Susceptibility of wild mice to Yersinia pseudotuberculosis and Yersinia enterocolitica.

Laboratory-born progeny and adult populations of old world woods mice (Apodemus speciosus) trapped in a certain area where Yersinia pseudotuberculosis was prevalent among wild animals, were challenged with Y. pseudotuberculosis and Yersinia enterocolitica to determine the ecology of Yersinia in wild mice. The laboratory-born progeny population was associated with infection with Y. pseudotuberculosis but not with Y. enterocolitica serotype O:3 and was found to be nearly 200 times more susceptible to infection with Y. pseudotuberculosis than were the adult populations. A. speciosus convalescing from infection with Y. pseudotuberculosis showed protection upon subsequent challenge with homogeneous and heterogeneous serotypes of Y. pseudotuberculosis. These data suggest that the prevalence of Y. pseudotuberculosis in wild mice may begin with the infection in the newborn and thus spread to the Y. pseudotuberculosis naive adult mice populations.

Rapid growth and spontaneous metastasis of human germinal tumors ectopically transplanted into scid (severe combined immunodeficiency) and scid-nudestreaker mice.

Xenograft acceptance, growth and spontaneous metastasis of ectopically transplanted human germinal tumors were compared among scid mice, athymic nude mice and F2 hybrids constructed from scid and nude mice, in relation to the impairments of T and B cell functions in these mice. In scid mice which are deficient in T and B cell functions, human yolk sac tumor (YST-2) that originated from the ovary grew to enormous sizes in 100% of the animals after both subcutaneous and intraperitoneal transplantation, while only half (59.1% and 51.9%) of the subcutaneous and none of the intraperitoneal transplants were accepted in usual athymic nude mice (BALB/c-nu/nu and CD1-nu/nu). The YST-2 grew rapidly in scid mice, developing 3 to 10 times larger tumors compared to nude-streaker (AKR/J-nustr/nustr) and usual nude mice, respectively. Furthermore, ectopically transplanted tumors spontaneously metastasized to distant organs (mostly to the lung) in scid mice (but less frequently in leaky scid mice), while metastases have never been found in nude mice. Although a xenograft of human classic (typical or pure) seminoma of the testis has never been established in nude mice, it grows slowly in one-third (36.4%) of scid mice and very rapidly in all of scid-nustr (scid/scid; nustr/nustr) double mutant mice. Spontaneous metastases of xenografted seminomas were also observed in distant organs (lymph node, lung, liver, spleen, and kidney). The metastastic distribution of the two human germinal tumors in scid and scid-nustr mice mimics that found in human. These results (xenograft acceptance, growth of transplanted tumors and degree of metastatic spread) were compatible with the level of T and B cell impairments indicated by FACS analysis, as well as mitogen responses, serum IgG and morphological features of the thymus.

Modified PCR-RFLP method for HLA-DPB1 and -DQA1 genotyping.

We previously developed a new technique for HLA class II genotyping by digestion of polymerase chain reaction-amplified genes with restriction endonucleases (PCR-RFLP method). This PCR-RFLP method is an efficient and convenient typing technique for class II alleles. However, small fragments or bands located close to each other on polyacrylamide gels sometimes prevent precise analysis of the RFLP bands. Furthermore, the restriction enzymes we have reported in the previous papers are not sufficient to identify the genotypes of all heterozygous individuals. Here, we report an improved PCR-RFLP method using some informative restriction enzymes which have either a single cleavage site or, alternatively, no cleavage site in the amplified DNA region, depending on the HLA alleles, making reading of RFLP band patterns much easier. Each second exon of the HLA-DQA1 or -DPB1 gene was selectively amplified from genomic DNAs of 70 HLA-homozygous B-cell lines and 100 healthy Japanese by PCR. Amplified DNAs were digested with restriction endonucleases and then subjected to electrophoresis assaying simply for cutting, or no cutting, of the DNA. ApaLI, HphI, BsaJI, FokI, MboII and Mn1I can discriminate eight alleles of the DQA1 gene. Similarly 19 alleles of the DPB1 gene can be discriminated with Bsp1286I, FokI, DdeI, BsaJI, BssHII, Cfr13I, RsaI, EcoNI, and AvaII enzymes. This modified PCR-RFLP method can be successfully applied to heterozygotes. Thus, the method is technically simpler and more practical for routine HLA typing work than our previous PCR-RFLP method.

Intestinal carriage of Yersinia pseudotuberculosis by wild birds and mammals in Japan.

