administration of granulocyte colony-stimulating factor (G-CSF) are presented. Subsequent to the nadir of granulocytes, and at the stage of rapid increase of granulocytes, the symptoms of fever, hypotension, dyspnea, pleural effusion and edema appeared, and laboratory data revealed hypoxia, hypocapnia and hypoalbuminemia. In addition, an abscess-like lesion was observed in the liver in one patient. After the administration of G-CSF was ceased or decreased, and pulse therapy with methylprednisolone was initiated, these symptoms disappeared quickly.

Fatal aplastic anemia caused by Epstein-Barr virus infection after autologous bone marrow transplantation for non-Hodgkin malignant lymphoma.

We report a case of severe and fatal aplastic anemia during an episode of infectious mononucleosis caused by Epstein-Barr (EB) virus infection. The 13-year-old female patient had shown normal hematological findings and had previously undergone repeated chemotherapy and autologous bone marrow transplantation for refractory non-Hodgkin malignant lymphoma (NHL). She was probably in an immuno-suppressed condition prior to this episode of infection. The possible causal relationship of the EB virus infection in the pathogenesis of aplastic anemia was documented by the clinical course, demonstration of EB virus genome in the bone marrow cells, and an elevated plasma interferon (IFN)-gamma level.

Effects of Mg and Mn ions on the inhibitory action of cyclic nucleotides in the longitudinal myometrium of rat.

Contractile response, membrane activity, and fluorescence signal from mag-fura-2 were measured on the longitudinal myometrium taken from estrogen-treated rat, and the influence of Mg and Mn ions on the inhibitory effects caused by db cAMP, 8-bromo cGMP, and db cGMP was investigated. Taking the Kd value of mag-fura-2 with Mg to be 2.0 mM, the intracellular Mg concentration was estimated to be 380 microM in the tissue incubated with Mg-free Krebs solution and 420 microM in the tissue exposed to Ca-free 40 mM K solution containing 15 mM Mg for 5 min, respectively. The phasic contractions generated by electrical stimulation in Mg-free Krebs solution were more strongly depressed by 30 microM db cAMP, 10 microM 8-bromo cGMP, and 1 mM db cGMP after the Mg loading treatment. The muscle was exposed to Ca-free 40 mM K solution containing 0.6 mM Mn for 10 min, and it was assumed that the muscle was loaded with Mn ions. After the Mn loading treatment, the contractile depression caused by 8-bromo cGMP and db cGMP was enhanced, whereas that by db cAMP was attenuated. These results were discussed in relation to the influence of intracellular load with Mg or Mn on the inhibitory actions caused by isoprenaline, forskolin, and relaxin.

Laboratory correlates of chemoimmunotherapy with low-dose recombinant interleukin-2 and mitomycin C in patients with advanced carcinoma.

Based on our clinical findings that the ability of cancer patients to generate lymphokine-activated killer (LAK) cells was remarkably augmented after mitomycin C (MMC) administration, we designed a treatment regimen that consisted of MMC 12 mg/m2, i.v. on day 1 and recombinant interleukin-2 (IL-2) 700 U/m2, i.v. every 12 hr from day 4 through day 8. Of 29 patients with advanced carcinoma treated with this regimen, 10 had a partial response (PR) and 4 had a minor response. The correlation of hematological and immunological changes associated with this treatment with the antitumor response to this therapy was investigated. Pretreatment values of total white blood cell and lymphocyte counts, and the level of increase of eosinophil counts in responder patients who showed a PR, were significantly greater than those in nonresponder patients. However, there was no correlation between clinical response and cytotoxic activities of peripheral blood mononuclear (PBM) cells, including NK and LAK activity, and the ability to generate LAK cells after the treatment. The capacity of adherent cells in PBM to produce IL-1-beta was increased after the treatment in both responders and nonresponders, whereas IL-1-alpha production was not increased. In addition, a significant increase in the ability to produce TNF-alpha was observed only in responders, indicating the correlation of TNF-alpha production with clinical response to this therapy. Since these correlations had been reported in the previous studies using IL-2, the present results suggested that the therapeutic effectiveness of this therapy against advanced carcinoma, is due to IL-2 probably augmented by its combination with MMC. In addition, these parameters might be predictive of therapeutic efficacy of this treatment.

