[Surgical treatment of fresh injuries of the Achilles tendon].

We report 35 injuries of the Achilles tendon that have been operated on immediately. Clinical follow-up examinations of 23 patients showed the following results: very good and good in 19 cases, satisfactory in 3 cases and unsatisfactory in one case. Immediate surgery of an injury of the Achilles tendon is clearly preferable to conservative therapy. The functional and the cosmetic results are better after an interwoven Bunell's suture than after the interweaving with the plantaris muscle's tendon. Because of the high rate of infection we do not use the "fibrin adhesive" any more.

[The influence of primary closed reduction on the incidence of posttraumatic femoral head necrosis (author's transl)].

24 patients with fractures of the femoral neck were younger than 65. Eight of them were operated on the day of the accident. The fractures of the other 16 patients were immediately reduced, the leg placed in traction and the operation carried out between 2 and 8 days later. Osteosynthesis was performed by means of cancellous bone screws without opening the joint. If this internal fixation was stable, the incidence of femoral head necrosis was reduced to one tenth. For fractures of the group Pauwels III this method was not suitable. Early reductions seems to be of more importance for the final results than immediate surgery.

Vasoactive intestinal polypeptide (VIP) in children with neural crest tumours.

The concentration of vasoactive intestinal polypeptide (VIP) in plasma was measured in 22 children with neural crest tumours (NCT) during a 5-year period; the mean concentration of VIP in plasma was 22.5 pmol/l (interval 2.0-95.0 pmol/l). To establish a reference interval the plasma concentration of VIP was measured in 41 children without tumours; the mean concentration of VIP in plasma was 6.2 pmol/l (interval 0.5-19.0 pmol/l). Of the 22 children with NCT 16 had a plasma concentration of VIP within the normal range while 6 children (27%) had elevated plasma concentration of VIP between 28 and 95 pmol/l. Only one child, whose plasma concentration of VIP was 95 pmol/l had diarrhoea. Elevated plasma concentration of VIP in children with NCT but no diarrhoea has not previously been described. The urinary excretion of vanillylmandelic acid (VMA) was increased in 18 of the children with NCT (82%). In 2 of the children with normal excretion of VMA the concentration of VIP in plasma was elevated. Thus, the plasma concentration of VIP may be a supplement to VMA as a tumour marker in some cases of NCT.

[New classification of epiphyseal-plate injuries with regard to therapy and prognosis (author's transl)].

In this report a new simple classification of epiphyseal-plate injuries with regard to therapy and prognosis is pointed out. The classification can be used for each epiphysis and will be demonstrated on the injuries of the distal lower leg. Type I: There is complete separation of the epiphysis from the metaphysis without and with a metaphyseal fragment. The prognosis for growth is excellent after conservative or operative treatment. Type II: The fracture which is intraarticular (epiphyseal fragment) and the fracture across the full thickness of the epiphyseal plate and through a portion of the metaphysis needs accurate reduction. The prognosis for growth is good only after open operation. Type III: This relatively uncommon injury results from a very severe crushing force applied through the epiphysis to one area of the epiphyseal plate. The prognosis in this type is decidedly poor. The need for regular follow-up observation in these cases is obvious. At the end the good results of 40 patients after epiphyseal-plate injuries of the distal lower leg are presented.

[Late results of radius head resection].

The results after excision of the radial head were good and fair in every case. In our 15 re-examined patients we saw no poor results. Indications for excision were comminuted fractures and fractures of the radial neck without possibility of reduction. Two patients had subluxations of the distal radio-ulnar joint. The subluxations were no more than three millimeters of proximal displacement of the radius after injury. At follow-up the subluxations were no more than 5 millimeters. There was no correlation between the degree of subluxation and the presence of symptoms. Valgus deformity we saw in 12 of 15 cases at follow-up, but the patients had no limitations of motion, no instable elbow and no pain. Prosthesis implantation is not necessary at present because there are no poor end results after only excision.

[Does the fibrin glue accelerate the healing of tendons? (author's transl)].

The question of an accelerating influence of the fibrin adhesive (Immuno AG, Vienna) on healing has been investigated in the suture of heel tendons of 69 rabbits. The achilles-tendon cut through was anastomosed in typical way, but besides in the one group a homologous fibrinogen-cryoprecipitate was applied. The histological and biomechanical evaluations of the anastomosis were carried out after one, two and four weeks. By means of the fibrin adhesive the loading capacity of the healing tendons could not be accelerated. The data of rupture stress were in the control group after one week 1.65 kg (+/- 1.14) and after two weeks 8.66 kg (+/- 1.38) and in the fibrin group 2.13 kg (+/-1.28) respectively 7.76 kg (+/-2.19). The difference between the two groups is not significant. Four weeks postoperatively the test could not be carried out, for the anchorage of the tendons at the material testing machine was not possible for over 15 kg stress-strain. Histologically a distinct difference of the repair process of the tendons operated on could not be seen in the two groups either.

[Plastic repair of the lateral ligament of the upper ankle joint: methods and results (author's transl)].

A review is given about common techniques for plastic repair of the lateral ligament of the upper ankle joint. Own experiences are presented. Indications for plastic repair age: old injury of the ligaments with clinical symptoms; fresh lesions of the ligament; if the tissues of the articular capsule or of the ligament are completely destroyed or if during surgery a chronic insufficiency of the ligaments becomes evident. Contraindications against plastic repair are: a marked arthrosis of the ankle joint, a permanent valgar displacement of the talus, old age and a high risk for surgery. The results of 13 plastic operations of the lateral ligament could be classified as "very good" in ten cases and as "good" in three cases (evaluation according to Marti). In seven cases a modified technique of Watson-Jones has been used, twice Weber's method and in four cases a direct ligament plastic. Those procedures are favoured which come close to the original anatomy, are easy to perform without destroying functional structures and which do not cause an unnecessary restriction of movement.

[Indication for surgical treatment of recent fibular ligament lesions. Comparative study after surgical and conservative therapy].

Results of clinical and or radiological follow-up examination of 134 patients with recent rupture of the lateral ankle ligaments are analysed. 69 cases have been treated conservatively, 65 cases by surgery. With regard to the findings at the first examination after the trauma conclusions are drawn concerning the indication for surgery. Operative findings and results of the treatment show conclusively that surgery is indicated when the opening of the injured ankle joint at strain is 10 degrees or more when compared with the intact side.

Neuroblastoma. Evaluation of 60 cases.

60 cases of neuroblastoma from the departments of paediatrics and paediatric surgery, Rigshospitalet, are reviewed. The overall cure rate was 35%. In localized disease (stages I--II) the prognosis was favourable (cure rate 78%), while the prognosis for disseminated disease (stages III--IV) was poor (cure rate 10.8%). Neuroblastomas in stage IV-S had a favourable prognosis in spite of metastases to the liver, skin and bone marrow. The clinical stage at the time of diagnosis was the only independent prognostic factor in this study.

[Hydrothorax after catheterization of the right internal jugular vein (author's transl)].

Two cases of hydrothorax are reported which developed as the result of intrapleural catheter malposition after cannulation of the right internal jugular vein. Predisposing factors for this rare, but frequently misdiagnosed, complication are discussed. In both cases control chest radiography with small doses of contrast medium as well as aspiration of blood failed to indicate malpositioning. To prevent this complication the use of larger doses of contrast medium and/or control aspiration shortly after the start of infusion are recommended.
