[Hemolytic anemia due to the dysfunction of the protection against oxidative attack].

The most important products of the hexose monophosphate pathway is reduced nicotinamide adenine dinucleotide phosphate (NADPH). Reduced glutathione (GSH) maintained by the reduction of oxidized glutathione (GSSG) using NADPH as a cofactor, is a major reducing agent in the red cell and the ultimate source of protection against oxidative attack. In the syndromes associated with dysfunction of the hexose monophosphate pathway and glutathione synthesis and metabolism, oxidative denaturation of hemoglobin is the major contributor to the hemolytic process. Glucose-6-phosphate dehydrogenase (G6PD) plays a key role in the generation of NADPH. G6PD deficiency is the most common metabolic disorder, and it is associated with chronic hemolytic anemia and/or drug- or infection-induced acute hemolytic attack. It is estimated that 400 million people are affected worldwide. The mutations responsible for 101 variants have been determined. Some of them have polymorphic frequencies in different populations. Most variants are produced by one or two nucleotide substitutions. Molecular studies have disclosed that most of the class 1 G6PD variants associated with chronic hemolysis have the mutations surrounding the site of dimer formation.

[Congenital dilatation and stricture of the bile duct as a cause of primary intrahepatic calculi].

Further studies on image techniques and hepatobiliary scintigraphy for diagnosis of biliary diseases revealed details of the whole biliary ductal system and appeared some kinds of biliary malformation, which are clinically important and had been unknown yet, such as anomalous arrangement of the pancreatico-biliary ductal system, congenital biliary strictures and congenital dilatation of the intrahepatic bile duct associated with choledochal cyst. We had 42 cases with primary intrahepatic calculi for these 12 years and they had Alonso-Lej Type I cysts with dilatation of the intrahepatic bile duct and congenital biliary strictures in the-hepatic hilum. Furthermore, we had 30 cases with both the biliary malformations, but without intrahepatic stones. Consequently, coexistence of both these anomalies of the bile duct is thought to be the basis for the formation of primary intrahepatic calculi.

[Proton MR spectroscopy findings in herpes simplex encephalitis].

Localized proton magnetic resonance spectroscopy (1H-MRS) was conducted in two patients with herpes simplex virus type I encephalitis (HSE). MR spectra of bilateral temporal lobes were acquired by the single voxel method using 1.5T unit. Peaks indicating N-acetyl aspartate (NAA), choline (Cho) and creatine including phosphocreatine (Cr) were identified and ratios of NAA/Cr and Cho/Cr were calculated. These ratios were compared with those of the contralateral side showing normal MRI findings and also with the control spectra obtained from normal volunteers. Three abnormal findings were observed in the spectra of the patients suffering from HSE; 1. significant reduction of the NAA/Cr ratio at the involved temporal lobe, 2. mild reduction of the NAA/Cr ratio at the normal temporal lobe, and 3. elevation of the Cho/Cr ratio at the bilateral lobes, but more significant on the involved sides. These results indicated that neural loss and gliosis occurred in the contralateral area with normal MR images as well as the involved hemisphere. We concluded that 1H-MRS is able to show the specific histological findings of herpes simplex encephalitis. This is the first report assessing 1H-MRS for patients with HSE in Japan.

[Cerebral metabolic disturbance in hypoxic encephalopathy: evaluation with H-1 MR spectroscopy].

PURPOSE: To analyze metabolic changes in the brain during hypoxic encephalopathy. MATERIALS AND METHODS: MR imaging (MRI) and H-1 MR spectroscopy (H-1 MRS) were performed on three patients with hypoxic encephalopathy, who had need temporary mechanical ventilation, and five healthy age-matched volunteers. Spectra were obtained from bilateral fronto-parietal gray matter and basal ganglia voxels. N-acetyl aspartate (NAA), creatine (Cr) and choline (Cho) were analyzed. RESULTS: (1) MRI; T1 weighted MRI revealed high signal intensity areas in the diffuse cerebral cortex and basal ganglia in two patients. The former was thought to reflect cortical laminar necrosis. T2-weighed MRI showed high signal intensity in diffuse white mate. (2) H-1 MRS; significant reduction of the NAA/ Cr ratio and elevation of the Cho/Cr ratio were detected. CONCLUSION: H-1 MRS is able to quantify metabolic alterations in the hypoxic encephalopathy.

[Computed tomographic findings in septic pulmonary emboli secondary to renal abscess].

