A restrospective comparison between CT and scintigraphy with 99mTechnetium-phosphate (Tc-P) and 67-Gallium-citrate (GA) has been performed in 8 children with malignant extracranial tumours and in 9 children during concurrent control after treatment. CT was superior to scintigraphy in the primary tumour evaluation with regard size, type and invasiveness of the tumour. In the same way, regression, progression or local recurrence after treatment was better visualized by CT than by Ga and Tc-P scanning. In the detection of secondary spread of the tumour, CT is most sensitive of the modalities in the examination of the lungs, whereas Tc-P schintigraphy still is the method of choice to study metastases in the skeleton.
In an 8 month period 57 scans in 28 children have been performed using an EMI-CT 5005 General Purpose Scanner. Thirteen primary malignant tumours were evaluated, and scanning was of great value in localizing and delineating the lesion, and hence predicting the operability. Furthermore, secondary spread of malignant tumours was easily detected. In the continuing evaluation of treated malignant tumours CT has been used to estimate regression and relapse.
Thirteen patients with ventricular tachyarrhythmias after myocardial infarction underwent surgery at the heart wall or at the coronary vessels. Ten of these patients had an aneurysm of the anterior wall or akinesia of the anterior wall which in some cases was associated with stenosis of the right coronary artery or of the circumflex artery. One patient showed an isolated stenosis of the right coronary artery. One patient died from recurrent ventricular fibrillation during hospitalization, another patient died from reinfarction 4 months later. All the other patients could be discharged and were improved significantly.
A case of measles infection without a rash, which was followed by a severe encephalopathy after two months, is described in a 2 1/2 year old boy. At the age of 8 months he had been irradiated for an inoperable intrathoracic neuroblastoma, and at the time of exposure to measles he was being treated with cyclophosphamide and vincristine. This case closely resembles other cases recently described and termed immunosuppressive measles encephalopathy. The syndrome is believed to represent the effect of measles virus in patients with deficient cellular immunity induced by antineoplastic treatment. The importance of protecting children on immunosuppressive treatment for contracting measles is stressed.
A 6-year-old girl developed progressive symptoms of increased intracranial pressure starting 5 months after BCG vaccination. Thirteen months later craniotomy revealed an epithelioid cell granuloma of the arachnoid occluding the foramen of Magendie. No tubercle bacilli were found on histological examination. Insertion of a Pudenz shunt relieved the symptoms. Six months later generalized BCG infection developed, and in spite of treatment with ethambutol, rifampicin and isoniazid for 10 weeks, death occurred during an episode of increased intracranial pressure. Mycobacterium BCG could be cultured from several organs. The patient showed no obvious evidence of immuno-deficiency as judged on the basis of previous disease history, particle concentration of granulocytes, B and T lymphocytes in peripheral blood, concentration of immunoglobulins in serum, response of lymphocytes to transformation with mitogens and antigens, and histological findings in the thymus and BCG granulomas.

The technique of the locking nail as initially described by Klemm and Schellmann is shown for 12 cases. The advantages of this fixation device can be confirmed. This method presents a valuable adjunct in the assortment of osteosynthetic procedures.

A girl with various congenital malformations developed pancytopenia and hypoplastic bone marrow at the age of 6 year. A chromosome study of lymphocytes showed numerous breaks, gaps and rearrangements, allowing the diagnosis of Fanconi's anemia. Treatment with corticosteroids and splenectomy did not result in hematologic remission. Repeated immunologic studies showed increasingly deficient T cell function as judged by lymphocyte transformation studies and skin test reactivity, whereas T cell number, T/B cell ratio, immunoglobulins, complement factors and neutrophil function were normal. A sever Pneumocystis carinii pneumonitis developed, but was successfully treated with pentamidine, sulfametoxazole with trimetoprim and transfer factor. Improvement of T cell function followed transfer factor therapy. Combined therapy with corticosteroids and androgens caused partial remission of the hematologic abnormalities. The probability of a primary immune deficiency in the patient is discussed.
A new direct spectrometric micromethod for the determination of the available bilirubin binding sites of serum (S-ABBS) using bromphenol blue has been applied to 298 blood specimens collected from infants immediately before starting the first exchange transfusion. A relative number fraction of 0.07 of the material, characterized by having the lowest S-ABBS values, includes seven of the nine cases (i.e. a relative number fraction of 0.78) with autoptically verified kernicterus (KI). The two infants with KI but higher S-ABBS values differed in having severe neurological symptoms before the examination. The mean substance concentration of bilirubin in these nine sera was 288 mumol/l. The correlation of KI to the body mass at birth, to the substance concentration of serum albumin, and to the substance concentration of serum bilirubin is given, and the sources of error concerning the method are discussed. The present method is proposed for determining a type of quantity that is important, among other types of data, in deciding on the need for exchange transfusion in icteric (premature) newborns.
A simple and rapid direct spectrometric micromethod for the determination of the concentration of available bilirubin binding sites of serum is given. The method measures - by means of difference spectrometry - the maximal change in absorbance when serum is added to a buffered solution of bromphenol blue (BPB). This dye fulfills a set of requirements given and examined. The binding properties of bilirubin, BPB, and some related dyes are spectrometrically investigated, and differences between the properties of human and bovine albumin and between such albumin preparations and human serum are shown and discussed.
