[Noninvasive estimation of site of lesion of left anterior descending artery with stress 201Tl myocardial SPECT images].

Noninvasive estimation of site of lesion of left anterior descending artery (LAD) is important because of high morbidity of LAD lesion. Utility of stress 201Tl myocardial SPECT images for predicting site of LAD lesion characteristics of ischemic region induced by LAD lesion in the basal coronal image were studied in 53 patients with localized significant LAD lesion. Right margin of ischemic region at basal plane of SPECT images was determined by circumferential maximum count profile analysis of stress 201Tl myocardial images. Right margin of ischemic region induced by the just proximal lesion was 21 +/- 11 degrees, that by proximal lesion was -6 +/- 10 degrees and that by mid portion was -36 +/- 17 degrees. Sensitivity of right margin of ischemic region 0 for just proximal lesion was 100% (20/20) and that of right margin of ischemic region 0 for mid portion lesion was 100% (18/18). These results showed the possibility of detecting site of LAD lesion by stress 201Tl myocardial SPECT images.

Alpha-smooth-muscle actin and desmin expressions in human neuroblastoma cell lines.

The neural crest gives rise to a variety of tissues, including peripheral neurons, Schwann cells, melanocytes and ectomesenchymal cells, which include the smooth-muscle cells of large arteries. Cell lines derived from neuroblastoma (a neural-crest tumor) exhibit at least 2 distinct morphological cell types, a neuroblastic phenotype (N-type) and an epithelial-like phenotype (S-type) with characteristics of substrate-adhesiveness. We have analyzed 17 human neuroblastoma cell lines using a panel of monoclonal antibodies (MAbs) against cytoskeletal proteins. Three neuroblastoma cell lines (KP-N-SI, KP-N-YN and SMS-KCN) bound an alpha-smooth-muscle actin antibody. In addition, one of these lines (KP-N-SI) bound anti-desmin MAbs as determined by indirect immunofluorescence. A total of 8 cloned cell lines were obtained from the above parent cell lines. These were composed of either N- or S-type cells and were confirmed to have the same neuroblastoma origin as each parent cell line by chromosomal analysis. Alpha-smooth-muscle actin and desmin were demonstrated in the S-type cloned cells by indirect immunofluorescence, as well as by 2-dimensional Western blot analysis. These results were confirmed by Northern blot analysis using a specific probe (pSH alpha SMA-3' UT) to human alpha-smooth-muscle actin mRNA. These ascertain the presence of alpha-smooth-muscle actin and desmin in neuroblastoma cell lines. These data show that, in addition to giving rise to cells with neural, Schwann-cell and melanocyte markers, neuroblastoma can also give rise to the cells expressing smooth-muscle cell markers.

A high frequency of detection of Helicobacter pylori in whitish exudate of gastric ulcer.

Helicobacter pylori has been implicated in the pathogenesis of chronic gastritis and peptic ulcer. However, no report to date has been made on the presence of H. pylori in the whitish exudate of peptic ulcers. We examined the biopsy specimens of 52 patients (42 men and 10 women, aged 29-78, not taking steroids or nonsteroid antiinflammatory drugs), and evaluated the frequency of detecting the organism in the whitish exudate of gastric ulcers in comparison with that in the ulcer border, gastric fundic gland, and pyloric gland using microaerobic culture and acridine-orange stain. H. pylori was detected in high rates: 38 (73%) of 52 cases by culture, and 50 cases (96%) by staining of the whitish exudate, 42 (81%) of 52 cases by culture, and 46 cases (88%) by staining of the ulcer border. The frequency for the other regions were 81% by culture and 90% by staining of the fundic gland, and 81 and 90% of the pyloric gland, respectively. Of all sites studied by the staining method, H. pylori was detected at the highest frequency in the whitish exudate. The fact that many H. pylori live in the whitish exudate of gastric ulcer, suggests a causal relationship between H. pylori and gastric ulcer.

