[Has photocoagulation improved the prognosis of diabetic retinopathy?].

In 192 diabetic retinopathy cases, Xenon light coagulation was performed. Afterwards, the eyes treated were examined half yearly for at least 4 years. Fundus photography and fluorescein angiography were made and the visual acuity was checked. In 64 cases the comparison could have been made in the same patient between one treated and the other untreated eye. The statistical results of this study permit us to assess the therapeutic value of light coagulation for diabetic retinopathy.

[Echoculmetry in ablation surgery without subretinal drainage].

Based on echo-oculometric measurements of the maximum height of retinal detachments during the first hours and days after surgery without subretinal drainage, using a sound frequency of 12 MHz, final success (24 cases) or failure (2 cases) could be predicted within 24 h at the latest.

[Diabetic retinopathy. Analysis of progressive attacks, prophylaxis and therapy with light coagulation].

If one follows the course of diabetic retinopathy on the basis of retinal fluorescence angiography one can distinguish two main courses: Cases displaying an evolution mainly characterized by an increase of capillary hyperpermeability, and others in which the capillary occlusive processes outweigh hyperpermeability. Fluorescence angiography in addition provides paramount clues in regard to initiation of photocoagulation as well as its mode and extent of application and will furthermore render an adequate documentation of successful treatment.

[The fate of corneal infiltrations in cases of epidemic keratoconjunctivitis. A follow-up study over two and a half years (author's transl)].

34 out of 72 patients (47.2%) with epidemic keratoconjunctivitis due to an infection with the adeno-virus type 8 still showed, on average, corneal opacities after 2.3 years. The gamma2-test proved the statistical significance of the correlation between the prevention of persistent corneal changes and the abstention from topical treatment with corticosteroids in the acute phase of the disease. Yet, steroids mitigate the subjective discomfort, delay the development of keratitis by several days, or even prevent its manifestation as long as corticosteroids are applied. However, in one third of the cases, the keratitis recurs as soon as topical treatment with corticosteroids is stopped. Recurrent attacks of keratitis may flare up over years and the visual acuity may decrease to 6/12 causing transient incapability of working.

[Experimental ultrasonography of the rabbit eye (author's transl)].

In 20 adult rabbits of the Gelbsilber-strain the axial depth of the anterior chamber, the axial thickness of the lens, and 7 different diameters of the vitreous chamber were measured by means of bioechometry for standardization of the to date neglected experimental ultrasonography of animal eyes. For proper interpretation of echograms of the rabbit eye the particular anatomy and topography of the lens must be considered. Penetration of the large and almost spherical lens by the beam can only be avoided if the probe is placed behind the limbus or behind the equator of the globe. In the axial ultrasonogram the high single peaked echo of the posterior lens capsule appears between the third and fourth fifth of the entire axial length. In all other types of echogram the beam passes through peripheral sections of the lens oblique, and produces several small echoes, thus eventually simulating a pathologic echogram of the vitreous body.

[Experimental siderosis bulbi: an echographic-histological study (author's transl)].

In 20 eyes of 10 rabbits a siderosis was provoked by intravitreal injection of 0,1 ml of a 1% watery solution of Mohr's salt. The course of the following vitreous retraction was observed during 4 weeks by means of ultrasonography, and each echogram was compared with the corresponding histological finding. The results of these investigations show that by means of ultrasonography vitreous destruction or liquefaction can be ascertained and distinguished from an undulation, taught or funnel-shaped detachment of the posterior hyaloid membrane; in addition, this membrane and retinal detachments are well discernible by this method.

[Diagnosis of herpetic keratitis by immunofluorescence (author's transl)].

Between December 1973 and October 1974 scrapings of the corneal epithelium of 52 patients were examined by immunofluorescence. 41 of these corneal scrapings were suitable for examination. In 31 of 38 cases with the clinical diagnosis of a herpes simplex keratitis virus-specific antigen was found by means of immunofluorescence. The procedure takes only a few hours, and shows with 81,5% correct diagnoses a high reliability; scrapings of patients without herpetic affection never revealed a positive result. In 3 of 4 cases of deep herpetic keratitis, in which the herpetic etiology could only be supposed clinically, the diagnosis was proved by immunofluorescence.

[Treatment of the diabetic retinopathy with doxium-clofibrate (author's transl)].

28 patients with non proliferating exudative diabetic retinopathy were treated with 750 mg Doxium and 1500 mg Clofibrate daily during 8.6 months on an average. Before beginning--in the middle of the period of observation--and after finishing the treatment, a retinal fluorescein angiography was performed. The waxy exudates decreased in 11 patients (39.29%) and increased in 8 patients (28.57%). In 9 patients (32.14%) no change of the amount of the hard exudates was observed. The treatment was almost only in those angiopathies successful, in which the fluorescein angiography showed a preponderance of the hyperpermeability over the obliterating process of retinal capillaries.

[Pathogenesis and treatment of diabetic retinopathy (author's transl)].