Multiple malignant fibrous histiocytoma of bone. A case report.

A 27-year-old man had tumors in both the femoral and the tibial metaphyses of the left knee. Although histological examination of the biopsy specimens revealed different findings in the 2 lesions, the final diagnosis was malignant fibrous histiocytoma in both.

Electrophysiologic effects of intravenous E-4031, a novel class III antiarrhythmic agent, in patients with supraventricular tachyarrhythmias.

The electrophysiologic effects of intravenous (i.v.) E-4031, a new class III antiarrhythmic drug, were evaluated in 15 patients with supraventricular tachyarrhythmias [11 men, 4 women; mean age 41 +/- 19 (SD) years]. Eleven patients had accessory atrioventricular (AV) pathways, and 4 patients with no accessory pathway had paroxysmal atrial fibrillation. Electrophysiologic studies were performed before and after E-4031 administration (loading infusion 9 micrograms/kg for 5 min + maintenance infusion 0.15 microgram/kg/min). QT and QTc intervals were significantly prolonged by E-4031 from 0.40 +/- 0.03 (mean +/- SD) to 0.46 +/- 0.03 s (p < 0.0001) and from 0.43 +/- 0.03 to 0.49 +/- 0.04 s (p < 0.0001), respectively. No effect was observed on RR interval, PR interval, QRS duration, or AH and HV intervals. The effective refractory periods (ERPs) of the right atrium and ventricle were significantly prolonged from 219 +/- 27 to 236 +/- 26 ms (p < 0.001) and from 230 +/- 12 to 249 +/- 11 ms (p < 0.001), respectively. The ERP of the AV node did not change significantly after E-4031 administration. In patients with ventricular preexcitation, E-4031 significantly prolonged the ERP of the antegrade accessory pathway conduction from 340 +/- 101 to 362 +/- 106 ms (p < 0.001), but not retrograde accessory pathway conduction. AV reentrant tachycardia was induced in 3 of 11 patients with an accessory pathway, and repetitive atrial firing was induced in 3 of 4 patients with paroxysmal atrial fibrillation. E-4031 could prevent repetitive atrial firing in only 1 patient and could not prevent induction of AV reentrant tachycardia.(ABSTRACT TRUNCATED AT 250 WORDS)

Gold sodium thiomalate down-regulates intercellular adhesion molecule-1 and vascular cell adhesion molecule-1 expression on vascular endothelial cells.

We examined whether antirheumatic drugs alter cytokine- or lipopolysaccharide-induced expression of adhesion molecules on vascular endothelial cells. Human umbilical cord vein endothelial cells were co-cultured with various antirheumatic drugs in the presence of inflammatory cytokines, and adhesion molecule expression was measured by cell enzyme-linked immunosorbent assay and Northern blot analysis. Among these antirheumatic drugs, gold sodium thiomalate significantly inhibited intercellular adhesion molecule-1 and vascular cell adhesion molecule-1 expression on vascular endothelial cells and suppressed cellular binding between human monocytic cell lines, including U937 and HL-60 cells, and interleukin-1 beta-stimulated vascular endothelial cells. It is speculated that down-regulation of adhesion molecules might be one of the novel mechanisms of action of gold sodium thiomalate.

[The importance of pain control with morphine for terminally ill patient care for at home].

