The influence of renal sympathetic nerves on renal hemodynamic and renin responses during hypercapnia in dogs.

Studies were conducted in anesthetized dogs to examine the influence of the renal sympathetic nerves on renal hemodynamic and renin responses during controlled hypercapnia. The dogs were subjected to unilateral denervation and tested for their responses to hypercapnia induced by inhalation of 15% CO2 in air. Simultaneous measurements of the responses from both the denervated and innervated kidneys allowed an assessment of the influence of the renal nerves on the responses during acute hypercapnia. The data indicate that reductions in renal blood flow and glomerular filtration rate and increases in renin of the renal vein during respiratory acidosis are dependent, in part, on the presence of intact renal nerves. Other factors, however, are probably also present.

The cause of low voltage QRS complex in primary hypothyroidism. Pericardial effusion or thyroid hormone deficiency?

In order to clarify the cause of low voltage QRS complex seen on ECG (low voltage), thyroid hormones, LDH isozyme and pericardial effusion (PE) were studied in 39 patients with primary hypothyroidism. Low voltage and PE were found in 12 of 39 (30.8%) and 12 of 27 (44.4%), respectively. Serum T4 in patients with low voltage (Group 1) was significantly lower than that in patients without low voltage (Group 2) (T4 1.4 +/- 1.7 vs 2.8 +/- 2.3 micrograms/dl, p less than 0.05). Group 1 had a higher incidence of large amounts of PE than Group 2 (6/8 vs 1/19, p less than 0.002). However, there was no significant difference in thyroid hormone levels between patients with and without PE. No significant difference in LDH isozyme pattern was found among the groups. Low voltage without PE was found in only one patient. Two patients with low voltage and PE demonstrated that in spite of the presence of large PE, low voltage improved after thyroid replacement therapy. By multi-variate analysis, low voltage was related to large PE, patient age and low T4 levels. From these results, it was suggested that in hypothyroidism low voltage was brought about by a combination of both severe thyroid hormone deficiency and large PE. In addition, elderly patients over 60 years had low voltage more frequently than did patients under 59 years.

[Studies on the occupational maladjustment syndrome (Part 2). In reference to relationship between psychophysiological characters and clinical features].

In a previous report (Natsume et al., Jpn. J. Ind. Health, Vol. 24) on the occupational maladjustment syndrome (O.M.A.S.), on the basis of clinical data, we classified patients with O.M.A.S. into 5 subcategories; core, drop-out, transient reaction, special job maladjustment and other. The core and drop-out types are typical of O.M.A.S. In order to test our hypothesis regarding the clinical and psychophysiological character of patients with O.M.A.S., 39 subjects (25 core type and 14 drop-out type) were studied by polygraphic techniques such as EEG, EMG of facial muscles and electrooculogram, and the results were compared with those of 16 normal subjects. We were specifically interested in their behavior in mental arithmetic and at rest. The results are summarized as follows: At rest, less frequent irregular rapid eye movement (especially "r-type") and smaller amplitudes of the corrugator supercilii muscle characterized the patients compared to the normals. During mental arithmetic these two indicators showed a greater tendency to increase in the core type than in the normals, but they remained unchanged in the drop-out group. In the core type subjects, the increased levels in these indicators persisted briefly after the arithmetic sessions. We discuss the relationship between the clinical character of the two types and the above results. We consider that the psychophysiological character of the two types may play an important role in the pathogenetic mechanism of O.M.A.S.

[Study of somatosensory evoked response (SER) in patients with occupational vibration disease. Effect on SER induced by load of hot and cold water, and depressing the brachium].

Somatosensory evoked responses (SERs) to mechanical stimulation were recorded in 5 patients with occupational vibration disease and 10 grader drivers, and the results were compared with those obtained from 48 normal subjects. SERs were recorded at room temperature of 20-23 degrees C and after immersing the hand in hot (40 degrees C) or cold (10 degrees C) water for 5 minutes. SERs were also recorded while depressing the brachium and soon thereafter to examine the effect of ischemia. The following results were obtained. In SERs recorded on the finger of patients with occupational vibration disease, it was observed that some peaks of SERs were absent in 6 out of the 10 cases and that peak latency time was abnormally prolonged in 3 out of them. On the other hand, in SERs recorded on the forearm, abnormality was observed in only 1 out of them. In recording SERs in 6 patients with occupational vibration disease after immersing the hands in hot water, it was observed that abnormal SERs became normal in all 6 cases. However, these normalized SERs became abnormal in 5 out of the 6 cases within 25 minutes after removing the hands from hot water. No appreciable change could be observed in SERs recorded after immersing the hands in cold water. Partial absence of SER peaks could be observed in 2 out of 9 grader drivers while depressing the brachium. On the other hand, all SERs in 7 normal cases did not show any abnormality while depressing the brachium. The results suggest the presence of a close relationship between peripheral vascular dysfunction and peripheral somatosensory disturbance in patients with occupational vibration disease. However, it was observed that peripheral vascular dysfunction could not always trigger peripheral neural dysfunction. These findings have led to the conclusion that SERs to mechanical stimulation are more useful in the diagnosis of dysfunction of the peripheral nerve in patients with occupational vibration disease than SERs to electrical stimulation. The findings suggest that immersion in hot water and cold water and depression of the brachium are useful sensory tests for patients with occupational vibration disease and grader drivers.