The initial disorder leading to any kind of diabetic retinopathy is hypoxia. After exhaustion of the compensatory autoregulation (vasodilatation and increase of the volume flow), the blood-retina barrier breaks down, and parts of the central retinal capillary network fall out (pre-retinopathy). In the non-proliferative and pre-proliferative stage, either disturbance of permeability or occlusion of capillaries is the prevailing process. The stage and predominance of one of the two pathogenic factors, which eventually may be jointed by vascular proliferation, are decisive for the application of light coagulation. The plan of the procedure must be based upon fluorescence angiography. If all progressive changes are treated, and fluorescence angiographic controls are made in intervals of several weeks or months, according to the stage and form (primary exsudative or primary occlusive) of the retinopathy, this procedure is preferable to the coagulation of big retinal areas (retinal ablation), and a maximum of retina with normal vascularisation can be saved. Medical treatment comprising calcium phosphate, the vasoprotecting Doxium, and clofibrate as a depressor of the lipid level in the blood, play a secondary role. Finally, hypophysectomy could not maintain its place in modern treatment of diabetic retinopathy.

[Treatment of malignant lid tumours by intrapalpebral injection of urea (author's transl)].

36 malignant tumours of the eyelids (basaliomas, basal-cell carcinomas and squamous-cell carcinomas) have been treated by intrapalpebral injections of a 5% to 25% solution of urea as recommended by E.D. Danopoulos. 1.5 to 3.0 ml of the solutions were injected around the tumours twice per week. Six malignant tumours disappeared completely after a treatment of eleven months; their largest initial diameter averaged 0.53 cm. Considering the protracted treatment, the uncertainty of success, without histological examination of the excised scar tissue, and finally because of the low rate of verified success, injections of dissolved urea around malignant tumours of the eye lids cannot replace radical excision.

[Echooculometry in retinal detachment surgery without subretinal drainage (author's transl)].

Changes of the height of retinal detachments up to 0.1 mm can be visualized by A-scan-ultrasonography; such small changes cannot be ascertained by means of ophthalmoscopy. Based on echooculometric measurements of the maximal height of retinal detachments during the first hours and days after surgery without subretinal drainage, the final success (24 cases) or failure (2 cases) could be predicted 24 hours at latest. Thus in cases of retarded resorption of the subretinal fluid it can be decided by means of echography, wether a reattachment of the retina can be expected or a failure, the cause of which needs early detection.

[Beginning atrophy of the eye bulb. An echographic-histologic comparative study (author's transl)].

In 21 cases of beginning atrophy of the eye bulb, ultrasongraphy revealed abnormal echoes reflected by equatorial and preequatorial parts of the bulbar shell. In addition, ultrasonography revealed a shortening of the bulbar axis by 0.88 mm and a thickening of the retroequatorial sclera up to 2.1 mm on an average. By correlation of the echogram and microscopic sections the presumption of an edema of the peripheral choroid and the ciliary body could be verified in 6 cases. Thus echography makes possible an earlier diagnosis of beginning bulbar atrophy than does the clinical symtomatology. In such cases surgical procedures for improvement of hypotony are useless or even detrimental.

[The diagnosis of pseudotumours of the macula by means of ultrasonography (author's transl)].

The echograms of 35 patients affected with pseudotumours of the macula were analysed in detail. Pseudotumours and malignant melanomas can be distinguished by means of ultrasound with high probability if the minimal prominence of the tumour is not lower than 1.1 mm, and with certainty if the minimal prominence is not less than 1.5 mm. The echogram of pseudotumours is mainly characterized by its extraordinary irregularity.

[Ultrasonic biometry and oculopression].

The changes of the intraocular distances provoked by oculopression (10 min, 40 mm HG) were explored by ultrasonic biometry. Immediately after removing the oculopressor, a flattening of the anterior chamber and a deepening of the vitreous cavity occurred. The maximal deepening of the anterior chamber together with the shortening of the vitreous axis appeared 9 min after oculopression on an average. The causes of the predominance of the initial lengthening (0.24 mm on an average) and of the following shortening (0.16 mm on an average) of the vitreous cavity compared with contrary changes of the anterior chamber are discussed.

Atypical cotton-wool spots.

Atypical cotton-wool spots in four selected cases of diabetic retinopathy are discussed. The most striking characteristic distinguishing them from typical cotton-wool spots is their size, which ranges from about 2 to 4 disc diameters. They develop after stenosis or a complete obstruction of a first order arteriole at the point at which it emerges from the parent arterial branch. The size of the lesion corresponds with the size of the affected arteriole. Restoration of local circulation is attempted by neighbouring arterioles and venules, the latter carrying a reversed blood flow. Arterio-venous communications along the border of the infarcted area are a prominent feature, most likely attributable to the necessity of free drainage of the arterial influx. The same haemodynamic principle applies to the coarsening of the adjacent capillary bed. Secondary changes of the bordering venous branches include aneurysm formation and the staining of the venous wall.

[Exudative diabetic retinopathy. A fluorescence angiography study].

For elucidation of the pathogenesis of the lipoidic depostis due to diabetic retinopathy, 40 patients (44 eyes) were submitted to fluorescence angiography in intervals of 4-6 months; the follow-up covered 1-3 years, the average time being 1.75 years. In 43 of 44 cases, disturbances of the permeability of retinal blood vessels were found, their extent correlating with the size of the exudate foci. An increased perfound, thier extent correlating with the size of the exudate foci. An increased permeation of the dye was closely connected with an increased exudation, and a decreased vascular permeability was connected with a regression of lipoidic deposits.
