[Radiological diagnosis of visceral manifestations in Hippel-Lindau syndrome (author's transl)].

The efficiency of radiological methods in the diagnosis of visceral manifestations of Hippel-Lindau Syndrome is discussed by means of a case report as well as by the results from studies of other authors. The importance of detecting small renal malignancies (often occurring bilaterally and multifocally in this disease) is stressed since benign (cystic, adenomatous, angiomatous) lesions of visceral organs are of minor clinical relevance. Pheochromocytomas, found in about 20% of cases, are primarily diagnosed clinically. The diagnostic goal of precise quantification of neoplastic renal tumors and of unequivocal differentiation between cystic and solid lesions appears to be achieved by the combination of computertomographic and pharmaco-angiographic techniques. A prerequisite for the alternative use of ultrasound is optimal imaging of all parts of renal parenchyma.

[The value of angiography, computerized tomography and sonography in the diagnosis of chronic renal failure (author's transl)].

Concerning information with regard position, size and contour, there are no significant differences in sonography, computertomography and angiography. Sonography and CT have the advantage in demonstrating changes of the pyelon whereas angiography is the only method, which permits analysis of the vascular branches. Sonography should be used as primary investigation to detect morphological changes in chronic renal failure. CT seems important if, due to technical problems, sonography is not possible. Angiography is restricted to problems concerning the vascular tree.

[Sonography in the Diagnosis of Intrahepatic Cholangiectasis (author's transl)].

The sonographic pattern of signs associated with cholangiectasis is analysed in 25 patients. The extent of dilatation of the bile ducts correlates with the bilirubin level and the duration of the disease. Discrepancies occur in fibrose processes in the biliary tract region and in localised cholangiectases. Sonography is a basic radiologic examination method if there is any suspicion of obstruction to flow in the biliary tract. Findings obtained via sonography are the starting-point of further examinations.

[Differential sonographic diagnosis of retroperitoneal tumors].

Presentation of the sonographic findings of 170 retroperitoneal masses: primary and metastatic neoplasms, abdominal aortic aneurysms, hematomas and abszesses. A typical diagnostically reliable sonographic pattern was seen only in retroperitoneal hematomas in the area of the psoas and in aortic aneurysms, provided the aorta itself could be visualized. In all the other masses the ultrasound pattern did not provide reliable clues as to their histology; ultrasound, however, proved to be highly successful in demonstrating the presence and location of the lesion.

[Radioimmunological determination of parathyroid hormone: regional localization in hypercalcaemic hyperparathyroidism (author's transl)].

Angiographic or scintigraphic localization of parathyroid adenomas is successful in only a small number of patients with hypercalcaemic hyperparathyroidism. This report is concerned with the preoperative localization of parathyroid adenomas by regional catheterization of the thyroid veins in patients with hypercalcaemic hyperparathyroidism (n = 7). PTH was determined radioimmunologically, using an antiserum specific for PTH1-84 and the carboxyl-terminal fragment of the hormone. Determination of regional differences in the plasma concentration of PTH permitted the preoperative localization of PTH secreting adenomas. The preoperative localization of parathyroid adenomas was confirmed in each instance by surgery. Thus, the regional determination of plasma PTH represents a tool to improve the preoperative localization of parathyroid adenomas in patients with hypercalcaemic hyperparathyroidism and, hence, to reduce the risk of an unsuccessful operation.

[Double-contrast examination in hypotonia in the operated stomach (author's transl)].

The technique of examination, as well as the extent and quality of visualisation are described, basing on a selected case material of patients who had been subjected to stomach surgery; the extensive possibilities of diagnosis are studied. Hypotonia and double contrast offer considerable advantages over the conventional examination of the operated stomach. Assessment of mucosal conditions, especially in the anastomosis range, is facilitated, thus offering a chance to improve the method of diagnosis. Hypotonic double-contrast examination should always be performed if there are no possibilities of endoscopic examination.

[Acute left decompensation--significance of thoracic bed roentgenography for assessing pulmonary venous pressure (author's transl)].

25 thoracic bed x-ray films from 20 intensive-care patients were classified roentgenologically according to three degrees of severity of left decompensation, and compared with the end-diastolic pulmonary arterial pressure. Very good correlation was seen in 60% of the cases and a satisfactory agreement in 30%, whereas a discrepancy compared with haemodynamic parameters was observed in about 10% of the patients. Possibilities of explaining discrepancies are discussed within the framework of error analysis.

[Exploration of pancreaticojejunal anastomosis by postoperative endoscopic pancreaticography (author's transl)].

Endoscopic pancreaticography (EP) was performed in 9 patients after pancreaticojejunal anastomosis had been established surgically; in 2 of these patients EP was not successful. The anastomosis could be demonstrated in 5 cases to be functioning 1-6 years after operation; in 3 cases the duct system was found to be dilatated, in 2 cases diameters were normal. EP and clinical symptoms correlated well in 4 of 5 cases.

[Parathyroid hormone and 25-hydroxycholecalciferol concentrations in the serum of patients with kidney transplantations and their radiological correlation].

In 40 renal transplant patients the serum concentration of parathyroid hormone, 25-hydroxycholecalciferol, calcium and inorganic phosphate were estimated. The results of these biochemical studies were compared to X-ray soft tissue and bone pathologic findings. The serum immunoreactive parathyroid hormone (iPTH) was normal in 37 patients and increased in 3 cases. 19 (47.5%) renal transplant recipients showed decreased serum 25-hydroxycholecalciferol levels. In 23 out of 40 patients (57.5%) serum phosphate was lower than 3.0 mg/100 ml. X-ray soft tissue changes were observed in 28 (70%) and bone changes in 18 (45%) patients; both frequencies were higher when compared to occurrence during regular dialysis treatment. Features of hyperparathyroid osteopathy were frequently observed in renal transplant recipients. Femoral head necroses were predominant in men, whereas osteomalacia with Looser's zones were found mainly in women. The pathogenesis of X-ray bone changes in patients with renal transplants seems to be multifactorial and related to the duration of secondary hyperparathyroidism before renal transplantation as well as to phosphate depletion and 25-hydroxycholecalciferol deficiency.