A 73-year-old woman with diabetes mellitus and hypothyroidism had been well until 4 days before she presented with fever, productive coughing, and general malaise. Dyspnea gradually worsened, and she was admitted to Kobe City General Hospital. She appeared toxic and was in moderate respiratory distress on admission. There were scattered rales in both lung fields. The abdomen was soft with no tenderness. A chest roentgenogram showed peripheral infiltrates in both lung fields. Arterial blood gas analysis showed a Po2 of 48.6 Torr and Pco2 of 27.2 Torr. A blood culture on admission showed Escherichia coli. Computerized tomography of the chest showed multiple nodules of various sizes in the peripheral lung fields. Some nodules had necrotic centers and feeding vessels. Wedge-shaped lesions abutting the pleura were also seen. These findings strongly suggested septic pulmonary emboli. An abscess in the left kidney, which was considered to be the source of the septic pulmonary emboli, was found with Ga scintigraphy, ultrasonography, and computerized tomography of the abdomen. Treatment with antibacterial drugs was effective against the lung lesions but not the renal abscess. A left nephrectomy was done. In the proper clinical setting, characteristic CT features of septic emboli can contribute to early diagnosis of this disease.

Inhibition of tumor invasion and metastasis by peptidic mimetics of Arg-Gly Asp (RGD) derived from the cell recognition site of fibronectin.

The partially modified retro- and retro-inverso peptides of the Arg-Gly Asp (RGD) sequence of fibronectin, in which the direction of the Arg residue is reversed and/or the chirality of the amino acid residue is inverted, i.e., mainly R(rev)-COCH2CO-D and DR(rev)-COCH2CO-D, have been synthesized to examine their antimetastatic effects in murine lung or liver metastasis models, as well as their inhibitory effect on tumor cell invasion in vitro. R(rev)-COCH2CO-D inhibited lung metastasis produced by i.v. coinjection with B16-BL6 melanoma more potently than did other pseudo-peptides or the original RGDS peptide. Rrev-COCH2CO-D also showed antimetastatic effects against several different types of tumor cells such as B16-BL6 melanoma, Colon26 M3.1 carcinoma, and L5178Y-ML25 lymphoma cells, in a dose-dependent manner, and multiple administrations had a therapeutic effect on spontaneous lung metastasis. The invasion of melanoma cells into reconstituted basement membrane Matrigel in vitro was suppressed by R(rev)-COCH2CO-D more effectively than by RGDS. These results indicate that the antimetastatic effect by R(rev)-COCH2CO-D was in part due to the inhibition of tumor invasion. The RGDS peptide decomposed when incubated with fresh plasma in vitro, whereas R(rev)-COCH2CO-D was not affected by this treatment. Thus, the reversion of the Arg-Gly linkage in the RGD sequence resulted in protease resistance leading to the retardation of the clearance of the peptide in vivo, and consequently augmented its antimetastatic and antiinvasive properties. Designed peptide analogues may provide various advantages and be useful for preventing cancer metastasis.

Measurement of optic nerve head circulation: comparison of laser speckle and hydrogen clearance methods.

Microcirculation in the optic nerve head (ONH) in the ocular fundus was measured noninvasively using the recently developed laser speckle circulation analyzer. Results were compared with capillary blood flow (CBF) measurements made by the hydrogen clearance method. The ONH blood flow of six normal rabbits was measured simultaneously by both methods. Inhalation of 10% carbon dioxide (CO2) increased the CBF by 22% (hydrogen clearance method). Intravenous injection of endothelin-1 (ET-1) (10(-10) mol/kg) decreased the CBF by 19%. Significant correlations were found between the NB (normalized blur) value by the laser speckle method and CBF (r = 0.38, P < 0.05, n = 30) and also between the relative change of NB and CBF (r = 0.92, P < 0.01, n = 30) in the ONH. This indicates that evaluation of the NB change in the ONH is as accurate with the laser speckle method as with the hydrogen clearance method, giving a basis for the clinical application of the laser speckle method of measuring ONH circulation.

[The long-term results of the irradiation therapy for the dural arteriovenous malformation].

Although radiotherapy has been used to treat non-neoplastic vascular diseases, it is not widely accepted as a standardized treatment for dural arteriovenous malformations (DAVM). The uncertainties regarding the therapeutic outcome and the benign nature of DAVM are the major factors that limit the indication of radiotherapy. In an attempt to assess the long-term benefits and risks of the treatment, we analyzed 24 patients with DAVM treated with radiotherapy. The cavernous sinuses were involved in 12 and the transversesigmoid sinuses in 12. Doses ranging from 10 to 56 Gy (median 44.9 Gy) were delivered by multiple fractionation. In terms of clinical improvement, complete response was achieved in 19 patients and partial response in five. Four patients had recurrence of symptoms; two patients were reirradiated with the result of good symptomatic relief, one patient was successfully treated with transarterial embolization and the other patient with less disabling symptoms was managed conservatively. No adverse side effects attributed to radiation were encountered during the follow-up period of 0.5-17 years (median 8.5 years). Radiotherapy is a safe and effective method for the treatment of DAVM and is warranted in patients who are unsuitable for transarterial embolization or surgical procedure.

