The complete amino acid sequence of ribonuclease from the seeds of bitter gourd (Momordica charantia).

The complete amino acid sequence of ribonuclease (RNase MC) from the seeds of bitter gourd (Momordica charantia) has been determined. This has been achieved by the sequence analysis of peptides derived by enzymatic digestion with trypsin, lysylendopeptidase, and chymotrypsin, as well as by chemical cleavage with cyanogen bromide. The protein contains 191 amino acid residues and has a calculated molecular mass of 21,259 Da. Comparison of this sequence with sequences of the fungal RNases, RNase T2, and RNase Rh, revealed that there are highly conserved residues at positions 32-38 (TXHGLWP) and 81-92 (FWXHEWXKHGTC). Furthermore, the sequence of RNase MC was found to be homologous to those of Nicotiana alata S-glycoproteins involved in self-incompatibility sharing 41% identical residues.

Retinoic acid induces polarizing activity but is unlikely to be a morphogen in the chick limb bud.

Retinoic acid is a putative morphogen in limb formation in the chick and other vertebrates. In chick limb formation, it is thought that retinoic acid is released from the zone of polarizing activity (ZPA) and the concentration gradient of retinoic acid formed from the posterior to the anterior provides positional cues for digit formation. Implantation of a bead containing retinoic acid at the anterior margin of the limb bud induces a mirror-image symmetrical duplication of the digit pattern similar to that observed when the ZPA is grafted into the anterior margin of the host limb bud. Also, the level of endogenous retinoic acid (25 nM on average) is higher in the posterior one third of the limb bud. We found that when the bead containing either retinoic acid or an analogue but not the ZPA, was implanted in the anterior margin of the chick limb bud, expression of the retinoic acid receptor type-beta gene was induced around the bead within 4 h. These results indicate that exogenous retinoic acid is not identical with the ZPA morphogen. As the anterior tissue exposed to retinoic acid has polarizing activity, we conclude that the primary function of exogenous retinoic acid is to induce polarizing activity in the limb bud.

Processing of DNA base damage by DNA polymerases. Dihydrothymine and beta-ureidoisobutyric acid as models for instructive and noninstructive lesions.

The processing of unrepaired DNA lesions is a key to understanding and predicting the biological end points of particular DNA damages. In this study, we prepared single-stranded f1 phage (f1-K12) DNA containing dihydrothymine or beta-ureidoisobutyric acid as models for instructive or noninstructive base lesions and assessed the potential biological consequences of these lesions in vitro and in vivo. To determine the effect of the two lesions on in vitro DNA synthesis, the extent of DNA synthesis was measured by 3H-labeled nucleotide incorporation, and the newly synthesized DNA was analyzed by DNA sequencing gels. The results showed that dihydrothymine in the template was at most a weak block to in vitro DNA synthesis catalyzed by Escherichia coli DNA polymerase I Klenow fragment (Pol I) and T4 DNA polymerase. In contrast, beta-ureidoisobutyric acid constituted a very strong (probably absolute) replicative block in vitro. With Pol I, termination bands were observed either opposite or one base prior to (3' to) the putative beta-ureidoisobutyric acid depending on its position in the template. However, when DNA synthesis was catalyzed by Pol I lacking a 3'----5' exonuclease activity, termination bands were only observed opposite beta-ureidoisobutyric acid, with purine nucleotides being incorporated preferentially opposite the lesion. With T4 DNA polymerase that contains a very active 3'----5' exonuclease activity, DNA synthesis was arrested almost exclusively one base prior to (3' to) the putative beta-ureidoisobutyric acid site in the template. We also measured survival of transfecting DNA containing dihydrothymine or beta-ureidoisobutyric acid in an attempt to correlate the in vitro data with in vivo processing. In keeping with the results obtained in vitro, dihydrothymine present in transfecting f1-K12 DNA did not constitute an inactivating lesion. On the other hand, it took 0.9 beta-ureidoisobutyric acid residues per molecule to inactivate transfecting f1-K12 DNA, indicating that the lesion was an absolute replicative block in vivo. When host cells were ultraviolet-irradiated to induce the SOS response, a slight increase (about 2-fold) in survival of transfecting f1-K12 DNA containing beta-ureidoisobutyric acid was observed. The potential effects of the structures of base lesions on lesion-polymerase interactions are discussed.

