[Thoracoscopic unilateral fold plication method in lung volume reduction surgery for diffuse pulmonary emphysema].

We have devised a 'Fold Plication Method', which helps ensure safe and simple lung volume reduction surgery (LVRS) for pulmonary emphysema and prevent air leakage from pulmonary stumps, which is most crucial problem, in addition to postoperative interstitial pneumonia that is occasionally caused by the use of the bovine pericardium. We performed LVRS on seven patients with the disease, using this method and the thoracoscopic surgery, which is based on the 'Two Windows Method' we had previously developed. The operation was performed without observable bleeding within approximately one hour for all patients, who then recovered favorably.

[A case of adult-onset neurenteric cyst presenting as chronic progressive muscular atrophy in lower legs].

A 50-year-old man was admitted, because of motor weakness of the lower limbs, dysesthesia of the left lower extremity, and anuresis. He had an episode of pain in his gluteal region 17 years ago, and then, no abnormalities were detected including myelography in a hospital, followed by slowly progressive muscular atrophy of his lower legs. At 50 years of age, dysuria appeared. He was diagnosed as having neurogenic bladder by urologists, and was admitted to our hospital. On admission, abnormal neurologic findings included: severe muscular atrophy in his lower legs, pes cavus, dysesthesia at the left S1 level, and autonomic bladder. Magnetic resonance imaging (MRI) showed mass lesion involving lower conus and cauda equina. After resection, pathological study revealed the mass was a neurenteric cyst. It is said that the neurenteric cyst causes an asymmetrical and sequential loss of specific neurological functions, with a subsequent return of these functions in the reverse order. That mechanism is not clear. However, in our case, the course of the illness was slowly progressive. We speculate that, because of the cyst's adhesion to cauda equina and perforation through the cyst by a nerve root, the cyst was fixed and caused slowly progressive neurological deficits in proportion to increase of the cyst's size. Our report suggests that a neurenteric cyst, involving the lower conus and cauda equina, can produce severe muscular atrophy in the lower legs.

Estimation of free calcium levels after thyroidectomy.

Total calcium is routinely measured after thyroidectomy in a clinical setting, while the measurement or calculation of the free calcium level is not generally performed. We reviewed total and free calcium levels in patients who underwent lobectomy (n = 15), subtotal thyroidectomy (n = 15) and total thyroidectomy (n = 15). Postoperative total calcium levels decreased significantly in comparison to preoperative levels in all thyroidectomies (p < 0.01), and this fall was significantly related to the extent of surgery (p < 0.01). In contrast, there was no significant difference between preoperative and postoperative free calcium levels in patients undergoing lobectomy, although we found a decrease in free calcium levels after both subtotal and total thyroidectomy. Total protein levels decreased regardless of the type of operation. Serum total calcium levels were thought to be altered by serum protein levels through the change of protein-bound calcium levels. When examined for free calcium levels, some patients were administered unnecessary calcium supplementation because hypocalcemia had been judged from the total calcium level. Since the wrong diagnosis may be given with regard to hypoparathyroidism by measurement of total calcium levels alone, we propose that free calcium levels should be routinely measured or calculated after thyroidectomy.

[A case of renal cell carcinoma found after skin metastasis].

A case of renal cell carcinoma, found after skin metastasis is presented. A 76-year-old woman came to our hospital complaining of two painless subcutaneous tumors in her left chest. Histopathological diagnosis was clear cell carcinoma. She underwent left nephrectomy and histopathological findings revealed renal cell carcinoma, alveolar type, clear cell subtype, grade 1. Lung metastasis was proved soon after the operation. We removed skin metastasis several times after the nephrectomy.

[Creutzfeldt-Jakob disease with a widespread presence of kuru-type plaques after cadaveric dural graft replacement. An autopsy case].

A case of Creutzfeldt-Jakob disease (CJD) is reported in a 48-year-old woman who had received a cadaveric dural graft after a clipping procedure of a cerebral artery aneurysm in September 1985. In November 1994, she noticed unsteady gait and blurred vision at first. She successively developed ataxic gait, dementia and myoclonus, and became mute. Serial CT scans revealed no abnormal findings, and serial EEGs showed diffuse slow activity without periodic discharge. The patient died in March 1996, 17 months after the initial symptoms. A brain autopsy demonstrated extensive spongiform degeneration in the cerebral neocortex, thalamus, striatum, and cerebellum especially in the granular layer, with associated astrocytosis and marked neuronal loss. Immunohistochemically, PrP plaques, so called kuru-type plaques, were extensively distributed throughout the cerebrum and cerebellum. Moreover, some of these plaques resembled "florid" plaques, which were surrounded by a zone of spongiform change. The PrP gene analysis of her blood and brain tissue revealed no mutations with homozygosity, Met/Met, at codon 129. The unusual features of this case, that is the absence of PSD on EEG and the widespread presence of kuru-type plaques including "florid" plaques, may be similar to the features of "new variant" CJD.

Long-term results of Charnley low-friction arthroplasty for coxarthrosis with congenital hip dysplasia. 15 year follow-up study.

