[Uteroplacental circulation and functional status of the fetoplacental unit in risk pregnancies with special reference to late gestosis].

Trial to define position and variability of radioisotope functional diagnostics of placental perfusion in comparison with urinary estrogen excretion by serial examinations. - In case of placental insufficiency estrogen values are significantly reduced starting from the 32nd to 35th gestational week as compared with normal values. In general, placenta perfusion shows delayed half-life periods of the increase in activity in all risk pregnancies beginning from the 28th week of pregnancy. A direct correlation between estrogen values and placenta perfusion can only be detected in cases of chronic placental insufficiency with intrauterine growth retardation. A prolonged perfusion time reveals a fetal hypotrophy in 53% of the cases. Late gestosis takes the first place about all risk pregnancies, connected with placental insufficiency in 30% of the cases. The tendency towards a development of placental insufficiency can be early detected by determination of placental perfusion time.

Antibodies in cerebrospinal fluid to white matter glycoproteins in multiple sclerosis patients.

Cerebrospinal fluid (CSF) specimens from 45 patients with multiple sclerosis (MS) and 45 age- and sex-matched controls with other neurological diseases (OND) were tested for antibodies to white matter (WM) membrane glycoprotein (GP) fractions prepared from MS and control WM membranes by lentil lectin chromatography. The binding of the CSF IgG to the 125I-labeled GP fractions was determined by immunoprecipitation using Protein A-Sepharose. CSF from patients with MS bound highly significantly more strongly to the GP fraction prepared from MS WM than did the OND CSF specimens (P less than 0.001). There was no such difference when control GP fraction was used as an antigen. No highly significant differences were observed when 20 paired serum specimens were tested. Electrophoresis of the immunoprecipitates showed that components with molecular weights (MWs) of 157,300, 135,600, 111,100, 93,000, 75,700, 63,300, 50,100, 24,300, 20,300 and 17,000 daltons were precipitated from the MS GP fraction by CSF specimens of both MS and OND groups, whereas components with MWs of 50,100, 24,300, 20,300 and 17,000 daltons were precipitated from the control GP fraction.

[Significance of the measurement of placental perfusion by radionuclides in prenatal diagnosis in pregnancies at risk].

The evaluation of placental perfusion measurements in 102 high-risk pregnancies from the 28th to 41st gestational week is described. --Following an injection of In113m-chloride, the half-life value of the rise in activity was determined by computer evaluation according to a single-compartment model. --Mean value of the half-life value of the rise of the placental perfusion curve is 12.55 +/- 2.42 seconds (33 normal test persons). --A half-life value of 20.62 +/- 2.43 seconds was determined in high-risk pregnancies. --In cases of EPH-gestosis a half-life value of 22.55 +/- 3.64 seconds was determined. Individual cases showed values of 50 seconds for gestosis. There was no correlation between the half-life value and the size of the placenta and or gestational age.

Treatment of psoriasis with somatostatin.

In 22 of 26 patients suffering from psoriasis the drip infusion therapy with Somatostatin, an inhibitor of the human growth hormone (HGH), leads to complete or partial remission of skin lesions. Fast reduction of joint pains in arthropathic psoriasis could be observed in four of four patients. The repeated measurement of HGH blood level showed a decrease of HGH in five cases following the Somatostatin therapy combined with the clinical remission.

Isoelectric focusing of myelin membrane proteins.

Human myelin was isolated from the white matter of autopsy brains. Myelin proteins were characterized by isoelectric focusing in ultrathin slab gels in a pH range from 3.5 to 10 after solubilization with urea and Nonidet P 40. The protein profile in the acidic region (pH below 6.2) revealed at least twelve faint bands which comprised only a few percent of the total myelin proteins. Most of the myelin proteins were focused in the neutral range (pH 6.2-7.8) which showed two sharper and three broader major bands, the total number of bands in this region being about twenty. The basic pH range (pH above 7.8) contained about 30% of the proteins, and revealed a very intense band near the cathode with seven to nine weaker bands below pH 9.0. When the myelin was partially delipidated prior to solubilization, an additional broad "band" was observed at the area pH 8.0-8.5.

