Quality assurance in hospitals.

The scarcer resources are in a hospital, the more carefully the quality of the services rendered--the type and the extent--must be ensured by means of resolute control. We must not reach a point where sparse resources lead to a poorer quality in services or where patients do not receive the treatment required. In this article the author gives a survey of the various methods of quality rating, giving the individual hospital the possibility of comparison with other hospitals.

Renal handling of nourseothricin.

Nourseothricin 1 is preferentially excreted via kidney and signs of nephrotoxicity can be observed after its administration. Renal handling of 1 was characterized in experiments on renal cortical slices under various experimental conditions. Following administration in vivo or in vitro the renal tubular transport system for organic anions (p-aminohippurate, PAH) is not influenced by 1. There is a high degree of accumulation of 1 in renal cortical slices. In contrast to PAH accumulation there is no influence of nitrogen atmosphere, simultaneous administration of PAH, probenecid or trishydroxyaminomethane on 1 accumulation. Age dependent differences in 1 accumulation does not exist. Our data show a distinct uptake of 1 in kidney tissue. Accumulation of 1 in renal cortical slices is caused by binding; an active tubular transport of 1 in renal cortical slices could be excluded.

Validation of biotechnological production processes.

Due to the biological synthesis of biotechnologically produced pharmaceuticals the product quality and safety of the drug is influenced by various factors. The correct nucleotide sequence and stability of the host cell/vector system provide the corresponding amino acid sequence of the protein. The posttranslational processing of the protein requires a well characterized production cell line. Suitable equipment for fermentation allowing a sterile production of the producing monoculture and consistent conditions are the basic requirements for the validation of the fermentation process. A constant specific productivity is one of the major criteria for the reproducibility of the production. For the validation of recovery and purification it is necessary to examine yield after each process step, product quality before and after each single process step and purification factors for removal of contaminating proteins, nucleic acids and potential viruses. In addition to the validation of the entire production process, reproducibility of quality of the formulated product has to be determined by a number of protein analytical, immunological and biochemical test methods concerning the identity, purity, safety and potency of the drug.

Use of alternative dose schedules for the determination of sublethal doses of potential antiviral drugs in mice.

In mice the ratio between maximally tolerated sublethal doses (MTD) after single and repeated administration of benzoxazolyl-2-formyl-S-ethyl-isothiosemicarbazone (ZIMET 111/74) was found to be different after s.c., p.o. or i.p. administration. Comparing the treatment schedules with relatively high or low initial doses, no differences of the total MTD were found. Using MTD after single drug administration (MTDs) as initial doses for repeated administration, the subsequent doses had to be reduced in a different manner for s.c. and p.o. administration to avoid lethality. As a result of these investigations a rule is proposed for compounds with similar mode of action and comparable pharmacokinetics to ZIMET 111/74 to calculate dose schedules for antiviral in vivo screening related to acute as well as to subacute toxicity data.

[Frequency and clinical relevance of delta virus infection in HBsAg carriers in the Berlin area].

The frequency of delta virus infection, immunoserological parameters and degree of liver inflammation were studied in 190 HBsAg carriers in the Berlin area from 1976 to 1986. Delta infection was detected in one of 50 HBsAg carriers (2%) in the period from 1976 to 1980. In contrast 19 of 140 HBsAg carriers (14%) with delta infection were found in the period from 1981 to 1986. This group included 12 Germans, 5 Turks and 2 Italians. Only 6 of these subjects belonged to a so-called risk group: 3 drug addicts, 2 homosexuals and 1 hemophiliac. Eighteen of the 20 patients with HDV infection showed progressive liver disease in the follow-up period. Nine cases developed complete liver cirrhosis over five years. Variable transaminase levels and elevation of immunoglobulin G were recorded. Humoral autoimmune phenomena were rarely observed. The increasing frequency of HDV infection in the Berlin area is presumably related to tourism, national origin and membership in specific risk groups. The data in our study underline the importance of effective prophylaxis by active immunization with HBsAg vaccines.

Common dimensions of psychopathology from the MMPI and the Basic Personality Inventory.

