[Genetic analysis of the class II region of rat major histocompatibility complex (RT1)].

Class II subregions of rat major histocompatibility complex (RT1) consist of RT1. B (HLA-DQ equivalent), D (HLA-DR equivalent), and a newly defined H (HLA-DP equivalent) subregions, arranged in the order of RT1.A (class I-H-B beta (beta 1, beta 2)-D (alpha, beta) RFLP analyses of RT1. B and D subregions confirmed that RFLP patterns were correlated well with the allelic specificities which have been determined by serology or MLR. RT1.B locus of BDIX was shown to have RT1.Bd. RFLF analyses of intra-RT1 recombinant rats revealed that recombinations occurred at the sites between RT1.H and B in NIG III, WIN, KGH, between RT1.A and H in DA. BI, BDIX, and between RT1. H alpha and H beta in LEJ. Taken together, the order of genes around H subregion was determined as follows: RT1.A (class I)-H beta-H alpha-B-. Expressions of H subregion genes has not been known. However, Northern hybridization analysis with DP alpha probe detected mRNA of approximately 1.7 kb, suggesting the transcription of RT1. H alpha gene.

[Neoplastic angioendotheliosis: a case of T cell lymphoma positive for S-100 protein].

A 63-year-old woman presented with painless, firm, subcutaneous nodules on her legs and trunk. The lesions were annular or serpiginous and their surface was livid-red to pale-red. Superficial lymph nodes were not palpable. Upper gastrointestinal series, barium enema, Ga scintigraphy, and CT findings were negative. Histological examination revealed deep dermal vessels occupied by neoplastic cells and fibrin thrombi. We diagnosed her as having neoplastic angioendotheliosis. Electron microscopically, tumor cells lacked Weibel-Palade bodies. Immunohistochemically, the tumor cells were positive for leukocyte common antigen, T11, MT-1, HLA-DR and S-100 protein. They were negative for Factor VIII-related antigen, epithelial membrane antigen, carcinoembryonic antigen, B-1, and B-2. Immunoelectron microscopic study showed that their nuclei, cytoplasma, and cell membranes were positive for S-100 protein. Based on these findings, we diagnosed her as having T cell lymphoma. The patient rejected chemotherapy and was discharged. Three months after discharge, right hemiplegia developed. Brain CT scan revealed enhanced masses in the left frontal, temporal, and occipital lobes. Craniotomy showed only gliosis and hemorrhage. She died of cerebral bleeding three months after craniotomy.

[Spontaneous remission of four months' duration in hypoplastic leukemia with tetraploid chromosome after blood transfusions and infection].

Spontaneous complete remission of four months' duration was observed in a 51-year-old male with hypoplastic leukemia. Cytogenetical analysis revealed that leukemia cells of this patient were tetraploid. The diameter of leukemia cells involving myeloid cells ranged from 30 to 50 mu. The remission was apparently associated with repeated blood transfusions and severe infection. Complete remission was confirmed by normal morphology and karyotype of the bone marrow cells, although in vitro marrow stem cell growth did not return to normal. Thus, normal hematopoiesis may not have recovered when the diagnosis of spontaneous remission was made.

[Renography and renal imaging with 123I-orthoiodohippurate; comparison with 131I-orthoiodohippurate].

Renography and renal imaging with 123I-orthoiodohippurate (OIH) were performed in 29 patients with various renal diseases, and a comparison made with 131I-OIH. In all patients, renograms and images with 123I-OIH were superior to those with 131I-OIH, because of the increased density and image excellence of 123I-OIH. In addition, dynamic images to evaluate renal blood flow, as well as high quality sequential excretion images, can be obtained with 123I-OIH. From our clinical results, 123I-OIH is expected to replace 131I-OIH in renal function studies.

Soluble interleukin-2 receptors in plasma cell dyscrasias.