[The diagnostic scope of ultrasonography in renal pathology (author's transl)].

The accuracy of ultrasonography in the diagnosis of different pathological renal conditions is evaluated. Correlations were established to the findings on intravenous urography and selective angiography. Ultrasonography is suitable for the determination of the exact size and site of the kidney and, therefore, should be used routinely in the follow-up of renal transplant recipients. The ability to differentiate between solid and cystic lesions is of partcular value. Moreover, perirenal changes are easily recognizable. The limits of B-mode sonography lie in the interpretation of central expanding lesions with a diameter of under 3 cm. This non-invasive method should be used selectively in the diagnosis of renal disease, whereas angiography should be limited to the classification of equivocal ultrasound findings and in the case of solid tumours.

[Ultrasound in gall bladder diagnosis (author's transl)].

An analysis of the ultrasound findings in 104 selected patients with suspected gall bladder disease showed a high accuracy of 92%. Limiting factors in the use of ultrasound are anatomical variations, high position of the gall bladder, which prevents demonstration of the gall bladder. The primary use of ultrasound is in the demonstration of radiologically invisible gall bladders. In our clinical material 58% had stones, with occlusion of the cystic duct as a cause, in 29% there was malignant disease in the porta hepatis, with evidence of bile obstruction amongst those patients in whom there was no radiological opacification of the gall bladder.

[Sonography for clarify complications after renal transplantation (author's transl)].

The diagnostic value of the ultrasound method was studied via 170 individual examinations in 75 patients. The method is particularly well suited for an accurate determination of the location and size of the transplanted kidney and for demonstrating perirenal growing and displacing processes such as cystic lymphangiomas, haematomas and abscesses. In addition, ultrasound supplies information on the area covered by lacunae. However, the use of the method for diagnosing casting-off reactions is limited on account of the great variability of the dimensional parameters. It can be used without risk in followup studies of patients with transplanted kidneys, since it is a non-invasive method. It deserves preference among the possible methods of investigation, especially when growing and displacing processes have to be identified.

[Pulmonary metastatic calcifications in malignant lymphoma demonstrated by imaging agents. Case report (author's transl)].

This report describes a patient with malignant lymphoma and hypercalcemic crises. A bone scan showed pronounced uptake of 99mTc-Methylene diphosphonate (MDP). This finding is thought to be due to metastatic lung calcification, which absorbs skeletal radiopharmaceuticals. The developement of metastatic calcifications in this case is most likely caused by phosphate applications to treat hypercalcemia. The mechanisms of formation and the detection of metastatic lung calcification with 99mTc-MDP are discussed.

[Therapeutic and diagnostic problems in a patient with insulinoma (author's transl)].

Surgical intervention in organic hyperinsulinism is based on prior accurate biochemical assessment. The emergent problems are discussed in a case with hypoglycaemic attacks, where "blind" pancreatic resections had been performed in two occasions without success. Finally, after demonstration of hyperinsulinaemia and localization of the tumour by selective angiography an adenoma was removed surgically. The postoperative complications, including the development of insulin-dependent diabetes, demonstrate the inherent risks of blind distal pancreatectomy for suspected insulinoma.

[Differential diagnosis of liver disease by means of sonography: correlation with scintigraphy and angiography (author's transl)].

The value of sonography of the liver is assessed and compared with angiography and scintigraphy on the basis of case material from 102 patients, representing localized and diffuse liver processes. The diagnostic accuracy depends on the characteristic acoustic impedance differences in the examined tissue. Diffuse liver disease and processes without clearly defined acoustic borderlines may cause diagnostic difficulties and differentiation can only be attempted by additional clinical information. Defined tumours represent the main field of application. The combined use of ultrasound, scintigraphy and angiography permits a correct diagnosis to be made in nearly 90% of cases. Differential diagnosis between cystic and solid processes is possible only by ultrasound.

[Exocrine pancreatic function in chronic alcoholics in Austria (author's transl)].

97 chronic alcoholic patients were investigated with regard to exocrine pancreatic function. Chronic calcifying pancreatitis was radiologically established in 7.2% of the cases. Secretin-pancreozymin tests were performed in 30 patients. Pancreatic insufficiency was found in 8 patients, while 8 patients showed marked hypersection. The daily faecal fat excretion in 10 other patients with partial gastrectomy and Billroth II operation was 5.3 g. Pancreatic disorders were, thus, detected in 53.2% cases of chronic alcoholism. This is higher than expected for Austria.

[The clinical relevance of scintigraphic adrenal imaging (author's transl)].

Labelled steroid precursors have been found to be valuable in scintigraphic imaging of localized adrenal disease or even in adrenal hyperplasia. Scanning procedures are non-invasive diagnostic methods and, in the case of the adrenals the images reflect anatomical as well as functional changes in the organs, constituting an advantage over angiographic methods. However, careful preparation of the patient, sophisticated technical prerequisites and a case-oriented programme of the procedure favouring either dexamethasone suppression, comparsion with phantom measurements adrenal profiles and/or multichannel analysis are necessary for the attainment of optimum results on scintigraphy.