From April 1987 to June 1994, 81 patients of terminally ill had been care at home. In these home care, 38 patients received pain control with morphine. In 29 patients, pain control had been started before home care but in 9 patients pain control had been started under home care. Routes of morphine administration were oral in 24 cases and rectal in 14 cases. In cases of oral routes, finally 2 cases converted to rectal routes and 2 cases converted to subcutaneous routes. Duration of treatment were from 2 days to 741 days (average 72 days). Dosage of morphine were 7.5 mg to 480 mg (average 116 mg). Two patients lived but 24 patients died at home and 12 patients died at hospital. We concluded that palliative care especially pain control with morphine was very important care for patients of terminally ill who wanted care at home and ultimately die at home. We had to make ourself master of method pain control with morphine.

Helicobacter pylori infection in gastric carcinoma.

PURPOSE: This study was undertaken to compare the pathoclinical findings in gastric adenocarcinoma with serum IgG antibody to Helicobacter pylori. MATERIALS AND METHODS: We examined 185 patients with histologically established gastric cancer. The presence of immunoglobulin (Ig)G antibody in the high molecular cell-associated antigen of H. pylori was determined by enzyme-linked immunosorbent assay. Pepsinogens I and II were measured by radioimmunoassay. The distribution of H. pylori on the gastric mucosa was assessed by the Campylobacter-like organism test and phenol red dye spraying. RESULTS: H. pylori IgG antibody was detected in 93.1% of patients with gastric cancer (mean age 61.7 years), 94.3% of patients with early gastric cancer and 91.2% with advanced gastric cancer. No statistical difference in serology was observed between type of gastric cancer, depth of cancer invasion, tumor size or histology. Only in patients with diffuse-type cancer of the cardia was there a lower percentage of positive results (80.0%). The ratio of pepsinogen I to pepsinogen II was higher in the patients who exhibited no H. pylori antibodies. CONCLUSIONS: H. pylori antibodies were common in patients with gastric cancer, and were not correlated with histological type nor stage of cancer. In the Niigata district, a higher percentage of patients with gastric carcinoma displayed H. pylori antibodies compared with other districts in Japan.

[A ornithine decarboxylase activity and proliferating cell nuclear antigen in gastric cancer].

To investigate ornithine decarboxylase (ODC) activity and proliferating cell nuclear antigen (PCNA) in gastric cancer, ODC activity and PCNA were measured in 50 resected samples. The relationship between both and clinicopathologic factors was examined. ODC activity was 473.3 +/- 54.7 pmol CO2/60 min/mg protein in tumors, and 273.5 +/- 38.3 pmol CO2/60 min/mg protein in normal mucosa. ODC activity in tumors was significantly higher than that of normal mucosa. ODC activity in tumors was significantly high in gross type 4, maximum diameter more than 10 cm, depth se, infiltrative growth (INF) gamma, positive lymph vessel invasion and positive lymph node metastasis. PCNA-LI was 24.7 +/- 1.5% in tumors, and 13.9 +/- 1.1% in normal mucosa. PCNA-LI of tumors was significantly higher than that of normal mucosa. PCNA-LI of tumors was significantly high in gross type 2, histological type tub 2 and por, depth ss beta and se, IFN beta, positive lymph vessel invasion, positive venous invasion, and positive lymph node metastasis. ODC activity and PCNA-LI were closely related in normal mucosa, showing a correlation coefficient of 0.730. On the other hand, their relationship was weak in tumors, showing a correlation coefficient of 0.417. These results suggest the differentiation of value between ODC activity and PCNA-LI in gastric cancer. In gastric cancer, ODC activity and PCNA-LI in tumors may be good markers of lymph node metastasis. Furthermore, PCNA-LI may be a good marker of hematogenous metastasis.

[Eosinophilic airway inflammation and airway hyperresponsiveness].