The MMPI and the Basic Personality Inventory (BPI) were administered to 235 male hospitalized psychiatric patients who were undergoing treatment for alcoholism. The 13 validity and clinical scales from the MMPI and the 12 scales from the BPI were subjected to principal axes factor analysis and Varimax rotation. The five factors retained for interpretation, which accounted for more than 96% of the common variance, each had loadings from both the MMPI and the BPI. Factors were identified as Inadequate Impulse Control, Generalized Anxiety, Depression and Somatization, Psychotic Processes, and Depressed Withdrawal. Given the high percentage of variance accounted for by the factors, it was concluded that the MMPI and BPI measure the same broad domain of psychopathology.

Further examination of ontogenetic limitations on conditioned taste aversion.

This study was to resolve a discrepancy in the literature as to the capability of infant rats in acquiring conditioned taste aversion. Previous studies had indicated that during the 1st postnatal week, an aversion to saccharin could be conditioned when paired with lithium chloride (LiCl). Analogous conditioning with sucrose did not seem to occur until the end of the 2nd postnatal week, however, even though sucrose is discriminated from water and preferred before then. We observed that 5- and 9-day old pups express conditioned taste aversion to both saccharin and sucrose flavors that previously were paired with illness induced by LiCl. This learning occurred only when several hours separated cannulation and conditioning. A number of other factors that seemed likely to determine this early learning were found to have no effect. Thus it appears that rats can learn taste aversions very early in life, but only under certain circumstances. The results are discussed with reference to Vogt and Rudy's (1984) conclusions on the ontogeny of taste guided behaviors in the rat.

Structure and elicitor or u.v.-light-stimulated expression of two 4-coumarate:CoA ligase genes in parsley.

We have isolated genomic clones encoding 4-coumarate:CoA ligase (4CL), a key enzyme of general phenylpropanoid metabolism, and have analysed the structure and regulation of the genes contained on these clones. Restriction enzyme and sequence analysis indicated that two distinct 4CL genes, Pc4CL-1 and Pc4CL-2, are represented on the clones and that additional 4CL genes are not present in parsley. Two lines of evidence suggest that each gene is transcriptionally activated by both elicitor and u.v. irradiation: cDNA clones corresponding to each gene were found in cDNA libraries made with RNA from both elicitor-treated and u.v-irradiated cells, and run-off transcripts homologous to a Pc4CL-2-specific intron probe were induced by both treatments. This induction was about half of the induction measured using probes homologous to both genes. The transcription initiation sites of both genes were determined. Comparison of the nucleotide sequences of the two genes 5' to these sites showed that they are highly homologous for several hundred base pairs and that they contain features potentially involved in regulation by elicitor and u.v. irradiation.

Evaluation of the kinin-induced pathomechanisms in the development of ARDS by kallikrein inhibition in vivo.

Our findings indicate that the kallikrein-kinin system might participate in the development of acute respiratory failure. They were assessed by measurements of plasma kininogen levels, blood pressure recordings and lung morphology. Aprotinin plasma concentrations ranging from 4 to 7 KIU/ml proved sufficient to inhibit effectively the hemodynamic and morphologic effects caused by tissue kallikrein infusion.

Effect of aprotinin and C1-esterase inhibitor on activation of the plasma kallikrein-kinin system in vivo.

Infusion of dextran sulfate in pigs rapidly activates the intrinsic cascade of the coagulation system including plasma prokallikrein and leads to release of kinin by limited proteolysis of kininogen. In this model, strong suppression of the DXS-induced activation of the plasma kallikrein-kinin system is seen at plasma aprotinin levels above 400 KIU/ml and C1INH levels above 4 U/ml.

[Beta blockade with celiprolol in tardive dyskinesia patients treated with neuroleptics].

The influence of celiprolol on the symptoms of tardive dyskinesia was compared with placebo in a randomized double blind study. 17 female patients were treated with a single daily dose of 200 mg celiprolol and 18 female patients received placebo for a period of 3 months. All patients got additional neuroleptic treatment. Celiprolol produced a small decrease in heart rate and systolic blood pressure, but had no influence on the diastolic blood pressure. The effects of celiprolol on the symptoms of tardive dyskinesia were similar to those of the placebo; in both groups improvements and deteriorations were observed. One patient of the celiprolol group became symptom free. Two cases of collapse occurred after 10 weeks of treatment with celiprolol. In one case collapse was associated with diarrhoea, in the other case the patient had preexisting hypotensive circulatory dysregulation. Sporadic reports about the effect of propranolol in patients with tardive dyskinesia might reflect the normal progress of the disease or the effect might depend on the different pharmacological profile of propranolol.
