[Clinical study of prophylactic cranial irradiation for small-cell lung cancer].

OBJECTIVE: To study the influence of prophylactic cranial irradiation (PCI) on survival and brain metastases in patients with limited small-cell lung cancer (SCLC). METHODS: Fiftyone patients with limited SCLC under complete remission after chemoradiotherapy were randomly divided into prophylactic cranial irradiation (PCI) group (n = 26) and control group (n = 25). Patients in PCI group received irradiation at a dose of 25.2-30.6 Gy. Survival rates were analyzed and compared by life table and Long-Rank, incidence of cranial metastases by chi 2 test. RESULTS: The clinical features of patients such as age, sex, effect of treatment before PCI were similar between the two groups. The incidence of cranial metastases was 3.8% in PCI group in contrast to 28% in the control group (P < 0.05). The 1, 2, 3-year survival rate was 84.6%, 73.1%, 42.3% respectively in PCI group and 72%, 40%, 32% respectively in the control group. The differences between the two groups of petients were statistically insignificant. No serious sequela was observed in patients receiving PCI. CONCLUSION: PCI decreases the incidence of cranial metastases for patients with limited SCLC following complete response to chemoradiotherapy, but it does not improve survival.

[Sequencing and cloning of human estrogen beta receptor (hERbeta) cDNA in human granulosa].

OBJECTIVE: To analyze the nucleotide sequence of cDNA and deduce the amino acid sequence of human estrogen receptor (hERbeta) in human granulosa cells. METHODS: Granulosa cells were prepared from the ovary of IVF-ET cases by Percoll technique with Dulbecco's modified eagle medium. RNA was extracted with the TRIzol reagent kit, and mRNA was purified with oligo-(dT)-cellulose in a sterile dispocolumn. cDNA was synthesized by RT-PCR using the SuperScript(TM) II RT kit. Amplified products were cloned into the pGEM-T vector and transfected into E. coli XL1-Blue. The nucleotide sequences were determined by repeated sequencing of both strands of alkaline-denatural plasmid DNA using the Sequenase Version 2.0 DNA sequencing kit. The obtained DNA sequence was compiled and analyzed using DNAMAN computer programs. RESULTS: Amplified cDNA of hER beta in human granulosa cells was composed of 1 495 bp, containing a 1 431 bp open reading frame. The predicted ER beta protein consisted of 477 amino acids. The predicted ER protein included 4 function domains: A/B, C, D, and E/F domains. Among these domains, C domain, richly containing cysteine, was the DNA-binding domain (DBD), and E/F domain was the ligand-binding domain (LBD). CONCLUSION: Detection of ERbeta in the ovary granulosa cells played an important role in explaining the self-endocrine function of estrogen.

[Evaluation of modified maze procedure by Doppler tissue imager].

Since 1998 the cardiac surgeons of WCUMS have performed modified maze procedure for treating the patients with rheumatic mitral valve disease accompanied with chronic atrial fibrillation (Af). In this study, echocardiography, especially Doppler Tissue Imager (DTI), was used to evaluate atria contraction and function of such patients before and after the operation. 14 rheumatic mitral disease patients, aged 27-60 years, were chosen; among them, 11 had mitral stenosis, 3 mitral regurgitation. The control group consisted of 16 cases, aged 24-67 years, with normal echocardiogram. Before and half-1 year after the operation, left atrium, left ventricle, right atrium and ventricle were measured, and cardiac function and posterior wall motion of left atrium were measured. Velocity of mitral valve annual and tricuspid valve annual was measured in DTI. The results showed: There was no age difference between the two groups. The motion of posterior wall of left atrium was 4.18 +/- 0.37 mm in the operation group, 4.09 +/- 0.29 mm in the control group (P > 0.05). No difference in velocity of mitral/tricuspid valve annual between the two groups was observed; 3 peaks of mitral valve annual were S 0.11 +/- 0.02, e 0.11 +/- 0.01, a 0.09 +/- 0.01 (m/s) in operation group, and S 0.08 +/- 0.00, e 0.10 +/- 0.01, a 0.08 +/- 0.01 (m/s) in control group, (P > 0.05, respectively); 3 peaks of tricuspid valve annual were S 0.10 +/- 0.01 vs 0.11 +/- 0.08, e 0.11 +/- 0.08 vs 0.10 +/- 0.01, a 0.10 +/- 0.10 vs 0.09 +/- 0.01 (m/s), (P > 0.05, respectively). The diameter of left atrium after operation in cases of mitral stenosis decreased significantly, compared with that before operation (P < 0.01). The diameter of right atrium after operation decreased significantly, compared with that before operation (P < 0.01). There was no significant change in right ventricle/left ventricle/EF and FS. The diameter of left atrium after operation in cases of mitral regurgitation decreased significantly, compared with that before operation (P < 0.01). The diameter of left ventricle after operation decreased significantly, compared with that before operation (P < 0.05). No significant change in right ventricle/right atrium/EF and FS was noted. The authors conclude the DTI, as a new diagnostic method in echocardiography, has high sensitivity and is less influenced when used to observe motion of atrium-ventricle annual influenced. It can avoid the error caused by deviation of blood displacement. In the rheumatic mitral valve disease patients who had mitral valve replacement and modified maze procedure, atrial contraction respores, the risk of thromboembolism decreases and prognosis improves. So the operation is worth doing.