Long-term effects of mass screening for neuroblastoma in infancy.

Eighty-four neuroblastoma patients were treated at the Department of Pediatrics, Kyoto Prefectural University of Medicine, during the 27 years from 1962 to 1988. They were divided into three groups: 35 cases in phase 1, the 12 years before mass screening from 1962 to 1974; 22 in phase 2, the 8 years after the onset of mass screening by a qualitative vanillylmandelic acid (VMA) test from 1974 to 1982; and 27 in phase 3, the recent 6 years from 1983 to 1988, after the introduction of mass screening by quantitative assay for VMA and homovanillic acid using high performance liquid chromatography. The clinical findings of these patients were compared for each phase. In Kyoto, the number of annual neuroblastoma cases diagnosed under 1 year of age increased from 0.58 in phase 1 to 1.50 in phase 2 and 3.17 in phase 3. The number of cases over 2 years of age decreased from 2.00 in phase 1 to 0.88 in phase 2 and 1.00 in phase 3. Survival rates increased from 17.1% (six of 35) in phase 1 to 54.5% (12 of 22) in phase 2 and 85.2% (23 of 27) in phase 3. The annual number of neuroblastoma deaths decreased from 2.42 in phase 1 to 1.25 in phase 2 and 0.67 in phase 3. Mass screening for neuroblastoma in infancy has increased the long-term survival rates of neuroblastoma cases treated at this hospital.

[Utility of rest Tl-201 myocardial SPECT images for estimating myocardial viability].

Rest Tl-201 myocardial SPECT images were underwent in 19 patients with anterior wall myocardial infarction under PTCR one month after the onset of acute myocardial infarction. The relationship between shortening rate (SR) of the left ventricle estimated by radial method and corresponding %Tl-201 uptake obtained by circumferential profile analysis was studied. For each patients 10 points on the anterior wall were taken into consideration. Seven patients showed depressive flat profile curve and 12 patients showed slant curve, in which %Tl-201 uptake continuously decreased from base to apex. Well correlation was obtained between SR and %Tl-201 uptake (SR = -41.2 +/- 1.03% Tl-201 uptake, r = 0.54, p less than 0.001). Average %Tl-201 uptake corresponding to SR = 0 was 46.3 + 6.8% (36-58). The sensitivity of %Tl-201 uptake greater than or equal to 60% for SR greater than or equal to 20% was 97% (87/90) and specificity was 69% (31/45). Tl-201 myocardial SPECT images were useful for estimating myocardial viability and %Tl-201 uptake was one of excellent parameters for quantitatively estimating myocardial viability.

[Human neuroblastoma cell lines having smooth muscle cell markers].

[Clinical study on long-term treatment of chronic hepatitis C with interferon].

We treated 41 chronic hepatitis C patients with recombinant interferon alpha 2a, 6 X 10(6) IU/day, for three weeks daily followed by intermittent therapy, 3 X 10(6) IU/day, three times weekly for 6 months and more. After 6 months of intermittent therapy, serum aminotransferases (AST, ALT) decreased to normal or nearly normal levels in 29 of 41 patients (70.7%) with histological improvement. The HCV (C100-3) antibody disappeared during and after IFN therapy in 7 of 34 HCV antibody positive patients (20.6%). Serum aminotransferases levels of all such patients were normalized or nearly normalized. The IFN antibody was detected in 8 of 41 patients (19.5%) including one whose IFN antibody was already present before starting IFN therapy. IFN treatment was discontinued in 22 of 41 patients (53.7%) because they responded completely or nearly completely to IFN therapy. All of the 22 patients have been maintaining normal aminotransferase levels for 1 to 24 months (mean, 12 months) after the treatment period. We conclude that long-term IFN therapy is greatly beneficial in controlling hepatic inflammatory changes in chronic hepatitis C.