[Growth inhibitory effects of ubenimex on leukemic cell lines resistant to chemotherapeutic agents].

Ubenimex (Bestatin, Ubx) has been shown to have anti-tumor activity and immuno-modulating activities. Ubx has been used in immuno-therapy in combination with remission maintenance chemotherapy after induction of complete remission for adult acute non-lymphocytic leukemia (ANLL, AML). Daunomycin (DNR), arabinosylcytosine (Ara-C) and 6-mercaptopurine (6-MP) are used for the standard chemotherapy for ANLL. It is, however, believed that emergence of resistant cells to chemotherapy cause minimal residual leukemia resulting in poor prognosis. Ubx has been administered in combination with these chemotherapeutic agents. We examined the combinatorial effect of Ubx with DNR, Ara-C, 6-MP and etoposide on K562 leukemic cell line and the chemotherapeutic agent resistant cells derived from K562 cell line. Ubx showed growth inhibitory effects on these cell lines. A synergistic effect was observed on growth inhibition and with colony formation of parent k562 cell line when DNR and Ubx were used in combination. A combination of Ubx with Ara-C or etoposide showed additional effects on parent cells and other resistant cell lines. The combined growth inhibitory effect of 6-MP and Ubx was stronger than the effect of 6-MP alone. These results show that Ubx has a direct growth inhibitory effect on leukemic cells and additional or synergistic effects are obtained on K562 leukemic cell line and on chemotherapeutic agent resistant cells derived from the K562 cell line when Ubx is used combination with the above chemotherapeutic agents.

Inhibition by galloylglucose (GG6-10) of tumor invasion through extracellular matrix and gelatinase-mediated degradation of type IV collagens by metastatic tumor cells.

Tumor invasion into extracellular matrix (ECM) and basement membrane (BM) is a crucial step in the complex multistage process that leads to metastasis formation. GG6-10 galloylglucose, isolated from Galla Rhois, inhibited the invasion of metastatic HT-1080 cells into a reconstituted BM, such as a Matrigel/fibronectin (FN)-coated filter, in a concentration-dependent fashion. GG6-10 affected neither the tumor cell adhesion and haptotactic migration to ECM components (Matrigel and FN), nor the growth of HT-1080 cells. The gelatin zymography revealed that GG6-10 was able to inhibit not only the degradation of gelatin mediated by matrix metalloproteinases (MMP)-2 and -9 in conditioned medium of HT-1080 tumor cells but also the production of MMP from the tumor cells in a concentration-dependent manner. MMP production is well known to be positively regulated by various cytokines, such as tumor necrosis factor-alpha (TNF-alpha). Thus, we examined the effect of GG6-10 on the TNF-alpha-mediated translation of the MMP-9 gene using HT-1080 cells transfected with the MMP-9 promoter linked to the luciferase gene as a reporter. Similarly to prednisolone, GG6-10 was found to inhibit the TNF-alpha-inducible promoter activity. In keeping with these results, GG6-10 might inhibit tumor cell invasion by inhibiting the gelatinolysis mediated by MMP-2 and -9 and interfering with the production of MMP via inhibiting transcription of the promoter for MMP.

Infected aneurysms--clinical study of 5 cases.

The infected aneurysm has been assumed to be a disease with a poor prognosis due to the occurrence of aneurysmal ruptures and sepsis, in contrast to the outcome of atherosclerotic aneurysms. In the present study, we conducted surgical treatment on five patients with infected aneurysms (infected abdominal aortic aneurysm in three cases and iliac artery aneurysm in two cases). In particular, two of the three patients suffering from infected abdominal aortic aneurysms underwent extra-anatomic bypass and the remaining one case underwent vascular graft replacement in situ. In the two patients who underwent an extra-anatomic bypass, an aneurysm was found at the site of aortic stump closure. In the patient who underwent in situ replacement, wrapping was carried out using the omentum after vascular graft replacement, and the postoperative course was uneventful. Accordingly, we consider that the optimum primary therapeutic intervention for infected aneurysms is in situ revascularization followed by wrapping with the omentum after removal of the aneurysm and debridement of the surrounding infected tissue to the maximum extent possible.

Spectroscopic identification of the heme axial ligation of cytochrome b558 in the NADPH oxidase of porcine neutrophils.

