Ophthalmodynamic studies in kinking of the internal carotid artery. Surgical implications.

In 6 patients with kinking of the internal carotid artery and 10 healthy volunteers ophthalmodynamographic studies were performed. Sideward head bending in the patients caused major ipsilateral decrease of internal carotid flow as did forward flexion on both sides. This effect was minimal in healthy humans. Intraoperative monitoring by the same method showed instant improvement of flow dynamics after kink resection and end-to-end anastomosis.

[Intraocular pressure during operations using the heart-lung machine (author's transl)].

The results of the investigations reported here show that contrary to expectations, the drop in mean arterial blood pressure (up to 57 mm Hg) during extracorporal circulation does not result in a simultaneous drop in intra-ocular pressure. Other factors, affect intra-ocular pressure during extracorporal circulation, causing it to rise. Postoperative studies of vision led to the conclusion that the elevations of intraocular pressure measured during extracorporal circulation with the heart-lung machine do not adversely affect ocular function.

[Experimental and clinical employment of an endovenous-myocardiac screwing electrode (author's transl)].

A common postoperative complication following transvenous endocardial pacemaker implantation is the dislodgement and displacement of the electrode. Following trial of a new endovenous-myocardiac screwing electrode in dogs we have tried this electrode since December 1976 on 105 patients, of which 84 cases were implanted primarily and 21 secondarily. The average stimulation threshold was 0.7 V at a pulse width of 1 ms. 6 patients had complications. By making a judicious use of this electrode the rate of electrode dislodgement can be reduced appreciably.

[Microbial emission, immission and changes in the germ count in the cooling water during operation of wet cooling towers ii. communication: measuring methods, emission values and changes in the germ count in the cooling system (author's transl)].

The second in the series "Microbial emission, immission and changes in the germ count in the cooling water of wet cooling towers" describes measuring methods and results of the measurements for determining the emission values and the changes in the germ count in the cooling system. The content of colony-forming units (KBE) in the cooling water varied widely, depending on the germ content of the surface water and the preparation of the cooling water (filtration, chemical conditioning). In summer the KBE values were as a rule higher than the values recorded in winter (factor of about 10). The introduction of cooling tower water into the surface water thus did not result in any noticeable changes in germ content, either in summer or in winter. An emission rate of 8.3 10(7) KBE/s (CSA) measured at the top of the cooling tower was the maximum emission value recorded. This resulted in a total germ content in the plume of 1.2 . 10(4) KBE/m3. In order to place this finding into perspective, it should be noted that fluctuations in the KBE values of the air of considerably more than 1000 KBE/m3 can occur because of biological release and meteorological conditions alone (see IVth communication). The KBE-P values recorded (content of particles with units which are capable of forming colonies) in the plume were of the same order as the KBE values. From this it can be concluded that the drops of cooling water which are swept away with the plume are normally only charged with individual colony-forming units. It is worth noting that the KBE values of the plume are only indirectly dependent on the KBE values of the cooling water used in the cooling tower.

[Wound healing after surgery by laser, scalpel and thermocautery (author's transl)].

Laser-, thermocautery- and scalpelsurgery when compared in respect to their wound healing procedure have qualified as prepared for clinical application. On minipigs wound healing of skin scissions has been followed up morphologically in light microscopy and by testing the stress resistance of 16 days old scars. The fastest wound healing occurs after scalpel incision resulting in an almost perfect reconstruction of subepethelial tissue architecture at the 20th postoperative day. The initial tendency of laser incisions to dilate, which is still more pronounced in thermocautery wounds, causes a flat funnel shaped deformation of the scars, again more distinct after thermocautery. This is reflected in a still remaining slight disarrangement of the subepithelial collagenous fibres at the 20 postoperative day. Stress resistance is maximal in thermacautery scars, i. e. almost 30% higher than in scalpel and laser scars.

[Aetiology of chronic aggressive hepatitis in Vienna, a contribution (author's transl)].

In 32 patients with clinically, biochemically and histologically proven chronic aggressive hepatitis, HBsAg, HBsAb, HBcAb and HA-Ab were determined by radioimmunoassays and HBeAg and HBeAb were estimated by immunodiffusion. HBsAg was positive in 18 patients [57%], HBcAb were found in 23 patients [72%], HBsAg and/or HBcAb were detected in 26 patients (82%). The incidence of HA-Ab was 88%, similar to the frequency of HA-Ab found in healthy blood-donors. The aetiology of chronic aggressive hepatitis was associated with hepatitis B in 82%, and with autoantibodies in 6% of our patients, while in 12% the aetiology remained cryptogenetic.

[Compensatory mechanisms of haemolysis after surgery with the heart-lung-maschine (author's transl)].

In 20 patients, suffering from various diseases, the extent of haemolysis and the correlation between the rate of haemolysis and various mechanisms of erythrocyte damage in the course of open heart surgery was studied. We investigated haemotological parameters, electrolytes, lactate dehydrogenase before, and 1, 3, 5, and 7 days after operation. Besides free haemoglobin, haptoglobin and haemopexin were determined 5 and 15 minutes after starting perfusior with the heart-lung machine and after finishing. The best index of haemolysis was free haemoglobin in the serum. With increasing duration of perfusion the rate of haemolysis increased. Compensating mechanisms were postoperative increase of haptoglobin and haemopexin.

[Comparative study of coronary vessel diameters in intravital and postmortal angiograms (author's transl)].

In a total of 21 mongrel dogs a comparison between intravital and postmortal coronary arteriograms was made so as to establish the difference in vessel diameter. Fresh postmortal hearts injected in situ showed an average increase in coronary vessel diameter of approx. 6 per cent. Examinations of 24 hour postmortal hearts which were removed from the animals showed a vessel diameter increase of barely 20 per cent in the mean. These 20 per cent correspond fairly well with literature information about the maximal widening of coronary vessels with nitroglycerin. Nerves which are still functioning, and humoral substances which may influence the still living muscle in vessel walls are supposed to be the reason for the smaller increase in fresh postmortal in situ illustrated coronary arteries. It must be stressed, that the measured differences between intravital and postmortal coronary artery diameters can only be representative of wall sections free of arterioslcerosis.

[Haptoglobin in the course of surgery with the heart lung machine (author's transl)].

In 20 patients, who underwent open heart surgery, the results of quantitavely and semi-quantitavely determined haptoglobin (hp) were evaluated. A rapid method (Rapi tex-Hp-Test, Behring-Werke) was used. Five minutes after the heart lung machine was cut off hp decreased slightly, and increased about 200% at the 4th day after operation, compared to the preoperative value. These changes were less pronounced in patients with prosthetic heart values. Blood transfusions did not influence the results. There is a significant correlation between the results of hp determined quantitatively and semi-quantitatively.