[Characteristics of Tl-201 myocardial SPECT images of patients with coronary artery lesion of diagonal branch].

Noninvasive detection of coronary artery lesion of diagonal branch is important. Utility of Tl-201 myocardial SPECT images for this purpose was studied in 9 patients with angina pectoris and 4 patients with myocardial infarction, in whom localized stenosis more than 75% was noted at diagonal branch. Position of basal margin of abnormal region in the coronal images lay between -30 degree and 30 degree and basal abnormal region was very narrow. Width of mid portion of abnormal region lay from 5 degree to 70 degree. Abnormal region due to diagonal branch was not noted in septal region. These results showed the utility of Tl-201 myocardial SPECT images for detecting coronary artery lesion of diagonal branch.

Characterization of an embryonal rhabdomyosarcoma cell line showing amplification and over-expression of the N-myc oncogene.

A parent rhabdomyosarcoma cell line designated SCMC-RM2 was established from bone-marrow tumor cells taken from an 11-year-old girl with an embryonal rhabdomyosarcoma. Subsequently a cloned SCMC-RM2-1 cell line was isolated from a parent line. These cell lines grew as adherent monolayers in liquid culture with a doubling time of 50 and 52 hr, respectively. In addition, colonies were established in soft agar, which grew in a dose-dependent fashion with a cloning efficiency of 0.7 and 0.8%, respectively. Chromosomal analysis showed these cell lines had neither double minutes nor homogeneously staining regions. Chromosome number ranged from 61 to 93, translocation; t(9;13)(p22;q14) was identified, and no alteration of chromosome 2 was observed. Surface membrane antigen profile of parent and cloned lines by using a panel of 24 monoclonal antibodies (MAbs) excluded the possibility of these being neuroblastoma cell lines. In addition, MAbs to the cytoplasmic protein desmin, myoglobin, muscle actin (alpha and gamma) and alpha-sarcomeric actin reacted with these cell lines, SCMC-RM2 and SCMC-RM2-1 being thus identified as rhabdomyosarcoma. Southern blot analyses revealed 8- and 7-fold amplification of the N-myc gene in SCMC-RM2 and SCMC-RM2-1 as compared with the promyelocytic cell line HL60. Over-expression of the N-myc mRNA was noted over control cell lines.

[A follow-up survey for hearing aids evaluation using speech maps].

A new method for assessing the selected hearing aids was developed and applied 53 patients with hearing impairment. Our previous paper showed that the average confusion distance which was introduced in this method was useful for estimating the hearing aids. We carried out a follow-up survey in order to measure the usefulness of this method after 3 or 4 years practical hearing aid use. There were 34 patients out of 53 from whom we could obtain adequate information about their hearing aids. They were asked to rate their satisfaction with the recommended instrument in five degrees. Fifteen of the 34 patients who had purchased the recommended hearing aid rated it very satisfactory; four rated the aid satisfactory; eight rated the aid helpful; six rated the aid sometimes helpful; and one rated the aid unsatisfactory. The Spearman's rank correlation coefficient between the change in the speech discrimination score with and without a hearing aid and the user's evaluation was 0.34. The Spearman's rank correlation coefficient between the change in the average confusion distance and user's evaluation was 0.41. The value 0.34 is not significant, but the value 0.41 is significant at the 1% level of significance. Therefore, we think that the average confusion distance should be taken into consideration as well as the speech discrimination score at least for predicting successful hearing aid use. Finally we feel that there is need for the inclusive system which is able to estimate both the speech mediated communication ability and user's satisfaction.

[Strength of teeth to fracture by impact. Testing of fractural strength against impact force, using replication models of human permanent teeth].

