Hemophilus influenzae biotypes and chronic bronchitis.

Hemophilus influenzae can be differentiated into 6 biotypes on the basis of 3 biochemical reactions. This study was undertaken to determine the biotypes of respiratory isolates from an adult population with chronic bronchitis and to compare the biotypes of upper and lower respiratory tract isolates of individual patients. For rapid biotyping of serologically nontypable H. influenzae, we used 2 commercially prepared kits (Minitek and IDS RapID Systems). Twenty-three of 29 isolates (79%) had biochemical characteristics of biotype II with the Minitek system. Eighteen of the same 29 isolates were retested with the IDS RapID System and reacted as biotype II organisms. Seven of 9 and 4 of 4 paired transtracheal/oropharyngeal samples had identical biotypes when tested with the Minitek and IDS RapID System, respectively. These findings suggest that H. influenzae organisms are exchanged between the oropharynx and tracheobronchial tree of elderly male patients with chronic bronchitis.

In vitro susceptibility of mycobacteria species other than Mycobacterium tuberculosis to amikacin, cephalosporins and cefoxitin.

The susceptibility of different species of mycobacteria, other than M. tuberculosis, to a range of cephalosporins and to amikacin was studied. Susceptibility patterns varied with species. M. fortuitum, M. marinum and M. szulgai were the most susceptible species to amikacin and to cefoxitin, where as M. kansasii, M. scrofulaceum and MAIS complex the most resistant. Cefoxitin appears to be much more active against mycobacteria than the other cephalosporins used in this study. Most of the cefoxitin-sensitive mycobacteria were inhibited by concentrations which can be easily attained in serum on standard dosage schedules.

[Craniopharyngioma: experience with combined operative and radiotherapy and problems of hormone substitution (author's transl)].

Craniopharyngioma, primarily a benign tumour, is associated with a variety of serious problems due to tumour localization. Access to the tumour during surgery is difficult and total resection can rarely be achieved. Growth of the remaining tumour continues and symptoms of increased intracranial pressure, further visual field loss and hormonal disturbances reappear. However, conservative operation in conjunction with radical irradiation shows good results as regards tumour regression and survival with satisfactory quality of life. Irradiation with 50 Gy on average was performed in 5 out of 6 children with remaining tumour or tumour recurrence, and satisfactory results were obtained. In all cases tumour size regressed. Endocrine disturbances could be ascertained by the combined LHRH-TRH-insulin test and the clinical picture. In nearly every case there was finally a complete lack of hormones of the pituitary-hypothalamic axis. Substitution of various hormones became necessary in all cases. Meticulous surveillance of children still undergoing developmental maturation is mandatory. All six children are well and lead a nearly normal life.

[Treatment of Wilms' tumor (author's transl)].

Uniform treatment based on the therapeutic approach of the 1st and 2nd US National Wilms' Tumor Study was decided on in March 1976 by paediatricians, surgeons, urologists and radiotherapists in Austria. Wilms' tumour was diagnosed in 34 children between 1 january 1976 an 29 february 1980 (stage I: n = 11, stage II: n = 8, stage III: n = 8, stage IV: n = 7). Parents of two children refused treatments; both children have since died of metastases. Of the remaining 32 children 29 (90.6%) are alive, 10 for more than 4, 15 for more than 3 and 19 for more than 2 years after diagnosis. 21 children are without need of treatment. Three children have died, one due to postoperative complications, one due to haemorrhagic chickenpox, but free of tumour, and one after insufficient treatment. Two of the five children with a recurrence between 2 1/4 to 15 months after diagnosis had been treated inadequately in the initial phase. The tumour free survival rate in 74.2%. Two children with early occurring or recurrent lung metastases have survived for 53 1/2 and 54 months up to now.

Diaphragm paralysis and ventilatory failure in chronic proximal spinal muscular atrophy.

Ventilatory failure occurred in a patient suffering from proximal neurogenic muscular atrophy resembling the adult form of Kugelberg-Welander syndrome. Diaphragmatic paralysis was diagnosed when patient was found to be unable to generate a transdiaphragmatic pressure difference on deep inspiration. Post-mortem examination revealed nearly complete replacement of the diaphragm by fat and fibrous tissue. Although uncommonly reported in the medical literature, ventilatory failure secondary to diaphragm dysfunction should be added to the list of complications encountered in the clinical setting of chronic proximal spinal muscular atrophy.

[Determination of triiodothyronine in serum with a heterologous enzyme immunoassay: results of a group survey].

