[Clinico-pathological studies of tumor-forming type of multiple myeloma].

Among 49 myeloma cases studied, 31 lesions were found to develop into extramedullary tumors in 8 cases and 22 lesions were found to develop into skeletal tumors in 14 cases. The locations of the extramedullary tumors were the lymph nodes, subcutaneous tissue, liver, kidney, stomach, and pancreas. The location of the skeletal tumors was the ribs, thoracic spines, and skull. These tumors were diagnosed as plasmacytomas by biopsy and/or touch- or aspiration cytology. The biological and clinical characteristics of the tumor-forming type compared with the non-tumor-forming type, were their occurrence in younger patients, a higher incidence of Bence-Jones proteinuria, more marked bone lesions and almost normal renal function. Concerning the monoclonal immunoglobulin (M-component) of the tumor-forming type, there were more of the Bence-Jones type and fewer of the IgA type than in the non-tumor-forming type. No relationship between tumor-formation and prognosis was seen.

Hexaprenyl pyrophosphate synthetase from Micrococcus luteus B-P 26. Separation of two essential components.

Hexaprenyl pyrophosphate synthetase was detected in extracts of Micrococcus luteus B-P 26. During the course of purification the enzyme was resolved into two components, each of which had no catalytic activity but restored the hexaprenyl pyrophosphate synthetase activity when combined with each other. Both fractions, designated components A and B in the order of their elution from hydroxyapatite, were purified free of farnesyl pyrophosphate synthetase co-occurring in the same bacterium. They appeared to be proteins of molecular weights of approximately 20,000 (component A) and 60,000 (component B). Component A was more stable as compared with component B which was easily destroyed by relatively mild heat treatment. The hexaprenyl pyrophosphate synthetase reconstituted of these two components catalyzed the synthesis of all-trans-hexaprenyl pyrophosphate from isopentenyl pyrophosphate and all-trans-farnesyl or all-trans-geranylgeranyl pyrophosphate, but it did not catalyze a reaction between isopentenyl pyrophosphate and either dimethylallyl or geranyl pyrophosphate.

Efficient enzymatic hydrolysis of polyprenyl pyrophosphates.

A method of efficient enzymatic hydrolysis of polyprenyl pyrophosphates was established. Polyprenyl pyrophosphates such as all-trans-octaprenyl and all-trans-nonaprenyl (solanesyl) pyrophosphate can be hydrolyzed completely to the corresponding primary alcohols by the action of potato acid phosphatase in the presence of 60% (v/v) methanol.

Ultrastructure of vacuolated plasma cells in macroglobulinemia associated with production of Mu-chain fragment.

In a unique case of macroglobulinemia associated with the production of mu-chain fragment, the authors observed vacuolated plasma cells (Vac-P) in the bone marrow. Immunofluorescence with anti-mu-chain showed positive staining for the cytoplasm but negative for the vacuoles. On electron microscopic study, clear vacuoles were localized in the vicinity of the Golgi complex, and the contents were amorphous membrane debris and concentric circular structures. The vacuolar membrane was partially disrupted and was contiguous with the outer surface of the concentric circular structure. From the findings, the authors thought these vacuoles were autophagic. Disruption of vacuolar membranes was also demonstrated in three other cases of Vac-P studies with electron microscope. Heterophagy of toxic substances, such as silica or salt or urate, leads to membrane disruption. Vac-P observed in abnormal mu-chain production may produce toxic substances. Once the substance is incorporated into the autophagic vacuole, the vacuolar membrane will be weakened, and the absorption of part of the membrane and cytoplasm or organelles follows.

A new glucose-6-phosphate dehydrogenase variant (G6PD Nagano) associated with congenital hemolytic anemia.

A new glucose-6-phosphate dehydrogenase (G6PD) variant associated with chronic nonspherocytic hemolytic anemia was reported. The patient, a 6-year-old Japanese male, was noticed to have hemolytic anemia soon after birth, and a diagnosis of G6PD deficiency was made at the age of 2. He had episodes of hemolytic crisis several times after upper respiratory infection. G6PD activity of the patient was 5.5% of normal. The enzymatic characteristics were examined when he was 5 years old, and his G6PD showed faster-than-normal electrophoretic mobility, low Km G6P, high Km NADP, low Ki NADPH, normal utilization of substrate analogues, heat instability, and a normal pH optimum curve. From these results, this was considered to be a new variant and was designated G6PD Nagano. Infection-induced hemolysis and chronic hemolytic anemia seem to be due to markedly impaired enzyme activity and thermal instability.

The induction of adenocarcinomatous differentiation in neoplastic bladder epithelium by an embryonic prostatic inductor.

Epithelial cells of non-glandular, transitional cell carcinomas from rat urinary bladders were associated with embryonic rat urogenital sinus mesenchyme (a prostatic inductor) and transplanted into syngeneic adult male hosts. Neoplastic bladder epithelium formed glandular (adenocarcinomatous) acini in a substantial number of recombinants, suggesting that the mesenchyme (stroma) may influence the histotypic organization of carcinoma cells and induce the neoplastic epithelium to express a novel phenotype.

Overproduction of structurally normal enzyme in man: hereditary haemolytic anaemia with increased red cell adenosine deaminase activity.