We studied more than fifteen-years' follow-up results of cases treated with low-friction arthroplasty (LFA) for coxarthrosis with congenital hip dysplasia. Thirty-four LFAs in 32 patients (2 males and 30 females) were followed up for an average of 18.8 years (range: 15 to 23.3 years). The mean age at surgery was 53 years (range: 36 to 67 years). In three patients, 4 hips needed revision surgery due to loosening of the prostheses. At the final follow up, using Hodgkinson's criteria, type 0 demarcation of the socket appeared in 9 cases, type 1 in 5 cases, type 2 in 4 cases, type 3 in 4 cases, and type 4 in 12 cases. According to Harris' criteria, there was no sign of loosening of the stem in 16 cases, however there was a possibility of loosening in 8 cases, probability in 2 cases, and definite loosening in 8 cases. The survival rate with socket loosening as the end point was 76.5% at the 10 year follow-up, 67.6% at 15-years' follow-up, and 50.9% at 20-years' follow-up. The survival rate with stem loosening as the end point was 82.4% at 10-years' follow-up, 79.4% at 15-years' follow-up, and 63.8% at 20-years' follow-up. The survival rate with prostheses loosening either socket or stem as the end point was 64.7% at 10-years' follow-up, 55.9% at 15-years' follow-up, and 34.6% at 20-years' follow-up. The survival rate with revision surgery as the end point was 100% at 10-years' follow-up, 94% at 15-years' follow-up, and 85.1% at 20-years' follow-up.

[A case of multiple sclerosis with relapses during the third trimester of pregnancy].

We report a 31-year-old woman with a relapse of multiple sclerosis during the third trimester of pregnancy. She had been treated palliatively after the relapse until occurrence of a new symptom and new lesions on MRI, and then she had Cesarean section. After surgery, steroid pulse therapy was started and the symptoms and lesions on MRI resulted in improvement. As a strategy to treat patients with multiple sclerosis in their third trimester of pregnancy, it is appropriate to evaluate disease activity using MRI and to start steroid pulse therapy after delivery.

Mechanistic studies on the neighboring base damage induced by KMnO4 oxidation of 8-oxoguanine in DNA.

New types of DNA substrates containing an 8-oxoguanine residue (8-oxo-G) were prepared in order to examine the mechanisms for the neighboring base damage initiated by KMnO4 oxidation of the 8-oxo-G. The results obtained from the reactions suggested that the damage at remote sites in the single strands can be explained by an electronic interaction (redox reaction) between an oxidized 8-oxo-G species and the base (to be damaged), which are close each other in a loop structure. For the inefficient damage observed in duplex substrates, electron transfer through stacked bases might be involved.

The effect of leukocyte function of streptozotocin-induced diabetes in naturally occurring gingivitis rat.

Although diabetes mellitus is known to aggravate periodontal disease, the precise relationship between these two entities is far from being completely understood. Further study of this relationship was therefore undertaken in the form of observation of both naturally occurring gingivitis in rats (ODUS/Odu) and effects produced by induction of experimental diabetes mellitus by injection of streptozotocin (STZ: 65 mg/kg, i.v.). At one and 3 mon after STZ injection, liquid paraffin was injected intraperitoneally. Four days thereafter, pocket probing depths of rats were measured and blood samples as well as peritoneal macrophages were collected from both experimental animals and non diabetic controls. Both chemotaxis and phagocytosis of macrophages were studied. At one and 3 mon after STZ injection, pocket probing depths of diabetic animals were significantly deeper than those of controls (p < 0.001). Pocket probing depths were deeper at 3 mon after STZ injection than after 1 mon in diabetic animals. At three months after STZ injection, there was a high degree of positive correlation between pocket probing depths, blood glucose levels, triglyceride, and hemoglobin A1c levels (p < 0.01). Also, macrophage chemotaxis was more suppressed in diabetic rats than it was in controls. Additionally, both phagocytosis ratios and phagocytosis indices of macrophages in the diabetes group were significantly more suppressed than those in controls in both experimental periods (p < 0.001). Findings suggest that both chemotaxis and phagocytosis are compromised in macrophages from rats rendered diabetic by STZ injection. Thus as host defense mechanisms become weakened, there is a corresponding progression of periodontal disease.

Fluorometric determination of phenol and p-cresol in urine by precolumn high-performance liquid chromatography using 4-(N-phthalimidinyl)benzenesulfonyl chloride.

A sensitive HPLC method for simultaneous determination of phenol and p-cresol in urine was developed. After acid hydrolysis of urine was conducted, phenol and p-cresol were extracted with diisopropyl ether and derivatized with 4-(N-phthalimidinyl)benzenesulfonyl chloride to give fluorescent sulfonyl esters. The labeling reactions were completed at 75 degrees C for 10 min. The fluorescent derivatives were separated on a reversed-phase column by a gradient elution with acetonitrile-water and detected by fluorescence measurement of excitation at 300 nm and emission at 410 nm. The detection limits (signal-to-noise ratio = 3) for phenol and p-cresol were 0.17 and 0.25 pmol per injection, respectively. The within-day and day-to-day relative standard deviations were 2.31-3.76 and 4.36%, respectively, for phenol and 1.99-4.56 and 3.71%, respectively, for p-cresol. The concentrations (means) of phenol and p-cresol in normal human urine were 67.3 and 167.9 nmol/mg creatinine, respectively.