This paper summarises the results of the evaluation of a heterologous enzyme immunoassay in antibody coated tubes for the determination of triiodothyronine (T3) by a group of 11 laboratories. The assay procedure is analogous to an established heterologous thyroxine enzyme immunoassay. The evaluation demonstrated that 1. the specificity of the assay and its analytical range between 0.46 and 9.2 nmol/l meet diagnostic requirements, 2. the intra- and interassay precision was in accordance with that commonly found in radio immunoassays, 3. the recovery of added T3 was between 90 and 103%, 4. the results agreed well with those of self-established radio immunoassays and were comparable with commercial RIA kits, 5. there was no interference by hyperbilirubinaemia or by high concentrations of bile acids, but interference can occur in very haemolytic and lipaemic samples. The great advantage of T3 enzyme immunoassays lies in the absence of restrictions and any authorization needed for working with radioactive substances.

[Thrombopoiesis and blood coagulation in pediatric patients with septicemia (author's transl)].

45 infants and children with thrombocytopenia and septicemia were studied. Many parameters of blood coagulation, the platelet diameters and the megakaryocytes of the bone marrow (Feulgen stained cytophotometry and maturity of the megakaryocytes) were examined. 15 patients had a consumption coagulopathy and 30 were classified as having an isolated septic thrombocytopenia. In both groups the number of the megakaryocytes of the bone marrow smears were normal. 81% of the megakaryocytes were mature. The patient group with isolated septic thrombocytopenia had significantly greater ploidy values of the megakaryocytes than a control group. In both groups the diameters of the platelets were also significantly greater than in an age matched control group of children with a normal platelet count. These results allow the conclusion that the thrombocytopenia in pediatric patients with septicemia is not caused by a diminished production of platelets.

[Hypercalcaemia as first symptom of acute lymphatic leukaemia: aetiology and treatment (author's transl)].

A hypercalcaemic crisis was the first manifestation of acute lymphatic leukaemia in a two-year-old girl. Admiinistration of prednisolone improved the child's general condition but obscured the true diagnosis which was made only when another hypercalcaemic crisis occurred. Unexplained hypercalcaemia should make one consider the possibility of a malignant blood dyscrasia. Forced diuresis, cortisone and mithramycin can be used therapeutically to achieve a rapid fall in calcium level.

[Visual acuity and intraocular pressure after surgery in congenital glaucoma (author's transl)].

In 124 eyes (82 patients) a total of 152 trabeculotomies were performed. Re-examinations were done after three and six months and then every year. After at least three years pressure was controlled in 67 out of 80 eyes (84%). Less favorable results were achieved in secondary glaucomas, in cases with previous surgery, if age at surgery was more than one year and in cases of known heredity. Corneal diameter and preoperative pressure did not influence the postoperative pressure. In 24 out of 60 eyes (44%) the final visual acuity was 0.4 or more. Vision and refraction were independent of the corneal diameter.

The variable region of human immunoglobulins. I. Serologic and structural correlations of antigenic markers common to V lambda I and V lambda IV proteins (isotypic cross-reactivities).

Sixty-eight human lambda-chain sources including representatives of the five different subgroups were studied for the distribution of the serologic markers ST, 111, and VOR The variable region character of these antigenic determinants has been demonstrated for those proteins whose primary structures had been partially or completely determined previously. The lambda-chain sub-groups I and IV are readily distinguished antigenically but the specificity designated VOR is shared between them and is absent in the II, III, and V subtypes. The results of these studies are discussed with respect to the possible relationships of the structural genes controlling the synthesis of the lambda-chain subgroups, and some potential phylogenetic and functional meaning of the heterogeneities is revealed.

[Acute lymphatic leukaemia in children: re-examination after remission of at least five years (author's transl)].

Full remission of more than five years was observed in nine of 24 children with acute lymphatic leukaemia. They had been treated by a scheme similar to the Pinkel VII one, and all had been discharged from treatment. The immune status, bone marrow, radiological, neurological and psychosomatic studies, as well as various laboratory tests indicated that in seven of the children the findings, are essentially normal. But two children have lasting severe organ damage, portal fibrosis in one and bilateral intracerebral calcification in the other, both presumably due to the methotrexate treatment.

Implantation of cigarette smoke condensate in the lungs of Syrian golden hamsters.

Different doses of benzo(a)pyrene, cigarette smoke condensate (CSC) and its nitromethane fraction (NMF), in wax pellets, were directly implanted into the lungs of Syrian golden hamsters. Pleomorphic sarcomas were induced in the B(a)P treated animals. Several epithelial proliferative lesions and a few bronchiogenic adenomas were induced in CSC and NMF treated hamsters. A species-specific effect has been observed in this investigation.