The mechanism of red cell adenosine deaminase (ADA) accumulation in a case of hereditary haemolytic anaemia due to increased red cell ADA activity was investigated. ADA activity of the younger cells was twice that of the older cells. Rate of ADA synthesis in erythroid colony cells cultured from the patient's bone marrow cells was 11-fold greater than that from the normal. The accumulation of ADA in the patient seems to be due to the increased synthesis in precursors of red cells in spite of the increased degradation in peripheral blood.

[Studies on the occupational maladjustment syndrome. With reference to the survey of examinations, etiological factors and treatment].

A survey of medical examinations has been made on occupational maladjustment syndrome (O.M.A.S.). We have examined and treated on 358 patients with O.M.A.S. during the past 10-15 years at two clinics; one was a company's clinic (this company has 15,000 employees) and the other was the clinic at the Dept. of Mental Health, Osaka Prefectural Institute of Public Health during the past 15 years. Discussions have been made on 150 patients whom we directly treated. Our 358 subjects having O.M.A.S. do not include those with secondary O.M.A. resulting from psychosis, depression, neurosis and physical impairment. From that reason, our subjects are categorized in a narrow sense, in a category which may be called as "adjustment disorder." The results are summarized as follows. 1. The patients with O.M.A.S. were found almost in males and most commonly found among patients in the twenties. 2. The average rate of patients with O.M.A.S. among all the patients who visited two clinics, was 4.1% at our clinic and 16.7% at the company's clinic. 3. At our clinic, the number of patients with O.M.A.S. was followed by the number of those with neurosis, schizophrenia and manic-depressive psychotic zone. This trend was also the same at the company's clinic. 4. As the cause of O.M.A.S., we found the following occupational factors; firstly, transposition by selective promotion, secondly, complication of job-quality, and thirdly, promotion to middle management position. As to personal factors, i.e. as to characteristic individual personality we found mostly rigid and serious trait, next immaturity and egotism, and timidity and nervousness. 5. The relation between the occupational and individual factors was examined from the psychodynamic point of view. From the result, O.M.A.S. was classified into 5 groups which consisted of the core group, dropout group, special job maladjustment group, transient reaction group and others. 6. In conclusion of the above mentioned data, the authors emphasize that it is important to explain and advise by the therapists not only for patients and their families, but also for their colleagues or superiors in their working place.

Ventricular size and isotope cisternography in patients with acute transient rises of intracranial pressure (plateau waves).

The size of the ventricular system and cerebrospinal fluid (CSF) flow were determined in 17 patients with plateau waves, using computerized tomography (CT) and isotope cisternography. Some patients had increased intracranial pressure (ICP) resulting from space-occupying lesions and other causes, and some had normal ICP observed in normal-pressure hydrocephalus. The size and shape of the ventricular system during plateau wave phases as ascertained by CT showed little or no change as compared with its size and shape during the interval phases between two waves. It was also noticed that, in patients with supratentorial masses, the midline shift showed no difference in degree between the two phases. These findings suggest that there is little change in the intracranial CSF volume between the two phases, that is, there is little compensatory outflow of the intracranial CSF for the ICP variations. These results may also support the assumption that the plateau waves are not caused by an intermittent obstruction of the CSF pathways. Isotope cisternography showed a marked delay of clearance of radioactivity from the intracranial CSF in 15 patients. The cisternographic pattern in patients with increased ICP and the absence of ventricular dilatation demonstrated an abnormally large accumulation of radioactivity over the cerebral convexities, and the pattern in patients with normal-pressure hydrocephalus showed complete obstruction of the subarachnoid space over both cerebral convexities. These observations suggest that, in patients with plateau waves, there is a marked delay in CSF absorption. The authors postulate that the reduction of CSF absorption may create a critically tight condition within the cranial cavity and act as a contributory factor in the development of the plateau waves.

Mesenchymal-epithelial interactions in hormone-induced development.

An hypothesis of hormone action upon epithelial growth, morphogenesis and cytodifferentiation is proposed in which certain hormonal effects upon epithelial cells are postulated to be mediated by nonsteroidal growth factors or morphogens elaborated by hormone-sensitive mesenchymal cells. Through this process mesenchymal cells induce specific patterns of epithelial morphogenesis and functional (biochemical) activity within urogenital epithelium.

Effect of progesterone and FSH on testes of immature chickens.

The effect of progesterone (P), estradiol benzoate (EB) plus P, and FSH on the testicular weight, macromolecular content, and incorporation of 3H-leucine, 3H-uridine and 3H-thymidine into macromolecules was examined in the testes of immature chickens. Not P alone but EB plus P lowered the testicular weight, and content of protein and RNA in the tissues, whereas FSH brought about an increase in the weight, and content of the two macromolecular elements. The DNA content was not affected by any hormones used. P, EB plus P and FSH brought about a decrease in the incorporation of radioactive leucine, uridine and thymidine into the macromolecules in the testes, with the exception of no change in the incorporation of 3H-uridine after EB plus P treatment and 3H-thymidine after FSH treatment. These results suggest that P, alone or in combination with estrogen, and FSH cause several changes in metabolic activity in the testes of immature chickens.