Levels of the soluble form of the interleukin-2 receptor (sIL-2R) were evaluated in the peripheral blood of 69 patients with plasma cell dyscrasias. A close relationship was seen between serum sIL-2R levels and clinical features. Among patients with normal BUN and creatinine levels, the mean (+/- 1SD) level of sIL-2R in 44 patients with multiple myeloma (MM) was higher than that of normal controls (457 +/- 227 U/ml vs 288 +/- 124 U/ml, P = 0.01). The mean level of sIL-2R in eight patients with primary macroglobulinemia was 722 +/- 251 U/ml. In MM, those with active or refractory disease showed a significantly higher mean level of sIL-2R than those in the remission phase (577 +/- 240 U/ml vs 335 +/- 103 U/ml, P = 0.01). There was a negative correlation between sIL-2R and hemoglobin levels in MM patients (r = -0.45, P = 0.01). Five patients with complications of renal insufficiency had elevated levels of sIL-2R. In a longitudinal study of a patient with plasmacytoma and an extremely high sIL2-R level, the sIL-2R level showed a strong relationship with tumor burden. Patients with high sIL-2R levels generally had a poor prognosis than those with normal levels. Thus a high sIL-2R level may be an indicator of a poor prognosis in MM.

[A clinical study of 55 cases of renal cell carcinoma].

Fifty five cases of renal cell carcinoma were treated by nephrectomy at our department between 1970 and 1987. A retrospective analysis was performed with TNM staging system of the General Rule for Clinical and Pathological Studies on Renal Cell Carcinoma, which was established by the Japanese Urological Association in 1983. Overall 3, 5 and 10-year survival rates by Kaplan-Meier method were 74, 67 and 52%, respectively. Several factors which might be related to prognosis were analyzed. The pathological T-stage and histological grade of tumors were found to be significant prognostic factors. As compared to these factors, venous invasion appeared to be less important. With respect to clinical findings, erythrocyte sedimentation rate and C-reactive protein were found to be prognostic factors.

[Aggressive natural killer cell leukemia/lymphoma--possible existence of a new clinical entity originating from the third lineage of lymphoid cells].

The morphologic, immunologic, genotypic, and functional properties of peripheral blood and bone marrow cells or cultured cells from two patients with a clinically aggressive non T, non B natural killer cell lymphoma/leukemia (ANKL/L) were described. The leukemic cells possessed medium to large granules in the cytoplasm, antigens against CD 38, CD 2, OKIal, and NKH-1 (N 901) monoclonal antibodies on their cell-surface, and also showed a high natural killer (NK) activity. In addition, these ANKL/L belonged to neither T-nor B-cell lineage, proved by studying clonal gene rearrangement for the T beta and T gamma receptor, and immunoglobulin. After we compared and investigated them with 9 cases of ANKL/L reported in other institutions, concerning immunophenotype, genotype and function, we reached the conclusion that the existence of ANKL/L originating from the third lineage in lymphoid cells is an obvious fact, suggesting this new clinical entity. It is important that all patients who have this type of a clinical disorder be diagnosed that there is no effective form of therapy at present.

[The modes of spread of pancreas head cancer based on difference of the pancreatic anlagen].

Embryologically the pancreas head consists of a dorsal and a ventral portion and these were able to be differentiated by the shape of the islets and the density of pancreatic polypeptide (pp) cells. We examined histologically the modes of spread of carcinoma of the head of the pancreas. The material were 15 cases with T1 and T2 cancer resected (T1;8;T2;7) and included 8 tumors arising from a ventral pancreas and 7 dorsal. We conclude that most of pancreas head cancers arising from a ventral pancreas involved the retropancreatic soft tissue at the early stage, and those arising from a dorsal pancreas showed an apparent tendency of a spread to the epithelial linings of the main pancreatic duct.

[Pathophysiology and laboratory tests of hemolytic anemia: with special reference to erythroenzymopathies].

Since the discovery of glucose-6-phosphate dehydrogenase (G6PD) deficiency and pyruvate kinase deficiency, erythroenzymopathies associated with hereditary hemolytic anemia have been extensively investigated. Kinetic and electrophoretic studies have shown that most erythroenzymopathies are caused by the production of a mutant enzyme. Single amino acid substitutions have been determined in G6PD and phosphoglycerate kinase variants by studies of the enzyme. Except for these two enzymes, it has been difficult to purify and to characterize the patient's enzyme because of the low protein contents in red blood cells. Recent advance in recombinant DNA technology has made possible the isolation of normal genomic DNA or cDNA for several enzymes. These results permit us to study the molecular basis of erythroenzymopathies at the nucleotide level. Single base substitutions have been identified in aldolase, triosephosphate isomerase, G6PD and adenylate kinase variants by the cloning and nucleotide sequence of the patients' genes. To date, all of the enzyme-deficient variants which have been investigated are caused by point mutations. An exception is a hemolytic anemia secondary to increased adenosine deaminase (ADA) activity. Red cell ADA activity increases on the order of a hundred-fold in affected individuals. The basic abnormality appears to result from overproduction of structurally normal enzyme due to abnormal translational efficiency.

