Pathology of the uterine body.

During the past year, evidence has compiled to suggest that uterine carcinosarcomas are, in fact, metaplastic carcinomas and that mitotic counts are of no value in distinguishing between high- and low-grade endometrial stromal sarcomas in stage I cases. The pathologic and biologic behaviour of uterine müllerian adenosarcomas has been comprehensively reviewed. Prognostic factors have been identified in a large series of surgically staged endometrioid adenocarcinomas of the endometrium, while the sinister nature of the serous papillary and clear cell carcinomas of the endometrium have been confirmed. It has been claimed that a papillary pattern of growth is an independent prognostic factor in endometrial neoplasia and shown that the accuracy of macroscopic estimation of depth of myometrial invasion decreases with increasing tumor grade. The value of peritoneal cytologic examination in early stage endometrial adenocarcinoma has been questioned, as has the value of DNA flow cytometry in endometrial neoplasia. The prognostic significance of steroid receptor status has been confirmed and studies of oncogene amplification are beginning to yield prognostically useful information. Evidence is also beginning to emerge concerning chromosomal abnormalities in many women with endometrial adenocarcinoma.

Electronic monitoring of fetal heart rate and determination of fetal scalp blood pH in prediction of intrapartum fetal distress.

A prospective study on consecutive fetal monitoring in 559 deliveries was carried out. The difference between the first 30-minute and the last 30-minute monitoring was compared. Abnormal fetal heart rate (FHR) pattern in the last 30 minutes combined with abnormal fetal scalp blood pH has a good predictability of 1- and 5-minute Apgar scores. The correlation of fetal heart rate-uterine contraction (FHR-UC) monitoring (525 cases) and fetal scalp blood pH determinations (FSB-pH) (79 cases) with the fetal condition was assessed. It is indicated that the first 30-minute FHR tracing can reflect the fetal status in labor, and FHR-UC monitoring assessment is evidently superior to FSB-pH in the prediction of a vigorous fetus. Whereas, FSB-pH is superior to FHR-UC monitoring in the prediction of a compromised fetus.

Quantitative electroencephalographic correlates of cognitive decline in normal elderly subjects.

We obtained a topographic computer analysis of the electroencephalogram in 53 normal elderly subjects. Normal aging was not associated with an increase in slow (delta) activity. However, cognitive performance correlated positively with fast (beta) activity particularly in frontal leads, even after controlling for age, education, occupation, and medication. Five subjects who showed early signs of cognitive decline, had all a marked reduction in beta activity suggesting that this may be an early indication of intellectual loss.

Do AgNOR counts reflect cellular ploidy or cellular proliferation? A study of trophoblastic tissue.

There is considerable debate as to whether AgNOR counts reflect cellular ploidy or cellular proliferation. Trophoblastic tissue from hydropic abortions and from hydatidiform moles offers an excellent model for analysing this problem. Thus, complete moles show considerable cell proliferation but are diploid, whilst partial moles show markedly less cell proliferation but are triploid: hydropic abortuses are diploid and show no cellular proliferation. AgNOR counts in villous cytotrophoblastic cells from hydropic abortions and from complete hydatidiform moles are similar, but those in partial hydatidiform moles are 50 per cent higher than in either hydropic abortions or complete moles. Thus, in non-neoplastic trophoblastic tissue AgNOR counts are clearly a reflection of ploidy rather than of cell proliferation.

Nucleolar organiser regions in normal, hyperplastic, and neoplastic endometria.

Nucleolar organiser regions (NORs) can be visualised by a silver stain that shows NOR-associated proteins as black intranuclear dots (AgNORs). NORs are loops of DNA that transcribe to ribosomal RNA, and the number of AgNORs is thought to reflect cellular proliferative activity. A study of AgNOR counts in normal, hyperplastic, and neoplastic endometria is reported. The highest AgNOR counts were found in normal proliferative endometrium and in well-differentiated endometrioid adenocarcinomas that were invading the myometrium. A mean AgNOR count of greater than 9 in curettage material from an atypical proliferative lesion of the endometrium is highly suggestive of an invasive neoplasm.

Current problems in the pathology of intra-epithelial lesions of the uterine cervix.

Problems in the interpretation and diagnosis of intraepithelial lesions of the uterine cervix include interobserver differences in respect of grading, uncertainty as to the significance of human papillomavirus infection and debate about the current terminology. The role of human papillomaviruses in cervical intra-epithelial neoplasia (CIN) remains unclear and the specific type of virus present does not correlate closely with the behaviour of these lesions. There are defects in the current terminology and in proposed new terminology for CIN; it is suggested that the term 'borderline CIN' should be introduced in cases where there is uncertainty as to whether or not CIN 1 is superimposed on a flat condyloma.