[Study of spontaneous echo contrast in patients with rheumatic mitral stenosis by transesophageal echocardiography].

Spontaneous echo contrast (SEC) is common in the areas of slow blood flow. This study was conducted to examine the correlation of SEC and left atrial appendage function, atrial fibrillation (Af) and thrombus in patients with rheumatic mitral stenosis. The transthoracic and multiplane transesophageal echocardiograms were performed on 117 patients(mean age 42.22 +/- 10.79 years). Left atrial size, left atrial appendage area, mitral valve area, left atrial appendage flow peak emphatic velocity and full velocity were measured. The presence of SEC and thrombus in the left atrium and appendage was noted. The results showed that among the patients, 57 had normal sinus rhythm, 60 had Af; 19 had thrombus, 98 had no thrombus; 71 had SEC and 46 had no SEC. Compared with the patients without SEC, the patients with SEC showed greater incidence of thrombus (28.76% vs 0, P < 0.05), higher rate of Af (69.01% vs 23.91%, P < 0.05), larger LAD (46.93 +/- 0.85 mm vs. 41.76 +/- 1.05 mm, P < 0.05), larger LAA area (8.06 +/- 0.32 cm2 vs 6.92 +/- 0.31 cm2, P < 0.05), smaller MVA area (1.07 +/- 0.23 cm2 vs 1.32 +/- 0.35 cm2, P < 0.05), lower LAA flow peak emphatic velocity (0.17 +/- 0.01 m/s vs 0.32 +/- 0.02 m/s, P < 0.05), and lower LAA full velocity (0.18 +/- 0.01 m/s vs 0.31 +/- 0.03 m/s, P < 0.05). These findings indicate that the presence of SEC is associated with significantly greater incidence of thrombus, higher rate of Af, larger LAD and LAA area, smaller MVA area, lower LAA flow peak emphatic velocity and full velocity. Atrial fibrillation, the diameter of left atrium and the area of mitral valve are independent predictors of SEC. SEC is significantly associated with LAA function and thrombus formation.

[Relationship between the changes of serum free fatty acids and insulin resistance in type 2 diabetics].

The purpose of this study was to investigate the changes of serum free fatty acids (FFA) in fasting state and absorptive state and the relationship between FFA changes and insulin resistance in the patients with type 2 diabetes (DM2). 75 g of glucose were given to 60 patients with DM2. Fasting serum FFA, serum insulin levels and the same parameters 2 h after glucose load were measured by calorimetric and RIA methods. Fasting and 2 h after oral glucose tolerance test (OGTT) plasma glucose(PG) levels were also assessed by using glucose oxidase method. The results showed the levels of serum FFA, PG and insulin of fasting and after glucose load in the patients were significantly increased and their insulin-sensitive (ISI) was remarkably decreased as compared with those in 30 normal controls, P < 0.05. Multiple stepwise regression analysis showed that the FFA level of 2 h after OGTT was negatively correlated with ISI, P = 0.0304. The results suggest that in patients with DM2, the levels of fasting and absorptive FFA are significantly elevated, which is negatively correlated with the decline of ISI, implying the association of abnormal fasting and absorptive FFA and insulin resistance in patients with type 2 diabetes.

[A computer-aided analysis system for counting the synaptic numerical density based on disector technique].

The change in the numbers of synapse is of importance in various kinds of physiological and pathological processes. The numerical density is the structural parameter measuring up the numbers of the particles in the space. A new stereological technique, the disector technique, has been described to count the number of particles in a given volume of tissue. It is a technique for counting the number of arbitrary particles in 3-dimensional space using adjacent parallel sections at known distance apart. In this article also discussed is the software for computer assisted count of the synaptic numerical density based on the physical director technique with Visual Basic 4.0.

[Induction of skin immune tolerance of mouse to rat xenogeneic transplantation].

OBJECTIVE: To explore a moderate and effective project which will be clinically suitable to induce donor-specific tolerance across xenogeneic barriers. METHODS: 4 x 10(7) Lewis rat bone marrow cells were infused to 300 rad total body irradiated (TBI) C57BL/6 (B6) mice, combined with intraperitoneal administration of cyclophosphamide (CTX) and intravenous injection of anti-mouse CD4 monoclonal antibody. Recipient B6 mice were characterized for the tolerance status with donor Lewis rat skin graft, mixed lymphocyte reaction (MLR), and delayed-type hypersensitivity (DTH) assays after 30 days. Adoptive transfer and the effect of IL-2 on MLR were detected to further explore the tolerance mechanism. RESULTS: Survival time of donor rat skin grafts was specifically prolonged in the tolerant B6 mice. The results of MLR and DTH assays showed donor-specific hyporeactivity, while the tolerant B6 mice were still immunocompetent to MHC-disparate third party BALB/c mouse or DA rat stimulator cells. CONCLUSIONS: A reliable xenogeneic transplantation tolerance of mouse to rat was achieved by this project of tolerance induction. The result of in vitro and in vivo adoptive transfer of spleen cells from tolerant B6 mice demonstrated that the suppressor cells were unlikely to exert effect in the tolerance. The inhibition of specific MLR could be reversed by adding IL-2, indicating that clonal anergy instead of clonal deletion is responsible for the tolerant maintenance.