Membrane protein antigens in multiple sclerosis.

Rabbits were immunized with white matter (WM) membrane fractions isolated from autopsy brain specimens of three patients with multiple sclerosis (MS), and three controls. All the rabbits developed high serum antibody titers to the MS and control WM fractions, as tested by enzyme immunoassay. Antibodies against WM membrane components were analyzed further by immunoprecipitation of radio-labeled WM proteins and subsequent polyacrylamide gradient gel electrophoresis. Antigenic membrane components with molecular weights of 138 000, 111 000, 86 500, 79 600, 69 000, 63 000, 58 000, 53 400, 45 700, 24 500 and 22 300 were found in both MS and control WM. Although there may have been some quantitative differences in these immunogenic proteins of MS and normal WM, no multiple sclerosis-specific membrane antigen could be demonstrated. The hyperimmune anti-WM sera did not precipitate 35S-labeled polypeptides from cells infected with herpes simplex type 1, adeno type 5, measles, mumps, rubella, respiratory syncytial, parainfluenza type 2 or cytomegaloviruses, which suggests that the MS brain WM membrane proteins do not share common antigenic determinants with the viral polypeptides.

Bioluminescent assay of total and brain-specific creatine kinase activity in cerebrospinal fluid.

A bioluminescent assay based on the firefly luciferase reaction has been used for determination of creatine kinase activity in CSF. Activities as low as 0.1 U/L can be measured. The coefficient of variation at an activity level of 0.3-0.4 U/L was between 5 and 6%. The assay conditions optimized for serum specimens can be used for CSF. The adenylate kinase activity is almost completely inhibited, which simplifies the procedure. The creatine kinase/(CK) isoenzyme distribution was obtained using the bioluminescent assay in combination with immunoinhibition or ion exchange chromatography. All specimens contained both MM and BB activity, but no MB was found. The study indicates that the bioluminescent assay is useful in the determination of CK isoenzymes in CSF. The clinical importance of the observed CK levels will be reported in a separate communication.

Association of ophthalmoplegic migraine with familial acute anterior uveitis.

In a family with 3 cases of acute anterior uveitis (AAU) the proband had 13 episodes of recurrent AAU associated 4 times with severe, throbbing ipsilateral headache. During one such episode he had an oculomotor palsy, hyperaesthesia of the cornea, and lack of spontaneous pulsations of the central retinal vein in the eye with AAU. An oculomotor palsy occurred twice intermittently between the AAU episodes. Carotid and vertebral angiographies confirmed the diagnosis of ophthalmoplegic migraine. Prostaglandins liberated by trauma, AAU, or other inflammation may be involved as mediators in ophthalmoplegic migraine attacks. All 3 patients with AAU had sacroiliitis and seronegative polyarthritis. One of their siblings had polyarthritis and one incomplete Reiter's disease. All of them were HLA-B27 positive whereas one healthy sibling was HLA-B27 negative. These findings support the hypothesis that HLA-B27 itself or a pleiotropic HLA-B27 linked gene predisposes the carrier to AAU, sacroiliitis, ankylosing spondylitis, and Reiter's disease.

The endocrine response to physical activity.

Physical activity is characterized metabolically by a relatively greater demand for combustion of carbohydrate and in addition by increased strain on several other physiological processes. Training tends to reduce the impact of physical activity on most organ systems. Physical activity, when it is not exhaustive, usually leads to elevated blood levels of "hormones of stress and activity" (catecholamines, cortisol, growth hormone), while thyroid and gonadal secretions are less involved. Following a period of training the endocrine response seems to abate, whether due to increased peripheral hormone sensitivity or to reduced need.
