Endogenous endotoxin absorption and the role of intestinal lymphatics.

We investigated the absorption routes of endotoxin derived from the intestine of rabbits with or without thoracic duct lymph drainage (TDLD). The intestinal circulatory disorders induced were superior mesenteric artery occlusion (SMAO) and superior mesenteric vein occlusion (SMVO). Detection and quantitation of endotoxin in plasma and lymph were carried out using a synthetic chromogenic substrate (peptide-4-methyl-coumarin amide, MCA). In the SMAO group, endotoxin levels in portal plasma exceeded levels in lymph from the thoracic duct throughout the experiment, and in the SMVO group, the relationship was reversed. In peripheral arterial blood, endotoxin levels were significantly lower in rabbits with TDLD than in those with intact lymphatic system. Intestinal lymphatics probably play an important role in appearance of systemic endotoxemia in non-septic shock.

Effects of laminin, fibronectin and type IV collagen on liver cell cultures.

The effects on mouse liver cells of laminin, fibronectin and type IV collagen, all of which are the main matrix of the basement membrane, were studied. Laminin, a glycoprotein isolated from cultures of rat yolk sac carcinoma cells, promoted the attachment of mouse fetal liver cells to laminin-coated dishes, but did not have a strong influence upon the attachment of normal adult liver cells. On the other hand, fibronectin which was purified from mouse plasma promoted the attachment of adult liver cells but not that of fetal liver cells. The number of neonatal liver cells attached to the surfaces coated was intermediate between those of fetal and adult liver cells in each matrix. DNA synthesis and cell proliferation during the culture of full-term fetal liver cells in laminin-coated dishes were higher than those in fibronectin- or type IV collagen-coated dishes. The amount of alpha-fetoprotein secreted in the laminin-coated dishes was more than in other groups. No differences in secretion of albumin into media, however, were observed in either group. These results suggest that laminin may be necessary for cell growth, tissue organization and cell differentiation during the normal development of liver in vivo.

Modulation of fetal mouse liver cells cultured on a pigskin substrate.

Mouse fetal liver cells cultured on a pigskin epidermal substrate grew for 7 weeks. Different enzymes and proteins, i.e. gamma-glutamyl transferase (GGT), nonspecific esterase (NE), lactic dehydrogenase (LDH), alpha-fetoprotein (AFP), and albumin were studied histochemically and/or biochemically. The activity of GGT was high at the beginning of culture and then decreased rapidly. The activities of NE and LDH were high during the culture. Release into the media and localization of AFP suggested active synthesis during the early stages. AFP levels gradually decreased and could be demonstrated only in trace amounts after 3-4 weeks of culture. On the other hand, the production of albumin was weakly evident early and became more and more evident after the second week in culture. Hydrocortisone modulated AFP and albumin production. The effect of hydrocortisone was to prolong expression of AFP and to reduce expression of albumin. Electron microscopic observations showed that the cultures consisted of organelle-rich parenchymal cells associated with the pigskin basement membrane by pseudopod-like structures. These results indicate that fetal mouse parenchymal cells were cultured and modulated on a pigskin epidermal substrate.

Effects of basement membrane matrix on the culture of fetal mouse hepatocytes.

The effects of laminin, fibronectin and type IV collagen, all of which are major matrix components of the basement membrane, upon fetal mouse hepatocyte culture were studied. Among these matrices, laminin showed the greatest effect on DNA synthesis, cell proliferation, the secretion of alpha-fetoprotein and cell attachment. The effects of fibronectin and type IV collagen were slight with regard to the promotion of growth and cell attachment. However, there were no differences in the secretion of albumin into the media among the groups. These results suggest that laminin may be necessary for the cellular growth and functional maintenance of immature liver cells during normal development of the liver in vivo.

[Relationship between T cell infiltration in breast cancer tissues and prognosis].

The relationship between the degree of T cell infiltration in cancer tissues and prognosis was studied in 26 patients with breast cancer. The degree of T cell infiltration was divided into 4 groups according to the number of infiltrating T cells. There was a good correlation between the degree of T cell infiltration and the survival period or disease-free period. It may be clinically useful to check the degree of T cell infiltration in cancer tissues for predicting the prognosis of patients with breast cancer.

[Development of MOF as result of DIC. Clinical and experimental studies].

Sixty three percent of 60 cases of clinically diagnosed Disseminated Intravascular Coagulation (DIC) developed to Multiple Organ Failure (MOF) and the mortality was high in the group which showed organ failure before the manifestation of DIC. The mortality of MOF cases was 91.6% and the presence of organ failure before or after DIC manifestation did not affect the mortality. Among 41 autopsy cases, microscopic examination showed pathohistological DIC in 19 cases even in basic lesion bearing cases without any clinical DIC signs (31.3%). Clinically and experimentally it was suggested that intrinsic and extrinsic endotoxin, namely, hepatic failure, infection etc. may play an important role in manifestation of DIC and development of MOF. Once DIC appears, hypercoagulability and hyperfibrinolysis will develop, then microthrombus in organs will appear in quite early phase of clinical course. Organ specificity and intraorganic specificity might be explained by anatomical factor and local fibrinolysis, etc.. In conclusion, the early detection of DIC and employment of protective treatment for organ failure are though necessary to improve prognosis.

Carcinoid of the stomach: a case report and review of 100 cases reported in Japan.

