[Use and effectiveness of scintigraphy, sonography and angiography in circumscribed disease of the liver (author's transl)].

The effectiveness of scintigraphy, sonography and angiography is compared in a material of 140 cases of circumscribed disease of the liver. Typical clinical starting points are defined as points of reference of the diagnostic techniques. Clinical diagnosis is very safe in cases of secondary blastoma and hepatocellular carcinoma of the liver. Since the great majority of these conditions cause space-occupying lesions of the liver, problems of radiologic diagnosis are confined to the smaller number of clinically ill-defined lesions. For the diagnosis of secondary blastomas and cystic process of all kinds, scintigraphy and sonography are not improved by angiography. Solid space-occupying lesions of unknown origin, however, can only be recognized with angiography. Histology of both benign and malignant processes will be correct in more than 90% of cases.

[Differential diagnosis of space-occupying lesions of kidneys (comparison between IVP, angiography and ultra-sound) (author's transl)].

On 77 IVP's it can be shown that correct assessment of the identity of space-occupying lesions of the kidneys is difficult (correct diagnosis in 39% of cystic and 10% of solid conditions). Improvement can be expected from early tomography of IVP's by improved pictures of the parenchyma, mainly with ultra-sound which separates cystic from solid conditions. Correlation of these techniques appears sufficient for cystic processes. Angiography is indicated for solid tumors and doubtful ultra-sound findings.

[Pharmacoangiography of the pancreas (author's transl)].

The value of pancreatic pharmacoangiography was assessed in 269 visceral angiograms comparing pre- and postpharmacoangiographic series. Angiotensin as a vasoconstrictor was studied in 179 patients, Adrenalin in 50 patients. Experiences with vasodilators based on 40 angiograms carried out with Secretin and Tolazolin were evaluated respectively. After application of Angiotensin, enhanced filling of small arteries in the head of the pancreas was achieved in 75% of patients. Good filling of small arteries of the body of the pancreas rose from 26 to 55%, while angiography with vasoconstrictors seems of low value in the tail of this organ (20% improvement). The diagnostic information of angiography was improved in 4 of 43 carcinomas (10%). Confirmation or exclusion of questionable pathologic alterations are a further, though not measurable advantage of this method. Pharmacoangiography with constrictors is recommended as an alternative method if superselective angiography is not possible. The use of vasodilators for better imaging of the pancreatic parenchyma is of no significant diagnostic value. The use of vasodilators for better presentation of the spleno- and mesenterico-portal axis is advisable because dilatation of the capillary system permits application of larger doses of contrast medium and a higher flowrate. This causes early and more intensive imaging of the veins.

[The importance of magnification angiography as an investigation for the arterial supply of the femoral head after medial neck fracture (author's transl)].

In observing the development in 20 patients who had been operated upon for fracture of the neck of the femur a significant interrelation between the angiographic result and the formation of a necrosis of the head of the femur has been ascertained. The magnification angiography enables the terminal arterial region of the femoral vessels to be assessed with a still higher degree of certainty and thus appears to be a valuable diagnostic measure for the prognostic assessment of the medial fracture of the femoral neck.

[Autologous renal transplantation for renovascular hypertension (author's transl)].

Renovascular hypertension was diagnosed in a 20-year-old man as being due to bilateral renal-artery stenoses and infrarenal aortic coarctation. Reconstruction of the renal arterial supply by vein graft and aortorenal bypass failed to reduce the hypertension. But after bilateral autotransplantation of the kidneys into the iliac fossa and a dacron lay-on graft over the coarcted region the arterial blood pressure returned to normal. Pre-operative measurement of plasma-renin concentration in renal vein blood and the patient's age are essential factors in deciding on reconstructive surgery of a stenosed renal artery.

[Angiography, scintigraphy and ultra sound in the diagnosis of splenic or hepatic disease in Hodgkin's disease (author's transl)].

The combined use of selective angiography, scintigraphy and ultra-sound is not reliable enough for the certain exclusion of visceral involvement in Hodgkin's disease. Since this is of importance in the planning of treatment, it confirms the recommendation made by Wagenknechts, Glatsteins and others that a diagnostic splenectomy be carried out in order to determine the presence of visceral involvement.

[The basis of tumor verification using thermography].

The gradient between interior body temperature and external temperature was gradually measured in young pigs, since their skin resembles the properties of human skin. The interior temperature appeared to extend to 2.5 cm under the surface of skin. Then it drops, at first slowly and from 1 cm under the surface rapidly. Tumors or inflammations can only warm the skin directly when their site is 1 cm or less under the skin. Tumors which lie deeper may warm the skin via reflex zones. Warming by heat conduction is prevented by convection. Heating plates inserted 1.8 cm and 2 cm under the skin surface and heated 5 degrees beyond surrounding temperature do not warm the skin surface. This can be demonstrated by thermography. The importance of these results for thermographic tumor diagnosis is discussed.

[Pulmonary perfusion, ventilation, and gas exchange during modified cobalt-60 teletherapy of operated on breast carcinomas].

Post ablationem mammae, 22 patients were exposed to a modified telecobalt-irradiation because of mammary carcinoma. (Tangential pendulum irradiation of the thoracic wall with deflection of the central beam off the pendulum axis, parasternal irradiation with pendulum fields, irradiation of the supraclavicular and axillar area with deflection of the central beam to lateral and cranial directions.) During radiation therapy and up to six months after its termination, periodical pulmonary function tests were accomplished, consisting in a radiogram of the thorax, in spirometric check-up and blood gas analysis, and in an examination of pulmonary perfusion and ventilation using radionuclide techniques. In one patient with metastases to the ribs, the spirographic and nulear-medical ventilatory results changed for the worse in the course of time, whereas the other 21 patients did not show any significantly abnormal alterations at follow-up examinations.