Cerebral vasospasm and vasoconstriction caused by endothelin.

We investigated the histological changes between arteries constricted by endothelin for 7 days and vasospastic arteries induced by the double injection of autologous blood. Group 1 was a sham-operated group. Group 2 animals received a continuous cisternal injection of endothelin-1 (1.7 x 10(-9) mol/7 days) by a miniosmotic pump implanted in the neck musculature for 7 days. Group 3 received double injections of cisternal blood administered 48 hours apart. Angiography showed severe constriction of the basilar artery, 34.6% and 43% in Groups 2 and 3, respectively, on Day 7. Histological study showed marked constriction of the basilar artery in both Group 2 and Group 3. Degenerative changes in endothelial cells and smooth muscle cells were observed in both Group 2 and Group 3. Immunohistochemical study demonstrated endothelin-1 in the endothelial cells in Group 2, but not in Group 1 or in Group 3. It is suggested that endothelin-1 may play an important role in the pathogenesis of cerebral vasospasm.

Changes in phosphorus-31 magnetic resonance spectra caused by epidural balloon in cats.

Changes in brain high energy phosphate metabolite and pH levels were studied using serial phosphorus (P)-31 magnetic resonance spectroscopy (MRS) in an epidural balloon cat model. The balloon was inflated with 1, 2, 3, or 4 ml of water at 1 ml/sec and deflated after obtaining a spectrum 4 minutes later. Serial spectra, intracranial pressure (ICP), physiological parameters, and the pupil sizes were observed for 1 hour. The mean ICP after balloon inflation increased to between 36 +/- 3 and 130 +/- 6 mmHg. Some animals showed oculomotor paralyses. Phosphocreatine, adenosine triphosphate, phosphodiester, and phosphomonoester levels suddenly decreased after inflation and gradually recovered after balloon deflation. Acidosis progressed with cerebral compression for 4 minutes and gradually recovered. All changes and recovery were volume related. This study demonstrates the potential of P-31 MRS for noninvasive, serial, in vivo measurements of critical high energy phosphate metabolites and intracellular pH after head injury.

Long-term results of immunochemotherapy for advanced ovarian cancer.

Fifty-one patients with stage III and IV ovarian adenocarcinoma underwent initial cytoreductive surgery at Kurume University Hospital between January 1982 and October 1985. They were assigned postoperatively by randomized trials to immunochemotherapy or chemotherapy alone. Of all 51 patients, long-term results of immunochemotherapy for 43 patients were evaluated versus radicality of initial cytoreductive surgery. As a result, the group of patients treated with immunochemotherapy tended to have a better prognosis than the group of patients treated with chemotherapy alone, whereas no statistical difference was observed between the two groups. Moreover, no significant difference was observed in the monitoring of OKT 4/8 ratio between the two groups. However, immunochemotherapy produced a favorable prognosis in the patients with residual disease of greater than 2 cm in diameter at initial surgery. In conclusion, these data suggested that immunochemotherapy may have some impact on survival of patients with ovarian adenocarcinoma.

Effect of chemical sympathectomy on cerebral blood flow in rats.

Thirty male Wistar rats, weighing 350 to 400 gm each, received stereotactic injections of 6-hydroxydopamine (300 micrograms/kg) into the left lateral ventricle. The same amount of saline was injected into a control group of 15 rats. Seven days after this procedure, cerebral blood flow (CBF) was measured by the hydrogen clearance method. A hypertensive condition at a mean arterial pressure of about 160 mm Hg was maintained for 1 hour by intravenous infusion of phenylephrine. In the 6-hydroxydopamine-treated group, CBF increased significantly after the elevation of systemic blood pressure compared with that in the control group, and cerebral autoregulation was impaired. After a 1-hour study, the specific gravity of the cerebral tissue in the treated group significantly decreased; electron microscopic studies at that time revealed brain edema. It is suggested that depletion of brain noradrenaline levels causes a disturbance in cerebral microvascular tone and renders the cerebral blood vessels more vulnerable to hypertension.

[Clinico-pathological studies on the effects of preoperative hyperthermo-chemoradiotherapy of advanced esophageal carcinoma].