The combination of electron paramagnetic resonance (EPR), near-infrared magnetic circular dichroism (NIR-MCD) and resonance Raman (RR) spectroscopies at cryogenic temperatures has been used to identify the axial heme ligation of the low spin cytochrome b558 component of NADPH oxidase from porcine blood neutrophils. The EPR and NIR-MCD results indicate the presence of two distinct forms in frozen solution; one with a low field g-value at 3.23 and porphyrin(pi)-to-Fe(III) charge transfer maximum at 1660 nm and the other a low field g-value at 3.00 and porphyrin(pi)-to-Fe(III) charge transfer maximum at 1510 nm. On the basis of these properties and the RR studies, both are attributed to forms of cytochrome b558 with bis-histidine axial ligation. The origin of the observed heterogeneity, the location and identity of the specific histidines involved in ligating the heme, and the role of the heme prosthetic group in O2- production are discussed in light of these results.

The POU domain transcription factor Brn-2 is required for the determination of specific neuronal lineages in the hypothalamus of the mouse.

We generated mice carrying a loss-of-function mutation in Brn-2, a gene encoding a nervous system specific POU transcription factor, by gene targeting in embryonic stem cells. In homozygous mutant embryos, migratory precursor cells for neurons of the paraventricular nuclei (PVN) and the supraoptic nuclei (SO) of the hypothalamus die at approximately E12.5. All homozygous mutants suffered mortality within 10 days after birth, possibly because of a complete deficiency of these neurons in the hypothalamus. Although neither developmental nor histological abnormalities were observed in heterozygous mice, the levels of expression of vasopressin and oxytocin in the hypothalamus of these animals were half these of wild-type mice. These results strongly suggest that Brn-2 plays an essential role in the determination and development of the PVN and SO neuronal lineages in the hypothalamus.

Structural elucidation of two novel amphoteric glycosphingolipids from the earthworm, Pheretima hilgendorfi.

The novel amphoteric glycosphingolipids containing a choline phosphate were purified from whole tissues of the earthworm, Pheretima hilgendorfi. Their chemical structures were completely characterized as cholinephosphoryl-->6(Man alpha 1-4)Gal beta 1-6Gal beta 1-1Cer (cholinephosphorylmannosylneogalabiaosylceramide, named PGL3a) and cholinephosphoryl-->6Gal beta 1-6Gal beta 1-6Gal beta 1-1Cer (cholinephosphorylneogalatriaosylceramide, named PGL3b) by compositional sugar, fatty acid and sphingoid analyses, hydrogen fluoride degradation, partial acid hydrolysis, methylation analysis, exoglycosidase degradation, proton magnetic resonance spectroscopy and fast atom bombardment mass spectrometry. The ceramide moieties of these lipids consisted of 22:0, 23:0 and 24:0 acids as major fatty acids, and branched octadeca- and nonadeca-4-sphingenines and octadeca-4-sphingenine as main sphingoids. Since the oligosaccharides and the ceramide moieties of PGL3a and PGL3b were identical with those of neutral glycosphingolipids found in this organism, the biosynthesis of the amphoteric glycolipids may occur by the addition of a choline phosphate residue to the corresponding neutral glycolipids, Man alpha 1-4Gal beta 1-6Gal beta 1-1Cer or Gal beta 1-6Gal beta 1-6Gal beta 1-1Cer.

Pyruvate kinase deficiency of mice associated with nonspherocytic hemolytic anemia and cure of the anemia by marrow transplantation without host irradiation.

Mutant mice with splenomegaly and nonspherocytic hemolytic anemia were found in an inbred colony of the CBA/N (hereafter CBA) strain maintained in the Japan SLC Haruno farm (Shuchi-gun, Shizuoka, Japan). The activity of pyruvate kinase (PK) in red blood cells (RBCs) of the anemic mutants decreased to 16.2% of normal (+/+) CBA mice. Because the mutant CBA mice showed a remarkable reticulocytosis (41.6%) and because the PK activity of reticulocytes is much higher than that of mature RBCs, the PK activity in mature RBCs of the mutant CBA mice was calculated to be 2.8% that of mature RBCs of CBA-(+/+) mice. Because RBC type PK is encoded by the Pk-1 locus of the mouse (chromosome 3), we designated the mutant locus as Pk-1slc. The anemia and PK deficiency of CBA-Pk-1slc/Pk-1slc mice were cured by bone marrow transplantation (BMT) from CBA-(+/+) mice. Prior irradiation was not necessary for the curative BMT. On the other hand, the BMT from CBA-Pk-1slc/Pk-1slc mice to nonirradiated CBA-(+/+) mice did not result in the decrease of RBCs and the reduction of PK activity. The present results indicate that CBA-Pk-1slc/Pk-1slc mice are a potentially useful animal model for studying pathophysiology of PK deficiency and for developing new therapeutic methods to correct PK deficiency.
