Ultrasonically guided percutaneous interstitial implantation of iodine 125 seeds in cancer therapy.

A new method for ultrasonically guided percutaneous interstitial implantation of 125I seeds in abdominal tumours is described. A linear array puncture transducer mounted on a special x-y-z co-ordinate frame is used in combination with a special insertion technique. Following in vitro experiments the technique was applied in two patients with pancreatic cancer. There were no immediate complications after the implantation of 36 and 37 125I seeds.

Pharmacokinetics of ketoprofen in patients with chronic renal failure.

The pharmacokinetics of ketoprofen were studied in 7 volunteers and in 5 patients suffering from varying degrees of renal insufficiency. All the subjects received a single oral dose (50 mg) of ketoprofen. Plasma concentrations were measured up to 24 hours after administration and urinary excretion for 2 days. Ketoprofen concentrations were determined by mass-fragmentography with a minimum level of detection of 0.02 mg/l. As expected, a reduction of the [51Cr]EDTA clearance was correlated with an increase in the elimination half-life of ketoprofen. The urinary excretion corresponded to results reported by other groups.

A case-control study of hypertensive women in a post-disaster community: Wyoming Valley, Pennsylvania.

Among 396 female flood victims participating in a five-year post-disaster survey, 31 cases were identified who developed hypertension during that time period. A case-control study of hypertension was carried out utilizing age and ponderal index as matching criteria. The purpose of the study was to assess risk factors for hypertension associated with the early recovery period. Pairwise matching yielded 29 case-control pairs for analysis. Such factors as property loss, financial difficulties, physical work, use of alcohol, and perceived distress--all associated with the recovery period--were significantly associated with hypertension. Two mental health dimensions--somatization and anxiety--and the respondent's perceived effect of the flood on health assessed at the time of the survey also demonstrated significant positive correlations with hypertension.

Heterogeneity of cell lineages in L3 leukemias.

Five cases of adult leukemia with L3 morphology in bone marrow were studied for the presence of immunologic, metabolic, and enzymatic markers. Among the five patients, there were four males and female. Median age was 66 with a range of 16-80 yr. Median survival was only 5 mo. Serum lactate dehydrogenase (LDH) levels, 3H-thymidine labeling indices, and DNA/RNA content of the L3 lymphoblasts were markedly elevated. B-cell markers were found in three cases, two exhibiting surface membrane IgM-lambda, and one IgG-K. Terminal deoxynucleotidyl transferase (TdT) enzymatic activity was consistently low in this group. In one case, the L3 lymphoblasts displayed only surface Fc receptors demonstrated by the binding of aggregated IgG. TdT activity was found to be significantly increased. In another instance, the lymphoblasts formed spontaneous rosettes with sheep erythrocytes and exhibited paranuclear staining with acid phosphatase. TdT activity was found to be low. Although most of the L3 leukemias are neoplasias of B lymphocytes, other lineages may also express this morphology.

Clinical and social status following injury to the lateral ligaments of the ankle. Follow-up of 144 patients treated conservatively.

Out of 174 patients treated conservatively for injuries to the lateral ligaments of the ankle 144 were seen at follow-up after 3.1--6.1 years (mean 4.2 years), and clinical as well as social sequelae were recorded. The diagnostic criterion was a difference in talar tilt of 6 or more degrees between the injured and uninjured foot on inversion stress radiographs. Occupational and athletic injuries, almost equally represented, made up a total of 64 per cent. Residual symptoms were present in 21 per cent, mainly in the form of functional instability, but only four patients (3 per cent) reported daily complaints. One patient had developed reflex dystrophy, and this was the only patient who had been referred for further orthopaedic treatment. There was a close correlation between pain on movement in the ankle joint and residual symptoms which were, incidentally, unrelated to the degree of primary talar tilt. All the patients seen at follow-up had normal range of movement in the talocrural and subtalar joints. Two patients with residual symptoms had stopped working, while in the others the working ability was unaffected. The period off work had been twice as long for patients having heavy and fairly heavy work as for those doing light work. All football players with residual symptoms had had to give up playing, but only a few had daily complaints. This indicates that some symptom-free patients have in fact latent symptoms, elicited only by fairly severe strain.

Radiological and muscular status following injury to the lateral ligaments of the ankle. Follow-up of 144 patients treated conservatively.