We report clinico-pathological studies on the effect of preoperative hyperthermia and chemotherapy combined with radiotherapy (HCR) for progress of the local curability of advanced esophageal carcinoma. The subjects of these studies were 17 patients who underwent subtotal esophagectomy after preoperative irradiation 40 Gy from 1980 to 1989, of which 8 patients had HCR, 6 patients irradiation only (R), 3 patients both irradiation and chemotherapy (CR). The clinical response rate of the patients with R or CR was 33% (PR 3, MR 3, NC 3), and the histological effective (Ef3 or Ef2) rate was 56% (Ef3 1, Ef2 4, Ef1 4). The clinical response rate of the patients with HCR was 88% (PR 7, MR 1), and the histological effective rate was 100% (Ef3 1 Ef2 7). HCR was more effective than R or CR for the local lesion of esophageal carcinoma histopathologically (p less than 0.05). However, the survival rate of patients with HCR was similar to R and CR, respectively. These results suggest that further improvement of the heating methods and the methods of combining hyperthermia with irradiation and chemotherapy is needed.

[A case of Swyer-James syndrome diagnosed by selective alveolobronchography].

A 33-year-old male was admitted to our hospital for further investigation of unilateral hyperlucency of the left upper and middle lung fields on chest X-ray. A pulmonary scan showed decreased perfusion of the left upper and middle lung fields. Pulmonary arteriography demonstrated small left pulmonary arteries with diminished peripheral vascular branches. Left bronchogram showed almost normal distribution of main and subsegmental bronchi, but poor filling by contrast in the peripheral bronchi. From these findings, the patient was diagnosed as having Swyer-James syndrome. In order to demonstrate the alveoli in this syndrome, selective alveolobronchography (SAB) was performed. SAB revealed coarse ring shadows of B1+2.

Characterization of antisense DNA derivatives having stereoisomeric linkages.

The isomer separation of oligonucleoside phosphoramidates (OPA) by RPLC was studied. All stereoisomers of OPA-tetramer (dATCG) modified with isopropylamine could be separated and they showed different CD spectra each other. OPA-decamer (dGGGCATCGTC) modified with 3-amino-1-propanol could be separated into five fractions. Each fraction was found to have different ability to form hybrids with its complementary oligonucleotide, indicating that it is possible to exclude stereoisomers which can hardly bind to target nucleic acids.

Effects of base damages on DNA replication.

Understanding the response of DNA polymerase to the encountered damage in a template is a key to assessing lethal and mutagenic events of cells exposed to genotoxic agents. In the present study M13 (or f1) DNA templates containing 4 types of thymine damages were prepared, and DNA synthesis was carried out in vitro with the templates. The extent of inhibition of DNA synthesis by the damages was evaluated by measuring [3H]dTMP incorporation. Furthermore, newly synthesized DNA was analyzed on a sequencing gel to determine termination sites of DNA synthesis. The results showed that DNA synthesis was differentially inhibited by the damages, and the termination sites of DNA synthesis were dependent on the structures of the damages and the 3'-5' exonuclease activity of DNA polymerase used.

Unusual signal broadening in NMR of oligonucleotides.

Solution structure of single-stranded small oligonucleotides (up to pentamer) has been studied by 1H-NMR (600 MHz) and it has been found that when those having some specific base sequences produce unusual broadening of the lines attributed to the protons of guanine residues. These broadened signals did not become sharp even at elevated temperature. This extremely unusual behavior has been attributed to the interaction between the guanine residues.

[Clinical experience with omniscience and omnicarbon prosthetic heart valves].

Omniscience valves were implanted in sixty-two patients. Twenty-eight of these patients underwent aortic valve replacement (AVR), 15 had mitral valve replacement (MVR) and 8 had aortic and mitral valve replacement (DVR). Post-operative events occurred in nine (5.9%/patient year) of the AVR group, in three (1.7%/patient year) of the MVR group and in three (5.4%/patient year) of the DVR group. The actuarial freedom from all events at five years in the AVR, MVR and DVR was 74 +/- 8%, 88 +/- 6%, 67 +/- 16%, respectively. Cardiac death occurred in four (2.5%/patient year) of the AVR, one (0.6%/patient year) of the MVR and two (3.6%/patient year) of the DVR. The freedom at five years in the AVR, MVR and DVR was 88 +/- 6%, 96 +/- 4%, and 77 +/- 14%, respectively. Valve-related complications were noted in four patients. Post-operative cerebral hemorrhage was seen in three of the AVR. Maximum opening angle of the Omniscience valve was 39.1 +/- 4.5 degrees at the aortic position and 44.6 +/- 9.7 degrees at the mitral position. Omnicarbon valves implanted in ninety-five patients, fifty-eight of these patients underwent AVR, 24 had MVR and 13 had DVR. Events occurred post-operatively in four (2.6%/patient year) of the AVR group, in three (12.2%/patient year) of the MVR group, but in none of the DVR group. The freedom at five years was 89 +/- 6% in the AVR and 84 +/- 8% at three years in the MVR. Post-operative cardiac death occurred in one (0.7%/patient year) of the AVR and in two (8.1%/patient year) of the MVR.(ABSTRACT TRUNCATED AT 250 WORDS)