Fecal specimens were obtained from wild birds and mammals in the eastern part of Shimane Prefecture, Japan, an area where serotype 1b, 2b, 3, and 4b strains of Yersinia pseudotuberculosis were found to be prevalent in humans. Each of 869 animals, including 259 wild birds and 610 wild mammals, was screened for yersiniae. A total of 37 strains of Y. pseudotuberculosis were isolated from 34 (5.6%) mammals, including 23 raccoon dogs (Nyctereutes procyonoides), eight deer (Cervus nippon), two hares (Lepus brachyurus), and one marten (Martes melampus), and from two (0.8%) birds, including one eastern spot-billed duck (Anas poecilorhyncha) and one wigeon (Anas penelope). The Y. pseudotuberculosis isolates collected belonged to virulence plasmid-positive (serotypes 1b, 2b, 3, 4b, and 6) and virulence plasmid-negative (serotype 5a) strains, the most predominant serotype being 4b. The close relationship between the regional distributions of Y. pseudotuberculosis in wild animals and humans suggests that wild animals are an important source of infection.

First isolation of Yersinia enterocolitica serotype O:8 in Japan.

A 4-year-old Japanese boy was infected with Yersinia enterocolitica serotype O:8, H:befiv, biotype 1B, phage type Xz, restriction endonuclease analysis of plasmid DNA type B, restriction endonuclease analysis of chromosomal DNA type 8. He presented with acute gastroenteritis with elevated body temperature (40 degrees C), rigor, and shivers. The diagnosis was septicemia. This is apparently the first report of this serotype from a human infection outside North America.

Acute Disease↗

Acute mesenteric lymphadenitis due to Yersinia pseudotuberculosis lacking a virulence plasmid.

A serotype 4a strain of Yersinia pseudotuberculosis lacking the virulence plasmid pYV (pYV- strain) was isolated from the mesenteric lymph nodes but not from the stool or the appendix of a 10-year-old girl with a diagnosis of acute mesenteric lymphadenitis. Microscopically, reticulocytic abscess and lymphadenitis were persent in the enlarged mesenteric lymph nodes. Antibody against the isolate was detected in the serum. The isolate was negative for the presence of plasmid pYV and plasmid pYV-mediated properties, including autoagglutination and calcium dependency, but was positive for chromosome-mediated properties, including invasion into HeLa cells and tissues of mice and the Serény test. Mice were orally infected with this pYV- strain, and rapid elimination from the intestine occurred 14 days later. Hence, the potential to inhibit the phagocytosis encoded by plasmid pYV was lacking. As the pYV- strain was recovered from the mesenteric lymph nodes and the spleen, the invasiveness was encoded by chromosomal genes. The count of the pYV- strain in the mesenteric lymph nodes increased to 10(4.6) cells per g within 4 days. These findings suggest that pYV- Y. pseudotuberculosis was the causative agent of acute mesenteric lymphadenitis in the absence of gastroenteritis.

Subrenal capsule assay using liver cancer specimens obtained by fine needle biopsy.

Twenty-seven liver cancer biopsies (20 hepatocellular carcinomas, 4 metastatic liver cancers and 3 cholangiocellular carcinomas) were obtained using a 21-gauge fine needle biopsy guided by ultrasonography. These cancers were subcutaneously transplanted to the subrenal capsular region of BDF1 mice premedicated with immunosuppressive agents to modulate the host immune reaction. The SRCA was based on the change in tumor size (delta TS) and the tumor growth inhibition rate (TGIR). The transplantation rate of the 27 liver cancer specimens was 85% by delta TS and 67% by TGIR. The efficacy rates of Adriamycin, cis-platinum, and mitomycin were respectively 71%, 58%, and 43% by delta TS, and 73%, 56% and 50% by TGIR. Thus, liver cancer specimens obtained by fine needle biopsy and examined by SRCA had a fairly high transplantation rate, and this method can be useful for patients with inoperable liver cancer, as a general chemosensitivity test for anticancer drugs.

Studies on cephalosporin antibiotics. IV. Synthesis, antibacterial activity and oral absorption of new 3-(2-substituted-vinylthio)-7 beta-[(Z)-2-(2-aminothiazol-4-yl)-2- (carboxymethoxyimino)acetamido]cephalosporins.

A series of new 7 beta-[(Z)-2-(2-aminothiazol-4-yl)-2-(carboxymethoxyimino)ace tamido] cephalosporins (1) having various substituted-vinylthio groups at the C-3 position of the cephen nucleus was synthesized and evaluated for antibacterial activity and oral absorption in rats in comparison with cefixime. Of these, the cephalosporins (1a and 1c) with a lower alkoxycarbonylvinylthio group (Z-form) at the C-3 position showed a potent antibacterial activity against Gram-negative bacteria, improved activity against Staphylococcus aureus as well as good oral absorption in rats. The structure-activity relationships of 1 are also presented.

Platelet adhesion to a biomimetic prosthesis prepared from canine arterial wall.