Traumatic injury anterior teeth is often encountered clinically. A number of clinical and statistical studies have so far been undertaken for the incidences, causes and ages in terms of the site of fracture. However, little has been attempted at fundamental estimation of the fractural resistance of anterior teeth that are susceptible to fracture. This has been studied in only an erect position of eruption by Nose or Chin. Changes in resistance of teeth in a twisted position or at different angles of eruption still remain unstudied. We took an interest in the relation between physicopositional conditions of human teeth and changes in their fractural strength against impact. Determination of the resistance of teeth to a variety of experimental conditions was attempted by use of an Izod-type tester. Epoxy resin replication models of human anterior teeth were used as experimental specimens, since human teeth were considered inappropriate because of their large individual differences and an anticipated difficulty in precise reproduction of the angles of eruption and torsion. The following results were obtained; (1) In the replication models of teeth erected positioned at 90 degrees vertically to the horizontal plane, the fractural strength against impact was highest for the maxillary cuspid model and was lowest for the mandibular central incisor model. (2) In the replication models of teeth with their mesial or distal incisal margin twisted 45 degrees medially, the fractural strength was reduced for all the teeth except the maxillary central incisors. (3) In comparison of the strength between the replication models with the mesial incisal margin twisted 45 degrees medially and those with the distal incisal margin twisted likewise am marked reduction in strength was found for the maxillary cuspid model prepared in the former condition of torsion. For the mandibular cuspid model, a relatively large reduction of the strength was found in the latter condition of torsion. (4) In comparison of the strength of teeth positioned at various angles, the strength was decreased by slanting either labially or lingually for all the replication models except the maxillary and mandibular cuspid models.

[Relationship between molecular structure of halogenated methane compounds and their hypotensive effect in rats].

In this study gases of twelve halogenated methane compounds were administered by inspiration to Wistar rats in order to elucidate the relationship between molecular structure of the compounds and their hypotensive effect, and to develop new inhalational circulatory control agents. The correlation between the hypotensive effect of the compounds evaluated from the experimental data and computed van der Waals volume as well as surface area following molecular mechanics calculations of structure and energy, was investigated. The results showed that the compounds with van der Waals volume and surface area more than 73 A3 and 100 A2, respectively, had the hypotensive effect. In conclusion, our data suggest that the hypotensive effect of inhalational agents requires greater molecular sizes than regular ones.

The 24 h-urinary excretions of albumin, beta 2-microglobulin and N-acetyl-beta-D-glucosaminidase activity in children with IDDM.

Microalbuminuria in diabetics is considered to be a sensitive indicator of early diabetic nephropathy. In this paper, 24 h-urinary excretions of albumin, BMG and NAG activity were measured in forty-one children with insulin-dependent diabetes mellitus. Moreover, the relationships between the urinary excretions of these substances and various clinical parameters of diabetes were analyzed. The mean values (mean +/- SD) of 24 h-urinary albumin, BMG and NAG activity in the diabetic children were 8.0 +/- 10.6 mg/day, 61.2 +/- 69.0 micrograms/day and 1.87 +/- 1.30 U/day, respectively. No significant differences were found between diabetic children and normal controls for these mean values. Nor were significant correlations between the various clinical parameters of diabetes and the urinary excretions of any of these substances found. However, nine of the forty-one diabetic children (22.0%) had higher levels of these urinary substances than those (mean + 2SD) in normal controls. Screening of the 24 h-urinary albumin, BMG and NAG activity should be performed routinely in young patients with diabetes.

Hearing aid evaluation using psychoacoustical proximity between Japanese monosyllables.

To facilitate hearing aid selection, a new method was developed to assess the qualitative changes in speech perception with and without a hearing aid. This method utilizes maps of the Japanese monosyllables where the degrees of confusion are represented by distances between the monosyllables. On these maps, perceptual failures of the monosyllables can be simply analyzed by connecting confused stimuli and the corresponding wrong responses. The qualitative characteristics of monosyllable confusion for 53 patients with hearing impairment were comparatively analyzed with and without hearing aids. Results show that this method is useful for describing the qualitative effects of hearing aids on speech perception, and that it thus provides important information for hearing aid selection.
