[Application of multi-lines fitting technic for ethenzamide elimination with capacity-limited process in the rabbit plasma].

Intravenous and oral administrations of ethenzamide (EZ) were carried out in the rabbit, and elimination process pharmokinetically discussed. When the drug dose increased, plasma clearance and extent of bioavailability were reduced and plasma peak times delayed after oral dose of EZ. Michaelis-Menten type elimination parameters were estimated from the plasma concentration-time profiles after intravenous dosing of EZ. The first-order absorption rate constant (ka), hepatic available fraction (FH) and approximate elimination rate constant (K) for hepatic first-pass metabolism of EZ were estimated using computer multi-lines fitting technique by iterative nonlinear least squares regression program, MULTI (RUNGE).

Multiple myeloma in alcoholic liver cirrhosis.

A case of multiple myeloma (Bence Jones, lambda) associated with alcoholic liver cirrhosis is reported. A 56-year-old Japanese male died of hepatic failure and hypercalcemia. Autopsy revealed alcoholic liver cirrhosis and plasma cell myeloma. Immunoelectrophoretic analysis of his reserved serum disclosed the presence of M component of lambda Bence Jones protein. IgA and lambda light chain were demonstrated in the cytoplasm of the myeloma cells. Complications such as generalized amyloidosis, metastatic calcification, myeloma kidney and hemorrhagic pancreatitis were noted. The coexistence of multiple myeloma and liver cirrhosis has rarely been reported. On the basis of a review of the reported cases, a possible association between both diseases was discussed.

Different responses of albumin-containing and alpha-fetoprotein-containing cells to estriol in the adult mouse liver.

A single intraperitoneal injection of 10 mg estriol (E3) rapidly induced a transient decrease in the serum albumin (ALB) level in 3 to 5 days in adult mice. At a dose of 5 mg, E3 showed a remarkable threshold effect on the decline of serum ALB. By immunohistochemistry, two types of ALB-positive cells were found in the liver. Type 1 cells were stained intensely and almost homogeneously throughout all or nearly all their entire cytoplasm, and type 2 cells showed only granular or vesicular cytoplasmic deposits. After administration of 10 mg E3, the type 1 cells diminished rapidly and markedly in direct proportion to the fall in serum ALB from the centro- and mediolobular zones, where they had been normally localized either singly or in small groups. Simultaneously, the granular or vesicular deposits that had been normally stained very weakly in the type 2 cells in the centro- and mediolobular zones became intensely stained along with cells in the perilobular zone. Alpha-fetoprotein (AFP)-containing cells in the liver appeared in maximal numbers at 5 to 7 days after E3 administration. These serological and cytological changes after E3 administration are here discussed on the basis of a possible inverse relationship between ALB and AFP gene expression.

[Changes of muscle activities in neck and shoulder region during sustained isometric contractions of jaw muscles].

Eight healthy participants performed horizontal isometric jaw movements with maximum voluntary contractions of the jaw muscles until they could not tolerate the task. Electromyographic activities bilaterally recorded from the superior and inferior regions of the sternocleidomastoid muscle and the nuchal and shoulder regions of the trapezius muscle were observed. The results obtained were as follows: 1. In more than 94% of 32 trials, the amplitude of EMGs during the task were higher than those at rest, and in more than 79%, temporal changes in EMG burst were synchronized with the onset-offset of the exercise. 2. In more than 72% of all trials, small amplitude EMG lasted after cessation of the task. 3. Muscle activities of the lateral cervical region during the exercise were superior to the activity of the nuchal and shoulder regions, and amplitude of EMGs in the neck and shoulder regions tended to be the highest during isometric retrusion. 4. These results don't contradict with our hypotheses that occlusal habits might be one cause of the unpleasant sensation in the neck and shoulder regions and that sustained isometric retrusion might have heavier effects on it.

[Influence of a repetitive commanded acoustic signal with constant frequency on jaw tapping movement].

Influence of a repetitive commanded acoustic signal with constant frequency of 1.3 Hz on jaw tapping movement was studied. 10 healthy subjects were made to perform anticipated jaw tapping movement corresponding to the signal. Jaw tapping movement with and without signal were analyzed for their opening distance and a ratio of duration of opening phase I (depressing phase), opening phase II (plateau phase), closing phase and tooth contact phase to the cycle time. 1. Opening distance in loaded tapping was shorter than that in the tapping ad lib. (P not equal to 0.01), and independently of commanded signal it became to be longer with increasement of trial numbers (P not equal to 0.01). 2. The ratio of duration of tooth contact phase in loaded tapping became smaller with increasement of the trials, although that in the tapping ad lib. became greater (P less than 0.001). 3. The ratio of duration of opening phase I in loaded tapping became greater with increasement of the trials, while that in the tapping ad lib. became smaller (P not equal to 0.01). 4. It could be interpreted that in the loaded tapping "an instant performance control" acted from the beginning of the trials and the signal might make it facile or easy to detect and confirm errors in movement.