Proposed new criteria for early carcinoma of the esophagus.

The criteria of early carcinoma of the stomach, defined in 1981, has gained worldwide acceptance. A few proposals of a criteria for early carcinoma of the esophagus have been made; however, these criteria have not been internationally accepted, as the rate of recurrence is high in patients with early carcinoma of the esophagus, as determined by these criteria. In patients with submucosal carcinoma of the esophagus, metastasis to lymph nodes was present in 32.5 per cent at the time of the operation, and the five year survival rate was only 54.5 per cent. On the other hand, while the incidence of epithelial or mucosal carcinoma, or both, is only 2.2 per cent (46 of 2,130) in patients with carcinoma of the esophagus, the incidence has been remarkably increased and the five year cumulative survival rate for patients with epithelial carcinoma or mucosal carcinoma, or both, is now 88.4 per cent. Thus, we consider that the criteria for early carcinoma of the esophagus to be mucosal carcinoma as well as epithelial carcinoma regardless of the presence or absence of lymph node metastasis, and that a submucosal carcinoma should be excluded from the classification of early carcinoma of the esophagus.

[A case report on the perforation of a gastric tube ulcer after esophageal reconstruction for cancer].

The development of peptic ulcers in gastric tubes used for esophageal reconstruction for cancer are rare. However, they can potentially cause serious complications, including perforation and hemorrhaging. We experienced a case in which an ulcer of large size developed in the gastric tube, and it was very difficult to treat. The patient was a 72-year-old man, who had undergone an esophagotomy of the thoracic portion, an gastroesophagoplasty through the anterior portion of the sternum, and postoperative radiotherapy 2 years ago. Eight days after his admission, the ulcer began excessive bleeding, and we performed an emergency operation. The ulcer perforated into the mediastinum, and developed an abscess involving the intrathoracic artery. After hemostasis and debridement, we resected the upper half of the gastric tube, closed the oral side of the remaining portion, and converted the cervical esophageal stump into an external fistula. The wound was covered with a flap. A histological examination showed an ulcer with no evidence of malignancy, and without any signs of healing. Five months were required for the wound to heal completely. After that, we succeeded in reconstruction through jejunal free-transfer, using the microvascular surgery technique. Some pathogenesis is suggested for gastric ulcers after gastroesophagoplasty. Despite performing a vagotomy, the secretion of acid from the gastric mucosa is common. Both this condition, and the deterioration of the mucosal barrier caused by surgery, can play significant roles in the development of the ulcer. The histological influence of postoperative radiotherapy is also important, as this treatment aggravates the circulatory disturbance, and disturbs the histological reaction necessary for healing.

[CABG in 2 patients with severe left ventricular dysfunction].

Coronary artery bypass grafting was performed in two patients with left ventricular dysfunction (EF 16%). In both cases they had chest pain at rest and ischemic myocardium were detected by the SPECT. We considered them candidate for the operation regardless of LV dysfunction. They were eventful in the postoperative course. One case was discharged and is free from chest pain now, but another unfortunately died for multiple organ failure.

[Spontaneous aortic cusp rupture].

A 54-year-old man with sudden onset of aortic regurgitation due to aortic cusp rupture is presented. Urgent operation was undertaken and mitral valve replacement was performed with good post-operative course. Intraoperative examination revealed the detachment of right coronary cusp, prolapsing into left ventricle. There was no findings suggestive of bacterial endocarditis. Histological examination of excised valve revealed non-specific chronic inflammatory changes with focal area of myxomatous changes.