[Effect of anticancer agents on rat prostate. Evaluation of organ weight, histological finding and 5 alpha-reductase activities].

To evaluate the effect of anticancer chemotherapeutic antigens on rat prostate, ten kinds of anticancer agents corresponding to the dose generally used for humans were intraperitoneally injected to 63-day-old Wistar rats. The anticancer agents were administered as follows: Cyclophosphamide (CPM) was used at the dose of 8 mg/kg for 7 days. Methotrexate (MTX), actinomycin-D (ACD) and cis-platinum (CDDP), 163 micrograms/kg, 8 micrograms/kg and 833 micrograms/kg for 5 days, respectively. Nitrogen mustard (NM), bleomycin (BLM), peplomycin (PLM), adriamycin (ADM), vincristine (VCR), and vinblastine (VBL), 500 micrograms/kg, 250 micrograms/kg, 170 micrograms/kg, 2.5 mg/kg, 33 micrograms/kg and 83 micrograms/kg, twice in a week, respectively. The rats were killed on the fifth day after completion of the schedule. Then, the weight of the body, the prostate, the epididymis and the adrenal gland were measured. In addition, 5 alpha-reductase activities and histological findings in the prostate were examined. For determination of 5 alpha-reductase activities, cell-free homogenate obtained from the rat ventral prostate was incubated with C14-testosterone at 37 degrees C for 30 minutes in an atmosphere of 95% of O2 and 5% of CO2. Subsequently, the metabolites from testosterone were separated and purified with thin layer chromatography using the solvent system with benzene acetone, 4:1 (v/v). 5 alpha-Reductase activity was determined with the sum of dihydrotestosterone (DHT) and androstanediol converted from testosterone and indicated as pmol product/mg protein. The 5 alpha-reductase activity was employed as a biological marker for the degree of androgenic dependency in the prostate. The results were summarized as follows. CDDP significantly reduced the weight of the body (p less than 0.001, n = 7), but not the activity of 5 alpha-reductase. NM and VBL had a specific action to reduce the weight of the prostate (p less than 0.01, n = 8) without causing loss of body weight. NM and VBL showed no influence on 5 alpha-reductase activities. The activity of 5 alpha-reductase was markedly damaged by BLM (p less than 0.05, n = 6) and PLM (p less than 0.05, n = 5). However no significant reduction was recognized in the weight of the body and the prostate. CPM, MTX, ACD, ADM and VCR were ineffectual on the body and the prostate weight and 5 alpha-reductase activities. In the histological examination, atrophy and degeneration of the glandular epithelium were revealed in the prostate treated with NM, VBL and CDDP.(ABSTRACT TRUNCATED AT 400 WORDS)

[Retrospective study of short survival cases in hepatocellular carcinoma after transcatheter arterial embolization therapy].

Twenty-two patients with hepatocellular carcinoma were treated with transcatheter arterial embolization therapy. In the six who died within about a month, serum albumin and Ch-E were lower, and total bilirubin and ICG R15 were higher than in the other cases. In four of them, more than 50% of the liver was occupied by tumor, and tumor thrombosis were found in the portal trunk or bilateral first portal branch. Five patients died of hepatic failure followed by upper gastrointestinal bleeding. One died of cachexia. The causes of short survival were 1) severe liver cirrhosis, 2) portal obstruction, 3) large tumor, 4) widespread TAE, 5) retrograde flow of gelfoam.

[Alteration of blood coagulation and fibrinolysis after transcatheter arterial embolization of hepatocellular carcinomas].

Alteration in blood coagulation and fibrinolysis were observed serially after TAE 37 times by measuring PT, APTT, FDP, fibrinogen, AT III, plasminogen, and alpha 2-macroglobulin. Immediately after TAE, PT and APTT were prolonged, Fbg AT III, alpha 2-M, and platelets decreased, and FDP increased. These changes within 24 hours suggested that coagulation activities were accelerated, and fibrinolysis followed. Within four weeks after TAE, PT and APTT became most prolonged one to three days after, while AT III, Plg, and alpha 2-M became lowest on the third to seventh days. It is suggested that these decrease were mainly due to suppressed production resulting from damage of the residual liver.

Isozymes of human erythrocyte pyrimidine 5'-nucleotidase--chromatographic separation and their properties.

The biochemical properties of two isozymes of human erythrocyte pyrimidine 5'-nucleotidase (P5N) separated by ion-exchange chromatography were studied. These two isozymes, P5N-I and P5N-IIB, can be distinguished from each other by substrate specificity, pH dependent activity and thermostability. P5N-I is thermolabile and dephosphorylates various pyrimidine 5'-monophosphates optimally around pH 7. UMP and CMP were found to be most effective substrates. P5N-IIB is thermostable and characterized by its high Michaelis constant and maximum velocity for dUMP or dTMP and acidic pH optima. The molecular weight was estimated to be 39,000 for P5N-I and 44,000 for P5N-IIB by gel filtration. These data suggest that these two isozymes are independent proteins and probably encoded by distinct structural gene loci.
