Hemodialysis in methyprylon overdose. Some pharmacokinetic considerations.

Serial measurements of serum methyprylon concentration were made in the case of a 14-year-old girl who ingested an overdose of the drug. Our data indicate that, in the presence of high plasma levels of the drug, much longer half-lives than the usually reported four hours may be observed. It is postulated that saturation kinetics may be a possible mechanism for this observation. The striking clinical improvement after six hours of hemodialysis demonstrated the efficacy of this mode of treatment in our patient.

Genetic control and distribution of leucine aminopeptidase in the cultivated chenopods (Chenopodium) and related weed taxa.

Isozymes of leucine aminopeptidase (LAP) in leaf tissue of the cultivated chenopods (Chenopodium quinoa and C. nuttalliae) and their sympatric weedy relatives (C. hircinum and C. berlandieri) can be electrophoretically resolved into a sum total of five anodally migrating bands. These are the products of two unlinked gene loci which apparently reflect genetic duplication via allotetraploidy. Accessions from the Andean and Mexican areas of cultivation are characterized by a lack of electrophoretically detectable variation. Andean weed and cultigen accessions appear to be genetically identical at both Lap loci, as do weed and cultigen material from Mexico. The two cultigens, and their sympatric weeds, can be differentiated by variation at the Lap-B locus, whereas they are monomorphic at Lap-A. This locus is, however, highly polymorphic in weedy C. berlandieri populations of western North America.

Septicemia due to Neisseria lactamica.

Neisseria lactamica was isolated from the blood of a pediatric patient who had signs of septicemia and otitis media. Organisms morphologically resembling Neisseria, as well as gram-positive cocci, were seen on a Gram stain of fluid from the middle ear. It is hypothesized that the N. lactamica septicemia was secondary to infection of the middle ear by this organism.

Etiologic diagnosis of intrapleural empyema by counterimmunoelectrophoresis.

Cultural methods failed to identify the infecting organism in 4 patients with intrapleural empyema. Antimicrobial drugs had been administered to 3 of the patients before their admssion to the hospital. In each case, soluble polysaccharides believed to be those of pneumococcus or hemophilus were detected in the empyema fluid by counterimmunoelectrophoresis. These findings provided a rational basis for management of antimicrobial therapy for 3 of the patients and useful information on the origins of the empyema in the fourth patient. Counterimmunoelectrophoresis is a useful additional method for presumptively identifying the infecting organism in patients with intrapleural empyema.

Ampicillin in Haemophilus influenzae meningitis. Clinicopharmacologic evaluation of intramuscular vs intravenous administration.

Sixty-two children with Haemophilus meningitis were treated with ampicillin sodium, 200 mg/kg/day for ten days. Thirty-one received the drug intravenously (IV) for ten days; the other 31 patients received ampicillin IV for five days (except for one test dose given intramuscularly [IM] on day 2) followed by IM administration of ampicillin the last five days. Ampicillin concentrations in cerebrospinal fluid (SCF) were higher one hour after IV administration, but at two and four hours, concentrations were greater after IM doses. Responses to therapy and rates of complications were similar in the two groups. Fourteen patients had positive CSF cultures on day 2. Ten of these were isolated only on Levinthal medium and would have been overlooked with routine culture methods. All organisms were ampicillin-susceptible and all CSF cultures were negative by 48 hours. More patients with delayed sterilization had neurologic sequelae (78%) than patients with prompt CSF sterilization (13%). The schedule of five days of IV treatment followed by five days of IM therapy was pharmacologically and clinically as effective as ten days of IV therapy and has practical advantages.
