[Favorable, unfavorable and indifferent side effects of pharmacotherapy of psychoses].

Not all side-effects are undesirable side-effects of a treatment. Under certain conditions allergic side-effects during the therapy of phasic psychotics affect a turn in the course of the basic illness and bring a premature end to depression. As a result of the observation of a sudden cure of a deep vital depression in a 73 year old patient in direct chronological connection with the appearance of an allergic urticaria, studies were carried out, in the 8 years following, of a total of 18 cases of similar circumstances; during the treatment of an endogenous depression the development of intercurrent allergic skin reactions showed: in 6 cases the psychosis completely disappeared with the development of the urticaria; in 8 cases the symptoms were considerably relieved so that only the remains of vegetative disorders persisted; in 4 cases no effect could be determined. In 2 cases the same allergic reaction was consciously provoked by a reapplication of the medicaments concerned during a following depressive phase; again with favourable results since under these conditions coincidences are not very probable, the conclusion can be made, and experimentally supported, that allergic drug reactions may be employed in the course of depressions, with careful avoidance of complications.

[Ground force in relation to the activity of various leg muscles in the supporting foot phase of human gait].

By means of using biomechanical methods we investigated the relations between muscle activity and ground forces that are produced during walking. Our special attention was directed forwards the touching phase of the foot which was registered via integrated electromyogram (surface electrodes) and force plateform (Kistler). The initial spike of vertical force and its dependence of m. tibialis anterior activity was examined. In addition we observed connections between m. tibialis anterior activity and surface condition of the ground. Further, our results documented a damped movement of the touching fore foot and a steady take over of weight during pressing which was caused by increased tibialis activity. The pressure phase (first part of the vertical force component) was separated in two curves and defined by means of two angles. More data was given for interpretation the activities of m. gastrocnemius, m. peroneus longus and vertical and transversal force components. In further experiments we intend to analyse pathologic conditions.

Cigarette smoking and sperm quality.

The effect of cigarette smoking on conventional semen parameters (volume, total sperm count, sperm count/ml, total motility, progressive motility, morphology) was studied retrospectively in 90 healthy patients (50 non-smokers, 40 smokers) of our infertility clinic. Both groups, comparable in age, height and weight, were subdivided into two age groups (less than 28 years, greater than or equal to 28 years). Smokers were found to have sperm volumes significantly smaller than non-smokers of the same age. No additional effects on sperm parameters were found. Cigarette smoking revealed no detrimental effect on spermatogenesis.

Long-term results of corpus cavernosum autoinjection therapy for chronic erectile dysfunction.

Between May 1985 and March 1992, 172 patients suffering from chronic erectile dysfunction (21-70 years old) underwent constant corpus cavernosum autoinjection therapy (CAT) with a standardized papaverine phentolamine mixture (16385 injections). Thereafter 41 patients continued CAT with the single agent papaverine (1257 injections). On the basis of both these 17642 protocol auto-injections, and over 6 years of experience with intracavernosal autoinjection therapy we conclude that, especially with the papaverine-phentolamine mixture, CAT constitutes an effective therapy (full rigidity in 95.8%) with tolerable side-effects for chronic erectile dysfunction when preceded by careful patient selection and thorough multi-disciplinary evaluation. This is especially so in the case of arterial and/or neurogenic aetiology of the erectile dysfunction. In addition, the contraindications must be strictly observed, the treatment and technique fully explained, and a regular follow-up instituted. CAT is generally well accepted by the patients and their partners (98.8%/97.6%) and has distinct positive effects on self-esteem (77.8%), performance anxiety (84.4%), and partnership (79.5%). The most serious side-effect was prolonged erection (25 out of 17642 injections). In 6 patients reversible fibrotic changes near the tunica albuginea were observed.

Accidental radiation exposure and azoospermia.

Seven Georgian male soldiers (19-25 years old) had accidentally been exposed to radiation by Cs-137 between April 1996 and May 1997. No information about the exact time and duration of exposure was available. All patients presented with the subacute stage of Cutaneous Radiation Syndrome with deep painful ulcers on different body sites, predominantly on the legs. Semen analyses showed complete azoospermia in 4 patients, with elevated follicle-stimulating hormone (FSH) in 3 and elevated luteinizing hormone (LH) in 2 of them. One patient had severe oligozoospermia of 7 million sperm per mL, with normal sperm motility and morphology; his FSH and LH levels were elevated. One patient had complete normozoospermia, and the seventh patient had polyzoospermia of 340 million per mL; both of these patients had normal serum hormone levels. Only the patient with oligozoospermia reported a history of delayed testicular descent; his physical examination showed relatively soft and small testicles and a varicocele with considerable reflux. The physical andrological examinations were normal in the other 6 patients. It is very likely that the azoospermia in the 4 patients can be attributed to the radiation accident. In conclusion, it is essential to perform andrological examinations in patients who have been exposed to radiation even if there are only cutaneous injuries detectable, as a high percentage of them can show azoospermia.

Detection of cutaneous varicella zoster virus infections by immunofluorescence versus PCR.

Detection of localized, clinically atypical cutaneous infections with varicella zoster virus (VZV) has proven difficult, as serum antibody tests sometimes are not sensitive and specific enough for that purpose. Therefore immunofluorescence and an internally controlled PCR for VZV are compared for sensitivity. Detection of PCR products was done by ELISA, and if positive, additionally by agarose gel electrophoresis. Of 60 samples 44 were PCR-positive by ELISA (44 = 100%), of which 37 (84%) were also positive on the agarose gel. Thirty-four samples (77%) were positive by immunofluorescence. No sample was positive by immunofluorescence and negative by PCR. A combination of immunofluorescence and PCR with agarose gel analysis detected 42 samples out of 44 positive by PCR ELISA (95%). These results demonstrate that immunofluorescence is a suitable, fast and inexpensive method for routine diagnostics. Additional sensitivity can be achieved by screening immunofluorescence-negative samples by PCR, which is extremely sensitive but time-consuming and labor-intensive.

[Spermatologic studies in 300 patients with varicocele of different degrees of severity and in various age classes].

Spermatologic investigations were made in 227 patients with varicocele at the age of 19-23 years (average age 20.4 years) and in 73 patients with varicocele at the age of 24-40 years (average age 28.0 years). The spermatologic findings of the patients with varicocele were compared with the results of 55 andrologic healthy young men without varicocele. The diverse degrees have a different effect to the sperm quality. Only patients with the clinical signs of the varicocele-orchidiopathy present a decreased sperm quality. The degree varicocele combined with other testicular disorders shows the worst sperm quality. The results in the higher age group explain, that the damage of the testicle increases with longer duration of the varicocele. Differentiating the spermatozoal morphology according to the "Düsseldorf morphological classification" you can find characteristic forms with hyperelongation of the head in patients with varicocele and spermatozoa with acrosomal defects in the degree varicocele combined with other testicular disorders.

[Circadian rhythm of testosterone level in plasma. I. Physiologic 24-hour oscillations of the testosterone level in plasma].

The testosterone level in plasma is measured in 10 males by radioimmunoassay (RIA) at the following times: 07.00 h, 08.00 h, 09.00 h, 10.00 h, 13.00 h, 16.00 h, 19.00 h, 23.00 h and 03.00 h. A circadian rhythm can be noted. Peak levels are reached between 07.00 h and 10.00 h. At 19.00 h testosterone values reach their minimum and rise at night. In the morning between 07.00 h and 10.00 h testosterone levels seem to have a plateau. Between the morning and the evening the values show great differences.
