Concealed quadrigeminy.

Concealed quadrigeminy has been observed in a 24-year-old pregnant woman without apparent heart disease during 10-hour Holter monitoring; in a 33-year-old female with prolapse of the mitral valve during 0.5-hour continuous electrocardiographic recording, and in an 80-year-old man with anterior myocardial infarction. In all these continuous electrocardiographic recordings 391, 437, 25 consecutive interectopic intervals were correlated and the number of sinus beats in each interval fitted the formula s=4n - 1. The 2nd case showed a variant of concealed quadrigeminy. The number of sinus beats in 9 interectopic intervals fitted the formula s=4n when the first ectopic beat of the interval was interpolated and did not interfere with the propagation of the next first sinus beat.

Some factors affecting the measurement of blood inorganic phosphorus in cattle.

Sixty cows were studied over 3 days, to determine the effect of site of sampling, handling stress and the tranquilliser xylazine hydrochloride, on blood inorganic phosphorus (P). Jugular venous and coccygeal mixed arterial and venous blood were compared. Saliva was also analysed for P. Buffered and unbuffered trichloracetic acid (TCA) were compared as deproteinising agents. Tail blood P was consistently 12% (range 7-17%) higher than that from the jugular vein (P less than 0.01). This difference decreased when jugular blood was sampled first but increased following administration of the tranquilliser. The drug significantly (P less than 0.01) increased the concentration of P in the saliva on 2 out of 3 days. The possible role of salivary P in affecting jugular P values, is discussed. The buffered TCA analyses gave consistently lower P readings than the unbuffered analyses. The magnitude of this effect was time dependent, being greater when the samples were left for 4 days (P less than 0.05) than when analysed the next day. There was a very strong correlation between plasma and whole blood P. It was concluded that the tissue of choice in phosphorus studies is tail blood (plasma or serum) treated with buffered TCA.

Practice-education synergy: a research focus on continuity of care.

In this article we have described one focus of an ongoing partnership between a VHA medical center and a university-based school of nursing: to improve patient outcomes through a research program focused on patient-centered continuity of care. A nursing effectiveness and patient outcomes matrix was introduced as a framework to guide variable identification, including nursing-specific outcomes. Opportunities and challenges inherent in developing this program of research were discussed. The synergy of a successful practice-education collaboration not only strengthens the links between practice and education but may also directly affect patient-centered outcomes of nursing care.

Doctor Dean's prescription for health care reform.

One of the nation's smallest states is taking big steps to ensure universal access to health care for all of its citizens. Vermont will create a purchasing pool of public employees, individuals, and businesses who will enroll in a consolidated community-rated insurance plan. A new dispute resolution system will try to remove malpractice suits from the courtroom. Vermont's efforts are needed to show that states can and should develop universal systems of care, so that when a national system is produced states will be the only logical choice to run it.