[Treatment of urinary tract infection with cefixime].

Cefixime (CFIX) was administered orally in the treatment of 59 cases of urinary tract infection (UTI). According to the response criteria defined by the Japanese UTI committee, the rate of clinical efficacy for 26 cases of uncomplicated cystitis was 96.2%. The rate of clinical effectiveness of five patients with complicated UTI and five with uncomplicated pyelonephritis was 80.0%. In the other 23 cases which did not meet the response criteria, the efficacy rate was also high. The results indicated that CFIX was an effective drug for the treatment of UTI.

[Anesthetic management of thrombotic thrombocytopenic purpura].

A 46-year-old man, with TTP and renal failure, received splenectomy because complete remission was not achieved after the treatments such as plasma exchange, antiplatelet agents and corticosteroid. After the administration of methylprednisolone, anesthesia was induced with thiamylal and succinylcholine chloride and maintained with nitrous oxide, enflurane, fentanyl and vecuronium. Total blood loss was 1710 gram. Four units of packed red cell and 9 units of fresh frozen plasma were infused. The anesthetic course was uneventful. TTP is a rare and usually fatal disease and its etiology is unknown. Careless infusion of platelet or fresh blood may accelerate the disease process and multiorgan dysfunction may present other anesthetic problems. In this report, problems of anesthetic management for TTP were discussed.

[Relation of karyotypic findings to chemotherapy response in patients with preleukemia].

In 1982, the French-American-British (FAB) Cooperative Group proposed the myelodysplastic syndrome (MDS), which comprised a group of heterogeneous hematologic disorders characterized by various degrees of cytopenias and a possibility of leukemic transformation. And chromosome findings of bone marrow cells from patients with this syndrome are reported to have one of the greatest prognostic significance. In our chromosome study of 97 patients with MDS, we found clonally abnormal karyotypes in 61 patients (63%), including the whole chromosome or partial loss of long arm of chromosome 7 (-7/7q-) in 18 patients and a partial loss of chromosome 5 (5q-) in 14 patients. Among these 97 patients, 26 received cytostatic chemotherapy (small dose of cytarabine; 23 patients, small dose of behenoyl Ara-C; 2, and intensive combined chemotherapy; 1) as well as supportive care. Among these 26 treated cases, pretreatment hematological characteristics or FAB subclassifications were not associated with response to chemotherapy. In contrast, only one of 9 patients with -7/7q- and/or 5q- achieved a partial remission, while 5 of 8 patients with other abnormal karyotypes and 4 of 9 patients with normal karyotype achieved a complete (4 patients) or a partial remission. In conclusion, pretreatment cytogenetic findings can predict the potential responders to this chemotherapy, and may be one of the most important factors in the selection of the treatment for this ill-managed syndrome.

Papillary cystic tumor of the pancreas: an immunohistochemical and ultrastructural study of 14 patients.

We report herein the clinicopathological, immunohistochemical and ultrastructural studies on 13 female patients and one male patient with papillary cystic tumors of the pancreas. Their ages ranged from 12 to 60 (mean 25) years. Most patients complained of abdominal mass or abdominal pain. Following complete resection of the tumor, all have remained well for between 3 months and 19 years (maen 5 years). In one patient the tumor was malignant and, 10 years after the initial partial resection, there was a recurrence with involvement of the colon, metastasis to the lymph nodes and venous invasion. Immunohistochemically, most tumor cells were positive for neuron specific enolase, synaptophysin, alpha-1-antitrypsin and vimentin and sometimes for the estrogen receptor related antigen, ER-D5, and the oncogen product of c-Ha-ras, c-Ha-ras P21. Ultrastructurally there were zymogen-like intracytoplasmic granules, intercellular junctions and intercellular spaces. These results support the hypothesis that the tumor originates from undifferentiated cells capable of differentiation toward acinar, endocrine or ductular cells. Estrogen and the c-Ha-ras oncogene presumably are linked to the development.