A questionnaire investigation of anxiety and depression in early dementia.

We report findings on a study of anxiety and depression by questionnaire in 50 patients with mild dementia and 134 control subjects using the Hospital Anxiety and Depression Scale. Thirty-eight percent of patients and 9% of controls had a possible or probable diagnosis of an anxiety disorder. Possible or probable depression was found in 28% of the patients and 3% of the controls. These rates for the patients were above those in normal populations. All patients and control subjects were tested with the Extended Scale for Dementia (ESD). Neither group showed a significant relationship between depression and ESD scores. In the control subjects there was a negative correlation (P less than .006) between anxiety and cognitive scores, one that was not found in the patients.

Cellular elements of the dermis and collagen remodelling during larval life of anurans.

The differentiation of the collagenous dermis has been followed by electron microscopy in larvae of three anuran species. Events can be divided into five phases, depending on the organization of the cellular elements. Initial secretion of collagen appears to be from the epidermis but fibroblasts are present from early stages. Apart from secretion, there is phagocytosis of fibrils by fibroblasts, seen most frequently when the operculum is formed and at metamorphosis. In most larval stages, the dermis consists of regular plies of collagen fibrils, penetrated by processes from underlying fibroblasts and crossed by nerves, with a cellular layer defining the deep face. In Rana, these cells contain enormous vacuoles and have been called "compartmented cells", but the vacuoles are absent in Bufo and Xenopus. Just before metamorphosis, fibroblasts and chromatophores migrate into the dermis, and establish a stratum laxum. A few plies of larval collagen fibrils are left under the epidermis, the bulk of the larval dermis becomes the stratum compactum. During larval life, the hypodermis contains chromatophores, blood capillaries and wandering leucocytes. The dermis gains a blood circulation only at metamorphosis, when subdermal lymph sacs and vertical smooth muscles are established.

A mouse chromosome 17 gene encodes a testes-specific transcript with unusual properties.

We have characterized a novel mouse gene (D17Si11) on chromosome 17 that expresses a major transcript observed uniquely in the testes. The D17Si11 locus has been mapped to the central region of chromosome 17 between H-2 and C3. Sequence analysis demonstrates several unusual features of this locus and its transcript: first is the presence of complementary sets of alternating purine and pyrimidine residues within the 3' region of the transcript that could form double-stranded, hairpin-like secondary structures with properties similar to that of Z-DNA; second is the existence of a hypothetical, long open reading frame in the nucleotide strand that is complementary to the testes transcripts. This complementary strand open reading frame is three times the size of the longest potential open reading frame present in the transcript itself. Although a function for D17Si11 has yet to be determined, the gene is relatively nonpolymorphic in mice and appears conserved in mammals.

Modulation of the antigenicity of amylase in cervical glandular atypia, adenocarcinoma in situ and invasive adenocarcinoma.

Using a polyclonal anti-salivary alpha-amylase antibody and a biotin-avidin-peroxidase detection system immunoreactive amylase was detected in 39/40 cases of invasive cervical adenocarcinoma, 6/7 cases of adenocarcinoma in situ, 6/6 cases of high-grade cervical glandular atypia and 12/14 cases of low-grade cervical glandular atypia. Expression was seen in only 2/18 normal endocervices and this was weak and patchy. In contrast, strong amylase activity was demonstrated in 18/18 normal endocervices using a starch film assay on frozen sections. It is proposed that antigenic modulation of amylase occurs during endocervical neoplastic transformation.

The psychiatrist in the obstetric unit. Establishing a liaison service.

The establishment and first 18 months of a psychiatric liaison service to a general hospital obstetric unit are described. The service was initially intended to predict antenatally those at risk of post-natal depression and to offer follow-up throughout pregnancy and in the post-natal period. However, a large proportion of referrals were of women currently unwell. Furthermore, post-natal follow-up in the community proved difficult. Referrals, diagnoses and disposal are analysed, leading to conclusions about the effectiveness and problem of such a service. The components of a comprehensive obstetric liaison provision are proposed, emphasising the need for strong community links built around a CPN service, allowing adequate postnatal psychiatric care.

Sensitivity and specificity of the extended scale for dementia.

The Extended Scale for Dementia was introduced as a test for grading the intellectual function of patients with dementia. Its use in discriminating demented patients from nondemented control subjects has been explored. The test had a sensitivity of 93% and a specificity of 96% in patients over the age of 65 years, but the sensitivity fell to 75% in those younger than 65 years. The scale may be useful, particularly for patients over the age of 65 years, in helping to distinguish dementia from normality.