A case of carcinoid of the stomach is described and a review of 100 patients with carcinoid of the stomach reported in Japan up to 1980 is made. Of the 100 cases, the male to female ratio was 2.54. Typical clinical manifestations of the carcinoid syndrome were quite rare. Metastases to the liver were observed in 76% of cases with tumours larger than 5 cm in diameter. The liver and lymph nodes were common sites of metastases. Carcinoids arising from the anterior wall of the stomach also showed a high incidence of metastasis. As a result of histochemical examinations, it was revealed that the nonreactive cell type was found in 31% of cases, while the argentaffin cell type was found in only 10%. Confirmation of the diagnosis of carcinoid is sometimes difficult. Occasionally, therefore, electron-microscopic demonstration of specific secretory granules becomes essential for the final diagnosis.

[Study on the serum concentrations of gentamicin after intravenous drip infusion].

Serum concentrations of gentamicin (GM) were monitored after intravenous drip infusion of 60 mg over 30 minutes, 1 hour and 2 hours. These concentrations were compared with those after intramuscular injection. The mean peak serum concentration obtained at 30 minutes after intramuscular injection (5.09 micrograms/ml) and those obtained at the end of intravenous drip infusion (5.17--6.66 micrograms/ml) were comparable. Serum concentrations decreased to less than 2.0 micrograms/ml at 6 to 8 hours after injection or infusion in all cases except 1 with less body weight than others. Pharmacokinetic analysis showed that the T1/2 of GM ranged from 2.49 to 4.33 hours and that the AUC ranged from 19.66 to 27.09 hr X micrograms/ml. Results obtained in this study suggested that the usefulness of intravenous drip infusion over the time from 30 minutes to 2 hours is equal to that of intramuscular injection in the GM therapy.

Acute and subacute toxicity studies on collagen wound dressing (CAS) in mice and rats.

Single administration of collagen wound dressing (CAS) made from bovine derm in the form of finely ground powders was given to mice and rats via i.p., s.c. and p.o. routes and via i.v. route in the form of physiological saline extracts and it was continuously injected into mice for 28 days via s.c. route to study its acute and subacute toxicity. Examinations were made of on general conditions, body weight, food and water consumption, hematology, serum biochemistry, organ weight, and gross and microscopic findings. Results showed no marked toxicity except for local irritation which was seen only after parenteral administration. We concluded on the basis of these animal experiments that there should be no problem in regard to safety after somewhat more extensive therapeutic application of CAS as a wound dressing in clinical practice.

[Effect of levamisole hydrochloride on advanced and recurrent breast cancer--LMS randomized controlled study of a group responding to CAF (cyclophosphamide, adriamycin, and 5-FU) therapy].

The effect of levamisole HCL on advanced and recurrent breast cancer was investigated cooperatively by a randomized controlled study using envelope method. Patients with advanced and recurrent breast cancer who had showed either complete response or partial response to the previous CAF therapy (cyclophosphamide 100 mg/body/day p.o. from day 1 to 14, adriamycin 30 mg/body/day i.v. on day 1 and 8, 5-FU 500 mg/body/day i.v. on day 1 and 8) were entered in this study and divided randomly into a control group and a levamisole group. No further treatment was given to patients of the control group until they fell into progressive disease. Patients of the levamisole group were given the drug at a daily dose of 150 mg for 3 consecutive days every fortnight. Fifty-nine cases were entered in this protocol, but 9 cases of them were ruled out as exclusions or dropouts. Therefore, the number of eligible cases was 31 in the levamisole group and 19 in the control group. Both the duration of remission and the survival time were significantly (p less than 0.05) prolonged in the levamisole group compared with the control group. There was no difference in side effects between these two groups.

Studies on the etiologic mechanism of anisakiasis. --1. Immunological reactions of digestive tract induced by Anisakis larva.

In an attempt to determine the etiological mechanism of anisakiasis, immunological studies were performed using guinea pigs and rabbits. In order to carry out these examinations, we used a specific antigen, hemoglobin of Anisakis larva. We demonstrate that the etiological mechanism of anisakiasis involves anaphylactic reaction as well as Arthus reaction in the digestive tract. Cell-mediated immune reaction also have to consider the relation of establishment of allergic condition in experimental anisakiasis. These results suggest that the three types allergic reaction may play the main role in clinical symptomes of human anisakiasis.

Effect of endotoxin on rat liver. Analysis of acid phosphatase isozymes in the liver of normal and endotoxin-treated rats.

Lysosomal enzymes in parenchymal and sinusoidal cells in rat liver were sequentially studied following administration of endotoxin and correlated with fine structural changes in liver cells. In normal rat liver, there are three types of acid phosphatase isozymes, two present only in parenchymal cells and the other specifically in sinusoidal cells. The parenchymal acid phosphatase isozymes hydrolyze preferentially AMP and CMP, while the sinusoidal isozyme hydrolyzes phenylphosphate. After an intraperitoneal injection of an LD50 dose of endotoxin, the activity of the sinusoidal acid phosphatase isozyme reached a maximum at 1 hour, and was followed by an elevation of the activity of the isozymes derived from parenchymal cells. Within 1 hour after the injection, morphologic changes were most prominent within sinusoids, including swelling of the Kupffer cells and accumulation of polymorphonuclear leukocytes, platelets, and fibrin clumps on and around the Kupffer cells. Degenerative changes in parenchymal cells were observed following the initial reactions within sinusoids. The results of the present study are compatible with the hypothesis that the hepatic parenchymal cell damage induced by endotoxin is mediated by the initial changes within sinusoids, rather than by the direct aciton of endotoxin on the parenchymal cells.