[Clinical evaluations on removability of removable partial dentures].

The following results were obtained from studying the removability of 53 removable partial dentures. 1. All of their retentive forces were less than 2,000 gf. 2. In 5 out of 31 dentures whose retentive forces were graded D2 and less, the wearers could remove the dentures easily without help of their fingers. Out of 22 dentures graded D4 and more, 3 conic dentures were difficult in removal. 3. The wearers were dissatisfied with 3 dentures graded D2 and less, and with 2 conic dentures graded D4. 4. In 12 dentures graded D1, 9 dentures were evaluated to be too loose by the operators. In 12 dentures graded D4 and more, 2 conic dentures were evaluated to be too tight by both the wearers and the operators. 5. All conic dentures complained had 4-degree-tapered conic studs as the retainer. 6. It is considered that in terms of subjective evaluations, an adequate retentive force for the dentures might be in the range from 800 gf to 2,000 gf.

[An experimental study on retentive force of a cone type denture retainer].

This study was made to clarify the retentive force of a denture retainer, i.e., a cone-type stud and sleeve attachment. In order to measure the wearing effect, an engaging and disengaging cycle test of 1,000 times was performed. The effects of taper angle (cone angle), sleeve height and thrusting force at engaging stud and sleeve on retentive force were also analyzed. 1. The cycle test had little effect on the retentive force (P not equal to 0.41). 2. The retentive force became greater with decrease of taper angle (P less than 0.0001). 3. The effect of the sleeve height on retentive force differed with taper angle (P less than 0.0001). 4. The retentive force became greater with increase of thrusting force (P = 0.0009), and this effect was greater as the taper angle decreased (P less than 0.0001). 5. The contributions of the factors were approximately 38% in taper angle, 30% in thrusting force and 2% in sleeve height. 6. It could be interpreted that we had better use the retainer with taper angle of 6 to 8 degrees, although the retainer with taper angle of 5 degrees could be applicable to the tooth on which the less occlusal force transmitted.

[Histochemical study of external urethral sphincter in the rabbit. Analysis with construction of histograms].

Male rabbit's external urethral sphincter was examined by histochemical muscle fiber typing (myosin ATP-ase staining), and the analysis with construction of histograms regarding to muscle fiber types were performed. Rabbit's external urethral sphincter was predominantly composed of fast twitch (type 2) fibers (87.3%) as a whole. But the proportion of constituent fiber types varied according to the layers, i.e., the slow twitch (type 1) fibers constituted a relatively high percentage (33.4%) in the inner third layer, while few of the type 1 fibers were found in the outer third layer. The all histograms regarding to fiber type in different layers were normal bell-shaped distribution curves. The mena diameter of type 2 fibers (14.7 microns) was evidently larger than that of type 1 fibers (20.5 microns). All three kinds of muscle fibers equally tended to increase in size toward the outer direction, and in every three layers, the diameter of type 2 was larger than that of type 1 also. The definite differences in the proportion of fiber types and fiber sizes between layers may implicate that the inner and outer layers play different roles, i.e., continuous tonic constriction in the former and sporadic strong constriction of short duration in the latter, under different neural regulations. As far as rabbit's external urethral sphincter is concerned, sporadic strong constriction should be mainly dependent on the muscle fibers of large size composing the outer layer, especially the fast twitch fibers. It is possible that the rabbit is so adapted that it could interrupt urination promptly.

In vitro immunosuppressive properties of spergualins to murine T cell response.

Deoxyspergualin has strong immunosuppressive activity in animals. However, it shows less in vitro immunosuppressive activity at the therapeutic concentration used for in vivo administration. Recently, we reported that there are some technical problems with in vitro experiments. In this report, the effects of spergualins were examined under in vitro conditions which excluded these problems, and compared with cyclosporine A (CYA). Spergualins have suppressive effects on mixed lymphocyte response (MLR) and cytotoxic T lymphocyte induction. Furthermore, interleukin-2 (IL-2) induced proliferation of concanavalin A blasts and CTLL-2 were inhibited at low concentration. However, spergualins have little effect in the early stage of MLR or the mitogen response. These results suggest that spergualins act on the proliferation and differentiation of T cells which respond to growth factors, such as IL-2.

[A case of hepatocellular carcinoma treated by the combined therapy of ultrasonic guided ethanol injection and transcatheter arterial embolization].