Cell cycle-dependent regulation of early developmental genes.

Cell cycle phase at the onset of development in Dictyostelium influences cell fate. Cells in the G2 phase, which tend to become spores, show a more rapid induction of expression of the cell surface receptor involved in the chemotaxis. We show that differential induction of developmental expression is restricted to some transcripts, including those encoding proteins required for chemotaxis, and thus is not due to general transcriptional repression during mitosis. We also show that cells showing rapid induction of one such gene are preferentially located at the centre of early aggregates. These results are consistent with cells derived from G2 phase being at the centre of early aggregates because selective differences in gene regulation render them more efficient at aggregation.

Germ-line transmission of transgenes in Xenopus laevis.

Adult Xenopus laevis frogs made transgenic by restriction enzyme-mediated integration were bred to test the feasibility of establishing lines of frogs that express transgenes. All of the 19 animals raised to sexual maturity generated progeny that expressed the transgene(s). The patterns and levels of expression of green fluorescent protein transgenes driven by a viral promoter, rat promoter, and four X. laevis promoters were all unaffected by passage through the germ line. These results demonstrate the ease of establishing transgenic lines in X. laevis.

Interaction of the N-terminus of sterol carrier protein 2 with membranes: role of membrane curvature.

Although neither the physiological function nor the mechanism of action of sterol carrier protein 2 (SCP(2)) is yet completely clear, it is thought that SCP(2) interacts with membranes to elicit its biological effects. The results presented here show that the SCP(2) N-terminus, composed of two amphipathic alpha-helices, interacted preferentially with highly curved but not lower-curvature membranes containing anionic phospholipid. CD spectra of SCP(2) showed up to 1. 2-fold increased alpha-helical content, on the interaction of SCP(2) with small unilamellar vesicles (SUV) (median radius 10-14 nm) but less with large unilamellar vesicles (LUV) (median radius 52-60 nm). Although enhanced interaction with the SUV membranes was due in part to the radius of curvature and to the greater exposure of acidic phospholipid in the outer leaflet of the bilayer, simply increasing the molar percentage of acidic phospholipid in the LUV membranes had much less effect on SCP(2) binding. A similar preferential interaction was observed with highly curved SUV as opposed to LUV for the SCP(2) N-terminal peptide (1-32)SCP(2) as well as structurally modified peptides in the order (1-32)SCP(2)=(10-32)SCP(2)>(1-24)SCP(2)>>(1-E20-32)SCP(2). The CD results were confirmed with an independent filtration binding assay, which showed that SCP(2) bound 5-fold more to SUV than LUV, whereas its N-terminal peptides bound up to 4-fold better in the order (1-32)SCP(2)=(10-32)SCP(2)>(1-24)SCP(2)>(1-E20-32)SCP(2). Finally, cholesterol potentiated the binding of SCP(2) and N-terminal peptides to anionic-phospholipid-containing SUV but not LUV. These findings were consistent with the SCP(2) N-terminus being a membrane-binding domain that was highly dependent on membrane surface curvature as well as on lipid composition.

Human uterine leiomyomata express higher levels of peroxisome proliferator-activated receptor gamma, retinoid X receptor alpha, and all-trans retinoic acid than myometrium.

Uterine leiomyomata are the main indication for a hysterectomy in the United States and occur in 25% of women >35 years. Because uterine leiomyomata can form when ovariectomized guinea pigs are exposed to estradiol and retinoic acids, we tested whether human leiomyomata had high levels of retinoic acids and related nuclear receptors. Compared with normal human myometrium, leiomyomata had 3- to 5-fold higher levels of peroxisome proliferator-activated receptor gamma (PPARgamma), retinoid X receptor alpha proteins, and all-trans retinoic acid, but only during the follicular phase of the menstrual cycle. 9-cis Retinoic acid was undetectable in either leiomyomata or myometrium. PPARgamma mRNA levels were lower in leiomyomata than myometrium, but only during the luteal phase of the cycle. A PPARgamma agonist, troglitazone, was given to guinea pigs along with estradiol and all-trans retinoic acid and produced the largest leiomyomata seen to date in this model. By contrast, no tumors formed when troglitazone was given alone or with estradiol or when troglitazone was given with estradiol and 9-cis retinoic acid. New therapies for human leiomyomata may emerge by combining antagonists for PPARgamma and retinoid X receptor alpha with selective estrogen receptor modulators.