A biomimetic prosthesis was prepared from canine arterial wall by extraction with 70% formic acid. Under these conditions, the arterial architecture consisting of elastic lamella was conserved and the tensile strength was retained. Attachment of 51Cr-endothelial cells to the surface of the prosthesis was enhanced in the presence of collagen (50 micrograms/ml), fibronectin (20 micrograms/ml), and fibrinogen (500 micrograms/ml). These agents also enhanced the adhesion of 51Cr-platelets. However, preattachment of endothelial cells to prostheses prevented platelet adhesion.

[A case of ruptured distal arch aneurysm associated with postoperative unilateral respiratory disorders].

This paper describes a patient with ruptured distal arch aneurysm associated with postoperative unilateral respiratory disorders whose life was able to be saved by surgery. The patient was a 68-year old male who underwent emergency surgery due to sealed rupture of distal arch aneurysm while preparing for selective surgery. Artificial graft replacement of the distal arch was carried out under centrifugal pump bypass. Respiration was controlled using differential lung ventilation (DLV) techniques. This was done because weaning from the respirator was difficult due to decreases in lung-thorax compliance on the opened chest side after surgery. Weaning from the respirator was achieved on the 10th day of illness after surgery. Postoperative prognosis of ruptured thoracic aneurysm is extremely poor, with high mortality being attributable to the fact that most patients are elderly and the incidence of respiratory and cardiovascular complications is very high. In addition, problems are often encountered in postoperative respiration management in patients with thoracic aneurysm who need open chest surgery. DLV seems to be a useful support in such patients who have underlying respiratory disorders.

[Oral condition of infantile acute lymphatic leukemia with microbisme substitute].

The oral condition of a 7 year old boy at the 12th and 13th remission induction therapy sessions since contracting acute lymphatic leukemia was investigated clinically and microbiologically. The findings obtained were as follows: 1) The appearance of gingivitis coincided with the decrease of white blood cells 7 to 9 days after administration of the anti-leukopenic drugs. 2) Gingivitis appeared on the same site of the oral cavity at both remission induction therapies sessions. 3) The bleeding from the gingival lesions corresponded to the decrease of blood platelet at both remission induction therapies sessions. Conversely, gingivitis disappeared with the improvement of peripheral blood conditions. 4) The total number of oral bacteria decreased after administration of the antibiotics, although the ratio of Candida in the oral microbial flora increased markedly. 5) Typical symptoms of oral candidiasis were not observed after the appearance of the microbisme substitute.

An artificial endocrine pancreas--problems awaiting solution for long-term clinical applications of a glucose sensor.

The artificial endocrine pancreas is a feedback controlled system regulating insulin delivery on a minute-by-minute basis according to measured blood glucose levels. The bedside-type artificial endocrine pancreas has been proven to be useful not only as a therapeutic tool for diabetic patients but also as an elegant research tool for investigating the pathophysiology of the disease. Recently, a wearable-type, closed-loop system has been developed, of which the breakthrough was the establishment of a needle-type glucose sensor. However, for the long-term clinical use of needle-type glucose sensors, several obstacles remain to be solved, such as oxygen dependency, biocompatibility of the membrane, etc. The most promising method for the non-invasive determination of blood glucose is infrared spectroscopy. The trend in the development of a closed-loop glycemic control system which enables perfectly physiological glycemic regulation on a long-term basis is directed to an implantable device. Much research is being conducted to realize such devices.

[Cardiac performance before and after Jatene procedure for transposition of the great arteries: with specific reference to post-operative supravalvular pulmonary stenosis].

The cardiac performance was evaluated in patients who had received arterial switch operation (AS-op) for transposition of the great arteries (TGA) in their infancy with special attention to postoperative supravalvular pulmonary stenosis (SVPS). AS-op was performed in 36 infants; 13 with simple TGA and 23 with TGA accompanied with ventricular septal defect. Nine patients had undergone pulmonary arterial banding with systemico-pulmonary shunt. The mean age at surgery was 7 months. Postoperative catheterization was performed on average of 15 months after surgery. The end-diastolic volume (EDV) and ejection fraction (EF) of the left and right ventricles (LV, RV) were calculated from biplane cineangiograms. LVEDV and RVEDV were normalized and expressed as % of normals. SVPS was defined as peak systolic pressure gradient of 50 mmHg or higher between the RV and the pulmonary artery (PA). The mean LVEDV, RVEDV, LVEF and RVEF values were all within the normal range. Inverse correlation was observed between the peak systolic RV-PA pressure gradient and RVEF (n = 29, r = -0.84, p less than 0.001). SVPS after AS-op was observed in 8 patients (22%). Stenosis was observed at the site of anastomosis of the artery. In these cases, the diameter of the PA at the stenotic site decreased to 52% of the preoperative values. Branch stenosis of the PA was observed in 3 patients who had undergone surgery during the neonatal period. Generally, the results after AS-op were satisfactory in terms of cardiac performance. Postoperative SVPS was observed in 8 patients (22%) with decreased RVEF. Careful observations of the growth of PA after AS-op are mandatory.
