A recent analysis of data from earlier papers on the relationship between dental caries and drinking water fluoride concentration suggested that the commonly accepted inverse relationship did not exist. Our reanalysis of those data, however, confirms the well-known association between fluoride concentration and dental caries. It also shows that the contrary result arose misleadingly from three simultaneous methodological errors: use of a unifactorial instead of a multifactorial model; omission of or over-aggregation of some data, and analysis of homoscedastic probits instead of heteroscedastic counts.
BACKGROUND: Both the potential analgesic effects and side-effects of topical diclofenac sodium 0.1% are points of interest after excimer laser phototherapeutic keratectomy. METHODS: Excimer laser phototherapeutic keratectomy was performed in 134 eyes of 134 patients. In 65 eyes (65 patients), the effects of topical diclofenac given three times a day for 3 to 4 days were compared to a control group of 69 eyes (69 patients). All patients received paracetamol for systemic analgesia and were patched after surgery until reepithelialization. RESULTS: Twenty-eight patients (43%) of the diclofenac group needed additional systemic analgesics compared to 67 patients (95%) in the control group. Seventy-two hours after surgery we found no significant differences in corneal epithelial wound healing and no severe complications. CONCLUSION: Topical diclofenac sodium reduces postoperative pain in patients after phototherapeutic keratectomy.
BACKGROUND: The 193-nanometer excimer laser is used for photoablative reprofiling of the cornea to correct refraction errors and to treat corneal pathologies. To use the unique properties of the excimer laser, some technical features should be provided by the excimer laser system. A new excimer laser system is presented. METHODS: The instrument has a fluence sensor for precise measurement of the actual fluence just before surgery, a new delivery system including an integrator system to homogenize the laser beam, a steelband with apertures and a zoom optic to get the desired form of central reprofiling, and an alignment system to reduce the possibility to decenter the optical zone. RESULTS: The fluence before surgery can be measured very precisely with the fluence sensor. The ablation zone size is at least 8 mm in diameter with a homogenous laser beam profile resulting in a smooth surface of the treated material. Different forms of ablations can be created using the steelband with different forms of apertures. The center of the ablation zone can be found using the alignment system. CONCLUSION: The new excimer laser system fulfills some important technical features for optimal use of the excimer laser in corneal surgery.
We present our clinical strategy to treat corneal diseases with the excimer laser. Nine eyes with recurrent erosion after trauma, three eyes with band keratopathy, and one eye with secondary crystalline deposits and a leukoma adherens after perforating injury underwent excimer laser phototherapeutic keratectomy (PTK). The patients with recurrent erosion had at least six episodes in the 12 months, the other patients suffered from severe pain. The recurrent erosions were treated with 15 pulses (193 nm, 180 mJ/cm2, 10 Hz) after removing the epithelium. There was only one new episode within 6 months after treatment in a patient who failed to follow the postoperative therapeutic regimen. In the patients with band keratopathy we removed the calcium deposits mechanically and then used masking fluids to obtain a smooth surface. The crystalline deposits in the patient with leukoma adherens could be removed but recurred after 4 months; the patient still suffered from glare.
