[An investigation into the levels of a trimethoprim-sulphamethoxazole combination in the ventricular fluid of neurosurgical patients (author's transl)].

Results of measurements of Trimethoprim-Sulphamethoxazole (Bactrim, Eusaprim) levels in the CSF of seven patients with intact meninges are reported. The maximum fluid concentration of non-protein-bound SMZ was approximately 46.5%. The TMP value was 68.9%. With this combination of drugs it is possible to achieve a high CSF level of broadspectrum chemotherapeutic substances in neurosurgical patients.

[Investigation of the c. s. fluid levels of two newer antibiotics; cefazolin and sisomicin (author's transl)].

In ten patients continuous estimations of the C.S.F. and serum levels were performed after the administration of a Cephalosporin derivative (Cefazolin - 4 cases) and a new alpha-amino-glycoside antibiotic (Sisomicin - 6 cases). After the evaluation of the results some doubts must be cast on the value of both these antibiotics for the treatment of meningitis or for per-operative chemo-prophylaxis in neurosurgical procedures. The values found were compared with those well-known in the literature, regarding the cephalosporins and the alpha-amino-glycoside Gentamycin.

Studies on HLA antigens and cellular and humoral autoimmune phenomena in patients with myasthenia gravis.

The HLA phenotype of fifty-four patients with myasthenia gravis (MG) was compared to that of 600 controls. When male and female patients were compared separately with the control group, HLA-B12 was increased in MG males (P less than 0-023) and HLA-A1, B8 in MG females (P less than 0-023). In addition, HLA-A1, B8 was correlated with the early onset type of MG and with a more severe clinical course (Osserman scale IIb-III) in female patients. Cell-mediated immune responses were studied in a twenty-one hospitalized MG patients. Specific cell-mediated immunity to highly purified muscle proteins was investigated using the two-step-leucocyte migration agarose test and general cell-mediated immunity was studied by determining the cutaneous response to four extrinsic antigens. Cellular immune activity to muscle antigens occurred in fourteen out of twenty-one patients (67%) with MG and with one exception, in none of the controls. There was no significant difference in the LIF inducing ability of the different muscle antigens. A statistically significant correlation between HLA-A1 and B8 and either cell-mediated immune reactivity to muscle antigens or humoral autoimmune phenomena or clinical parameters was not found. Only a trend toward an association between HLA-A1 and B8 and a cell-mediated immune response to muscle antigens could be demonstrated. Positive delayed skin reactions to extrinsic antigens were observed with the same frequency as in healthy blood donors.

[Ski boot rim fracture: tension-optical study].

Most of the models of ski-boots used today end up at the weakest area of the tibia. They should be disregarded for medical and statistical reasons because the maximal bending point lies at the minimal diameter of the tibia. A ski-boot with a joint, splinting more than the distal third of the calf and ending approximately at the mid-point of the tibia elasticity, offers an important safety to the skier. Together with a safety binding the risk of an accident of the so-called "fracture of the border of the ski-boot" can be further reduced.

[Microsurgery in pre-pontine tumours (author's transl)].

On the basis of some selected examples, the problems of micro-surgical operations on parasellar and pre-pontine tumours are discussed. The tumours are divided into five types, according to their localisation. The particular anatomical aspects are discussed in detail.

Development of a liposarcoma out of the capsules of a hygroma.

We treated a patient who had a very rare finding: the development of a malignoma out of the capsules of the subdural hygroma covering the right occipitotemporal brain. Lumbar encephalography and arteriography confirmed the fact that surgery was necessary. A subdural-auricular shunt was performed and revised 2 years later. During the second operation, the external and internal capsules were removed in addition to the shunt. 3 years later the hygroma and large capsules were removed again and a ventricular-subdural drainage performed. 1 year later once more a subdural-auricular shunt was performed. 10 months later a tumour was found to be a pseudomucinous liposarcoma with large hyperchromatic nuclei and oval cells with large vacuoles of neutral fat. Renewed growth of the tumour led to the child's death. The histological and macroscopic findings of the capsules suggest the development of a malignoma within the last 3 years.
