[Analysis of the progressive phases of diabetic retinopathy. Prevention and treatment by photocoagulation (author's transl)].

An attempt has been made to clarify various unresolved problems concerning the pathogenesis, course and treatment of diabetic retinopathy (d.r.) on the basis of an analysis of 2320 fluorescein angiograms performed on 150 patients at intervals of 1 week to 6 months during the period January 1967 to April 1975. The characteristic changes of diabetic angiopathy are already visible in the angiogram before the ophthalmoscopic and biomicroscopic features of d.r. become manifest (preretinopathic stage). Like in the later stages of d.r., hyperpermeability of the retinal capillaries predominates in certain patients, whilst stenosis and obstruction of the precapillaries and arterioles prevail in the remaining patients. Hyperpermeability frequently causes a decrease in, or a failure of perfusion of the retinal capillaries due to compression by the swollen perivascular tissue. D.r. is characterised by phasic development. In cases of primary exudative angiopathy an increase in the capillary hyperpermeability can precede phases of progression. Cases of primary occlusive angiopathy display a jerky type of involvement of larger and larger retinal vessels. An additional exudative component accelerates progression of the angiopathy. Large zones without visible capillaries or multiple small areas with deficient capillarisation, IRMAs and shunt vessels herald the imminent onset of the proliferative stage (preproliferative stage). Relation between histological structure and ultrasonogram in malignant melanoblastoma of the choroid.

The extreme polymorphy of the histological structure of the malignant melanoblastoma of the choroid, and the diverse forms of the tumor echogram stimulated the comparative investigation of such tumors in 22 cases. The complicated combination of different histological details cannot be revealed through clinical echo-ophthalmography. However, connections between the histological preparation and the interference picture can be demonstrated subsequently.

[Secotrial diabetic retinopathy in a case of partial atrophy of the optic nerve (author's transl)].

In a 35-year-old man diabetic patient with atrophy of the upper half of the right optic nerve and loss of the lower half of the visual field, no signs of diabetic retinopathy developed in the upper half of the eyeground. The diabetic retinopathy affecting the inferior half of the fundus was strictly confined to the inferior half of the eyeground; in addition the retinopathy took a more benign cours compared with the left eye showing a normal disc. After photocoagulation in the left fundus, the retinopathy improved to a grade comparable to the influence of the partial optic atrophy upon the course of the diabetic retinopathy in the right eye.

[Malignant melanoma of the iris - 23 years after incomplete excision of the tumour (author's transl)].

A 54-year-old male was 1948 submitted to a sectorial iridectomy because of a histologically proved malignant melanoma of the iris. 1971 an absolute secondary glaucoma had developed, and the eyeball was excised. Though histologically tumour issue of the original type was found in the chamber angle and ciliary muscle as well as around scleral blood vessels and subconjunctivally any clinical manifestation of generalization never was observed since the iridectomy 23 years ago.

[Diagnosis of tumor-like fundus changes by means of a combination of echography, infrared photography and fluorescein angiography (author's transl)].

Using a combination of echography, infrared photography and fluorescein angiography in tumor simulating changes of the fundus an optimum combination of these diagnostic methods was achieved. The echography allows exact measurements of the prominence and in lesions exceeding 1.1 to 1.5 mm also a tissue differentiation. By infrared photography melanin could be differentiated from the bloodpigments and lipofuscin and the extension of pigmented tissues could be revealed. Fluoresceinangiography visualizes the vascular pattern and the permeability of the vessels of tumors or pseudotumors. Up to now 100 cases with tumorlike fundus lesions were subjected to this combined examination method and many of these cases preserved from enucleation.

[Fundus changes in scleroderma (author's transl)].

12 patients with scleroderma were submitted to an ophthalmological examination. 9 patients displayed fundus changes. In 7 cases defects of the retinal pigment epithelium near the macula were observed, one of these patients showed an associated serous detachment of the pigment epithelium. In three patients localized retinal vascular changes, especially on the capillary level, were notices. Obviously, alterations of the collagen as well as vasopathological changes being part of the systemic disorder do have their counterpart of the systemic disorder do have their counterpart within the eye.

[Epidemic keratoconjunctivitis: a report on a clinical epidemic (author's transl)].

After description of the clinical and serological features of an epidemic keratoconjunctivitis caused by adeno-virus (Type 8) the author compares results of 4 methods of treatment. The schedule consisted of "Okuzell-Coldan" eye drops as placebo, eye drops of corticosteroid content, intramuscular inoculation with Interferonogen and a combined treatment with 0.3 percent "Neutralrot" eye drops and radiation with daylight lamp HPR 125 W produced by Philips Co. Ltd. The transition from the conjunctival form to the keratic form could be best prevented through the last named therapy.
