Surgical treatment of complete atrioventricular canal defect with an endocardial cushion prosthesis.

In order to obtain better operative results with less postoperative atrioventricular valve regurgitation, we have developed a new method of repair for the complete atrioventricular canal with a composite prosthetic patch called an endocardial cushion prosthesis, in which horizontal wings are attached to a vertical patch on both sides to compensate for the defective atrioventricular valve tissue. Ten patients ranging in age from 8 months to 8 years (mean 28.9 months) have been operated on with this technique since 1975. All patients except one showed severe preoperative pulmonary hypertension (over 50 mm Hg) in mean pulmonary artery pressure. Six patients had trivial-to-moderate mitral regurgitation before surgery. Nine patients survived the operation (operative mortality 10%). Postoperative mitral regurgitation was found in three of seven patients who underwent cardiac catheterization and angiocardiographic examination at an average of 4.4 months after surgery. The grades of mitral regurgitation were trivial in all. Mild-to-moderate tricuspid regurgitation was observed in five patients and there was a correlation between the grades of tricuspid regurgitation and pulmonary/systemic peak pressure ratio (r = .757, p less than .05). This method appears to be advantageous in preventing postoperative mitral regurgitation with low operative mortality. However, tricuspid regurgitation was not well avoided among the patients with persisting pulmonary hypertension.

Interaction between articulatory movements and vocal pitch control in Japanese word accent.

In order to investigate the interaction between articulatory and vocal pitch controls, EMGs of the cricothyroid (CT) and sternohyoid (SH) muscles were recorded during utterances of two-mora Japanese words with rising and falling accent types in the Tokyo dialects. The test words consisted of combinations of open and closed vowels. It was revealed that the changes in CT activity were consistently observed in correspondence with the F0 shift according to the accent types, while the SH muscle was observed to be active both for jaw opening and F0 lowering, with some subject-to-subject difference. It was also observed that the time lag between the decrease in CT activity and the F0 fall varied with a shift in the relative timing of SH activity in one subject, but not in the other. Articulatory jaw opening with SH activation was considered to positively contribute to F0 lowering, but the use of this mechanism appeared to be optional at least in the Tokyo accent.

Reconstructive surgery for acquired valve disease.

There has been a continuing controversy between reconstructive valve surgery and replacement. In this paper, an analysis of clinical and hemodynamic late results of the patients undergoing open mitral commissurotomy (OMC, 135 patients), repair of mitral regurgitation (MAP, 32 patients), and combined valvuloplasty (62 patients) was presented. 1) OMC: The 10-year actuarial survival rate was 86.5% postoperatively. None of the patients required reoperation in this series. Clinical and hemodynamic improvements mainly depended upon the anatomical severity of a stenosed mitral valve. 2) MAP: The 10-year actuarial survival rate was 94% postoperatively. Especially, good to excellent results were obtained in the patients with mitral regurgitation due to ruptured chordae or dilated annulus. 3) Combined valvuloplasty: The 10-year actuarial survival rates postoperatively were 90% in OMC + tricuspid annuloplasty group (31 patients), 100% in MAP + tricuspid annuloplasty group (7 patients), and 87.2% in OMC + aortic commissurotomy group (24 patients). The reconstructive valve surgery provides very small operative risk, very low late mortality and morbidity. Therefore, it should be the treatment of choice for most of the patients with symptomatic valve disease.

[Chemotherapy of hepatocellular carcinoma--with special reference to one-shot intra-arterial infusion of a high dose of adriamycin].

Twenty-two cases of non-operable hepatocellular carcinoma (HCC) were treated with three types of chemotherapy mainly with Adriamycin (ADM). In the group (A), one shot intraarterial infusion of 100 mg ADM into the hepatic artery (9 cases), group (B), one shot intraarterial infusion of 30 mg ADM (3 cases), and group (C), 20-60 mg of systemic administration of ADM (10 cases) were examined. The therapeutic effects and side effects were studied comparatively among these three groups. In terms of tumor regression, RR was 4 cases (44.4%), and MR was 4 (44.4%) in group (A), and PR was 1 (10%) in group (C). The average survival time was 8.3 months (5 cases are still alive) in group (A), 5.7 months in group (B), and 4.2 months in group (C), respectively As side effects, alopecia was found 100% in (A), 33% in (B); leukocytopenia was found 100% in (A), 33% in (B); but no serious complication due to leukocytopenia was observed in any of them. One shot of 100 mg of ADM intraarterial infusion therapy seemed to be superior in therapeutic effects against HCC over other therapies.

X-ray microanalysis of hydrated biological specimens.

Using a wide angle backscattered electron detector (BED), glutaraldehyde fixed or unfixed specimens of biological soft tissues such as hen oviduct, kidney, liver, duodenum of mouse as well as mitochondrial fraction from rat liver were observed under low vacuum (0.3 to 0.5 torr) at magnifications from x300 to 10,000. The backscattered electron images (BEI) of glutaraldehyde fixed hen oviduct were correlated to X-ray microanalysis. Intracellular secretion granules in such unhydrated, uncoated and unstained slices of hen oviduct preserved Ca even after glutaraldehyde fixation. Backscattered electron images of unfixed duodenum and kidney of mice were not satisfactory for observing intracellular structures, though X-ray microanalysis could detect P,S,Cl and K over cut surfaces of such hydrated unfixed tissue. Compared with frozen dehydrated specimen, the hydrated unfixed tissue tends to preserve more potassium and less chlorine. Cytochemical reaction product of succinate dehydrogenase (SDH-ase) activity in isolated mitochondria from rat liver was detectable in hydrated condition. Quick and easy assessment of localized elements in surgical specimens by combination of wet-SEM and X-ray microanalysis will be a new tool for clinical application of SEM. In spite of obvious limitations of this method, especially of its spatial resolution both in backscattered electron image and in bulk specimen X-ray microanalysis, the combination of wet-SEM and X-ray microanalysis provides information which has not been available in the past.