In order to investigate the effects of accumulation and activation of eosinophils on the airway responsiveness we developed an animal model of eosinophilic airway inflammation by administration of polymyxin B. Bronchoalveolar lavage confirmed the presence of significantly increased numbers of eosinophils in polymyxin B-treated guinea pigs compared with that in saline-treated guinea pigs. Contrary to our expectation, no significant increase in airway responsiveness was obtained in polymyxin B-treated group. The results also showed that both intratracheal instillation and inhalation of FMLP to polymyxin B-treated guinea pigs enhance the airway responsiveness to intravenous 5-HT. The indice of eosinophil activation, EPO activity after the FMLP administration in polymyxin B-treated guinea pigs was significant increased. It suggested that accumulated eosinophils be highly activated. The results indicate that eosinophil accumulation does not necessarily cause AHR and accumulated eosinophil activation plays an important role in the development of AHR. Accumulated eosinophil activation would be one of the trigger for asthma attack.

Turn-over of PAF in cultures of human paranasal sinus mucosa.

The change of PAF concentration in the culture medium was investigated by radioimmunoassay when 10(-8) M PAF or 10(-8) M lyso-PAF was incubated with a piece of normal human paranasal sinus mucosa. The PAF concentration in the medium of the former group was halved within 11.3 min and reduced to less than 5% of the initial concentration within 60 min. However, there was no significant difference in the reduction of PAF concentrations in the medium between groups with or without the mucosa. When 10(-8) M lyso-PAF was incubated with a piece of mucosa, PAF gradually increased and reached the maximum of 0.36 x 10(-8) M at 20 min, and thereafter quickly decreased to a non-detectable level.

[A case of acute idiopathic mediastinitis that responded rapidly to treatment].

A 50-year-old man had a feeling of a foreign substance such as fish bone in his throat at breakfast. The feeling gradually changed to retrosternal pain and pain on swallowing, so he went to hospital, but the results of all examinations were normal. The next day he was referred to our emergency center because of the increasing pain and disturbance in swallowing. On admission he had a fever of 37.9 degrees C, a WBC of 35,000 cells/mm3, and a CRP level of 18.3 mg/dl. There was no widening of the mediastinum visible on the chest radiograph, but on the chest CT a tumorous shadow was seen around the esophagus from the cervical to the gastric portion, and was suspected to be an abscess. Some antibiotics were given, based on the diagnosis of acute mediastinitis. The pain gradually subsided, and the tumorous shadow was obscured, without mediastinal drainage. Prompt therapy resulted in rapid improvement, and the patient was discharged on hospital day 23.

A highly enhanced thrombopoietic activity by monomethoxy polyethylene glycol-modified recombinant human interleukin-6.

Interleukin-6 (IL-6) is a novel cytokine with activities that can affect hematopoietic cells including those of the megakaryocytic lineage. We have examined the effects of monomethoxy polyethylene glycol-modified recombinant human interleukin-6 (Peg-IL-6) on thrombopoiesis in vivo. To compare the thrombopoietic activity between Peg-IL-6 and unmodified IL-6, each was administered subcutaneously to mice every 24 hours at various doses. A dose-response experiment showed that Peg-IL-6 and IL-6 increased platelet counts in a dose-dependent fashion at a plateau stimulation level of 0.5 micrograms/day and 10 micrograms/day, respectively. This dose of Peg-IL-6 and IL-6 induced the elevated platelet counts by approximately 140% to 160% and 50% to 60%, respectively. Peg-IL-6 treatment (0.5 micrograms/day) for x-ray (6.0 Gy) irradiated mice induced an increase in the rate of platelet recovery, and a higher dosage (5 micrograms/day) completely blocked the induction of thrombocytopenia in this model. In contrast, IL-6 (10 micrograms/day) could not protect the animals from platelet nadir but reduced the period of thrombocytopenia after x-ray irradiation. Furthermore, when administered to 5-fluorouracil-treated mice, 5 micrograms/day of Peg-IL-6 diminished the platelet nadir and increased platelet counts on individual days during the recovery phase. The potent thrombopoietic activity of Peg-IL-6 were due to prolongation of circulating IL-6 levels that were reverted from Peg-IL-6 in vivo. These findings indicate that reduction of total body clearance of IL-6 induces potent thrombopoiesis and that Peg-IL-6 may be a useful thrombopoietic agent.