Increased sensitivity of EBNA2-transformed rat fibroblasts to ionizing radiation.

To clarify whether expression of Epstein-Barr virus(EBV) nuclear antigen 2 (EBNA2) correlates with sensitivity to ionizing radiation, we tested EBNA2-transformed rat fibroblast clones for their radiosensitivity to X-rays. These transformed clones reproducibly generated tumors in mice. X-irradiation suppressed the growth of the tumors, and irradiated mice survived longer than non-irradiated ones. In contrast, tumors formed by activated H-ras or E6-E7 genes of human papillomavirus type 16 (HPV16) were strongly resistant to the same dose of X-irradiation. In in vitro culture, these EBNA2-expressing clones also showed higher radiosensitivity than cell lines transformed by activated H-ras and E6E7 genes. The averaged D(o) of EBNA2-expressing clones was 2.3 times lower than that of non-expressing and control clones. These results suggest that expression of EBNA2 is responsible for the radiosensitivity.

Relaxation of insulin-like growth factor 2 gene imprinting in esophageal cancer.

Paternal allele-specific expression is identified for the insulin-like growth factor 2 (IGF2) gene. Relaxation or loss of IGF2 imprinting, however, has been reported in several neoplasms. We studied the expression of IGF2 mRNA in 35 squamous cancers of the esophagus and searched for the presence or absence of relaxation of IGF2 imprinting. In 28 (80%) cases, IGF2 mRNA was overexpressed in the tumor tissues (T) compared to the normal tissues (N). The patients whose tumor invaded the adventitia showed a higher T/N ratio than those whose tumor was restricted to the musculi propria layer. Heterozygosity was determined by using the Apa I polymorphism in exon 9. Thirteen of 35 cases showed heterozygosity. In these 13 cases, a similar analysis was performed on cDNA obtained by reverse transcriptase-polymerase chain reaction. Consequently, 7 cases disclosed relaxation of IGF2 imprinting in the tumor tissue. The cases of esophageal cancer with relaxation of IGF2 imprinting showed a higher T/N ratio and deeper invasion than those without relaxation. The results suggest that overexpression of IGF2 mRNA plays an important role in esophageal cancer and, in certain cases, is associated with relaxation of IGF2 imprinting.

Adhesion-dependency of serum-induced p42/p44 MAP kinase activation is released by retroviral oncogenes.

G1/S transition of the cell cycle is blocked when cells are cultured without cell adhesion, even if nutrients and serum are present. Shift experiments involving transfer from adhesion to suspension cultures have revealed that cell adhesion in the early half of G1 phase is required for induction of DNA synthesis. We found that activation of p42/p44 mitogen-activated protein (MAP) kinases, one of the earliest responses induced by serum stimulation, was also dependent on cell adhesion in rat F2408 and mouse Swiss3T3 fibroblast cell lines, suggesting that MAP kinase activation is a critical step in adhesion-dependent G1 progression. F2408 cell lines transformed by the v-src, v-K-ras, and v-mos oncogenes showed constitutively high MAP kinase activity, even in the absence of serum and cell adhesion, while both serum stimulation and cell adhesion were necessary to induce intensive activation of MAP kinase in a F2408 cell line transformed by the E6 and E7 genes of human papillomavirus type 16, as well as in untransformed F2408.

Osteotropic factor-stimulated synthesis of thrombospondin in rat dental pulp cells.

The amount of thrombospondin (TSP) mRNA in confluent clonal rat dental pulp cells was increased by transforming growth factor-beta (TGF-beta), 1 alpha-25-dihydroxyvitamin D3 (1,25(OH)2D3), and phorbol 12,13-didecanoate (PDD), with maximal levels 6, 36 and 2 h, respectively, after stimulation. These increases were accompanied by enhanced syntheses of TSP proteins which were found in the different forms in cell layer/matrix fraction (198 and 165 kDa TSP) and the culture medium (180 kDa TSP). These three factors also raised the mRNA level of osteopontin, which is thought to play an important role in mineralization in dentin and bone. The order of effectiveness of these factors was PDD > TGF-beta > 1,25(OH)2D3 for all the stimulations described above. These results suggest that the osteotropic factors enhance TSP synthesis at the pretranslational level and that TSP produced by dental pulp cells participates in formation of reparative dentin.

Conversion of recent-onset Atrial Fibrillation by a single oral dose of Pilsicainide (Pilsicainide Suppression Trial on atrial fibrillation). The PSTAF Investigators.

The efficacy and safety of a single oral dose of 150-mg pilsicainide, a new class Ic antiarrhythmic drug, in converting recent-onset atrial fibrillation to sinus rhythm were evaluated in 75 patients (51 men, 24 women; age 23 to 74 years). Conversion to sinus rhythm was achieved within 90 minutes in 45% of patients given pilsicainide and in 8.6% of those on placebo (p < 0.01), with no major adverse effects.