Reported is the case of a 72-year-old man admitted to hospital on February, 1986, because of a liver tumor. The patients laboratory data showed the following values: HBs Ag (-), ICG (R15) 34%, AFP 2650 ng/ml. A celiac angiogram revealed hypervascular tumor in the posterior lobe (s6), and a CT showed an intra-hepatic metastasis in the left lobe (s4). The diameters of these two lesions, measured ultra sound (US) were 50 mm (s6) and 30 mm (s4). These two lesions were treated by a US-guided ethanol injection with the posterior lesion also receiving by a transcatheter arterial embolization (TAE). After this combined therapy, serum AFP level decreased to 20 ng/ml and the two lesions became reduced in size. The patient has survived for two years though he is now receiving additional ethanol injection therapy for another metastasis lesion in the s5 region.

[The clinical course of nodular type hepatocellular carcinomas after transcatheter arterial embolization (TAE) therapy].

TAEs were performed for 14 cases of nodular type hepatocellular carcinomas. The tumor size in 10 of the 14 cases was less than 5.0 cm in diameter. After 2 to 3 TAEs for each of the cases, tumor regression was noted in 11 cases and a decrease in the serum AFP in every case. The 1-year survival rate (1 YSR) was 79%, the 2YSR was 42%, and 3YSR was 10%, Re-growth of the tumors was observed in 45% of the cases 1-year after, and in all cases 2-year after. In cases which had multiple nodules or an incomplete capsule or a poor accumulation of Lipiodol, re-growth occurred even earlier.

[CD3-, OKM1+, Leu7-, Leu11+ large granular lymphocyte leukemia with ascites and CNS involvement].

A case of large granular lymphocyte (LGL) leukemia with ascites and CNS involvement was reported. A 39-year-old Japanese female was admitted to our hospital in March, 1987 because of high fever. Her clinical and hematological features were characterized by generalized lymphadenopathy, marked hepatosplenomegaly, high serum LDH level (3,257 mU/ml), marked leukocytosis (71,000/microliters) with 74% LGLs and bone marrow infiltration with 57% LGLs. Despite of chemotherapy, ascites, retroperitoneal mass and CNS involvement developed and she died of sepsis after three months. LGLs from the patient's blood, marrow and ascites, stained positively for acid phosphatase. These LGLs were E rossete+ and Fc (IgG) receptor+ and were positive for CD2, OKM1, HLA-DR and Leu11, but were negative for CD1, CD3, CD4, CD8 and Leu7 as well as for terminal deoxynucleotidyl transferase activity. The natural killer activity against K562 target cells was high and was significantly augmented after stimulation by recombinant human interleukin 2. These LGLs also demonstrated normal antibody-dependent cytotoxicity activity. Cytogenetic study on bone marrow cells and ascitic cells revealed clonal chromosomal abnormalities. These clinical, hematological, immunological and cytogenetic findings suggest that this patient had a neoplastic proliferation of natural killer cells.

[Multiple primary neoplasms in patients with multiple myeloma and primary macroglobulinemia].

A review of 65 patients with multiple myeloma (MM) and 10 patients with primary macroglobulinemia (PM) seen at our clinic from 1972 through 1988 revealed further cancers that were found in 11 patients with MM and in 2 patients with PM. In 4 of 13 patients with multiple primary neoplasms, either MM or PM was an antecedent. In 6 patients, MM or PM was coincident with the cancers. In 3 patients, the cancer was antecedent. Only one patient with multiple myeloma developed secondary leukemia following treatment with alkylating agents.

[Multiple myeloma of IgG-lambda type associated with asymptomatic primary biliary cirrhosis].

Multiple myeloma associated primary biliary cirrhosis (PBC) is very rare and only two cases have been reported. In this paper, we reported the first case of male patient with asymptomatic PBC and multiple myeloma. A 66 year-old Japanese male was referred to our hospital for the further examination of a monoclonal gammopathy. He was diagnosed of multiple myeloma (IgG-lambda type) because of 2.3 g/day of Bence Jones proteinuria (lambda type), 3,401 mg/dl of monoclonal IgG (lambda type), 12.8% of bone marrow plasmocytosis and generalized osteoporosis. The alkaline phosphatase was 314 mU/ml and serum IgM level (polyclonal) 937 mg/dl. The patient was started on intermittent courses of melphalan and prednisolone, achieving transient improvement. After two years, hepatosplenomegaly developed gradually and the levels of serum ALP elevated increasingly. At that time, relevant investigation results were: serum ALP 663 mU/ml, serum IgG 4,144 mg/dl, serum IgM 823 mg/dl, positive anti-mitochondrial antibody test x 320. The liver biopsy showed chronic nonsuppurative destructive cholangitis. PBC (stage 1-2 according to Sheuer's criteria) associated with multiple myeloma was diagnosed. A pathogenetic relationship such as loss of immunoregulatory function could be speculated although the simultaneous occurrence of PBC and multiple myeloma could be coincidental.