An average of 4.2 years after conservative treatment of injury to the lateral ligaments of the ankle 144 patients attended a follow-up examination which included radiography and measurement of the strength of plantar flexion. Prior to treatment all of the patients had a difference in talar tilt of 6 or more degrees between the injured and uninjured side. At follow-up there was instability in 28 patients, or 19.4 per cent, but no correlation between instability and residual symptoms. Nine patients had an anterior "drawer sign" exceeding 2 mm, but this was also without relation to residual symptoms. Radiological evidence of osteoarthrosis was found in five patients, but four of them had a stable ankle joint and only one had residual symptoms. The mean age of the patients with osteoarthrosis was twice that for the entire material. Two patients had osteochondritis of the talus. The strength of plantar flexion was measured in 124 patients. There was significantly less strength in the injured than in the uninjured leg, but there was no statistically significant correlation between residual symptoms and reduced strength or between radiological instability and reduced strength. According to the results of the present study and a previous one (Hansen et al. 1979) and to the results reported in the literature, we find no reason to alter our present principles regarding the diagnosis and conservative treatment of injury to the lateral ligaments of the ankle.

Biologic markers in breast carcinoma. IV. Serum fucose-protein ratio. Comparisons with carcinoembryonic antigen and human chorionic gonadotrophin.

Serum fucose-protein ratio was evaluated as a potential biologic marker for patients with metastatic breast cancer. By analysis of the same blood samples, comparisons were made with carcinoembryonic antigen (CEA) and human chorionic gonadotrophin (hCG). For 150 patients with metastatic breast cancer, 85% had a value for serum-fucose protein ratio above the normal range in comparison to 75% for CEA and 40% for hCG. Serum fucose-protein ratio was exclusively increased in 12% of the patients, CEA in 4% and hCG in 2%. Both serum-fucose protein ratio and CEA were elevated in 39% of the patients, and together, either in combination of alone, were increased in 93% of the patients. Raised values for serum fucose-protein ratio as well as for CEA decreased with change in disease status from pretreatment to response for patients with measurable disease parameters and increased correspondingly with overt disease progression. Preliminary data indicate both serum fuxose-protein ratio and CEA frequently become elevated when patients progress from a disease free interval after surgery to recurrence.

Variability of reproductive casualty in maternal phenylalaninemia.

A biochemical survey of 4,000 parents of mentally retarded children did not identify any mothers with elevated phenylalanine blood levels. This finding and an analysis of nearly 100 reported cases of maternal phenylalaninemia suggest a low risk of reproductive casualty in phenylalaninemic mothers who somehow escaped the usual effects of phenylketonuria. Recent observation on cases of maternal phenylalaninemia who were detected through unselected screening indicate that the reproductive risk associated with the condition may have been overstimated in the past. This risk is concentrated in mothers who had been affected themselves. Mental level appears to be a stronger predictor of reproductive outcome in phenylalaninemic women than the degree of phenylalaninemia. Selective prenatal intervention based on maternal mental level is not possible, however, where the natural history of the condition has been altered. This is the case in phenylketonuric girls who has been treated during childhood. These girls are now reaching child-bearing age in fast growing numbers, as a result of screening for phenylketonuria. Prenatal intervention is appropriate for all of them until predictors are established that can identify those at risk of reproductive casualty.

School achievement: risk factor in teenage pregnancies?

A review of live births, spontaneous fetal deaths, and induced abortions in residents of Upstate New York ages 12--17 shows that pregnancy rates increased during the period 1971 through 1974. This increase was attributable to pregnancies ending in induced abortion while live births remained relatively stable. White teenagers had a higher frequency of induced abortions than non-white teenagers, but induced abortions increased more rapidly among non-whites over the four-year period. School achievement as reflected by highest grade completed at the end of pregnancy was related to risk of pregnancy as well as to election of induced abortions. The distribution of pregnancies by age and school grade suggests that an increased risk of pregnancy is associated with below average but also, and unexpectedly, with above average grade attainment. Incongruity of age and school achievement may identify groups of teenage schoolgirls with special needs for preventive programs.

Brief reports decline of Down's syndrome after abortion reform in New York State.

Incidence rates derived from prospective studies of 120,000 newborns were applied to live births and induced abortions in order to estimate the trend of Down's syndrome in New York State after liberalization of the abortion law in 1970. Women aged 35 years of older, at higher risk of Down's syndrome births, sought terminations of their pregnancies more frequently than did younger women. The estimated number of newborns with Down's syndrome in the state declined 20 percent from 1971 to 1975. In New York City, more pregnancies in the high-risk age range were aborted than were carried to term. These trends indicate that abortion reform may have made a significant contribution to the reduction of severe mental retardation.
